Como a mídia aborda o suicídio de idosos? E o que as notícias sobre suicídio de idosos nos dizem? Estas foram as perguntas que guiaram esta reflexão, que se debruçou sobre matérias de suicídios de idosos publicadas na mídia nacional, entre abril de 2010 e abril de 2013. Observamos que a palavra “suicídio”, até pouco tempo atrás “aquela-que-não-deveria-ser-nomeada” na imprensa, aparece na maior parte das notícias, no título e nos textos, demonstrando que o suicídio, como problema de saúde pública, afasta-se do campo privado para ocupar e compor o espaço público. Constatamos que entre as principais situações de risco que levam os idosos a cometer um “gesto de comunicação” extremo estão os efeitos de uma política econômica recessiva e o prolongamento da vida sem dignidade, mediante o medo da dependência e do provável “trabalho” aos demais; situações que indicam serem essenciais o desenvolvimento e o fortalecimento das redes de suporte social.

Depuis que l'électricité a été découverte et que le secteur électrique s'est considérablement développé dans le XVIIIe siècle, jamais l'homme n'a pu se détacher de cette énergie. Pendant longtemps le système d'énergie électrique a fonctionné sans grand changement, mais avec le développement rapide des technologies, de nouvelles améliorations apparaissent. Ce mémoire traite des nouveaux concepts de réseaux d'énergie électrique appelés Réseaux intelligents ou Smart Grid. À travers un panorama de leurs développements dans le monde, cette étude porte d'une part, sur l'avancée des projets dans quelques pays et d'autre part, du niveau de développement de ce réseau au Brésil et de son intérêt pour le pays. L'étude a comme point central un des composants de ce réseau intelligent, le compteur communicant, qui est l'élément essentiel des interconnexions entre consommateurs et producteurs. Ce rapport apporte un éclaircissement sur les compteurs conventionnels et les compteurs intelligents et sur leur mode de fonctionnement. Enfin il aborde la question des consommateurs : par quels moyens leur transmettre tous ces changements à venir, puisque jusqu'à présent, leur seule participation était de payer l'énergie consommée. Avec le nouveau réseau, ils deviendront de véritables acteurs puisqu'ils seront informés en temps réel de leur consommation d'énergie électrique. Pour terminer, le mémoire montrera comment ils pourront s'adapter à cette nouvelle façon de gérer leur consommation en ésperant les inciter à une utilisation plus raisonnable de l'énergie et à modifier leur comportement en gérant de manière active leur consommation en intégrant notamment les énergies renouvelables

Increased plasma osmolality by food intake evokes augmentation of plasma oxytocin (OT). Ovarian steroids may also influence the balance of body fluids by acting on OT neurones. Our aim was to determine if estrogen influences the activity of OT neurones in paraventricular nucleus (PVN) and supraoptic nucleus (SON) under different osmotic situations. Ovariectomized rats (OVX) were treated with either estradiol (E-2) or vehicle and were divided into three groups: group I was fed ad libitum, group II underwent 48 h of fasting, and group III was refed after 48 h of fasting. On the day of the experiment, blood samples were collected to determine the plasma osmolality and OT. The animals were subsequently perfused, and OT/FOS immunofluorescence analysis was conducted on neurones in the PVN and the SON. When compared to animals which were fasted or fed ad libitum, the plasma osmolality of refed animals was higher, regardless of whether they were treated with vehicle or E-2. We observed neural activation of OT cells in vehicle-or E-2-treated OVX rats refed after 48 h of fasting, but not in animals fed ad libitum or in animals that only underwent 48 h of fasting. Finally, the percentage of neurones that co-expressed OT and FOS was lower in both the PVN and the SON of animals treated with E-2 and refed, when compared to vehicle-treated animals. These results suggest that E-2 may have an inhibitory effect on OT neurones and may modulate the secretion of OT in response to the increase of osmolality induced by refeeding. The enteric nervous system regulates autonomously from the central nervous system all the reflex pathways that control blood flow, motility, water and electrolyte transport and acid secretion. The ability of the gut to function in isolation is one of the most intriguing phenomenons in neurogastroenterology. This requires coding of sensory stimuli by cells in the gut wall. Enteric neurons are prominent candidates to relay mechanosensitivity. Surprisingly, the identity of mechanosensitive neurons in the enteric nervous system as well as the appropriate stimulus modality is unknown despite the evidence that enteric neurons respond to sustained distension. Objectives: The aim of our study was to record from mechanosensitive neurons using physiological stimulus modalities. Identification of sensory neurons is of central importance to understand sensory transmission under normal conditions and in gut diseases associated with sensorimotor dysfunctions, such as Irritable Bowel Syndrome. Only then it will be possible to identify novel targets that help to normalise sensory functions. Methods: We used guinea-pig ileum myenteric plexus preparations and recorded responses of all neurons in a given ganglion with a fast neuroimaging technique based on voltage sensitive dyes. To evoke a mechanical response we used two different kinds of stimuli: firstly we applied a local mechanical distortion of the ganglion surface with von Frey hair. Secondarily we mimic the ganglia deformation during physiological movements of myenteric ganglia in a freely contracting ileal preparation. We were able to reliably and reproducibly mimic this distortion by intraganglionic injections of small volumes of oxygenated and buffered Krebs solution using stimulus parameters that correspond to single contractions. We also performed in every ganglion tested, electrical stimulations to evoke fast excitatory postsynaptic potentials. Immunohistochemistry reactions were done with antibodies against Calbindin and NeuN, considered markers for sensory neurons. Results: Recordings were performed in 46 ganglia from 31 guinea pigs. In every ganglion tested we found from 1 to 21 (from 3% to 62%) responding cells with a median value of 7 (24% of the total number of neurons). The response consisted of an almost instantaneous spike discharge that showed adaptation. The median value of the action potential frequency in the responding neurons was 2.0 Hz, with a recording time of 1255 ms. The spike discharge lasted for 302 ± 231 ms and occurred only during the initial deformation phase. During sustained deformation no spike discharge was observed. The response was reproducible and was a direct activation of the enteric neurons since it remained after synaptic blockade with hexamethonium or ω-conotoxin and after long time perfusion with capsaicin. Muscle tone appears not to be required for activation of mechanosensory neurons. Mechanosensory neurons showed a response to mechanical stimulation related to the stimulus strength. All mechanosensory neurons received fast synaptic inputs. There was no correlation between mechanosensitivity and Calbindin-IR and NeuN-IR (44% of mechanosensitive neurones Calb-IR-/NeuN-IR-). Conclusions: We identified mechanosensitive neurons in the myenteric plexus of the guinea pig ileum which responded to brief deformation. These cells appear to be rapidly accommodating neurons which respond to dynamic change. All mechanosensitive neurons received fast synaptic input suggesting that their activity can be highly modulated by other neurons and hence there is a low stimulus fidelity which allows adjusting the gain in a sensory network. Mechanosensitivity appears to be a common feature of many enteric neurons belonging to different functional classes. This supports the existence of multifunctional enteric neurons which may fulfil sensory, integrative and motor functions.

Die freien Endigungen von Spinalganglienneuronen sind für die Detektion schmerzhafter Reize verantwortlich. Dabei rufen thermische, chemische oder mechanische Reize Ionenströme über die Membran und dadurch Membranpotentialänderungen hervor. Diese noxisch induzierten Ströme sind in großem Ausmaß durch chemische Substanzen und andere Reize modulierbar. Der Ionenkanal TRPV1 ist für die Detektion zahlreicher chemischer Reize und zumindest eines Teils der noxischen Hitzereize verantwortlich. Im Rahmen dieser Arbeit wurden einige der Mechanismen geklärt, die zur schnellen Sensibilisierung hitzeevozierter Ionenströme führen. Hierfür wurden akut dissoziierte Spinalganglienneurone der Ratte als Modell ihrer peripheren Endigung verwendet und mittels Ganzzellableitung in der patch-clamp-Technik untersucht. Die Verwendung von Trypsin während der Präparation von Spinalganglienneuronen hat keinen funktionellen Einfluss auf hitze- oder capsaicininduzierte Ströme, verbessert aber die Untersuchungsbedingungen für das patch-clamp-Verfahren. Bei 144 akut dissoziierten Spinalganglienneuronen wurden die Stromantworten auf drei im Abstand von 40 s durch Überspülen mit 45,3 bis 46,3°C heißer Extrazellularlösung applizierte einsekündige Hitzereize gemessen. Dabei ließen sich repetitiv reproduzierbare hitzeinduzierte Einwärtsströme von etwa 160 pA erzielen; es konnte keine Tachyphylaxie und nahezu keine Inaktivierung beobachtet werden. Direkt vor dem zweiten Hitzereiz wurden die Neurone für zwei Sekunden mit Extrazellularlösung überspült, die Kontrolllösung, 0,5 μM Capsaicin, 10 μM Natriumnitroprussid oder 10 μM YC-1 enthielt. Es fand sich kein Hinweis, dass Stickstoffmonoxid oder die Guanylatzyklase einen signifikanten Beitrag zur Sensibilisierung von hitzeinduzierten Strömen in Spinalganglienneuronen leisten, wobei ein durch den Versuchsaufbau bedingtes Auswaschen zytosolischer Faktoren, die für den Signalweg notwendig sind, nicht ausgeschlossen werden kann. Bei einer Konzentration von 0,5 μM löst Capsaicin für zwei Sekunden einen sehr kleinen Einwärtsstrom von etwa 33 pA aus und führt innerhalb von zwei Sekunden zu einer schnell reversiblen Sensibilisierung von hitzeinduzierten Einwärtsströmen in Spinalganglienneuronen (p<0,01). Das Ausmaß der Sensibilisierung ist proportional zur Größe des capsaicininduzierten Stromes (r=−0,7, p<0,001). Konstant halten der intrazellulären Calciumkonzentration mittels des Calciumchelators BAPTA verhindert die capsaicininduzierte Sensibilisierung hitzeinduzierter Ströme an Spinalganglienneuronen. Demzufolge beruht die capsaicininduzierte Sensibilisierung trotz der schnellen Kinetik nicht auf einer synergistischen Wirkung der beiden Agonisten Capsaicin und Hitze auf ihren gemeinsamen Rezeptor; vielmehr ist sie von einer Erhöhung der intrazellulären freien Calciumkonzentration abhängig. Funktionelle Änderungen der zellulären Funktion werden häufig durch Proteinkinasen vermittelt. Die zur Gruppe der MAP-Kinasen gehörende ERK (extracellular signal related kinase) wird bei Membrandepolarisation und Calciumeinstrom in die Zelle durch MEK (MAPK/extracellular signal related kinase kinase) aktiviert. Blockade der MEK/ERK-Kaskade durch den spezifischen MEK-Hemmstoff U0126 führt ebenfalls zu einer Aufhebung der Sensibilisierung der Hitzeantworten durch Capsaicin. Applikation von Capsaicin führt innerhalb von zwei Sekunden zu einer schnell reversiblen Sensibilisierung hitzeevozierter Ionenströme an nozizeptiven Spinalganglienneuronen. Diese Sensibilisierung wird durch einen Calciumeinstrom in die Zelle und die dadurch eintretende Aktivierung von Proteinkinasen hervorgerufen. Die MEK/ERK-Kaskade ist ein sehr schnell (deutlich unter 2 s) aktivierbares intrazelluläres Signalsystem, welches bei der Regulation der Empfindlichkeit nozizeptiver Spinalganglienneurone eine entscheidende Rolle spielt; die schnelle Kinetik ist dabei nur durch eine membranständige oder zumindest membrannahe Lokalisation dieser Proteinkinasen erklärbar. Durch Applikation zehnsekündiger Hitzereize lässt sich ebenfalls eine Sensibilisierung hitzeevozierter Ionenströme auslösen, die ebenso ausgeprägt ist, wie die Sensibilisierung durch 0,5 μM Capsaicin (p<0,005). Durch das immer größere Verständnis der Funktionsweise des nozizeptiven Systems ergeben sich ständig neue Ansätze für die Entwicklung neuer Analgetika. So könnte durch Modulation spezifischer intrazellulärer Proteinkinasen der Phosphorylierungszustand und damit die Aktivierbarkeit von Ionenkanälen, die der Transduktion noxischer Reize dienen, positiv beeinflusst werden. Neuere, noch spezifischere Inhibitoren der MEK können der Forschung und später auch der Therapie neue Möglichkeiten eröffnen.

The clinical signs, pathological and laboratory findings of cattle suffering from a tremorgenic syndrome are described. Animals on a farm with a total of 22 cows, 18 heifers and 9 calves were fed mouldy grass and spent malt-grain silage. Five heifers were affected with muscular tremor, hyperexcitability and hypersensitivity. They were ataxic or in sternal recumbency, while their appetite remained normal. Haematology and blood chemistry in two heifers as well as cerebrospinal fluid from one sick animal were unremarkable. The pathological examination of one animal brought no macroscopic changes to light. Histological examination, however, revealed the degeneration of motor neurones in the midbrain, brain stem and spinal cord. Analysis of a silage sample provided evidence of the presence of Aspergillus clavatus, a mould capable of producing neurotoxic tremorgenic mycotoxins. Epidemiology, clinical findings, pathology and microbiological examination suggest that the five cattle were suffering from neuromycotoxicosis.

AIM: To investigate the expression of E-cadherin, a major host cell receptor for Listeria monocytogenes (LM) internalin A, in the ruminant nervous system and its putative role in brainstem invasion and intracerebral spread of LM in the natural disease. METHODS: Immunohistochemistry and double immunofluorescence was performed on brains, cranial nerves and ganglia of ruminants with and without natural LM rhombencephalitis using antibodies against E-cadherin, protein gene product 9.5, myelin-associated glycoprotein and LM. RESULTS: In the ruminant brain, E-cadherin is expressed in choroid plexus epithelium, meningothelium and restricted neuropil areas of the medulla, but not in the endothelium. In cranial nerves and ganglia, E-cadherin is expressed in satellite cells and myelinating Schwann cells. Expression does not differ between ruminants with or without listeriosis and does not overlap with the presence of microabscesses in the medulla. LM is observed in phagocytes, axons, Schwann cells, satellite cells and ganglionic neurones. CONCLUSION: Our results support the view that the specific ligand-receptor interaction between LM and host E-cadherin is involved in the neuropathogenesis of ruminant listeriosis. They suggest that oral epithelium and Schwann cells expressing E-cadherin provide a port of entry for free bacteria offering a site of primary intracellular replication, from where the bacterium may invade the axonal compartment by cell-to-cell spread. As E-cadherin expression in the ruminant central nervous system is weak, only very locally restricted and not related to the presence of microabscesses, it is likely that further intracerebral spread is independent of E-cadherin and relies primarily on axonal spread.

OBJECTIVE: To compare the effects of intravenous methylprednisolone (IVMP) in patients with relapsing-remitting (RR-MS), secondary progressive (SP-MS), and primary progressive multiple sclerosis (PP-MS). METHODS: Clinical and neurophysiological follow up was undertaken in 24 RR-MS, eight SP-MS, and nine PP-MS patients receiving Solu-Medrol 500 mg/d over five days for exacerbations involving the motor system. Motor evoked potentials (MEPs) were used to measure central motor conduction time (CMCT) and the triple stimulation technique (TST) was applied to assess conduction deficits. The TST allows accurate quantification of the number of conducting central motor neurones, expressed by the TST amplitude ratio. RESULTS: There was a significant increase in TST amplitude ratio in RR-MS (p<0.001) and SP-MS patients (p<0.02) at day 5, paralleling an increase in muscle force. TST amplitude ratio and muscle force remained stable at two months. In PP-MS, TST amplitude ratio and muscle force did not change. CMCT did not change significantly in any of the three groups. CONCLUSIONS: In RR-MS and SP-MS, IVMP is followed by a prompt increase in conducting central motor neurones paralleled by improvement in muscle force, which most probably reflects partial resolution of central conduction block. The lack of similar clinical and neurophysiological changes in PP-MS corroborates previous clinical reports on limited IVMP efficacy in this patient group and points to pathophysiological differences underlying exacerbations in PP-MS.

Desferrioxamine inhibits cortical necrosis in neonatal rats with experimental pneumococcal meningitis, suggesting that iron-induced oxidative damage might be responsible for neuronal damage. We therefore examined the spatial and temporal profile of changes in cortical iron and iron homeostatic proteins during pneumococcal meningitis. Infection was associated with a steady and global increase of non-haem iron in the cortex, particularly in neuronal cell bodies of layer II and V, and in capillary endothelial cells. The non-haem iron increase was associated with induction of haem oxygenase (HO)-1 in neurones, microglia and capillary endothelial cells, whereas HO-2 levels remained unchanged, suggesting that the non-haem iron increase might be the result of HO-1-mediated haem degradation. Indeed, treatment with the haem oxygenase inhibitor tin protoporphyrin (which completely blocked the accumulation of bilirubin detected in HO-1-positive cells) completely prevented the infection-associated non-haem iron increase. The same cells also displayed markedly increased ferritin staining, the increase of which occurred independently of HO activity. At the same time, no increase in DNA/RNA oxidation was observed in infected animals (as assessed by in situ detection of 8-hydroxy[deoxy]guanosine), strongly suggesting that ferritin up-regulation protected the brain from iron-induced oxidative damage. Thus, although pneumococcal meningitis leads to an increase of cortical non-haem iron, protective mechanisms up-regulated in parallel prevent iron-induced oxidative damage. Cortical damage does not appear to be a direct consequence of increased iron, therefore.

Vom Nobelpreis für die Schilddrüsenoperation zur Osteosynthese als weltweit praktizierter Technik: Die Chirurgie in der Schweiz der letzten einhundert Jahre kann als Erfolgsgeschichte gelesen werden. Der Sammelband zum 100-Jahr-Jubiläum der Schweizerischen Gesellschaft für Chirurgie geht tiefer und analysiert in vier medizinhistorischen Detailstudien die Hintergründe von medizinischen Erfolgen und organisatorischen Herausforderungen. Einige Reflexionen beleuchten zudem die heutige Chirurgie von der Ökonomisierung bis zur neueren Sakralisierung des von Chirurgen behandelten Körpers. Beiträge aus der chirurgischen Praxis skizzieren technisch-therapeutische Trends dieses Faches von der Organtransplantation bis zur «Schlüsselloch-Chirurgie».

La collecte de l’eau est réalisée avec succès depuis des millénaires dans les régions du monde entier – certaines interventions récentes ont également eu un impact local significatif. Pourtant, le potentiel de cette technique reste largement inconnu, non reconnu et non apprécié. Il est temps de transposer à plus grande échelle les « bonnes pratiques » de collecte de l’eau, celles qui ont survécu au temps ou qui ont émergé des nouvelles expériences, après des décennies de focalisation presque exclusive sur la maîtrise des flux d’eau douce dans les rivières et les lacs grâce à des investissements dans des infrastructures d’irrigation.La collecte de l’eau offre des opportunités sous-exploitées pour les systèmes principalement d’exploitation pluviale des zones arides dans le monde en développement. Celle-ci fonctionne mieux précisément dans les zones où la pauvreté rurale est la pire. Quand elle est bien réalisée, son impact est à la fois de réduire la faim et de lutter contre la pauvreté, tout en améliorant la résilience de l’environnement. Ces connaissances sur les technologies de collecte d’eau et sur les milieux dans lesquels elles ont tendance à donner de meilleurs résultats, représentent une véritable richesse cachée. Pour la première fois, ces connaissances sont traitées, rassemblées et rendues disponibles à travers un tel outil organisé, illustré et instructif reliant les technologies aux réseaux des connaissances, outil qui servira aux utilisateurs présumés de ces directives pratiques pour mieux comprendre et mettre en oeuvre leurs choix
