Antecedentes: Se encuentran datos oficiales sobre trasplante de órganos en Colombia desde 2005 del Instituto Nacional de Salud y desde 2008 para riñón por la Cuenta de Alto Costo. Los datos no ofrecen explicación del número de inscritos en lista de espera para trasplante renal, al comparar con otros países: Colombia 4,68% vs. Argentina 27,58% o Estados Unidos 21,73%. Por otra parte no existen datos acerca de factores que influyen en el seguimiento pos-trasplante y no se tienen de sobrevida del receptor y del injerto. Metodología: Investigación de metodología mixta cuantitativa y cualitativa a partir de la información disponible en los informes oficiales y teniendo como fuente primaria pacientes y expertos en el cuidado de paciente antes y después del trasplante. Resultados: Los factores para acceder o no a lista de espera y seguimiento más mencionados son sistema de salud, aspectos éticos en cuanto remisión y autorización. Emerge como categoría la necesidad de un sistema de vigilancia en trasplantes. En la parte cuantitativa se trasplantan más personas si viven en una ciudad con grupo de trasplante 17,6 vs. 8,05%, si pertenecen al régimen contributivo O.R 3,37 p<0,00001, si no tienen diabetes O.R 3,22 p<0,0001 ni hipertensión arterial O.R 2,42 p<0,0001. No se encontró diferencia en cuanto a género O.R 1,01 p 0,57. Conclusiones: Se proponen ajustes al sistema de salud, de la normatividad, de la educación de la promoción de la donación y trasplante en todos los niveles del público y del personal de salud.

El retardo mental se caracteriza por limitaciones en el desempeño como resultado de significativas deficiencias de la inteligencia y la conducta adaptativa. En Colombia, la mayor parte de los pacientes con esta alteración no reciben evaluación genética. El objetivo de este trabajo es evaluar y caracterizar el retardo mental en un grupo de personas con esta condición en la población de Rovira (Tolima, Colombia) e identificar los posibles factores asociados. La metodología consistió en realizar un diagnóstico clínico preliminar de 25 pacientes con retardo mental y realizar la correspondiente toma de muestras de sangre y orina para efectuar los exámenes correspondientes. Se realizaron estudios bioquímicos (cloruro férrico, nitrosonaftol, nitroprusiato de sodio, Benedict, cromatografía para la detección de aminoácidos y carbohidratos) y citogenéticos (bandeo G). Para la detección de plaguicidas, se realizó un muestreo aleatorio en diferentes puntos de todo el recorrido del sistema de distribución de agua y ciertos lugares del centro del municipio de Rovira. Con este fin, se recolectaron 20 muestras de agua y 20 muestras de tomate, elegidas al azar, de los diferentes sitios de distribución y cultivos de la hortaliza. Se identificó una familia de tres hijos afectados (dos mujeres y un hombre) con retardo mental, lo cual sugiere un componente genético en este caso. Las pruebas metabólicas fueron negativas y los cariotipos normales. Se plantea la necesidad de realizar pruebas moleculares que incluyan el síndrome de X-frágil para complementar el estudio y realizar consejería genética. En cuanto a los resultados y el análisis pertinente de las muestras para organofosforados, el 100% de éstas resultaron positivas. Se reportó un 60% de positividad en las muestras de agua y del 100% en las muestras de tomate, para el caso de los carbamatos; sin embargo, para el caso de los organoclorados, el 100% de las muestras estudiadas resultaron negativas.

Introducción: Conocer y diagnosticar las variaciones más frecuentes de la vasculatura renal es de gran importancia para la planificación de la nefrectomía laparoscópica en el donante y para la reconstrucción vascular en el trasplante renal. De igual forma, considerar las variaciones vasculares −especialmente las del sistema venoso− es indispensable en reconstrucción vascular debido a la gran proporción de variaciones venosas asociadas a aneurismas de la aorta abdominal; además, es ideal en el estudio de condiciones clínicas tales como el síndrome de congestión pélvica y la hematuria. Metodología: Se trata de una revisión de la bibliografía sobre la proporción, diagnóstico, procedimientos quirúrgicos y síndromes clínicos asociados a las variaciones de la vasculatura renal, basada en el material encontrado con la siguiente estrategia de búsqueda: “Renal Artery/abnormalities”[Mesh] OR Renal Veins/abnormalities”[Mesh] AND “surgery”[Mesh] OR “transplantation”[Mesh] OR “radiography”[Mesh] “Kidney Pelvis/abnormalities”[Mesh] AND “Kidney Pelvis/blood supply”[Mesh]. Esta estrategia se modificó de acuerdo con las bases de datos: MEDLINE/PubMed, MEDLINE OVID, SCIENCEDIRECT, HINARI y LILACS. Desarrollo: Se revisó el origen y los tipos más frecuentes de variaciones de la vasculatura renal. Se investigó sobre las implicaciones quirúrgicas y los síndromes clínicos asociados.

Objetivo: Caracterizar la accidentalidad laboral en una clínica de tercer nivel de la ciudad de Bogotá, en el periodo 2010 a 2013. Materiales y Métodos: Se realizó un estudio de tipo corte transversal, utilizando una base de datos de accidentalidad en los trabajadores vinculados directamente con la institución. Resultados: Durante el periodo estudiado se generaron 191 accidentes, 57 casos en el año 2013, el personal de enfermería fue el grupo con mayor porcentaje de accidentes (70.2 %). El sitio donde ocurrieron con mayor frecuencia los accidentes fueron las áreas asistenciales (49.2 %). Al analizar los datos de los accidentes de riesgo biológico se encontró que el pinchazo es el más común (84.7%), seguido de las heridas abiertas y lesiones superficiales (77.2%). Las partes del cuerpo más frecuentemente lesionadas fueron extremidades superiores, siendo la mano la zona más afectada (60.2%); además identifica al paciente fuente del accidente en el 71.8% de los casos y no se logra determinar en un alto porcentaje, si el trabajador afectado usa elementos de protección personal al momento del evento (82.1%). Conclusiones Los accidentes de trabajo con exposición a riesgo biológico son frecuentes en los trabajadores del área asistencial, en especial en el personal de enfermería, dadas las características propias de sus tareas diarias y el cumplimiento de las mismas. El pinchazo fue causa más común, y la parte del cuerpo más frecuentemente afectada fueron las extremidades superiores, específicamente las manos. Dado que los accidentes laborales a riesgo biológico tienen el potencial de general consecuencias nocivas para la salud es esencial establecer estrategias tendientes a minimizar su frecuencia y severidad, por lo que se recomienda revisar e implementar nuevas tecnologías para la reducción de accidentes con elementos corto punzantes, aumentar la sensibilización de los trabajadores con respecto al uso de los elementos de protección personal y revisar la metodología utilizada para la investigación de accidentes laborales.

Este trabajo de grado aborda la contribución del periodismo en la configuración del debate en Colombia sobre política de drogas, a través de un análisis de contenido a las columnas y editoriales publicadas entre 2009 y 2013 en cuatro medios de referencia del país. Se identificaron los perfiles de quienes participaron, los argumentos y las políticas defendidas que plantearon, y la manera como presentaron sus opiniones en El Espectador, El Tiempo, La Silla Vacía y Semana. El desarrollo de la discusión en los textos demuestra una evolución respecto al tema en la medida que hay una transformación del paradigma punitivo hacia un camino menos restrictivo frente a la producción, el tráfico y consumo de drogas ilícitas. Se sugiere que los hechos coyunturales motivaron en parte dicho cambio al ejercer influencia en los giros y la visibilidad que toma el debate dentro de la prensa.

When allocating a resource, geographical and infrastructural constraints have to be taken into account. We study the problem of distributing a resource through a network from sources endowed with the resource to citizens with claims. A link between a source and an agent depicts the possibility of a transfer from the source to the agent. Given the supplies at each source, the claims of citizens, and the network, the question is how to allocate the available resources among the citizens. We consider a simple allocation problem that is free of network constraints, where the total amount can be freely distributed. The simple allocation problem is a claims problem where the total amount of claims is greater than what is available. We focus on consistent and resource monotonic rules in claims problems that satisfy equal treatment of equals. We call these rules fairness principles and we extend fairness principles to allocation rules on networks. We require that for each pair of citizens in the network, the extension is robust with respect to the fairness principle. We call this condition pairwise robustness with respect to the fairness principle. We provide an algorithm and show that each fairness principle has a unique extension which is pairwise robust with respect to the fairness principle. We give applications of the algorithm for three fairness principles: egalitarianism, proportionality and equal sacrifice.

Numerical results are presented and compared for three conservative upwind difference schemes for the Euler equations when applied to two standard test problems. This includes consideration of the effect of treating part of the flux balance as a source, and a comparison of different averaging of the flow variables. Two of the schemes are also shown to be equivalent in their implementation, while being different in construction and having different approximate Jacobians. (C) 2006 Elsevier Ltd. Previous experiments from our group have demonstrated that abomasal infusion of unsaturated free fatty acids (FFA) markedly decreases dry matter intake (DMI) in dairy cows. In contrast, experiments from other groups have noted smaller decreases in DMI when unsaturated triglycerides (TG) were infused postruminally. Our hypothesis was that unsaturated FFA would be more potent inhibitors of DMI than an equivalent amount of unsaturated TG. Four Holstein cows in late lactation were used in a single reversal design. Cows were fed a total mixed ration containing (DM basis) 23% alfalfa silage, 23% corn silage, 40.3% ground shelled corn, and 10.5% soybean meal. Two cows received soy FFA (UFA; 0, 200, 400, 600 g/d) and 2 received soy oil (TG) in the same amounts; cows then were switched to the other lipid source. Cows were abomasally infused with each amount for 5-d periods. The daily amount of lipid was pulse-dosed in 4 equal portions at 0600, 1000, 1700, and 2200 h; no emulsifiers were used and there was no sign of digestive disturbance. Both lipid sources linearly decreased DMI, with a significant interaction between lipid source and amount. Slope-ratio analysis indicated that UFA were about 2 times more potent in decreasing DMI than were TG. Decreased DMI led to decreased milk production. Milk fat content was increased linearly by lipid infusion. Milk fat yield decreased markedly for UFA infusion but was relatively unaffected by infusion of TG. Contents of short- and medium-chain fatty acids in milk fat decreased as the amount of either infusate increased. Contents of C-18:2 and C18: 3 in milk fat were increased linearly by abomasal infusion of either fat source; cis-9 C-18:1 was unaffected. Transfer of infused C18: 2 to milk fat was 35.6, 42.5, and 27.8% for 200, 400, and 600 g/d of UFA, and 34.3, 39.6, and 34.0% for respective amounts of TG. Glucagon-like peptide-1 (7-36) amide (GLP-1) concentration in plasma significantly increased as DMI decreased with increasing infusion amount of UFA or TG. Plasma concentration of cholecystokinin-octapeptide (CCK-8) was unaffected by lipid infusion. These results indicate that unsaturated FFA reaching the duodenum are more potent inhibitors of DMI than are unsaturated TG; the effect may be at least partially mediated by GLP-1.

Current gas-based in vitro evaluation systems are extremely powerful research techniques. However they have the potential to generate a great deal more than simple fermentation dynamics. Details from four experiments are presented in which adaptation, and novel application, of an in vitro system allowed widely differing objectives to be examined. In the first two studies, complement methodologies were utilised. In such assays, an activity or outcome is inferred through the occurrence of a secondary event rather than by direct observation. Using an N-deficient incubation medium, the increase in starch fermentation, when supplemented with individual amino acids (i.e., known level of N) relative to that of urea (i.e., known quantity and N availability), provided an estimate of their microbial utilisation. Due to the low level of response observed with some arnino acids (notably methionine and lysine), it was concluded, that they may not need to be offered in a rumen-inert form to escape rumen microbial degradation. In another experiment, the extent to which degradation of plant cell wall components was inhibited by lipid supplementation was evaluated using fermentation gas release profiles of washed hay. The different responses due to lipid source and level of inclusion suggested that the degree of rumen protection required to ameliorate this depression was supplement dependent. That in vitro inocula differ in their microbial composition is of little interest per se, as long as the outcome is the same (i.e., that similar substrates are degraded at comparable rates and end-product release is equivalent). However where a microbial population is deficient in a particular activity, increasing the level of inoculation will have no benefit. Estimates of hydrolytic activity were obtained by examining fermentation kinetics of specific substrates. A number of studies identified a fundamental difference between rumen fluid and faecal inocula, with the latter having a lower fibrolytic activity, which could not be completely attributed to microbial numbers. The majority of forage maize is offered as an ensiled feed, however most of the information on which decisions such as choice of variety, crop management and harvesting date are made is based on fresh crop measurements. As such, an attempt was made to estimate ensiled maize quality from an in vitro analysis of the fresh crop. Fermentation profiles and chemical analysis confirmed changes in crop composition over the growing season, and loss of labile carbohydrates during ensiling. In addition, examination of degradation residues allowed metabolizable energy (ME) contents to be estimated. Due to difficulties associated with starch analysis, the observation that this parameter could be predicted by difference (together with an assumed degradability), allowed an estimate of ensiled maize ME to be developed from fresh material. In addition, the contribution of the main carbohydrates towards ME showed the importance of delaying harvest until maximum starch content has been achieved. (c) 2005 Elsevier B.V. Gross cystic breast disease (GCBD) is the most common benign breast disorder, but the molecular basis of cyst formation remains to be identified. If the use of aluminium-based antiperspirant salts is involved in the etiology of gross breast cyst formation, it might be expected that aluminium would be at elevated levels in human breast cyst fluid (BCF). Aluminium was measured by ICP-MS in 48 samples of BCF, 30 samples of human blood serum and 45 samples of human breast milk at different stages of lactation (colostrum, intermediate, mature). The median level of aluminium in apocrine type I BCF (n:= 27, 150 mu g I-1) was significantly higher than in transuclative type II BCF (n = 21, 32 mu g I-1; P < 0.0001). By comparison, aluminium measurements gave a median concentration of 6 mu g I-1 in human serum and 25 mu g I-1 in human breast milk, with no difference between colostrum, intermediate and mature milk. Levels of aluminium were significantly higher in both types of BCF than in human serum (P < 0.0001). However when compared with human breast milk, aluminium levels were only significantly higher in apocrine type I BCF (P < 0.0001) and not in transudative type II BCF (P = 0.152). It remains to be identified why such high levels of aluminium were found in the apocrine type I BCF and from where the aluminium originated. However, if aluminium-based antiperspirants are found to be the source and to play any causal role in development of breast cysts, then it might become possible to prevent this common breast disorder. In this paper, we apply one-list capture-recapture models to estimate the number of scrapie-affected holdings in Great Britain. We applied this technique to the Compulsory Scrapie Flocks Scheme dataset where cases from all the surveillance sources monitoring the presence of scrapie in Great Britain, the abattoir survey, the fallen stock survey and the statutory reporting of clinical cases, are gathered. Consequently, the estimates of prevalence obtained from this scheme should be comprehensive and cover all the different presentations of the disease captured individually by the surveillance sources. Two estimators were applied under the one-list approach: the Zelterman estimator and Chao's lower bound estimator. Our results could only inform with confidence the scrapie-affected holding population with clinical disease; this moved around the figure of 350 holdings in Great Britain for the period under study, April 2005-April 2006. Our models allowed the stratification by surveillance source and the input of covariate information, holding size and country of origin. Influenza virus epidemics occur on an annual basis and cause severe disease in the very young and old. The vaccine administered to high-risk groups is generated by amplifying reassortant viruses, with chronologically relevant viral surface antigens, in eggs. Every 20 years or so, influenza pandemics occur causing widespread fatality in all age groups. These viruses display novel viral surface antigens acquired from a zoonotic source, and vaccination against them poses new issues since production of large amounts of a respiratory virus containing novel surface antigens could be dangerous for those involved in manufacture. To minimise risks, it is advisable to use a virus whose genetic backbone is highly attenuated in man. Traditionally, the A/PR/8/34 strain of virus is used, however, the genetic basis of its attenuation is unclear. Cold-adapted (CA) strains of the influenza virus are all based on the H2N2 subtype, itself a virus with pandemic potential, and again the genetic basis of temperature sensitivity is not yet established. Reverse genetics technology allows us to engineer designer influenza viruses to order. Using this technology, we have been investigating mutations in several different gene segments to effectively attenuate potential vaccine strains allowing the safe production of vaccine to protect against the next pandemic.
