Background/Objectives:To evaluate the impact of preoperative immunonutrition (IN) on postoperative morbidity in patients at risk of malnutrition undergoing major gastrointestinal (GI) surgery.Subjects/Methods:The combination of malnutrition and major GI surgery entails high morbidity. The Nutritional Risk Score (NRS) reliably identifies patients who need preoperative nutrition; the optimal nutritional formula for these patients still needs to be defined. In all, 152 patients with a NRS3 and undergoing elective major GI surgery were randomized between IN or isocaloric-isonitrogenous nutrition (ICN) given for 5 days preoperatively. Patients and caregivers were blinded for the allocated intervention. Thirty days complication rate was the primary endpoint. Infections, length of hospital stay and compliance were considered as secondary outcomes.Results:Overall, 145 patients were available for analysis; the 73 patients in the IN group matched well with the 72 ICN patients with regards to patient's and surgical characteristics. In all, 39 IN and 33 ICN patients experienced a total of 48 and 50 postoperative complications, respectively (P=0.723). Both groups did not differ significantly concerning infectious (13 vs 9) complications. Independent risk factors for overall complications were malignant disease (odds ratio (OR)=4.304; confidence interval (CI) 1.317-14.002) and operative time (OR=1.004; CI 1.000-1.008).Conclusion:In patients at nutritional risk, complications, infections and hospital stay after major GI surgery were comparable regardless of preoperative supplementation with IN or ICN.

The 2008-2012 Library Services and Technology Act (LSTA) Plan represents a blueprint for the State Library of Iowa’s federally funded activities over the next five years. We feel we have been successful in striking a balance between challenging ourselves to achieve more and being realistic about what we can actually accomplish with current human and financial resources. We have incorporated “Lessons Learned” from the previous five year plan. These include: • Continue to use LSTA funding primarily for projects of statewide impact, an emphasis based on input from the Iowa library community. • Consider carefully the ramifications of taking on additional projects. We have carefully selected a few, new projects. • Leverage staff resources and non-federal funding to carry out our LSTA plan. A number of programs and services identified in the plan also use state funding, including the State Data Center, Iowa Publications Online, Direct State Aid, Open Access and Access Plus. • Measure progress regularly and frequently. We plan to review our progress in implementing the plan at least quarterly and will ask the Iowa Commission of Libraries to review progress annually. • Write objectives that come closer to identifying impact. We believe our target outcomes are do-able and come closer to measuring impact.

stratégiques importants, le programme national de sécurité alimentaire du Cap-Vert, avec une orientation marquée pour la réduction de la pauvreté le PANA II en cours d’élaboration, le document UNDAF2 pour le Cap-Vert élaboré en septembre 2002, avec quatre axes d’intervention prioritaire et cinq dimensions transversales. la dimension genre, pour favoriser la participation effective de toutes les couches de la société (notamment les femmes) au processus de développement socio-économique du pays, particulièrement dans les politiques, stratégies et programmes du MAAP en cohérence avec la politique de décentralisation du Gouvernement. les dynamiques d’intégration sous-régionale en cours dans le cadre de la CEDEAO3, et du CILSS4, et continentales dans le cadre du NEPAD5. C'est en tenant compte de l’ensemble du cadre contextuel évoqué et de la complexité des tâches à accomplir que le Gouvernement a sollicité l'assistance de la FAO, au titre du programme de coopération technique, pour l’aider à élaborer une stratégie de développement agricole à long terme (horizon 2015) et préparer un Plan d’actions prioritaires à moyen terme.
