Menée dans une approche d'histoire culturelle, cette thèse de doctorat prend pour objet un corpus de guides de voyage en Suisse entre la fin du XVIIIe et le début du XXe siècle. Centrée sur les guides, ces livres qui entretiennent plus que d'autres des liens étroits avec le monde physique, elle a deux grands axes. Le premier est une lecture interdisciplinaire des guides de voyage, qui mêle littérature, bibliographie matérielle, histoire et histoire de l'art. Elle a cherché à comprendre les raisons et logiques du genre, en s'attachant particulièrement à ses fonctions et à ses formes (tant structurelles que textuelles et iconographiques). Cette partie de l'étude est importante, car elle n'a encore jamais été menée. Elle s'articule en deux volets : un volet théorique qui s'intéresse à l'histoire et à la forme des guides de voyage ; et une étude de cas qui s'attache à la lecture plus rapprochée de 6 guides : ceux de Thomas Martyr (1788, 1790 & 1794), Heinrich August Ottokar Reichard (1793 & 1802) et Johann Gottfried Ebel (1793, 1805, 1810-11 & 1817-18) pour la fin du XVIIIe siècle et le tournant du XIXe, et ceux de John Murray (1838 & 1886), Adolphe Joanne (1841, 1865, 1874, 1882 & 1908) et Karl Baedeker (1844, 1852, 1859, 1869, 1876, 1883, 1893, 1901 & 1913) pour le XIXe et la Belle Epoque. Le second axe de cette recherche est une réflexion sur les manières de mettre en scène l'espace dans un texte. En étudiant les itinéraires de voyage en Suisse (mais jusqu'au début du XXe siècle, « la Suisse »est pour les guides de voyage indifféremment un pays et une région supranationale : «les Alpes »), quatre types de mises en forme ont pu être identifiés : le voyage en boucle (linéaire, il part d'un point A pour y revenir), le voyage en marguerite (linéaire avec excursions), le voyage éclaté de l'ordre alphabétique, et enfin le voyage par «routes », fragments d'espace que l'on combine comme les pièces d'un puzzle, créant son chemin au fur et à mesure de sa progression. Ce faisant, on peut affirmer que les guides de voyage modernes (dont la forme se fixe dans les années 1830-1840 avec les premiers Murray, Baedeker et Joanne) se sont construits -malgré tout ce que l'on a pu dire sur la normativité prescriptive du tourisme -autour d'une liberté de plus en plus grande accordée aux voyageurs. Chacune de ces formes et chacun de ces types ayant une histoire et des conditions de possibilités, c'est en s'appuyant sur celles-ci que l'on peut mieux comprendre non seulement l'évolution du voyage et de ses pratiques, mais aussi la constitution de la forme littéraire qui l'a accompagné et permis. Ce faisant, des jalons pour une histoire culturelle du tourisme ont aussi été posés, histoire culturelle que j'appelle maintenant de mes voeux : il est quand même surprenant que, dans le pays de tourisme qu'est la Suisse, quand on s'est jusqu'à présent attaché à l'histoire du tourisme, on n'ait parlé qu'économie, société, infrastructures, loisirs ou santé, voire, plus récemment, écologie et bien-être. Redonner son creuset culturel à ce phénomène, c'est aussi retrouver une part du nôtre, car ces mémoires s'entremêlent indissociablement.

El análisis de los ensayos de John Tyndall, Fragments of Science, permite identificar la teoría atómica, el principio de conservación de la energía y el evolucionismo darwinista como los elementos constitutivos del andamiaje teórico del naturalismo científico. Así, en su ensayo “On the Study of Physics” se resumen sus brillantes facetas como educador y divulgador científico, desarrolladas fundamentalmente en el seno de la Royal Institution. En la lectura “On Force”, Tyndall da por finalizada la controversia Joule- Mayer sobre la primacía del descubrimiento del principio de conservación de la energía, a la vez que plantea algunas de las claves de la lucha por el liderazgo en el seno de la comunidad científica. El discurso presidencial ante la British Association de 1874 en Belfast ejemplifica el coraje de Tyndall en su empeño por demarcar los territorios de la ciencia y la religión, a la luz de los nuevos desafíos científicos. En el trasfondo subyacen los procesos de secularización de la sociedad y de profesionalización de una comunidad científica heterogénea. El compromiso cívico que Tyndall demuestra en “The Belfast Address” es digno corolario de una vida y obra que permite situarlo como paradigma de lo que, en la terminología de Turner, se ha dado en denominar científico público.

OBJECTIVE: To compare the distribution of congenital anomalies within the VACTERL association (vertebral defects, anal atresia, cardiac, tracheoesophageal, renal, and limb abnormalities) between patients exposed to tumor necrosis factor-α (TNF-α) antagonist and the general population. METHODS: Analysis for comparison of proportional differences to a previous publication between anomaly subgroups, according to subgroup definitions of the European Surveillance of Congenital Anomalies (EUROCAT), a population-based database. RESULTS: Most EUROCAT subgroups belonging to the VACTERL association contained only one or 2 records of TNF-α antagonist exposure, so comparison of proportions was imprecise. Only the category "limb abnormalities" showed a significantly higher proportion in the general population. CONCLUSION: The high number of congenital anomalies belonging to the VACTERL association from a report of pregnancies exposed to TNF-α antagonists could not be confirmed using a population-based congenital anomaly database.

Over the past decades, several sensitive post-electrophoretic stains have been developed for an identification of proteins in general, or for a specific detection of post-translational modifications such as phosphorylation, glycosylation or oxidation. Yet, for a visualization and quantification of protein differences, the differential two-dimensional gel electrophoresis, termed DIGE, has become the method of choice for a detection of differences in two sets of proteomes. The goal of this review is to evaluate the use of the most common non-covalent and covalent staining techniques in 2D electrophoresis gels, in order to obtain maximal information per electrophoresis gel and for an identification of potential biomarkers. We will also discuss the use of detergents during covalent labeling, the identification of oxidative modifications and review influence of detergents on finger prints analysis and MS/MS identification in relation to 2D electrophoresis.

We report two unusual cases of sudden unexpected death in children. Histopathologic examination showed intimal fibroplasias, a variant of fibromuscular dysplasia of the right coronary artery associated in both cases with fatty infiltration of the right ventricular myocardium. The significance of this particular combination of two lesions known to induce a sudden death in young people is discussed.

OBJECTIVES: To examine the association between socioeconomic status (SES) and several cardiovascular disease risk factors (CVRFs) and to assess whether this association has changed over a 15-year observation period. METHODS: Three independent population-based surveys of CVRFs were conducted in representative samples of all adults aged 25-64 years in the Seychelles, a small island state located east to Kenya, in 1989 (N=1081), 1994 (N=1067) and 2004 (N=1255). RESULTS: Among men, current smoking and heavy drinking were more prevalent in the low versus the high SES group, and obesity was less prevalent. The socioeconomic gradient in diabetes reversed over the study period from lower prevalence in the low versus the high SES group to higher prevalence in the low SES group. Hypercholesterolemia was less prevalent in the low versus the high SES group in 1989 but the prevalence was similar in the two groups in 2004. Hypertension showed no consistent socioeconomic pattern. Among women, the SES gradient in smoking tended to reverse over time from lower prevalence in the low SES group to lower prevalence in the high SES group. Obesity and diabetes were more common in the low versus the high SES group over the study period. Heavy drinking, hypertension and hypercholesterolemia were not socially patterned among women. CONCLUSION: The prevalence of several CVRFs was higher in low versus high SES groups in a rapidly developing country in the African region, and an increase of the burden of these CVRFs in the most disadvantaged groups of the population was observed over the 15 years study period.

The investigation of unexplained syncope remains a challenging clinical problem. In the present study we sought to evaluate the diagnostic value of a standardized work-up focusing on non invasive tests in patients with unexplained syncope referred to a syncope clinic, and whether certain combinations of clinical parameters are characteristic of rhythmic and reflex causes of syncope. METHODS AND RESULTS: 317 consecutive patients underwent a standardized work-up including a 12-lead ECG, physical examination, detailed history with screening for syncope-related symptoms using a structured questionnaire followed by carotid sinus massage (CSM), and head-up tilt test. Invasive testings including an electrophysiological study and implantation of a loop recorder were only performed in those with structural heart disease or traumatic syncope. Our work-up identified an etiology in 81% of the patients. Importantly, three quarters of the causes were established non invasively combining head-up tilt test, CSM and hyperventilation testing. Invasive tests yielded an additional 7% of diagnoses. Logistic analysis identified age and number of significant prodromes as the only predictive factors of rhythmic syncope. The same two factors, in addition to the duration of the ECG P-wave, were also predictive of vasovagal and psychogenic syncope. These factors, optimally combined in predictive models, showed a high negative and a modest positive predictive value. CONCLUSION: A standardized work-up focusing on non invasive tests allows to establish more than three quarters of syncope causes. Predictive models based on simple clinical parameters may help to distinguish between rhythmic and other causes of syncope

Adorno es un referente en la disciplina musicológica. Convencido de que el hombre a través de la dialéctica materialista podía entender el mundo y cambiarlo, consideraba que el arte no debía ser mero exponente de la sociedad, sino fermento de ese cambio. La sociedad debía transformarse a sí misma desde ella misma, y el arte, producto de la sociedad, era un medio para ello. Ante el fenómeno fetichista de la sociedad industrializada, éste debía presentarse como antítesis de la misma, siendo la única vía posible para la conservación de su carácter de verdad el aislamiento. En el ámbito musical fue Schönberg quien se mostró distante al público de su tiempo a través de una música antagónica y disonante que reflejaba el momento de horror, angustia y barbarie del hombre contemporáneo.

Cognitive errors (CE) and coping strategies (CS) are the focus of most cognitive-behavioral treatments for incarcerated child molesters. Several studies have reported differences in CEs and CSs between child molesters and controls. However, the vast majority of these studies assessed cognitive errors and coping using questionnaires, which are known to present a number of important limitations. This pilot study aimed to compare the CEs and CSs of N = 17 incarcerated child abusers and N = 12 controls using observer-rated methods, namely the Cognitive Error Rating Scale (CERS; Drapeau et al., 2005) and the Coping Action Pattern Rating Scale (CAPRS; Perry, Drapeau, & Dunkley, 2005). Results showed that child molesters presented more cognitive errors, in particular positive selective abstraction, and lower coping functioning, such as escape strategies. Treatment and research implications, including the use of observer-rated methods, are discussed.

Advances in clinical virology for detecting respiratory viruses have been focused on nucleic acids amplification techniques, which have converted in the reference method for the diagnosis of acute respiratory infections of viral aetiology. Improvements of current commercial molecular assays to reduce hands-on-time rely on two strategies, a stepwise automation (semi-automation) and the complete automation of the whole procedure. Contributions to the former strategy have been the use of automated nucleic acids extractors, multiplex PCR, real-time PCR and/or DNA arrays for detection of amplicons. Commercial fully-automated molecular systems are now available for the detection of respiratory viruses. Some of them could convert in point-of-care methods substituting antigen tests for detection of respiratory syncytial virus and influenza A and B viruses. This article describes laboratory methods for detection of respiratory viruses. A cost-effective and rational diagnostic algorithm is proposed, considering technical aspects of the available assays, infrastructure possibilities of each laboratory and clinic-epidemiologic factors of the infection.

The aim of this study was to determine human papillomavirus (HPV) types distribution in cervical preneoplasic lesions in a Southern Spanish population and their relationship between HPV type and grade of histopathological abnormality. Finally, 232 cervical samples from 135 women with previous cytological abnormalities were included in this study. Colposcopy studies and biopsies were performed. Haematoxylin-eosin stained slides were observed and detection of HPV DNA in cervical swabs was carried out with use of a polymerase chain reaction and microarrays technology. The relationship between the presence of HPV infection and diagnostic variables was evaluated. HPV 16 was the most common type followed by HPV 58, 51, 33 and 31. However, the two HPV types targeted in the prophylactic vaccines such as HPV type 16 and 18 were detected in only 37 (21.2%) and 2 (1.1%) cases respectively. Thirty-three (18.9%) of samples were infected with multiple types, the majority of them with two types. In addition, during the follow-up of patients many changes in type distribution were observed. Several studies will be necessary in order to evaluate the HPV type distribution for therapeutically and prophylactic purposes such as vaccine treatment. Also, because of the differences obtained depending of use of various DNA technologies, the performance of some comparative studies of the different methods from detection of HPV would be advisable in a high population of patients and with the most homogeneous conditions possible.
