A Alfa-galactosidase A (α-Gal A) humana é uma enzima lisossômica que quando deficiente causa a doença de Fabry. A doença de Fabry é uma esfingolipidose cuja principal causa de morbi-mortalidade é a insuficiência renal crônica (IRC). O objetivo deste estudo foi a implantação de um protocolo laboratorial que permita o diagnóstico da doença de Fabry em plasma e leucócitos, além da análise das características cinéticas da enzima α-Gal A em plasma e busca ativa da doença em 25 indivíduos com IRC de causa desconhecida. Também foram avaliadas a reprodutibilidade e a estabilidade do método enzimático. A padronização dos ensaios foi realizada com o substrato fluorescente 4-metilumbeliferil-α-Dgalactopiranosídeo. A reprodutibilidade foi avaliada utilizando amostras de plasma aliquotadas a 4ºC, -20ºC e -70ºC, analisadas uma vez ao mês por 6 meses e a estabilidade da fluorescência por até 24 horas após o término do ensaio enzimático. A padronização permitiu a implantação de valores de referência da α-Gal A para o estado do Pará, de 4 a 28 nmoles/h/mL (plasma) e de 20 a 96 nmoles/h/mg proteína (leucócitos). A enzima α-Gal A se mostrou termolábil, visto que com apenas 1 minuto de pré-incubação das amostras a 60ºC, sua atividade decaiu 71,09%. Com relação ao tempo de incubação, a atividade enzimática apresentou uma disposição linear crescente entre 15 a 180 minutos de incubação. A α-Gal A apresentou maior atividade no pH 4,8, o Km encontrado para a α-Gal A foi de 1,007 mM e a Vmáx foi 30,9 nmoles/h/mL. A melhor temperatura de armazenamento de plasma até o ensaio enzimático é -20ºC, onde foi observada menor variação em um período máximo de 6 meses. O método enzimático utilizado é estável, mesmo após 24 horas do término do ensaio, em temperatura ambiente. Com relação aos pacientes com IRC de causa desconhecida, todos apresentaram valor de atividade da α-Gal A dentro dos parâmetros de referência e, portanto, nenhum foi diagnosticado com doença de Fabry. O entendimento da cinética da α-Gal A e do seu comportamento in vitro possibilita um melhor diagnóstico laboratorial da doença de Fabry gerando dados para futuras comparações com indivíduos afetados por mutações nesta enzima

No presente trabalho implantou-se como modelo experimental para estudos de neurodegeneração crônica a doença prion induzida pelo agente ME7 em fêmea adulta no camundongo Suíço albino. As alterações comportamentais e neuropatológicas seguem de perto as previamente descritas para o camundongo C57BL/6j com duas exceções: 1) o septum ao invés do hipocampo é a região onde se detectou mais precocemente o maior número de microglias ativadas e astrócitos reativos e onde houve a maior redução de redes perineuronais nos estágios iniciais da doença; 2) Em relação ao C57BL/6j o curso temporal da doença é em média 4 semanas mais longo (26 semanas) e os sintomas iniciais começam a aparecer 4 semanas mais tarde (16 semanas) na variedade Suíça albina. Semelhante ao encontrado no C57BL/6j não se encontrou diferença nas estimativas do número de neurônios nos animais inoculados com o agente ME7 em relação aos inoculados com homogenado cerebral normal 15 e 1 8 semanas após a inoculação. A análise comparada do número de microglias ativadas astrócitos reativos e redes perineuronais empregando o fracionador óptico revelou diferenças significativas nos animais sacrificados na 15ª em relação aos sacrificados na 18ª semana pós-inoculação, com aumento do número das primeiras e redução do número das últimas na 18ª semana (teste T, bi-caudal p<0.05). A análise de cluster seguida da análise discriminante dos resultados dos testes comportamentais da variedade Suíça albina aplicada a cada quinzena ao longo do curso temporal da doença, revelou que a remoção de comida é a única variável discriminante para detecção de dois grupos distintos: um grupo menor (em torno de 40%, n=4), mais sensível, onde a doença cursa mais rápido e os animais atingem a fase terminal em 22 semanas, e outro maior (em torno de 60%, n=6), menos sensível, onde os animais atingem a fase terminal em 26 semanas. Os resultados são importantes para estudos comparativos de imunopatologia dentro da mesma e entre variedades de modelos murinos de neurodegeneração crônica na doença prion induzida pelo agente ME7.

The main feature of pulmonary emphysema is airflow obstruction resulting from the destruction of the alveolar walls distal to the terminal bronchioles. Existing clinical approaches have improved and extended the quality of life of emphysema patients. However, no treatment currently exists that can change the disease course and cure the patient. The different therapeutic approaches that are available aim to increase survival and/or enhance the quality of life of emphysema patients. In this context, cell therapy is a promising therapeutic approach with great potential for degenerative pulmonary diseases. In this protocol proposition, all patients will be submitted to laboratory tests, such as evaluation of heart and lung function and routine examinations. Stem cells will be harvested by means of 10 punctures on each anterior iliac crest, collecting a total volume of 200 mL bone marrow. After preparation, separation, counting and labeling (optional) of the mononuclear cells, the patients will receive an intravenous infusion from the pool of Bone Marrow Mononuclear Cells (BMMC). This article proposes a rational and safe clinical cellular therapy protocol which has the potential for developing new projects and can serve as a methodological reference for formulating clinical application protocols related to the use of cellular therapy in COPD. This study protocol was submitted and approved by the Brazilian National Committee of Ethics in Research (CONEP - Brazil) registration number 14764. Lower urinary tract disease (LUTD) is a multifactorial disease, wich may be caused by uroliths and calculations, bacterial and viral infections and anatomical abnormalities, among others. The disease can also be of unknown origin in wich case it is called intersticial cystitis. Knowing that the disease may have many causes, it is necessary to know the exact etiology and the best method of diagnosis, to establish specific therapeutic measures for each case. The diagnostic methods most commonly used are: urinalysis, radiography and ultrassonography, but the blood count and biochemistry can be used, trying to assess the overall status of the animal. The terapy used depends on the causative agent of disease, wich may include changes in supply until emergency treatment, azotemia in cats with post-renal. Abstract Introduction: Indications for induction therapy is not consensual in living donors. Objective: The objective of this study was compare no induction with thymoglobulin and basiliximab induction in the incidence of acute rejection in kidney transplantation with living donor. Methods: We select all cases of renal transplantation with living donor performed in Hospital das Clínicas de Botucatu da UNESP during the period of January 2010 to December 2013. The group was divided by the type of medication used for induction. Results: A total of 90 patients were evaluated. There were no differences in baseline characteristics of age and underlying disease. The rate of biopsy-proven acute rejection was higher in the group without induction (42.9%) compared to basiliximab group (20%) and Thymoglobulin (16.7%), p = 0.04. The rejection by compatibility shows that the identical had the lower rejection rate (10%). The haploidentical group without induction had the highest rejection rates (53.3%). In all distinct group the rejection rates were similar with basiliximab or Thymoglobulin, p = NS. The use of induction therapy was associated independently with a lower risk of rejection (OR = 0.32 CI: 0.11 to 0.93, p = 0.036). There were no differences in renal function at 6 months and patient survival and graft in the three groups. Discussion: The haploidentical patients without induction were those with higher rates of acute rejection. The group of patients induced with Thymoglobulin had a higher immunological risk, however showed low rates of rejection. Conclusion: The use of induction therapy resulted in lower rates of rejection in transplantation with living donor.

Multicentric carpotarsal osteolysis (MCTO) is a rare skeletal dysplasia characterized by aggressive osteolysis, particularly affecting the carpal and tarsal bones, and is frequently associated with progressive renal failure. Using exome capture and next-generation sequencing in five unrelated simplex cases of MCTO, we identified previously unreported missense mutations clustering within a 51 base pair region of the single exon of MAFB, validated by Sanger sequencing. A further six unrelated simplex cases with MCTO were also heterozygous for previously unreported mutations within this same region, as were affected members of two families with autosomal-dominant MCTO. MAFB encodes a transcription factor that negatively regulates RANKL-induced osteoclastogenesis and is essential for normal renal development. Identification of this gene paves the way for development of novel therapeutic approaches for this crippling disease and provides insight into normal bone and kidney development.

A doença de Dent é uma tubulopatia ligada ao X causada por mutações no gene que codifica o canal de cloro CLCN-5 e é caracterizada por proteinúria de baixo peso molecular, hipercalciúria, nefrocalcinose e insuficiência renal. Vários casos têm sido descritos, nos quais o único sintoma na apresentação foi proteinúria assintomática e glomerulosclerose global ou segmentar. A insuficiência renal nesses pacientes pode ser causada pela hipercalciúria e proteinúria persistente. Portanto, o inibidor da enzima de conversão da angiotensina e os tiazídicos poderiam ser úteis. O objetivo desta pesquisa é relatar os efeitos destas drogas em dois pacientes com doença de Dent tipo 1 com mutações novas. Neste relato não foram observadas correlações significativas entre dose de hidroclorotiazida e calciúria e entre enalapril e proteinúria. Este achado é importante, pois, sendo pacientes poliúricos, o uso destas drogas poderia prejudicar a função renal

Nephrogenic dopamine is a potent natriuretic paracrine/autocrine hormone that is central for mammalian sodium homeostasis. In the renal proximal tubule, dopamine induces natriuresis partly via inhibition of the sodium/proton exchanger NHE3. The signal transduction pathways and mechanisms by which dopamine inhibits NHE3 are complex and incompletely understood. This manuscript describes the role of the serine/threonine protein phosphatase 2A (PP2A) in the regulation of NHE3 by dopamine. The PP2A regulatory subunit B56 delta (coded by the Ppp2r5d gene) directly associates with more than one region of the carboxy-terminal hydrophilic putative cytoplasmic domain of NHE3 (NHE3-cyto), as demonstrated by yeast-two-hybrid, co-immunoprecipitation, blot overlay and in vitro pull-down assays. Phosphorylated NHE3-cyto is a substrate for purified PP2A in an in vitro dephosphorylation reaction. In cultured renal cells, inhibition of PP2A by either okadaic acid or by overexpression of the simian virus 40 (SV40) small t antigen blocks the ability of dopamine to inhibit NHE3 activity and to reduce surface NHE3 protein. Dopamine-induced NHE3 redistribution is also blocked by okadaic acid ex vivo in rat kidney cortical slices. These studies demonstrate that PP2A is an integral and critical participant in the signal transduction pathway between dopamine receptor activation and NHE3 inhibition. Renal reabsorption of inorganic phosphate (P(i)) is mainly mediated by the Na(+)-dependent P(i)-cotransporter NaPi-IIa that is expressed in the brush-border membrane (BBM) of renal proximal tubules. Regulation and apical expression of NaPi-IIa are known to depend on a network of interacting proteins. Most of the interacting partners identified so far associate with the COOH-terminal PDZ-binding motif (TRL) of NaPi-IIa. In this study GABA(A) receptor-associated protein (GABARAP) was identified as a novel interacting partner of NaPi-IIa applying a membrane yeast-two-hybrid system (MYTH 2.0) to screen a mouse kidney library with the TRL-truncated cotransporter as bait. GABARAP mRNA and protein are present in renal tubules, and the interaction of NaPi-IIa and GABARAP was confirmed by using glutathione S-transferase pulldowns from BBM and coimmunoprecipitations from transfected HEK293 cells. Amino acids 36-68 of GABARAP were identified as the determinant for the described interaction. The in vivo effects of this interaction were studied in a murine model. GABARAP(-/-) mice have reduced urinary excretion of P(i), higher Na(+)-dependent (32)P(i) uptake in BBM vesicles, and increased expression of NaPi-IIa in renal BBM compared with GABARAP(+/+) mice. The expression of Na(+)/H(+) exchanger regulatory factor (NHERF)1, an important scaffold for the apical expression of NaPi-IIa, is also increased in GABARAP(-/-) mice. The absence of GABARAP does not interfere with the regulation of the cotransporter by either parathyroid hormone or acute changes of dietary P(i) content.

BACKGROUND Heart failure with preserved ejection fraction (HFpEF) represents a growing health burden associated with substantial mortality and morbidity. Consequently, risk prediction is of highest importance. Endothelial dysfunction has been recently shown to play an important role in the complex pathophysiology of HFpEF. We therefore aimed to assess von Willebrand factor (vWF), a marker of endothelial damage, as potential biomarker for risk assessment in patients with HFpEF. METHODS AND RESULTS Concentrations of vWF were assessed in 457 patients with HFpEF enrolled as part of the LUdwigshafen Risk and Cardiovascular Health (LURIC) study. All-cause mortality was observed in 40% of patients during a median follow-up time of 9.7 years. vWF significantly predicted mortality with a hazard ratio (HR) per increase of 1 SD of 1.45 (95% confidence interval, 1.26-1.68; P<0.001) and remained a significant predictor after adjustment for age, sex, body mass index, N-terminal pro-B-type natriuretic peptide (NT-proBNP), renal function, and frequent HFpEF-related comorbidities (adjusted HR per 1 SD, 1.22; 95% confidence interval, 1.05-1.42; P=0.001). Most notably, vWF showed additional prognostic value beyond that achievable with NT-proBNP indicated by improvements in C-Statistic (vWF×NT-proBNP: 0.65 versus NT-proBNP: 0.63; P for comparison, 0.004) and category-free net reclassification index (37.6%; P<0.001). CONCLUSIONS vWF is an independent predictor of long-term outcome in patients with HFpEF, which is in line with endothelial dysfunction as potential mediator in the pathophysiology of HFpEF. In particular, combined assessment of vWF and NT-proBNP improved risk prediction in this vulnerable group of patients.

OBJECTIVE To validate a radioimmunoassay for measurement of procollagen type III amino terminal propeptide (PIIINP) concentrations in canine serum and bronchoalveolar lavage fluid (BALF) and investigate the effects of physiologic and pathologic conditions on PIIINP concentrations. SAMPLE POPULATION Sera from healthy adult (n = 70) and growing dogs (20) and dogs with chronic renal failure (CRF; 10), cardiomyopathy (CMP; 12), or degenerative valve disease (DVD; 26); and sera and BALF from dogs with chronic bronchopneumopathy (CBP; 15) and healthy control dogs (10 growing and 9 adult dogs). PROCEDURE A radioimmunoassay was validated, and a reference range for serum PIIINP (S-PIIINP) concentration was established. Effects of growth, age, sex, weight, CRF, and heart failure on S-PIIINP concentration were analyzed. In CBP-affected dogs, S-PIIINP and BALF-PIIINP concentrations were evaluated. RESULTS The radioimmunoassay had good sensitivity, linearity, precision, and reproducibility and reasonable accuracy for measurement of S-PIIINP and BALF-PIIINP concentrations. The S-PIIINP concentration reference range in adult dogs was 8.86 to 11.48 mug/L. Serum PIIINP concentration correlated with weight and age. Growing dogs had significantly higher S-PIIINP concentrations than adults, but concentrations in CRF-, CMP-, DVD-, or CBP-affected dogs were not significantly different from control values. Mean BALF-PIIINP concentration was significantly higher in CBP-affected dogs than in healthy adults. CONCLUSIONS AND CLINICAL RELEVANCE In dogs, renal or cardiac disease or CBP did not significantly affect S-PIIINP concentration; dogs with CBP had high BALF-PIIINP concentrations. Data suggest that the use of PIIINP as a marker of pathologic fibrosis might be limited in growing dogs.

La enfermedad renal crónica es una de las enfermedades con mayor prevalencia a nivel mundial debido al crecimiento y envejecimiento de la población de los países desarrollados. El número de pacientes con enfermedad crónica terminal aumenta cada año siendo la hemodiálisis el tratamiento de sustitución más común en esta población. A medida que progresa la enfermedad y el tiempo de tratamiento van apareciendo una serie de complicaciones que afectan a los principales sistemas del organismo, produciendo alteraciones musculo-esqueléticas que limitan la capacidad funcional del paciente afectando negativamente a la calidad de vida. Además, muchas de las alteraciones tienen relación con el sistema cardiovascular aumentando el riesgo de mortalidad en el paciente de hemodiálisis. Muchos factores están relacionados con la calidad de vida del paciente, entre ellos el ejercicio físico. Por ello se realiza una revisión de los artículos publicados que relacionan la enfermedad renal y el ejercicio físico con el objetivo de proporcionar unas directrices de trabajo para esta población.
