PURPOSE: The outer limiting membrane (OLM) is considered to play a role in maintaining the structure of the retina through mechanical strength. However, the observation of junction proteins located at the OLM and its barrier permeability properties may suggest that the OLM may be part of the retinal barrier. MATERIAL AND METHODS: Normal and diabetic rat, monkey, and human retinas were used to analyze junction proteins at the OLM. Proteome analyses were performed using immunohistochemistry on sections and flat-mounted retinas and western blotting on protein extracts obtained from laser microdissection of the photoreceptor layers. Semi-thin and ultrastructure analyses were also reported. RESULTS: In the rat retina, in the subapical region zonula occludens-1 (ZO-1), junction adhesion molecule (JAM), an atypical protein kinase C, is present and the OLM shows dense labeling of occludin, JAM, and ZO-1. The presence of occludin has been confirmed using western blot analysis of the microdissected OLM region. In diabetic rats, occludin expression is decreased and glial cells junctions are dissociated. In the monkey retina, occludin, JAM, and ZO-1 are also found in the OLM. Junction proteins have a specific distribution around cone photoreceptors and Müller glia. Ultrastructural analyses suggest that structures like tight junctions may exist between retinal glial Müller cells and photoreceptors. CONCLUSIONS: In the OLM, heterotypic junctions contain proteins from both adherent and tight junctions. Their structure suggests that tight junctions may exist in the OLM. Occludin is present in the OLM of the rat and monkey retina and it is decreased in diabetes. The OLM should be considered as part of the retinal barrier that can be disrupted in pathological conditions contributing to fluid accumulation in the macula.

Few episodes of suspected infection observed in paediatric intensive care are classifiable without ambiguity by a priori defined criteria. Most require additional expert judgement. Recently, we observed a high variability in antibiotic prescription rates, not explained by the patients' clinical data or underlying diseases. We hypothesised that the disagreement of experts in adjudication of episodes of suspected infection could be one of the potential causes for this variability. During a 5-month period, we included all patients of a 19-bed multidisciplinary, tertiary, neonatal and paediatric intensive care unit, in whom infection was clinically suspected and antibiotics were prescribed ( n=183). Three experts (two senior ICU physicians and a specialist in infectious diseases) were provided with all patient data, laboratory and microbiological findings. All experts classified episodes according to a priori defined criteria into: proven sepsis, probable sepsis (negative cultures), localised infection and no infection. Episodes of proven viral infection and incomplete data sets were excluded. Of the remaining 167 episodes, 48 were classifiable by a priori criteria ( n=28 proven sepsis, n= 20 no infection). The three experts only achieved limited agreement beyond chance in the remaining 119 episodes (kappa = 0.32, and kappa = 0.19 amongst the ICU physicians). The kappa is a measure of the degree of agreement beyond what would be expected by chance alone, with 0 indicating the chance result and 1 indicating perfect agreement. CONCLUSION: agreement of specialists in hindsight adjudication of episodes of suspected infection is of questionable reliability.

Splenic marginal zone lymphoma (SMZL) is an indolent B-cell lymphoproliferative disorder characterised by 7q32 deletion, but the target genes of this deletion remain unknown. In order to elucidate the genetic target of this deletion, we performed an integrative analysis of the genetic, epigenetic, transcriptomic and miRNomic data. High resolution array comparative genomic hybridization of 56 cases of SMZL delineated a minimally deleted region (2.8 Mb) at 7q32, but showed no evidence of any cryptic homozygous deletion or recurrent breakpoint in this region. Integrated transcriptomic analysis confirmed significant under-expression of a number of genes in this region in cases of SMZL with deletion, several of which showed hypermethylation. In addition, a cluster of 8 miRNA in this region showed under-expression in cases with the deletion, and three (miR-182/96/183) were also significantly under-expressed (P<0.05) in SMZL relative to other lymphomas. Genomic sequencing of these miRNA and IRF5, a strong candidate gene, did not show any evidence of somatic mutation in SMZL. These observations provide valuable guidance for further characterisation of 7q deletion.

BACKGROUND: Closures of atrial septal defects or a patent foramen ovale (PFO) are increasingly performed percutaneously. The experience of late migration of a new bio-absorbable device is presented here, followed by conceptual discussion. METHODS: Six months post PFO closure with a BioSTAR® device a patient presented with chest pain. Echocardiography showed a hyperechogenic structure perforating the aortic wall. RESULTS: Surgical exploration showed a perforation of the ascending aorta by one metallic, non absorbable arm. This is the second case of late (>6 months) dislocation of the residual framework of the occluder. CONCLUSIONS: The overall incidence of perforation of cardiac structures due to secondary dislocation is low. However this complication exists and should kept in mind in symptomatic patients with new onset of chest pain, after percutaneous procedures. The concept of biodegradation, with residual, non absorbable metal braiding, should be reviewed, analyzing in particular long term results and incidence of secondary dislocation.

Nykyään bakteerien identifikaatio- ja antibioottiherkkyysmääritykset tehdään kliinisen mikrobiologian laboratorioissa suurimmaksi osaksi manuaalisesti. Näytemäärät ovat lisääntyneet bakteriologian laboratorioissa, ja siksi diagnostiikan automatisointi on tullut tarpeelliseksi. BioMerieux on kehittänyt Vitek2-laitteen, jolla voidaan tehdä bakteerien ja sienien identifikaatio ja antibiootti-herkkyys-määrityksiä. Opinnäytetyön tarkoituksena oli verrata rutiinimenetelmien ja Vitek2laitteen identifikaatio- ja antibioottiherkkyystuloksia. Yksittäisiä vertailtavia kohteita olivat testireaktiot, ESBL (Extended Spectrum Beta-Lactamase) -ominaisuus ja määrityksiin kulunut aika. HUSLABin toimeksiannosta Vitek2-laitteella tutkittiin ESBL-kantoja sekä rutiinimenetelmillä vaikeasti tunnistettavia nonfermenta-tiivisia sauvabakteereja ja kasvuolosuhteiltaan vaativia bakteereja. Laitetta varten on kehitetty uusia testikortteja, jotka tunnistavat näitä bakteerik antoja. Aikaisemmat identifikaatio-, reaktio- ja antibioottiherkkyystulokset kerättiin HUSLABin potilastieto-järjestelmästä. Niitä verrattiin Vitek2-laitteesta saatuihin tuloksiin. Määritysten kestoa Vitek2-laitteella verrattiin aikaisempiin tutkimuksiin. Tutkimuksessa käytettiin yhteensä 183 bakteerinäytettä, joista 76 oli ESBL-kantoja, 40 nonfermentatiivisia sauvabakteereja, 41 kasvuolosuhteiltaan vaativia bakteereja sekä 26 uusia potilasnäytelöydöksiä. Vitek2 antoi 99 prosentille kaikista tutkituista ESB--kannoista saman identifikaation kuin rutiini-menetelmät. Uusista potilasnäytelöydöksistä Vitek2 tunnisti 96 Nonfermentatiivisista sauva-baktee-reista ja kasvuolosuhteiltaan vaativista bakteereista Vitek2 tunnisti noin 70 amalla tavalla kuin rutiinimenetelmät. Vitek2-laitteen antamat identifikaatioreaktiotulokset vastasivat paremmin Api20E-testin (94 kuin Api20NE-testin (70 reaktiotuloksia. Noin 80 aikista antibiootti-herkkyys-tuloksista vastasi perinteisillä menet elmillä saatuja tuloksia. Vitek2-laitteella saadut antibiootti-herkkyystulokset olivat yleensä rutiinimenetelmiä herkempiä. Vitek2-laitteella saadut ESBL-tulokset vastasivat rutiinimenetelmillä saatuja tuloksia noin 90-prosenttisesti. Verrattuna aikaisempiin tutkimuksiin Vitek2-laitteen suorittamat määritykset olivat rutiinimenetelmiä paljon nopeampia. Tulosten perusteella voidaan sanoa, että Vitek2-laite on käyttökelpoinen bakteriologian laboratoriossa identifikaatio- ja antibioottiherkkyystutkimuksissa. Nonfermentatiivisia sauvabakteereja ja kasvuolo-suhteiltaan vaativia bakteereja voisi kuitenkin tutkia enemmän.

Variation in temperature affects the biology of sea turtles at a range of scales. To elucidate the drivers of seasonality of nesting and duration of season, databases across four species of sea turtles (Caretta caretta n=37, Chelonia mydas n=64, Dermochelys coriacea n=44 and Eretmochelys imbricata n=36) at a global scale were created. By using remotely sensed sea surface temperature data, thermal profiles across the nesting season were generated. Duration of nesting season was correlated with latitude in all species but was more tightly coupled with temperature; seasons were significantly longer with increased mean SST. In general, nesting seasonality occurred at warmest time of the year. SST for the month before, month after and the month of peak nesting significantly affected the month of peak nesting.
