While the influence of HLA-AB and -DRB1 matching on the outcome of bone marrow transplantation (BMT) with unrelated donors is clear, the evaluation of HLA-C has been hampered by its poor serological definition. Because the low resolution of standard HLA-C typing could explain the significant number of positive cytotoxic T lymphocyte precursor frequency (CTLpf) tests found among HLA-AB-subtype, DRB1/B3/B5-subtype matched patient/donor pairs, we have identified by sequencing the incompatibilities recognized by CD8+ CTL clones obtained from such positive CTLpf tests. In most cases the target molecules were HLA-C antigens that had escaped detection by serology (e.g. Cw*1601, 1502 or 0702). Direct recognition of HLA-C by a CTL clone was demonstrated by lysis of the HLA class I-negative 721.221 cell line transfected with Cw*1601 cDNA. Because of the functional importance of Cw polymorphism, a PCR-SSO oligotyping procedure was set up allowing the resolution of 29 Cw alleles. Oligotyping of a panel of 382 individuals (including 101 patients and their 272 potential unrelated donors, 5 related donors and 4 platelet donors) allowed to determine HLA-C and HLA A-B-Cw-DRB1 allelic frequencies, as well as a number of A-Cw, B-Cw, and DRB1-Cw associations. Two new HLA-Cw alleles (Cw*02023 and Cw*0707) were identified by DNA sequencing of PCR-amplified exon 2-intron 2-exon 3 amplicons. Furthermore, we determined the degree of HLA-C compatibility in 287 matched pairs that could be formed from 73 patients and their 184 potential unrelated donors compatible for HLA-AB by serology and for HLA-DRB1/ B3/B5 by oligotyping. Cw mismatches were identified in 42.1% of these pairs, and AB-subtype oligotyping showed that 30% of these Cw-incompatible pairs were also mismatched for A or B-locus subtype. The degree of HLA-C incompatibility was strongly influenced by the linkage with B alleles and by the ABDR haplotypes. Cw alleles linked with B*4403, B*5101, B18, and B62 haplotypes were frequently mismatched. Apparently high resolution DNA typing for HLA-AB does not result in full matching at locus C. Since HLA-C polymorphism is recognized by alloreactive CTLs, such incompatibilities might be as relevant as AB-subtype mismatches in clinical transplantation.

The granules which appear in the nucleolar area in apoptotic HL-60 cells after camptothecin administration (Zweyer et al., Exp. Cell Res. 221,27-40, 1995) were detected also in several other cell lines induced to undergo apoptosis by different stimuli, such as MOLT-4 treated with staurosporine, K-562 incubated with actinomycin D, P-815 exposed to temperature causing heat shock, Jurkat cells treated with EGTA, U-937 growing in the presence of cycloheximide and tumor necrosis factor-alpha, and HeLa cells treated with etoposide. Using immunoelectron microscopy techniques, we demonstrate that, besides the already described nuclear matrix proteins p125 and p160, these granules contain other nucleoskeletal polypeptides such as proliferating cell nuclear antigen, a component of ribonucleoprotein particles, a 105-kDa constituent of nuclear spliceosomes, and the 240-kDa nuclear mitotic apparatus-associated protein referred to as NuMA. Moreover, we also found in the granules SAF-A/hn-RNP-U and SATB1 proteins, two polypeptides that have been reported to bind scaffold-associated regions DNA sequences in vitro, thus mediating the formation of looped DNA structures in vivo. Fibrillarin and coilin are not present in these granules or the PML protein. Thus, the granules seen during the apoptotic process apparently are different from coiled bodies or other types of nuclear bodies. Furthermore, these granules do not contain chromatin components such as histones and DNA. Last, Western blotting analysis revealed that nuclear matrix proteins present in the granules are not proteolytically degraded except for the NuMA polypeptide. We propose that these granules might represent aggregates of nuclear matrix proteins forming during the apoptotic process. Moreover, since the granules are present in several cell lines undergoing apoptosis, they could be considered a previously unrecognized morphological hallmark of the apoptotic process.

BACKGROUND: Diplopia related to neurosurgical procedures is often consecutive to oculomotor nerve lesions. We hereby report an oculomotor dysfunction secondary to an orbital roof effraction and its treatment. HISTORY AND SIGNS: Following surgery for a left anterior communicating artery aneurysm, a 45-year-old woman reported vertical diplopia associated with a left orbital hematoma. The diagnosis of third cranial nerve palsy was excluded by orbital imaging which revealed an orbital roof defect with incarceration of the levator palpebrae and superior rectus. THERAPY AND OUTCOME: As neurosurgeons advised against muscle adhesiolysis, diplopia was corrected by a two-step procedure on the oculomotor muscles. We first corrected horizontal and torsional deviations by operating on the healthy eye, before correcting the vertical deviation on the fellow eye. This two-step extraocular muscle surgery allowed restoration of binocular single vision in a useful field of gaze. CONCLUSIONS: Diplopia can occur as a rare orbital complication during neurosurgical procedures. Surgery of extraocular muscles can provide good functional results

BACKGROUND: We aimed to compare panitumumab, a fully human monoclonal antibody against EGFR, plus radiotherapy with chemoradiotherapy in patients with unresected, locally advanced squamous-cell carcinoma of the head and neck. METHODS: In this international, open-label, randomised, controlled, phase 2 trial, we recruited patients with locally advanced squamous-cell carcinoma of the head and neck from 22 sites in eight countries worldwide. Patients aged 18 years and older with stage III, IVa, or IVb, previously untreated, measurable (≥10 mm for at least one dimension), locally advanced squamous-cell carcinoma of the head and neck (non-nasopharygeal) and an Eastern Cooperative Oncology Group performance status of 0-1 were randomly assigned (2:3) by an independent vendor to open-label chemoradiotherapy (two cycles of cisplatin 100 mg/m(2) during radiotherapy) or to radiotherapy plus panitumumab (three cycles of panitumumab 9 mg/kg every 3 weeks administered with radiotherapy) using a stratified randomisation with a block size of five. All patients received 70-72 Gy to gross tumour and 54 Gy to areas of subclinical disease with accelerated fractionation radiotherapy. The primary endpoint was local-regional control at 2 years, analysed in all randomly assigned patients who received at least one dose of their assigned protocol-specific treatment (chemotherapy, radiation, or panitumumab). The trial is closed and this is the final analysis. This study is registered with ClinicalTrials.gov, number NCT00547157. FINDINGS: Between Nov 30, 2007, and Nov 16, 2009, 152 patients were enrolled, and 151 received treatment (61 in the chemoradiotherapy group and 90 in the radiotherapy plus panitumumab group). Local-regional control at 2 years was 61% (95% CI 47-72) in the chemoradiotherapy group and 51% (40-62) in the radiotherapy plus panitumumab group. The most frequent grade 3-4 adverse events were mucosal inflammation (25 [40%] of 62 patients in the chemoradiotherapy group vs 37 [42%] of 89 patients in the radiotherapy plus panitumumab group), dysphagia (20 [32%] vs 36 [40%]), and radiation skin injury (seven [11%] vs 21 [24%]). Serious adverse events were reported in 25 (40%) of 62 patients in the chemoradiotherapy group and in 30 (34%) of 89 patients in the radiotherapy plus panitumumab group. INTERPRETATION: Panitumumab cannot replace cisplatin in the combined treatment with radiotherapy for unresected stage III-IVb squamous-cell carcinoma of the head and neck, and the role of EGFR inhibition in locally advanced squamous-cell carcinoma of the head and neck needs to be reassessed. A transfusão sanguínea representa um dos possíveis tratamentos utilizados para restauração da oxigenação tecidual, restauração do volume sanguíneo no organismo e correcção dos distúrbios de coagulação. Quando realizada de forma insensata pode expor o receptor a sérias complicações, gerando aumento no custo do tratamento e propiciando desperdício de hemocomponentes. O presente trabalho objectiva analisar se as transfusões sanguíneas realizadas no Hospital Dr. Agostinho Neto foram coerentes com as normas internacionais de transfusão de hemocomponentes. Foi realizado um estudo transversal retrospectivo, envolvendo todos pacientes transfundidos durante o período de 91 dias. No período em causa foram realizadas transfusões de 452 unidades de hemocomponentes para o total de 509 unidades prescritas. Treze porcento das transfusões realizadas, abarcando pacientes com níveis de hemoglobina superiores a 7g/dL de sangue, referentes aos concentrados de hemácias e plasmas frescos congelados, não foram coerentes com as normas internacionais da transfusão sanguínea. A transfusão sanguínea é uma opção terapêutica em diversas situações clínicas e cirúrgicas, porém, o uso racional dos hemocomponentes promoveria a redução de riscos relacionados às transfusões, manutenção de estoques para situações de real necessidade, bem como diminuição de custos para o sistema de saúde.

Este trabalho foi realizado no âmbito dos regulamentos dos cursos de graduação para a obtenção do grau de licenciatura na universidade Jean Piaget de Cabo Verde. Nele abordamos os conceitos de Bussines Process Management (BPM), Bussines Process Automation (BPA) e Open Source, cujo seu principal objectivo é dar a conhecer o estudo e implementação de automatização de processo de negócio usando ferramentas de BPM Open Source e o seu impacto depois da implementação. No presente trabalho fez-se uma investigação sobre algumas ferramentas Open Source usadas na automatização de processos de negócio, e implementado um processo BPM no framework Activiti para automatização do processo de negócio segunda via dos números dos clientes da Unitel T+. Para a automatização do processo Segunda Via foi integrado os frameworks Activiti e o Liferay, visto que dado a realidade de uma operadora de telecomunicação existe uma plataforma de gestão dos dados dos clientes implementado no Liferay, e havia necessidade de integrar estes dois frameworks distintos. Um dos maiores desafios na integração dos dois frameworks foi a autenticação. Para dar resposta a este problema, obter maior nível de segurança e sincronizar os frameworks foi utilizado o Hook que permite uma única autenticação e balanceamento (sincronização) dos dados no Activiti e no Liferay.

Los resultados de fecundidad parcial, relativa y distribución espacial de cardúmenes activos de merluza Merluccius gayi peruanus durante el otoño 2003 (Cr. 0305-06), se comparan con los cruceros demersales de otoño 2002 y de verano 2003. Se discuten los valores hallados en cada parámetro, el estado de madurez gonadal en relación con la profundidad y las concentraciones de oxígeno de fondo. Se describe la probabilidad que la producción de huevos sea compensada por la fecundidad relativa en individuos de menor talla, ante la ausencia de hembras grandes en la población, por efecto de la pesquería.

Entre mayo y junio 2010, se desarrolló la evaluación poblacional de la macroalga Chondracanthus chamissoi “yuyo” de las praderas en las bahías de Pisco y Paracas. Las biomasas medias fluctuaron entre 2,2 y 67,0 g.m-2, con promedio de 43,6 g.m-2. Se estimó una biomasa total de 211,96 toneladas (±56,8%); de los cuales 179,74 t correspondieron a Atenas; 30,1 t a Puerto Nuevo y 2,1 t a Playa Lobería. El sustrato de fijación estuvo compuesto, principalmente, por piedras, conchuela y arena media. En las praderas de Atenas ocupó la mayor parte del área evaluada, en las praderas de Puerto Nuevo y Playa Lobería se encontraron reducidas, respecto al 2007. Se encontró ejemplares entre 1 y 25 cm de longitud. El peso total individual varió de 0,1 a 94,6 g. Se registró biodiversidad asociada conformada por moluscos. Las condiciones oceanográficas se registraron normales, lo que permite suponer que la disminución poblacional respondería a las actividades de extracción.
