The program of diagnosis and prevention of oral cancer of Univag - University Center aims to increase preventive and early oral diseases with emphasis on oral cancer, aiming to reduce the morbidity and mortality through educative actions, spreading of the auto-examination, performed by dental students of UNIVAG since the second half of 2005 to the present day. The dental students of 1st, 2nd, 3rd and 4th semesters were trained using practice-theoretical classes, for realization the exams preventives and diagnostics, such as self-examination, exfoliative cytology and biopsy of oral mucosa, and even present educative actions. Analyzing the preliminary results were observed increase in the number of diagnoses and referral of diagnosed with oral cancer for treatment in referral services. It was concluded that the program of diagnosis and prevention of oral cancer can make possible early diagnosis and adoption of preventive measures, consequently improving survival rates and life quality in population.

The peripheral giant cell granuloma (GPCG) is defined as a benign disorder of uncertain etiopathogenesis, and proliferative reaction of the fibrous connective tissue or the periosteum, which is characterized histologically by the presence of multinucleated giant cells. The purpose of this study is report a case of GPCG in a white women, with 56 years old, presenting nodule bleeding to the touch, red and white with small ulcerated areas, defined limits, resilient consistency, pedicled base with 2.0 cm diameter, asymptomatic, involving the permanent lower left third molar, that it was presented with mobility. Radiographically there was significant bone loss in this tooth region, whose initial diagnosis was pyogenic granuloma. The definitive diagnosis was obtained after excisional biopsy in which microscopic examination it was identified e presence of multinucleated giant cells. The clinical postoperative presented favorable cicatricial repairing of the operated area without recurrence after two years of monitoring.

To determine the profile of the patients who participated in the program of oral cancer diagnosis and prevention in Mato Grosso, from 2008 to 2009. Materials and Methods: The medical records of the treated patients were examined. Results: 1.293 records were analyzed, with a predominance of female patients (60%) and a greater participation of patients from 10 to 19 years of age (20%). Reports of pre-existing illnesses amounted to 302 recorded cases. In addition, 292 variations of normality could be also identified. During the period, 25 participants were recommended for exfoliative cytology and 133 for biopsy, as they presented oral lesions that raised the suspicion of malignancy. Conclusions: Campaigns to raise awareness of and prevent oral cancer are very important and should be extended to the entire population of the state of Mato Grosso, either by means of the local University Center (UNIVAG), or by other agencies and health institutions.

The Squamous Cell Carcinoma is what more happens among the oral cancers, being considered sweats incidence in more than 90%. As important prognostic factor competes the early diagnosis and the immediate institution of the treatment, that it can involve radiotherapy and chemotherapy besides the surgery. The initial lesions don’t offer great diagnostic difficulties and now complemental laboratory exams define the diagnosis quickly, In the present report, the initial medical conduct was inadequate, with incomplete interpretation of the laboratorial result and adoption of inadequate therapeutic conduct, what resulted in constant evolution of the lesion. The patient came to the consultation with that medical history, after six months of the first attendance accomplished in another center. To the physical exam, infiltrative ulcerated lesion was observed in the lower lip, with having accentuated tissue loss superficially. The patient accused spontaneous pain and she did use an ointment, recommended six months ago for occasion of the first attendance. As diagnostic conduct was indicated the accomplishment of cytology and biopsy, being defined the diagnosis of Squamous Cell Carcinoma. The patient was immediately guided to the medical service of its municipal district for manegement of the specialized oncologic treatment. The patient was submitted the surgery followed by radiotherapy, being verified involvement of another structures by contiguity. Now, the patient meets abed, without viable perspectives of cure. The present report emphasizes the importance of the clinical knowledge and professional responsibility in attendance of cases of that nature.

A displasia cemento-óssea é o tipo mais comum dentre as lesões fibro-ósseas, podendo apresentar-se de 3 maneiras: periapical, focal e florida. Paciente parda, de 53 anos, compareceu com queixa de “secreção na gengiva” e histórico de exodontia na região há 8 meses. À inspeção apresentou uma leve tumefação na região vestibular posterior esquerda da mandíbula e uma pequena fístula drenando pus. Realizou-se uma radiografia panorâmica, que revelou uma lesão de radiopacidade mista na região de molares inferiores direitos, medindo 2 cm de diâmetro; outra lesão radiopaca na região de pré-molares esquerdos, de 6mm de diâmetro - ambas bem delimitadas; e outra área radiolúcida com radiopacidade central, com halo radiopaco na região infectada, medindo 1,5 cm de diâmetro. O diagnóstico presuntivo foi de displasia cemento-óssea infectada, cisto periapical infectado associado à displasia e osteomielite. Após antibioticoterapia, realizou-se biópsia e curetagem da área. Microscopicamente apresentou áreas de tecido cemento-ósseo em forma trabecular e no local onde deveria haver tecido fibroso, presença de biofilme bacteriano, conteúdo hemorrágico e restos necróticos. No tecido de granulação removido observou-se intenso infiltrado inflamatório e áreas de calcificação cementóide. As características microscópicas juntamente com os achados clínicos e radiográficos levaram ao diagnóstico de osteomielite aguda supurativa secundária a lesão fibro-óssea, sugestiva de displasia cemento-óssea florida.

The real role of renal transplantation in hepatic fi brosis progression caused by hepatitis C virus is still unpredictable. Histological evaluation of the liver is the best form to estimate fi brosis evolution, although semiquantitative analysis carries important limitations. Objective: To apply a morphometric quantitative assay on hepatic fi brosis progression in renal recipients with hepatits C. Methods: Thirty patients were initially evaluated, but only seven were included. They underwent the fi rst biopsy near the transplantation date and the second biopsy at least 4 years later. The immunosuppressant therapy adopted in all cases was azatioprine and micofenolate. Fibrosis progression rate (FPR) was calculated before and after the surgery date in each patient according to Metavir score and morphometric analysis. Results: The FPR calculated by Metavir score showed no statistical difference between pre- and post-transplantation (p=0.9). The FPR calculated by the morphometric analysis was 0.58 ± 0.78 before transplantation and 3.0 ± 3.3 after the surgery, with statistical signi- fi cance between these values (p=0.0026). Conclusion: In the sample assessed, the progression of hepatic fi brosis was documented and quantifi ed only by the morphometric analysis, which is as a promising approach to histological evaluation of these patients.

There are only 10 reported cases of pancreatic hemangiomas in adults, only one of them causing digestive bleeding. We present a case of variceal bleeding and portal hypertension caused by a pancreatic hemangioma. The patient had 19 year-old and was received at her 16th week of pregnancy. She had massive hematemesis, controlled after variceal band ligation. Her image exams revealed a cystic lesion of 164 cm³ in the pancreas tail and signs of portal hypertension. Two months after, the ultrassonographic exam documented the lesion growth, achieving 200 cm³ at that time. The patient was submitted to distal pancreatectomy, and the histopathological analysis revealed a pancreatic hemangioma of 11 x 9 x 8 cm. Therefore, we report the second case of digestive bleeding caused by a pancreatic hemangioma, which had a well documented growth during the pregnancy. Additionally, we review the previous reports of pancreatic hemangiomas and discuss the hypothesis of hormonal infl uence on the natural history of these tumors.

Paciente feminino 36 anos, com queixa de febre noturna, astenia, parestesia de membros inferiores, artralgia, hepatoesplenomegalia, lesões cutâneas, anemia hipocrômica, microcítica e linfopenia. Sorologia negativa para Hepatite B,C e HIV; FAN e anti-DNA também negativos. Histopatologia da medula óssea mostrou-se discretamente hipercelular, com presença de focos de histiócitos com citoplasma vacuolado (células pseudo-gaucher-like),aspecto megaloblástico, linfoplasmocitico e lipogranulomatoso. A coloração Ziehl- -Nielsen positivo; diagnóstico morfológico: micobacteriose de medula óssea. A cultura do sangue e do aspirado de medula óssea foi negativo para M.tuberculosis. Biópsia das lesões cutâneas máculas e pápulas hipercrômicas dos membros inferiores e superiores revelou hanseníase de padrão Vichowiana. A pesquisa BAAR pela Faraco demonstrou numerosos bacilos, sendo a maioria de padrão granuloso. Pacientes com hanseníase virchowiana tem acometimento cutâneo em 100% dos casos, o baço é o órgão mais frequentemente acometido com disseminação através do sangue periférico. O quadro anêmico e a linfopenia provavelmente se deve à cronicidade da infecção, levando a infiltração medular. A ocorrência do comprometimento medular em paciente com hanseníase Virchowiana é excepcional, mesmo em países endêmicos, e pode indicar necessidade de tratamento mais prolongado.

Lissencephaly is a condition characterized by a lack of cerebral convolutions and sulci, which results from defective migration of nervous cells precursors in the telencephalus. The cause is presumably genetic. Lhasa-Apso dogs are most frequently affected, even though it may also occur in association with cerebellar hypoplasia in the Irish Setter, Wire-Haired Terrier and Samoieda breeds. This association was also reported in a cat. Clinical signs consist of dementia, aggressiveness, seizures, visual and olfactive dysfunctions, slow postural reactions and reduced menace response. Definitive diagnosis requires exams such as magnetic resonance imaging, cerebral biopsy or necropsy. There is no specific therapy for this disease, and seizures must be treated with anticonvulsants. The aim of this study is to review the literature regarding lissencephaly.

Contexto e objetivo: A hanseníase caracteriza-se por ter diagnóstico eminentemente clínico, principalmente através da constatação de hipo ou anestesia das lesões, ou seja, sem necessariamente o uso de exames adicionais, como baciloscopia e biópsia, utilizados para classificação clínica da doença. Buscou-se definir se há tendência à desvalorização da avaliação clínica em favor do excesso de exames complementares, investigar a adequação do registro nos atendimentos da atenção básica e compará-los com o obtido em serviço de média complexidade. Desenho e local: Estudo transversal, de base populacional, utilizando registros de prontuários, realizado em dois âmbitos de atendimentos de saúde da cidade de Campinas (SP): os Centros de Saúde e a assistência dermatológica especializada de hospital de ensino. Métodos: Os registros foram investigados por meio da aplicação de checklist, contendo itens indispensáveis ao atendimento a hansenianos. Associações foram testadas pela prova de Goodman e ao nível de 5% de significância. Resultados: Foram avaliados 33 prontuários oriundos dos centros de saúde e 36 do hospital, observou-se polarização entre registros adequados e ausentes, evidenciando a presença de efeito tudo ou nada nos atendimentos; de 26 variáveis, apenas em 5 (19,23%) obteve-se predomínio da avaliação do hospital sobre os centros de saúde; evidenciou-se dependência de exames complementares, solicitados em 67 dos 69 atendimentos (97,10%), para diagnóstico da doença. Conclusão: Confirmou-se, neste estudo, déficit da avaliação médica e indicação inadequada de exames adicionais. Por associar-se com excesso de solicitação de exames, a observação clínica mostrou-se incompleta, comprometendo a capacidade diagnóstica dos atendimentos a hansenianos.

PURPOSE: To compare the frequency of conjunctival HLA-DR expression (a surrogate marker for inflammation) in eyes treated with topical prostaglandin analogues versus eyes treated with other topical antiglaucomatous drugs. METHODS: Patients diagnosed with primary open-angle glaucoma presenting indication for trabeculectomy were divided in groups according to the use or not of prostaglandin analogues. All subjects were treated with the maximum tolerated dose of antiglaucomatous drugs until the date of the surgery. At the beginning of the surgical procedure, a 5 x 5 mm biopsy of the bulbar conjunctiva was collected, incubated with monoclonal anti-HLA-DR antibody and processed for histological analysis. RESULTS: Of the 31 eyes included (31 patients), 25 were under topical prostaglandin analogues (Group 1) and six under other topical pharmacological agents (Group 2). Fourteen eyes of Group 1 (56%) and three of Group 2 (50 %) were positive for the inflammatory marker HLA-DR (P=1.0). The percentage of stained cells ranged from 15.49 to 48.09% (median: 27.61) in Group 1, and from 18.35 to 28 (median: 20.71) in Group 2, with no differences statistically significant (p=0.33). CONCLUSION: The use of prostaglandin analogues did not increase conjunctival expression of HLA-DR compared to other topical antiglaucomatous agents.
