Os procedimentos invasivos recorrentes no tratamento do câncer na criança têm-se mostrado o momento de maior sofrimento tanto para o paciente quanto para o cuidador. Durante estes procedimentos, como no caso da punção venosa, grande parte das crianças apresenta reações caracterizadas como distresse comportamental. Intervenções comportamentais têm sido desenvolvidas com o objetivo de diminuir este distresse, podendo utilizar como agentes os próprios cuidadores das crianças. Esta pesquisa teve como objetivo analisar os efeitos de instrução e de treino parental sobre comportamentos observados em cuidadores e em crianças com diagnóstico de câncer durante procedimento de punção venosa em ambulatório. Foram selecionados nove cuidadores de crianças em tratamento quimioterápico em um hospital especializado em Belém-PA. Para a coleta de dados foram utilizados: Roteiro de entrevista, Questionário Sociodemográfico, Instrumento de Avaliação das Relações Familiares (Parental Bonding Instrument [PBI]), Child Behavior Checklist (CBLC), Protocolo de observação direta dos comportamentos da criança e do cuidador, Escala de Avaliação Comportamental, Manual de orientação para cuidadores sobre punção venosa em crianças e Protocolo de treino parental. Os participantes foram submetidos a uma dentre três condições: (1) Rotina, (2) Manual de Orientação ou (3) Treino Parental. A coleta de dados foi realizada por meio de entrevista e de quatro sessões de observação direta do comportamento. Os comportamentos das crianças foram classificados como concorrentes e não concorrentes. Os comportamentos dos cuidadores foram classificados como monitoria positiva, monitoria negativa e negligência. Os resultados sugerem que, na Condição Rotina, os participantes (cuidadores) não variaram seus comportamentos ao longo das sessões, com dois participantes mantendo alta frequência de comportamentos negligentes e um participante mantendo monitoria positiva. Na Condição Manual, observou-se mudança de comportamento em dois cuidadores como efeito imediato à utilização do manual, mas não em longo prazo. Na Condição 3, na qual os cuidadores foram submetidos ao Protocolo de treino parental, observou-se aumento na frequência de monitoria positiva em curto e longo prazo. Com relação às crianças, os resultados indicaram maior frequência de comportamentos não concorrentes, independente da condição a que o cuidador foi submetido. A maioria dos cuidadores foi classificada no PBI com estilo parental permissivo, confirmando a literatura sobre cuidadores de crianças com doenças crônicas. Ocorreram relatos de generalização das habilidades treinadas para contextos fora do hospital. Conclui-se que o uso do manual aumentou a aquisição de conhecimentos sobre punção venosa mas não foi suficiente para a manutenção de mudança de comportamentos. Por outro lado, o uso de treino parental mostrou-se eficaz na mudança de comportamentos (a longo prazo), assim como no desenvolvimento de novas habilidades sociais. Discute-se a importância do estilo parental como fator de proteção à criança com câncer.

A obesidade é uma doença crônica definida como o acúmulo de gordura anormal ou excessiva que pode prejudicar a saúde. Torna-se necessário intervir no seu combate e prevenção, especialmente entre as crianças. A literatura da área indica que, em geral, intervenções apenas com crianças ou com cuidadores, ou com ambos, apresentam resultados favoráveis. Neste estudo foram avaliados os efeitos de instruções, do treino de relato verbal (TRV) e do treino de automonitoração (TA), aplicados com e sem a participação do cuidador principal, sobre o seguimento de regras nutricionais em crianças com obesidade ou sobrepeso. Participaram duas crianças (9 e 11 anos) e suas cuidadoras primárias. O ambiente foi um consultório do ambulatório de psicologia de um hospital universitário. Foram utilizados: Prontuário dos pacientes, Roteiro de entrevista inicial, Inventário de estilos parentais (IEP), Roteiro de entrevista 2, Recordatório 24 horas, Manual informativo sobre obesidade e alimentação saudável, Teste de conhecimentos, Protocolo de orientação nutricional para crianças, Protocolo de automonitoração, Roteiro para análise do protocolo de automonitoração, jogos e brinquedos e Roteiro de entrevista final. O procedimento de coleta ocorreu em 10 sessões distribuídas em aproximadamente 15 semanas e consistiu de análise dos prontuários; entrevista no ambulatório com o cuidador e com a criança para assinatura do Termo de Consentimento Livre e Esclarecido, aplicação do roteiro de entrevista inicial e do IEP; inserção dos participantes em uma de duas condições (Condição 1, apenas a criança [P1] presente; Condição 2, tanto a criança [P2] quanto seu cuidador primário [C2] estavam presentes); aplicação do Roteiro de entrevista 2 seguido da aplicação do Recordatório 24 horas (Linha de Base 1 [LB1]); aplicação do Manual informativo sobre obesidade e alimentação saudável e do Teste de conhecimentos; TRV (Linha de Base 2 [LB2]); TA; entrevista de acompanhamento e entrevista de encerramento. Quanto aos efeitos das instruções, os resultados indicam que P1 manteve a mesma classificação em todos os itens, enquanto P2 melhorou seu desempenho nos itens conhecimento e seguimento das orientações nutricionais e C2 apresentou melhora no conhecimento sobre obesidade ao final do estudo. Os Índices de adesão à dieta obtidos por P2 foram mais elevados do que os obtidos por P1 em todas as fases da pesquisa. Comparando-se a média obtida pelos dois participantes em LB1 e LB2, observou-se aumento de 39,77% indicando mudança com significância clínica após intervenção. A combinação de variáveis nesta pesquisa mostrou-se favorável a ampliação do repertório dos participantes em relação ao comportamento alimentar, tendo como referência o próprio sujeito ao longo do estudo. Os resultados sugerem que há maior eficácia quando crianças e cuidadores são alvo de intervenção.

Objective: to identify the interference of acute myocardial infarction (AMI) in the quality of life of affected, interventions and understanding by health professionals. Method: an integrative review, aiming to answer << What are the interference in the quality of life of post-AMI customers? >> and << What are the interventions proposed in order to minimize them? >>. We selected 12 articles available in the LILACS database, between 2000 and 2011, based on the criteria of inclusion and exclusion. Results: we have selected a total of 12 articles selected according to the inclusion and exclusion criteria pre-established. We obtained a classification into two themes (1) interference with quality of life and (2) proposals for interventions to minimize interference. Conclusions: highlights the importance of patient involvement in care plan well structured, multidisciplinary team integration and quantity of publications by heterogeneous country on the subject.

Background: Pectus excavatum is characterized by a depression of the anterior chest wall (sternum and lower costal cartilages) and is the most frequently occurring chest wall deformity. The prevalence ranges from 6.28 to 12 cases per 1000 around the world. Generally pectus excavatum is present at birth or is identified after a few weeks or months; however, sometimes it becomes evident only at puberty. The consequence of the condition on a individual's life is variable, some live a normal life and others have physical and psychological symptoms such as: precordial pain after exercises; impairments of pulmonary and cardiac function; shyness and social isolation. For many years, sub-perichondrial resection of the costal cartilages, with or without transverse cuneiform osteotomy of the sternum and placement of a substernal support, called conventional surgery, was the most accepted option for surgical repair of these patients. From 1997 a new surgical repair called, minimally invasive surgery, became available. This less invasive surgical option consists of the retrosternal placement of a curved metal bar, without resections of the costal cartilages or sternum osteotomy, and is performed by videothoracoscopy. However, many aspects that relate to the benefits and harms of both techniques have not been defined. Objectives: To evaluate the effectiveness and safety of the conventional surgery compared with minimally invasive surgery for treating people with pectus excavatum. Search methods: With the aim of increasing the sensitivity of the search strategy we used only terms related to the individual's condition (pectus excavatum); terms related to the interventions, outcomes and types of studies were not included. We searched the Cochrane Central Register of Controlled Trials (CENTRAL), PubMed, Embase, LILACS, and ICTPR. Additionally we searched yet reference lists of articles and conference proceedings. All searches were done without language restriction. Date of the most recent searches: 14 January 2014. Selection criteria: We considered randomized or quasi-randomized controlled trials that compared traditional surgery with minimally invasive surgery for treating pectus excavatum. Data collection and analysis: Two review authors independently assessed the eligibility of the trials identified and agreed trial eligibility after a consensus meeting. The authors also assessed the risk of bias of the eligible trials. Main results: Initially we located 4111 trials from the electronic searches and two further trials from other resources. All trials were added into reference management software and the duplicates were excluded, leaving 2517 studies. The titles and abstracts of these 2517 studies were independently analyzed by two authors and finally eight trials were selected for full text analysis, after which they were all excluded, as they did not fulfil the inclusion criteria. Authors' conclusions: There is no evidence from randomized controlled trials to conclude what is the best surgical option to treat people with pectus excavatum.

Spontaneous adverse drug events (ADE) reporting is the main source of data for assessing the risk/benefit of drugs available in the pharmaceutical market. However, its major limitation is underreporting, which hinders and delays the signal detection by Pharmacovigilance (PhV). To identify the techniques of educational intervention (EI) for promotion of PhV by health professionals and to assess their impact. A systematic review was performed in the PUBMED, PAHO, LILACS and EMBASE databases, from November/2011 to January/2012, updated in March/2013. The strategy search included the use of health descriptors and a manual search in the references cited by selected papers. 101 articles were identified, of which 16 met the inclusion criteria. Most of these studies (10) were conducted in European hospitals and physicians were the health professionals subjected to most EI (12), these studies lasted from one month to two years. EI with multifaceted techniques raised the absolute number, the rate of reporting related to adverse drug reactions (ADR), technical defects of health technologies, and also promoted an improvement in the quality of reports, since there was increased reporting of ADR classified as serious, unexpected, related to new drugs and with high degree of causality. Multifaceted educational interventions for multidisciplinary health teams working at all healthcare levels, with sufficient duration to reach all professionals who act in the institution, including issues related to medication errors and therapeutic ineffectiveness, must be validated, with the aim of standardizing the Good Practice of PhV and improve drug safety indicators.

Parenting practices can be understood as the behaviors emitted by parents to educate and socialize their children. The present study investigates the influence of the parenting practices according to the baby sex and age, family configuration and number of children. For this, 50 adult mothers were questioned according to the Parental Styles Inventory for Mothers with Babies (IEPMB). An analysis with the results indicates that mothers with baby girls normally uses a more Positive Monitoring practice when compared with mothers with boy babies. A positive correlation (r = +0.54, p <0.001) was found, between the age of the baby and the practice of negligence, meaning that there is an increase of the usage in the practical as the baby age increases. Also a significant difference (p = 0.04) in the Inconsistent Punishment practice pointed out to the fact that mothers from nuclear families seem to use this practice more than mothers from other family configurations. And multiparous mothers use more Physical Abuse practical than primiparous mothers (p = 0.02). Concerning this analysis, we support a discussion in the present work about the importance of interventions that focus on the effect of specific positive practices, the reduction of negative practices and the role of the network support on the children development.

In contrast to the impressive advances in somatic research of erectile dysfunction (ED), scientific literature shows contradictory reports on the results of psychotherapy for the treatment of ED. Authors conducted a meta-analysis to evaluate the effectiveness of psychological interventions for the treatment of ED compared to oral drugs, local injection, vacuum devices, or other psychological intervention. Distinct sources of randomized controlled trials (RCTs) were searched: electronic databases (between 1966 and 2007), cross checking of references, and contact with scientific societies. For dichotomous outcomes the pooled relative risks were calculated and for continuous outcomes mean differences between interventions. Statistical heterogeneity was addressed. Eleven RCTs involving 398 men met the inclusion criteria. There is evidence that group therapy improves ED. Focused sex group therapy showed greater efficacy than control group. Men randomized to receive psychotherapy plus sildenafil showed significant improvement of ED and were less likely than those receiving only sildenafil to drop out. Regarding to the effectiveness of psychological interventions for the treatment of ED compared to local injection and vacuum devices no difference was found. Melnik T, Soares BGO, and Nasello AG. This qualitative study examines the social relationships between the Community Health Agents (CHAs) and the Family Health team (FH), highlighting cooperative interventions and interactions among workers. A total of 23 participant observations and 11 semi-structured interviews were conducted with an FH team in a city in the interior of Sao Paulo, Brazil. The results revealed that CHAs function as a link in the development of operational actions to expedite teamwork. These professionals, while creating bonds, articulate connections of teamwork and interact with other workers, developing common care plans and bringing the team and community together, as well as adapting care interventions to meet the real needs of people. In communication practice, when talking about themselves they talk about the community itself because they are the community's representatives and spokespersons on the team. The conclusion is that the CHA may be a strategic worker if his/her actions include more political and social dimensions of work in healthcare.
