Introducción: Las periodontitis son infecciones asociadas a microorganismos patogénicos y alteraciones en la respuesta del huésped. El estrés psicosocial y las conductas de afrontamiento inadecuadas pueden ejercer efectos inmunosupresivos, incrementando la susceptibilidad. El objetivo del estudio fue investigar la correlación entre estrés psicosocial, conductas adaptativas y tabaquismo con el estado periodontal. Materiales y método: Se reclutaron 166 sujetos voluntarios de ambos sexos, sanos sistémicamente, entre 25 a 65 años (41,2±11,3), que asistieron a la Facultad de Odontología de la UNCuyo. Para la valoración del estrés y conductas adaptativas se aplicaron: Escala de Reajuste Social (SRRS), Inventario de Síntomas de Estrés (SSI), cuestionario de conductas de afrontamiento (COPE). Se determinó el nivel de cortisol salival y se evaluó el hábito tabáquico. Los parámetros periodontales evaluados fueron: profundidad de sondaje, pérdida de inserción, índice gingival y de placa. El estudio estadístico incluyó análisis de regresión logística bivariado y multivariado, prueba de chi cuadrado, ANOVA. Resultados: Se demuestra asociación significativa por análisis bivariado: edad y estado periodontal (p<0,0001); conductas adaptativas y estado periodontal (p<0,0001) OR 5,3 IC95% (2,2-12,6); tabaquismo y estado periodontal (p<0,0001) OR 3,36 IC95% (1,6-7,0); cortisol y estado periodontal (p=0,0008). El análisis bivariado no encontró significancia entre estrés y estado periodontal. El análisis de regresión logística multivariado encontró asociación significativa de la relación estado periodontal con estrés (p=0,014), conductas adaptativas (p=0,030), tabaco (p=0,031), edad (p<0,0001). Conclusiones: Los sujetos que presentan estrés, inadecuadas conductas adaptativas y fuman son más susceptibles al deterioro periodontal. Valores altos de cortisol salival se asociaron a mayor deterioro periodontal.

Background: The prevalence of Diabetes mellitus (DM) is on a rise in sub-Saharan Africa and will more than double by 2025. Cardiovascular disease (CVD) accounts for up to 2/3 of all deaths in the diabetic population. Of all the CVD deaths in DM, 3/4 occur in sub Saharan Africa (SSA). Non invasive identification of cardiac abnormalities, such as Left Ventricular Hypertrophy (LVH), diastolic and systolic dysfunction, is not part of diabetes complications surveillance programs in Uganda and there is limited data on this problem. This study sought to determine the prevalence, types and factors associated with echocardiographic abnormalities among newly diagnosed diabetic patients at Mulago National referral hospital in Uganda. Methods: In this cross sectional study conducted between June 2014 and December 2014, we recruited 202 newly diagnosed adult diabetic patients. Information on patients\' socio-demographics, bio-physical profile, biochemical testing and echocardiographic findings was obtained for all the participants using a pre-tested questionnaire. An abnormal echocardiogram in this study was defined as the presence of LVH, diastolic and/or systolic dysfunction and wall motion abnormality. Bivariate and multivariate logistic regression analyses were used to investigate the association of several parameters with echocardiographic abnormalities. Results: Of the 202 patients recruited, males were 102(50.5%) and the mean age was 46±15 years. Majority of patients had type 2 DM, 156(77.2%) and type 1 DM, 41(20.3%) with mean HbA1C of 13.9±5.3%. Mean duration of diabetes was 2 months. The prevalence of an abnormal echocardiogram was 67.8 % (95% CI 60%-74%). Diastolic dysfunction, systolic dysfunction, LVH and wall motion abnormalities were present in 55.0%, 21.8%, 19.3% and 4.0% of all the participants respectively. In bivariate logistic regression analysis, the factors associated with an abnormal echocardiogram were age (OR 1.09 [95% CI 1.06–1.12], P <0.0001), type 2 DM (OR 5.8[95% CI 2.77-12.07], P<0.0001), hypertension (OR 2.64[95% CI 1.44-4.85], P=0.002), obesity (OR 3.51[955 CI 1.25-9.84], P=0.017 and increased waist circumference (OR 1.02[95% CI 1.00-1.04], P=0.024. On Multiple logistic regression analysis, age was the only factor associated with an abnormal echocardiogram (OR 1.09[95%CI 1.05-1.15], P<0.0001). Conclusion: Echocardiographic abnormalities were common among newly diagnosed adults with DM. Traditional CVD risk factors were associated with an abnormal echocardiogram in this patient population. Due to a high prevalence of echocardiographic abnormalities among newly diagnosed diabetics, we recommend screening for cardiac disease especially in patients who present with traditional CVD risk factors. This will facilitate early diagnosis, management and hence better patient outcomes.

Background: Adherence to controller therapy in asthma is a major concern during the management of the disease. Objective: To determine the adherence rate and identify the predictors of low adherence to asthma controller therapy. Methods: A cross-sectional study including asthma patients was conducted from November 1, 2012 to May 31, 2013 in 4 chest clinics in Cameroon. The adherence to asthma treatment was rated using Morisky Medication Adherence Scale. A multivariate logistic regression analysis was performed for the identification of factors associated with adherence to asthma treatment. Results: Among the 201 asthma patients included, 133 (66.2%) were female. The mean age of participants was 41.2 years. Sixty-one (30.3%) of the patients did not visit the chest physician during the last year prior to the study. Asthma was well controlled in 118 patients (58.7%). The prevalence of low adherence rate to asthma controller therapy was 44.8% and the absence of any chest specialist visit within the last 12 months was the only factor associated with the low adherence rate to asthma treatment (OR 5.57 ; 95% CI 2.84–10.93). Conclusion: The adherence rate to asthma controller therapy in Cameroon is low and it could be improved if scheduled visits are respected by patients.

La hemorragia intraventricular constituye una complicación que con mucha frecuencia se asocia a la prematuréz; a pesar de los avances en atención hospitalaria, que buscan disminuir el número de recién nacidos prematuros, éstos continúan presentándose y por lo tanto el desarrollo de hemorragia intraventricular se detecta cada vez con más frecuencia en este grupo de recién nacidos, es sabido además de la influencia de un número importante de factores de riesgo que contribuyen a la presencia de dicha patología. Objetivo: conocer la incidencia y los factores de riesgo que llevan al desarrollo de hemorragia intraventricular en recién nacidos prematuros atendidos en el servicio de neonatos y cuidados intensivos neonatales del Hospital de Niños Benjamín Bloom. Dicho hospital recibe continuamente recién nacidos prematuros, que en diferente grado presentan hemorragia intraventricular; por lo que resultó necesario determinar con qué frecuencia se desarrolló ésta entre los prematuros atendidos y además cuales fueron los factores de riesgo asociados. Método: se realizó un estudio analítico de casos y controles, de fuentes documentales a partir de los expedientes clínicos de los recién nacidos prematuros que fueron atendidos en el servicio de neonatos y cuidados intensivos neonatales del HNNBB de enero de 2012 a diciembre de 2013. Se llevó a cabo la revisión de 140 expedientes clínicos, uno de ellos se excluyó por presentar múltiples malformaciones congénitas, que constituía un criterio de exclusión, de éstos datos se utilizaron para el análisis 117 expedientes, ya que a 22 pacientes no se les realizó ultrasonografía transfontanelar, sin tener información de las razones por las que no se tomó dicho estudio. Resultados: el estudio demostró que la hemorragia intraventricular se presentó en 25 pacientes, con una frecuencia del 21.4%, similar a los datos encontrados en las referencias bibliográficas. Los factores de riesgo parto vaginal se presentó en 15 pacientes (60%), APGAR bajo al primer minuto (6.52±2.06), presencia de síndrome de dificultad respiratoria que se identificó en 21 pacientes (84%), la ventilación mecánica se necesitó en el 96% de los pacientes (24) y el uso de derivados sanguíneos con un promedio de 3.6±3.13 transfusiones, se relaciona al desarrollo de hemorragia intraventricular con mayor frecuencia, pero no se asociaron de forma significativa. Los factores que con mayor significancia se asociaron al desarrollo de esta patología en la población estudiada fueron: Edad gestacional ˂ 28 semanas (OR 4.9), peso menor de 1500 gramos (OR 5.48) y presencia de sepsis neonatal (OR3.77)

Background: Improper handling has been identified as one of the major reasons for the decline in vaccine potency at the time of administration. Loss of potency becomes evident when immunised individuals contract the diseases the vaccines were meant to prevent. Objective: Assessing the factors associated with vaccine handling and storage practices. Methods: This was a cross-sectional study. Three-stage sampling was used to recruit 380 vaccine handlers from 273 health facilities from 11 Local Government areas in Ibadan. Data was analysed using SPSS version 16 Results: Seventy-three percent were aware of vaccine handling and storage guidelines with 68.4% having ever read such guidelines. Only 15.3% read a guideline less than 1 month prior to the study. About 65.0% had received training on vaccine management. Incorrect handling practices reported included storing injections with vaccines (13.7%) and maintaining vaccine temperature using ice blocks (7.6%). About 43.0% had good knowledge of vaccine management, while 66.1% had good vaccine management practices. Respondents who had good knowledge of vaccine handling and storage [OR=10.0, 95%CI (5.28 – 18.94), p < 0.001] and had received formal training on vaccine management [OR=5.3, 95%CI (2.50 – 11.14), p< 0.001] were more likely to have good vaccine handling and storage practices. Conclusion: Regular training is recommended to enhance vaccine handling and storage practices.

Case-control studies evaluating the factors associated with childhood obesity are scarce in Brazil. We aimed to analyze the factors associated with obesity in Brazilian schoolchildren enrolled in the School Health Program.A case-control study was conducted on 80 schoolchildren aged 7 to 9 years, 40 of them obese and 40 of normal weight according to the cut-off points established by the World Health Organization (2007). Weight, height and waist circumference were obtained. Socioeconomic, demographic, health, eating behavior and lifestyle data were collected by applying a questionnaire to the person responsible and by determining his/her nutritional status. A binary unconditional logistic regression model (univariate and multivariate) was used for data analysis. The prevalence of obesity was 7.21%. The final model showed that duration of breast-feeding ≥6 months of age (OR 5.3; 95% CI: 1.3-22.1), excess weight of the person responsible (OR 7.1; 95% CI: 1.2-40.2), a sedentary level of physical activity (OR 4.1; 95% CI: 1.115.5), and fast chewing (OR 7.4; 95% CI: 2.1-26.9) were significantly associated with childhood obesity. The factors associated with obesity in schoolchildren were duration of breast-feeding ≥6 months, persons responsible with excess weight, and sedentary children who chew fast. The present study contributes information to be used for the health actions planned by the School Health Program.

Aim: This study was designed to determine the prevalence of and risk factors for schistosomiasis among a group of preschool children in Malawi. Schistosomiasis burden among preschoolers in Malawi is not well documented in the literature. Methods: This study used field research (in the form of a snail survey), laboratory work (urinalysis and microscopy for parasite identification), and questionnaireguided interviews to determine the prevalence of and risk factors for urinary schistosomiasis among children, aged between 6 and 60 months, in Malengachanzi, Nkhotakota District, Malawi. Results: Urinary schistosomiasis prevalence among preschool children was 13%. Of the factors evaluated, only age (P = 0.027) was statistically significantly associated with urinary schistosomiasis risk. Four-year-old preschool children were five times more likely to contract urinary schistosomiasis than two-year-old children (odds ratio [OR] = 5.255; 95% confidence interval [CI] = 1.014-27.237; P = 0.048). Increased contact with infested water among older children likely explains much of their increased risk. Infestation was evidenced by the presence of infected Bulinus globosus snails in the water contact points surveyed. Multiple regression analysis showed that visiting water contact sites daily (OR = 0.898, 95% CI = 0.185-4.350, P = 0.894), bathing in these sites (OR = 9.462, 95% CI = 0.036-0.00, P = 0.430) and lack of knowledge, among caregivers, regarding the causes of urinary schistosomiasis (OR = 0.235, 95% CI = 0.005-1.102, P = 0.066) posed statistically insignificant risk increases for preschoolers contracting urinary schistosomiasis. Conclusions: Urinary schistosomiasis was prevalent among preschool children in Malengachanzi, Nkhotakota District. Contact with infested water puts these children and the general population at risk of infection and reinfection. Inclusion of preschool children in treatment programmes should be considered imperative, along with safe treatment guidelines. To prevent infection, the population in the area should be provided with health education and safe alternative water sources.

Clostridium diffícile (CD) es el agente responsable de la mayoría de los episodios de diarrea de origen nosocomial. El objetivo de este estudio fue identificar los factores de riesgo asociados a la infección por C. difficile (ICD) en los pacientes con diarrea de origen nosocomial en el Hospital Universitario Dr. Manuel Quintela. Para ello se llevó adelante un estudio observacional, analítico, de casos y controles no pareados. Se incluyeron todos los pacientes mayores de 18 años que instalaron diarrea posterior a las 72 horas del ingreso al hospital y en quienes se solicitó investigación de CD durante el período abril a diciembre de 2013. Veintisiete pacientes (18,6%) cumplieron con la definición de caso, 105 (72,4%) cumplieron con la definición de control y 13 (9,0%) se excluyeron por presentar solo la prueba de glutamato deshidrogenasa (GDH) positiva. En relación con la presentación clínica, se destaca la asociación significativa de la ICD con la presencia de fiebre. En el análisis univariado el haber recibido antibióticos por más de 30 días y el uso previo de clindamicina estuvieron significativamente asociados a ICD. En el análisis multivariante solo el uso previo de clindamicina (p = 0,004 OR 5,881; IC95% 1,743-19,841) se mantuvo como factor de riesgo independiente. Este hallazgo pone en evidencia que en situación de endemia la implementación de una política de uso racional de clindamicina es una de las medidas necesarias para disminuir la frecuencia de ICD.

Antecedentes: el ataque cerebrovascular isquémico (ACVi) es causa de mortalidad y discapacidad importante en adultos, siendo la cardioembolia, sobre todo por fibrilación auricular (FA), una etiología importante. Se conocen otras etiologías, cardíacas y no cardíacas, que no siempre pueden definirse, refiriéndose como probables o posibles de cardioaortoembolia. El rol de alguna de ellas, como la ateromatosis aórtica (AA), continúa siendo discutido. Objetivo: analizar fuentes posibles de cardioaortoembolia en pacientes con ACVi reciente. Material y método: estudio caso-control. Se analizaron 100 sujetos con ACVi de menos de 30 días de evolución estudiados prospectivamente con ecocardiograma transesofágico (ETE), entre febrero de 2012 y marzo de 2014. Se apareó cada caso según edad, sexo, presencia de flutter/FA y ausencia de prótesis valvular cardíaca, con un control que se buscó retrospectiva y aleatoriamente entre sujetos con ETE estudiados entre 2010 y 2015 sin ACVi. Se compararon variables binarias mediante test de McNemar y las cuantitativas por test de t pareado; se realizó regresión logística múltiple para ACVi con variables clínicas y hallazgos de ETE y p < 0,1 en el análisis univariado. Se consideró significativo p < 0,05. Variables cuantitativas se expresan en media y desvío estándar (DE) y nominales en valor absoluto y porcentaje, índice de probabilidad (OR) en su valor e intervalo de confianza (IC) 95%. Resultados: se formaron finalmente 76 pares caso-control. Edad casos: 64,3±1,4 y control 64,9±1,3 años (p=0,19); 39 mujeres (51,3%) y 15 sujetos (19,7%) con flutter/FA en cada grupo (p=1). No hubo diferencias significativas según factores de riesgo cardiovascular (FRCV) globales (p=0,06) ni alcoholismo (p=0,80); se encontraron diferencias significativas en la prevalencia de dislipemia (p=0,03) e hipertensión arterial (HTA) (p <0,05). Las indicaciones principales del ETE en los controles fueron: previo a cardioversión eléctrica de FA, sospecha de endocarditis, y valoración de valvulopatía mitral. La ateromatosis aórtica proximal y compleja (AAPC) (p=0,002, OR 5,5, IC95% 1,9-15,9) y la AA en forma global (p=0,001, OR 4,1, IC95% 1,8-9,3), además de la dislipemia (p=0,02, OR 2,8 IC95% 1,2-6,4), se asociaron significativamente con la presencia de ACVi en el análisis multivariado. Conclusión: la AA aumenta significativamente las chances de ACVi y las quintuplica cuando es proximal y compleja. La dislipemia se asoció a ACVi.

Objetivo: Analizar las características asociadas al riesgo de feminicidio en España entre mujeres expuestas a la violencia de pareja o análogo y su posible asociación con las denuncias a los agresores. Métodos: Se realizó un estudio de casos y controles para el periodo 2010-2011. Los casos, 135 mujeres mayores de edad, asesinadas por su pareja o análogo durante dicho periodo, se identificaron a través de la página web de la Federación de Asociaciones de Mujeres Separadas y Divorciadas, y de los informes del Consejo General del Poder Judicial. Los controles, 185 mujeres expuestas a la violencia de pareja el último año, proceden de la Macroencuesta de Violencia de Género 2011. La asociación entre la denuncia y el riesgo de feminicidio se estimó mediante modelos de regresión logística multivariada. Resultados: No se encontró asociación entre denunciar al agresor y el riesgo de ser asesinada (odds ratio [OR]: 1,38; intervalo de confianza del 95% [IC95%]: 0,68-2,79). Las mujeres inmigrantes expuestas a la violencia de pareja registraron una mayor probabilidad de ser asesinadas (ref.: mujeres españolas; OR: 5,38; IC95%: 2,41-11,99). Esta asociación también se observó en las mujeres que vivían en zonas rurales (ref: zonas urbanas; OR: 2,94; IC95%: 1,36-6,38). Conclusiones: La denuncia judicial al agresor no parece modificar el riesgo de asesinato entre las mujeres expuestas a la violencia de pareja. Las medidas de protección a las mujeres deberían extremarse en las mujeres inmigrantes y las que viven en el medio rural.

We have shown Galanin(GAL) and Neuropeptide Y Y1(NPYY1) interactions at behavioural, cellular and receptor levels through GALR2/NPYY1R heterodimers in the amygdala. The aim of this work was to analyze GAL/NPYY1R interactions in the Dentate Gyrus(DG) of the Hippocampus, using autoradiographic, in situ hybridization and in situ proximity ligation assay(PLA). Rats(n=6) were sacrificed 15 minutes or 5 hours after icv injections of GAL(3nmol) and DG sections were incubated with NPYY1R agonist [I125]-[Leu31,Pro34]PYY(25 pM) or NPYY1R-33PdATP specific probe, for autoradiography and in situ hybridization respectively. Autoradiograms were analyzed using NIH image analysis system and Student’s unpaired t-test was used. For PLA, DG sections were incubated with anti-GALR2 Rabbit(1:100) and anti-NPYY1R Goat(1:200). PLA signals were detected with PLA PLUS or MINUS probes for rabbit or goat/mouse antibodies. PLA signals were visualized by using a confocal microscope Leica TCS-SL confocal microscope(Leica). We observed that GAL significant increased the NPYY1R agonist [I125]-[Leu31,Pro34]PYY binding in the DG by 20% (p<0,05) and the NPYY1R mRNA expression in the granular layer of DG by 31% (p<0,001). Moreover, PLA-positive red clusters were found specifically in the polymorphic layer and subgranular zone of the DG. No PLA clusters were observed neither in the molecular layer of the DG nor in the corpus callosum, an area that seems to lack of GALR2 receptor. These results demonstrate a novel mechanism of interaction between GAL and NPY1R in the DG at receptor level, probably involving the formation of GALR2/NPYY1R heteroreceptor complexes. El estudio se aplicó en niños escolares de 9-12 años de la parroquia urbano marginal de Narancay. La intervención se efectuó en dos meses y mediante las tres técnicas se evaluaron 120 molares con un total de 359 superficies examinadas. Resultados: el resultado global de la valoración de los diferentes índices de salud bucal presenta resultados positivos para calificarla como población de alto riesgo. El FDP detectó más lesiones de desmineralización del esmalte/caries incipientes que el examen clínico (311/359 vs. 168/359) p< 0.0001. El examen radiográfico detectó el menor número de lesiones incipientes (23/359 vs. 311/359) p< 0.00001. El FDP diagnostica más lesiones de desmineralización del esmalte y crisis incipiente que el examen clínico y radiográfico conjuntamente (311/359 vs. 191/359) p< 0.001 OR 5,70 (95% I C: 3,88 - 8,38). En cuanto a los costos, el diagnóstico clínico más Rx, por cada niño es de $4 y el diagnóstico clínico más FDP es de $1,50. Conclusiones: la localidad de Narancar de la parroquia Baños tiene una infraestructura sanitaria insuficiente, con baja escolaridad y con altos índices de enfermedad bucal. El diagnóstico dental de la población infantil que tanto los índices de CPOD-ceod, higiene buco dental, placa dental y los momentos de ingesta de azúcar, reflejan que el grupo de estudio tienen elevados factores de riesgo e indicadores de enfermedad buco denal

Top-down (grazing) and bottom-up (nutrient, light) controls are important in freshwater ecosystems regulation. Relative importance of these factors could change in space and time, but in tropical lakes bottom-up regulation has to been appointed as more influent. Present study aimed to test the hypothesis that phytoplankton growths rate in Armando Ribeiro reservoir, a huge eutrophic reservoir in semi-arid region of Rio Grande do Norte state, is more limited by nutrient available then zooplankton grazing pressure. Bioassay was conduced monthly from September (2008) to August (2009) manipulating two levels of nutrients (with/without addition) and two level of grazers (with/without removal). Experimental design was factorial 2X2 with four treatments (X5), (i) control with water and zooplankton from natural spot ( C ), (ii) with nutrient addition ( +NP ), (iii) with zooplankton remove ( -Z ) and (iv) with zooplankton remove and nutrient addition ( -Z+NP ). For bioassay confection transparent plastic bottles (500ml) was incubate for 4 or 5 days in two different depths, Secchi`s depth (high luminosity) and 3 times Secchi`s depth (low luminosity). Water samples were collected from each bottle in begins and after incubates period for chlorophyll a concentration analysis and zoopalnktonic organisms density. Phytoplankton growths rates were calculated. Bifactorial ANOVA was performance to test if had a significant effect (p<0,005) of nutrient addition and grazers remove as well a significant interaction between factors on phytoplankton growths rates. Effect magnitude was calculated the relative importance of each process. Results show that phytoplankton growth was in generally stimulated by nutrient addition, as while zooplankton remove rarely stimulated phytoplankton growth. Some significant interactions happening between nutrient additions and grazers remove on phytoplankton growth. In conclusion this study suggests that in studied reservoir phytoplankton growth is more controlled by ascendent factors than descendent

Hepatitis C infection (HCV) continues to disproportionately affect Hispanics/Latinos in the United States. Hispanic/Latino intravenous drug users (IDUs), because of their risky injection and sexual behaviors, are prone to HCV infection and rapid transmission of the virus to others via several routes. With a prevalence rate of approximately 75% among IDUs, it is imperative that transmission of HCV be prevented in this population. This study aims to examine the associations between demographic, injection and sexual risk factors to HCV infection in a group Hispanic/Latino IDUs in Miami-Dade County, Florida. Preliminary unadjusted results in this sample reveal that age (OR=4.592, p=0.004), weekly injection (OR=5.171, p=0.000), daily injection frequency (OR=3.856, p=0.000) and use of a dirty needle (OR=2.320, p= 0.006) were all significantly associated with HCV infection. Being born outside the U.S. was significantly negatively associated with HCV infection (OR=0.349, p=0.004). Additionally, having two or more sex partners in the past three months (OR=0.472, p=0.014) was negatively associated with HCV infection. After adjusting for all other variables, older age (AOR=7.470, p=0.006), weekly injection (AOR=3.238, p=0.007) and daily injection frequency (AOR=2.625, p=0.010) were all significantly associated with HCV infection. Being born outside the U.S. (AOR=0.369, p=0.019) was a significant protective factor for HCV infection, along with having two or more sex partners in the past three months (AOR=0.481, p=0.037). When analyzing the significant variables in a backward regression model, having 2 or more sex partners in the past three months was not significant at the p
