These employees are undoubtedly the most valuable resource for providing timely and quality services to Iowans. To strategically manage this resource, state departments and policymakers must have thorough and accurate information. The information in “Just the Facts for 2007” is a snapshot of the workforce, collected, compiled, and presented in a format that will aid agencies and decision makers in strategic planning and workforce planning. In many cases, data cover a number of years and are presented to give the reader a sense of trends. While the Department of Administrative Services wants to present data in its purest form so readers can draw their own conclusions, we also have a responsibility to clarify anything that may be confusing or misleading. The ill effects of second-hand smoke are now well documented. To protect the population from exposure to tobacco smoke, comprehensive smoking bans are necessary as expressed in the WHO Framework Convention on Tobacco Control and its guidelines. Switzerland has only a partial smoking ban full of exceptions which has been in effect since 2010, which reproduces the so-called Spanish model. In September 2012, the Swiss citizens refused a proposal for a more comprehensive ban. This case study examines the reasons behind this rejection and draws some lessons that can be learnt from it.

Medicine counterfeiting is a serious worldwide issue, involving networks of manufacture and distribution that are an integral part of industrialized organized crime. Despite the potentially devastating health repercussions involved, legal sanctions are often inappropriate or simply not applied. The difficulty in agreeing on a definition of counterfeiting, the huge profits made by the counterfeiters and the complexity of the market are the other main reasons for the extent of the phenomenon. Above all, international cooperation is needed to thwart the spread of counterfeiting. Moreover effort is urgently required on the legal, enforcement and scientific levels. Pharmaceutical companies and agencies have developed measures to protect the medicines and allow fast and reliable analysis of the suspect products. Several means, essentially based on chromatography and spectroscopy, are now at the disposal of the analysts to enable the distinction between genuine and counterfeit products. However the determination of the components and the use of analytical data for forensic purposes still constitute a challenge. The aim of this review article is therefore to point out the intricacy of medicine counterfeiting so that a better understanding can provide solutions to fight more efficiently against it.

Ara que ja ningú qüestiona les publicacions digitals, l'accés obert o els repositoris de tipus institucional o temàtic, cal co-mençar a treballar en els temes de preservació. Som responsables d'assegurar l'accés i l'ús futurs dels documents digitals, hagin nascut en format digital o siguin fruit dels projectes de digitalització. En aquest article es parla sobre la preservació digital i els tipus de continguts, així com els conceptes, tipologies, costos i usos de l'anomenat «núvol». També es mostra un exemple de núvol privat, la preservació de tesis doctorals del TDX i es comenta una experiència de núvol públic, el DuraCloud.

RESUM L’aigua és un recurs limitat necessari per a l’establiment de qualsevol forma de vida sobre la Terra. Aquest recurs, es veu regit pel cicle hidrològic i durant les diferents fases del seu cicle, l’aigua pot ser captada per ser aprofitada per l’ésser humà. En els darrers anys, la necessitat d’aigua per a la població ha estat incrementada provocant un impacte negatiu sobre aquest recurs. Per aquest motiu sorgeix la necessitat de crear polítiques globals per protegir l’aigua que vagin encaminades a minimitzar l’efecte negatiu provocat per l’home. Tota captació d’aigua destinada al consum humà ha de garantir uns paràmetres de qualitat i per tal de poder aconseguir aquest objectiu amb eficàcia és imprescindible l’aplicació d’una eina legal molt important: els perímetres de protecció. A l’article 6 de la Llei de policia i protecció de les aigües del 31 de juliol del 1985, s’estableix que tota captació d’aigua potable haurà de quedar aïllada mitjançant un perímetre de protecció, a l’interior del qual no podrà dur-se a terme cap activitat sense prèvia autorització de Govern. Els objectius principals d’aquest estudi son recopilar la informació existent en referència a les captacions d’aigua i els seus respectius perímetres de protecció, elaborar una base de dades que pugui contenir tota la informació existent i elaborar cartografia especifica amb la finalitat de realitzar una diagnosi de la situació actual de les captacions i perímetres associats i identificar els principals riscos potencials existents en aquestes àrees. Entre les conclusions mes rellevants obtingudes amb aquest treball, cal destacar que s’ha constatat que algunes activitats antròpiques potencialment contaminants es troben situades en zones properes a les captacions destinades a consum humà, inclòs dins dels seus perímetres de protecció, amb el conseqüent risc que aquesta situació comporta.

Background: Immunogenicity of standard infl uenza vaccine is suboptimal in lung transplant recipients. Intradermal vaccine may elicit stronger responses due to recruitment of local dendritic cells. We compared the immunogenicity of the infl uenza vaccine administered intradermally (ID) to the standard intramuscular (IM) vaccination. Methods: In this investigator-blinded, two-center, prospective trial, lung transplant patients were randomized to receive intradermal (6ug) or intramuscular (15ug) 2008/9 trivalent inactivated infl uenza vaccine. Immunogenicity was evaluated using a standard hemagglutination inhibition assay (HIA). Response to the vaccine was defi ned as a fourfold increase of the HIA levels for any of the 3 viral strains in the vaccine. Geometric mean titers (GMT) and seroprotection rate (HIA ≥32) were also analyzed. Patients were followed during 6 months for the development of infl uenza or acute rejection. Results: We randomized 84 patients to receive the ID (n=41) vs. IM (n=43) vaccine, respectively. Baseline characteristics were similar between groups. Median time from transplantation was 3.4 yrs (ID) vs. 3.3 yrs (IM) (p=0.84). Vaccine response to at least one antigen was seen in 6/41 (14.6%) patients in the ID vs. 8/43 (18.6%) in the IM group (p=0.77). In the ID group, GMTs (95% CI) after vaccination were 15.7 (11.1-22.3) for H1N1, 84.0 (52.0-135.7) for H3N2, and 14.5 (9.6-21.8) for B strains vs. in the IM group 17.5 (11.8-25.9) for H1N1, 108.9 (77.5-153.2) for H3N2, and 20.2 (12.8-31.9) for B (p=NS, all 3 strains). Seroprotection was 39% (H1N1), 82.9% (H3N2) and 29.3% (B strain) in the ID group vs. 27.9% (H1N1), 97.7% (H3N2) and 58.1% (B strain) in the IM group. No factors associated with vaccine response were identifi ed. Mild adverse events were seen in 44% of patients (ID) vs. 34% (IM) (p=0.38). Two patients (4.8%) in the ID group developed infl uenza infection compared to none in the IM group. Two patients in each group developed biopsy-proven acute rejection during follow-up. Conclusions: Immunogenicity of the 2008/09 infl uenza vaccine was poor in lung transplant recipients. ID administration of the vaccine elicited similar immune responses to standard IM vaccination. Novel strategies of vaccination are needed to protect lung transplant recipients from infl uenza.

The glyoxalase system is the most important pathway for the detoxification of methylglyoxal (MG), a highly reactive dicarbonyl compound mainly formed as a by-product of glycolysis. MG is a major precursor of advanced glycation end products (AGEs), which are associated with several neurodegenerative disorders. Although the neurotoxic effects of MG and AGEs are well characterized, little is known about the glyoxalase system in the brain, in particular with regards to its activity in different neural cell types. Results of the present study reveal that both enzymes composing the glyoxalase system [glyoxalase-1 (Glo-1) and Glo-2] were highly expressed in primary mouse astrocytes compared with neurons, which translated into higher enzymatic activity rates in astrocytes (9.9- and 2.5-fold, respectively). The presence of a highly efficient glyoxalase system in astrocytes was associated with lower accumulation of AGEs compared with neurons (as assessed by Western blotting), a sixfold greater resistance to MG toxicity, and the capacity to protect neurons against MG in a coculture system. In addition, Glo-1 downregulation using RNA interference strategies resulted in a loss of viability in neurons, but not in astrocytes. Finally, stimulation of neuronal glycolysis via lentiviral-mediated overexpression of 6-phosphofructose-2-kinase/fructose-2,6-bisphosphatase-3 resulted in increased MG levels and MG-modified proteins. Since MG is largely produced through glycolysis, this suggests that the poor capacity of neurons to upregulate their glycolytic flux as compared with astrocytes may be related to weaker defense mechanisms against MG toxicity. Accordingly, the neuroenergetic specialization taking place between these two cell types may serve as a protective mechanism against MG-induced neurotoxicity.

The oro-ocular cleft no. 4 according to the Tessier classification is one of the rarest facial cleft, and to this day, few cases have been reported in the literature. We describe the case of a 9-month-old girl with a complete bilateral facial cleft. On the right cornea protruded a hard lesion, a corneal staphyloma. We describe the 3 primary surgical steps used to restore the possibility of satisfactory feeding, to promote language acquisition, and to protect vision in the nonaffected eye. The psychological and social aspects of severe facial deformities in developing countries are also tackled.

Iowa's Drug Control Strategy serves as a comprehensive blueprint for coordinated prevention, treatment, and enforcement actions to protect citizens from the dangers posed by substance abuse. This holistic plan, developed by Iowa's Drug Policy Advisory Council, embraces a performance-oriented process to align resources with long-term goals, and supports three desired results: All Iowans are healthy and drug-free, Iowa communities are free from illegal drugs, all Iowans are safe from drug abusing offenders. This report also contains a mix of recent accomplishments and pending challenges.
