1.Pre-assessment data of the patient A 2-year-old boy, weighing 15 kg was admitted with a history of limited mouth opening(inter-incisor distance of 6 mm), hypoplastic and retrognathic mandible (bird face deformity) and facial asymmetry from left temporomandibular joint ankylosis (TMJA). He was born at term, after an uneventful pregnancy, and there was no report of trauma during caesarean section. No other possible aetiologies were identified. He was scheduled for mandibular osteotomy. Preoperative ENT examination revealed adenotonsillar hypertrophy. 2. Anaesthetic Plan A fiberoptic nasal intubation was performed under deep inhalation anaesthesia with sevoflurane, with the patient breathing spontaneously. Midazolam (0.05 mg.kg-1) and alfentanil (0.03 mg.kg-1) were given and anaesthesia was maintained with O2/air and sevoflurane. No neuromuscular blocking agent was administered since the surgical team needed facial nerve monitoring. 3. Description of incident During surgery an accidental extubation occurred and an attempt was made to reintubate the trachea by direct laryngoscopy. Although the osteotomy was nearly completed, the vocal cords could not be visualized (Cormack-Lehane grade IV laryngoscopic view). 4. Solving the problem Re-intubation was finally accomplished with the flexible fiberscope and the procedure was concluded without any more incidents. Extubation was performed 24 hours postoperatively with the patient fully awake. After surgery mouth opening improved to inter-incisor gap of 15 mm. 5. Lessons learned and take home message Two airways issues present in this case can lead to difficultventilation and intubation: TMJA and adenotonsillar hypertrophy. These difficulties were anticipated and managed accordingly. The accidental extubation brought to our attention the fact that, even after surgical correction, this airway remains challenging. Even with intensive jaw stretchingexercises there is a high incidence of re-ankylosis, especially in younger patients. One should bear that in mind when anaesthetizing patients with TMJA.

Introduction: Heterotopic pregnancy (HP) is defined as two gestational sacs simultaneously present in two different locations, being the uterus and the fallopian tubes the more common. Sporadic HP is a very rare condition (1:30,000 pregnancies). With the use of medically assisted reproduction the prevalence is significantly higher(1:7,000). Considering spontaneous pregnancy, HP is associated with risk factors, being prior inflammatory pelvic disease the most common. The clinical presentation is similar to that of ectopic pregnancy or spontaneous miscarriage although it is usually a more late diagnosis. Case report: 25 year-old pregnant woman, OI 0000, previously healthy; admitted at the Emergency Department (ED) with acute pelvic pain mainly at the right iliac fossa and moderate vaginal bleeding confirmed by speculum examination. She was hemodynamically stable and the bimanual palpation was painful; no prior medically assisted reproduction technique had been performed. The haemoglobin value was within normal range and the serum β-hCG was 2,763mUI/mL. The ultrasonography at the ED showed an in uterus gestational sac and another one inside the right fallopian tube; in both gestational sacs cardiac activity was absent. HP diagnosis was then established and the patient was admitted at the Obstetrics Ward for surveillance and ultrasonographic/laboratorial reassessment; complete miscarriage of the uterine pregnancy occurred but methotrexate was necessary for the treatment of persistent tubarian pregnancy. Conclusion: When evaluating a pregnant woman with pelvic pain and vaginal bleeding one should always be aware of several differential diagnosis amongst which HP should be considered. If the patient has in uterus viable pregnancy the treatment of the ectopic concomitant gestational sac should be as conservative as possible; methotrexate should not be used in that situation as it leads to uterine pregnancy miscarriage in about one third of the patients.

Introduction: Sciatic nerve variations are relatively common. These variations are often very significant in several fields of Medicine. The purpose of this paper is to present two such variants and discuss their clinical implications. Material and Methods: Three Caucasian cadavers with no prior history of lower limb trauma or surgery were dissected and found to present anatomical variants of the sciatic nerve. Results: In all cases the sciatic nerve divided above the popliteal fossa. In two cases (cadavers 1 and 2) it divided on both sides in the inferior portion of the gluteal region in its two terminal branches: the common fibular and the tibial nerves. In another case (cadaver 3) the sciatic nerve was found to divide inside the pelvis just before coursing the greater sciatic notch. The common fibular nerve exited the pelvis above the pyriformis muscle and then passed along its posterior aspect, while the tibial nerve coursed deep to the pyriformis muscle. Discussion: According to the literature, the anatomical variant described in cadaver 3 is considered relatively rare. This variant can predispose to nerve entrapment and thus to the pyriformis syndrome, sciatica and coccygodynia. The high division of the sciatic nerve, as presented in cadavers 1 and 2, can make popliteal nerve blocks partially ineffective. Conclusion: The anatomical variants associated with a high division of the sciatic nerve, must always be born in mind, as they are relatively prevalent, and have important clinical implications, namely in Anesthesiology, Neurology, Sports Medicine and Surgery.

Blastic plasmacytoid dendritic cell neoplasm is a rare and aggressive hematodermic neoplasia with frequent cutaneous involvement and leukemic dissemination. We report the case of a 76-year-old man with a 2 month history of violaceous nodules and a tumor with stony consistency, located on the head, and mandibular, cervical and supraclavicular lymphadenopathies. Multiple thoracic and abdominal adenopathies were identified on computerized tomography. Flow cytometry analysis of the skin, lymph node and bone marrow biopsies demonstrated the presence of plasmocytoid dendritic cell neoplastic precursor cells (CD4+, CD45+, CD56+ and CD123+ phenotype). After initial clinical and laboratorial complete remission with chemotherapy, the patient died due to relapse of the disease associated with the appearance of a cervical mass with medullary compromise.

A anestesia regional em ortopedia apresenta vantagens claras na estabilidade hemodinâmica, perfusão do território cirúrgico e analgesia de qualidade superior. o objectivo deste estudo foi avaliar o tipo e frequência de técnicas realizadas no ano de 2012 em anestesia para cirurgia ortopédica pediátrica. Um total de 662 crianças agendadas para cirurgia electiva foram retrospectivamente estudadas no que diz respeito às técnicas regionais utilizadas. Foram realizadas 248 técnicas regionais em 2012. Houve urn predominio de bloqueios do neuro-eixo (63%) em relação aos bloqueios dos nervos periféricos (BNP) (37%). A ultrassonografia foi essencial nos BNP realizados, correspondendoa 75% dos casos. Na anestesia do membro superior os bloqueios mais frequentes foram o bloqueio do plexo braquialvia supra-clavicular (61%) e os BNP na fossa antecubital (23%). No membro inferior os bloqueios mais comuns foram o bloqueio do nervo ciciticopopliteo (41%) e o bloqueio de nervo femoral (35%). Colocaram-se cateteres contínuos de bloqueio de nervo periférico em 5 bloqueios do nervo ciático popliteo. o uso de ecografo tornou-se preponderante para a realização de bloqueios dos nervos periféricos dos membros com as vantagens já amplamente descritas na literatura. Parece haver margem para diminuir o número de técnicas do neuro-eixo em relação a realização de BNP com uma maior taxa de colocação de cateteres contínuos de bloqueio de nervo periférico, diminuindo deste modo a invasibilidade do neuro-eixo. 0 número total de BNP realizados parece claramente satisfatório embora careça de estudos comparativos com outras instituições que o comprove.

INTRODUÇÃO A neoplasia do intestino delgado é uma entidade rara, apresentando uma incidência anual de cerca de 2.1 casos por 100,000 pessoas, sendo o adenocarcinoma, o segundo tipo histológico mais frequente (em 33% dos casos). A localização distal deste tipo de tumor é menos habitual, o que torna o adenocarcinoma do íleon num evento invulgar. A raridade deste tipo de patologia, associada a uma apresentação clínica pouco específica, origina habitualmente um atraso no diagnóstico e tratamento. CASO CLÍNICO Doente de 66 anos admitida no S.U. com quadro de dor abdominal localizada na fossa ilíaca direita com cerca de 6 dias de evolução e agravamento progressivo. Anorexia, náuseas e vómitos no dia de admissão. Empastamento doloroso à palpação na fossa ilíaca direita. Exames complementares revelaram tumor inflamatório do apêndice ileocecal. Intraoperatoriamente constatou-se neoformação com envolvimento do apêndice ileocecal, cego e segmento de íleon distal. Adenopatias na raíz do mesentério. Optou-se pela realização de hemicolectomia direita com ressecção em bloco de íleon distal. O resultado histológico revelou adenocarcinoma do íleon, que se estendia ao apêndice ileocecal e condicionando apendicite aguda (pT4N0). DISCUSSÃO/CONCLUSÃO Este caso apresenta uma patologia pouco frequente, caracterizada por um diagnóstico tardio e de difícil realização. Sublinhamos neste trabalho, a importância de um diagnóstico mais precoce e um tratamento adequado, de forma a obter um aumento da taxa de sobrevivência destes doentes.

O torcicolo adquirido na criança é uma manifestação clínica de múltiplas etiologias possíveis, que se podem agrupar em entidades musculoesqueléticas de origem traumática, infecciosa, inflamatória e tumoral, e entidades não musculo- -esqueléticas de origem neurológica, oftalmológica, otorrinolaringológica, gastroenterológica e tumoral do sistema nervoso central. Assim existe um amplo espectro de gravidade, desde processos benignos e autolimitados tal como o torcicolo traumático até quadros graves e potencialmente fatais como os tumores da fossa posterior. O torcicolo agudo é uma situação muito frequente no serviço de urgência. A maioria consiste em espasmo muscular, resultante de traumatismo minor e responde bem ao tratamento conservador. No entanto, um torcicolo persistente implica uma anamnese e um exame objectivo completos e minuciosos de forma a se poder colocar as hipóteses diagnósticas mais adequadas, independentemente da sua frequência, e estabelecer um programa de avaliação que nos permita chegar ao diagnóstico definitivo, pedido os exames complementares mais adequados. No torcicolo persistente/recorrente com anormalidades neurológicas devem ser efectuados exames neuro-imagiológicos para excluir patologia tumoral e eventualmente encaminhar para uma consulta de Neurologia ou Oftalmologia. Um torcicolo agudo acompanhado de febre deve orientar para uma etiologia infecciosa ou inflamatória da cabeça e pescoço. Apresenta-se uma revisão das principais causas de torcicolo adquirido com casos clínicos ilustrativos.

Estudo transversal, realizado de agosto/1998 a novembro/2000, envolvendo 207 pacientes hansenianos com o objetivo de analisar o perfil socioeconômico, demográfico e ambiental e as incapacidades físicas em decorrência da doença. O estudo foi desenvolvido em Buriticupu, área hiperendêmica em hanseníase, localizado na Amazônia do Maranhão. O grau de incapacidade foi determinado de acordo com o Ministério da Saúde do Brasil. A avaliação clínica e os resultados do exame físico foram registrados em uma ficha padronizada. Observou-se predomínio de pessoas casadas (45,9%), com escolaridade de 1º grau (56%), lavradores (40,1%), com renda familiar inferior a um salário mínimo (76,3%), na faixa etária de 14 a 44 anos (63,3%), do gênero masculino (60,9%) e da cor parda (67,6%). 44% residiam em casa de taipa, 82,6% destinavam os dejetos em fossa negra, 63,8% lançavam o lixo a céu aberto, 58% utilizavam água proveniente de poço e 51,7% não tratavam a água utilizada para ingestão. A maioria (75,4%) apresentava algum grau de incapacidade física, sendo predominante o Grau I (67,6%). Os segmentos mais afetados foram pés (62,3%), olhos (51,2%) e mãos (7,2%), sendo o maior percentual de incapacidades físicas observado entre os da forma dimorfa (93%) principalmente em mãos e pés, e na forma virchowiana maior freqüência de incapacidades oculares. Conclui-se que a hiperendemicidade associada a precárias condições socioeconômicas e ao elevado índice de incapacidades físicas podem interferir na qualidade de vida dos pacientes.

Female genital schistosomiasis is not uncommon in endemic areas for schistosomiasis, but there are few reports in the Brazilian medical literature. Here, we describe the case of a 31-year-old woman with lower abdominal pain who was diagnosed as presenting a fallopian tube tumor caused by Manson's schistosomiasis. The diagnosis was delayed because her symptoms were considered nonspecific. Involvement of the parietal peritoneum of the ovarian fossa was observed during laparoscopy and confirmed by histological analysis. The left tube and the tumor were excised and schistosomiasis was treated with praziquantel. She presented a full recovery and options for future reproduction are under evaluation.

Theropods form a highly successful and morphologically diversified group of dinosaurs that gave rise to birds. They include most, if not all, carnivorous dinosaurs, yet many theropod clades were secondarily adapted to piscivory, omnivory and herbivory, and theropods show a large array of skull and dentition morphologies. This work aims to investigate aspects of the evolution of theropod dinosaurs by analyzing in detail both the anatomy and ontogeny of teeth and quadrates in non-avian theropods, and by studying embryonic and adult material of a new species of theropod. A standardized list of terms and notations for each anatomical entity of the tooth, quadrate, and maxilla is here proposed with the goal of facilitating descriptions of these important cranial and dental elements. The distribution of thirty dental characters among 113 theropod taxa is investigated, and a list of diagnostic dental characters is proposed. As an example, four isolated theropod teeth from the Lourinhã Formation (Kimmeridgian‒Tithonian) of Portugal are described and identified based on a cladistic analysis performed on a data matrix of 141 dentition-based characters coded in 60 taxa. Two shed teeth are referred to an abelisaurid, providing the first record of Abelisauridae in the Jurassic of Laurasia and the one of the oldest records of this clade in the world, suggesting a possible radiation of Abelisauridae in Europe well before the Upper Cretaceous. The consensus tree resulting from this phylogenetic analysis, the most extensive on theropod teeth, indicates that theropod teeth provide reliable data for identification at approximately family level, and this method will help identifying theropod teeth with more confidence. A detailed description of the dentition of Megalosauridae is also provided, and a discriminant analysis performed on a dataset of numerical data collected on the teeth of 62 theropod taxa reveals that megalosaurid teeth are hardly distinguishable from other theropod clades with ziphodont dentition. This study highlights the importance of detailing anatomical descriptions and providing additional morphometric data on teeth with the purpose of helping to identify isolated theropod teeth. In order to evaluate the phylogenetic potential and investigate the evolutionary transformations of the quadrate, a phylogenetic morphometric analysis as well as a cladistic analysis using 98 discrete quadrate related characters were conducted. The quadrate morphology by its own provides a wealth of data with strong phylogenetic signal, and the phylogenetic morphometric analysis reveals two main morphotypes of the mandibular articulation of the quadrate linked to function. As an example, six isolated quadrates from the Kem Kem beds (Cenomanian) of Morocco are determined to be from juvenile and adult individuals of Spinosaurinae based on phylogenetic, morphometric, and phylogenetic morphometric analyses. Morphofunctional analysis of the spinosaurid mandibular articulation has shown that the posterior parts of the two mandibular rami displaced laterally when the jaw was depressed due to a mediolaterally oriented intercondylar sulcus of the quadrate. Such lateral movement of the mandibular ramus was possible due to a movable mandibular symphysis in spinosaurids, allowing the pharynx to be widened. A new species of theropod from the Lourinhã Formation of Portugal, Torvosaurus gurneyi, is erected based on a right maxilla and an incomplete caudal centrum. This taxon supports the mechanism of vicariance that occurred in the Iberian Meseta during the Late Jurassic when the proto-Atlantic was already well formed. A theropod clutch containing several crushed eggs and embryonic material is also assigned to this new species of Torvosaurus. Investigation on the maxilla ontogeny in basal tetanurans reveals that crown denticles, elongation of the anterior ramus, and fusion of interdental plates appear at a posthatchling stage. On the other hand, maxillary pneumaticity is already present at an embryonic stage in non-avian theropods.

As preocupações de projetistas na conceção de estruturas marítimas utilizando modelos numéricos como ferramentas de dimensionamento, prendem-se com a sua fiabilidade, tempo de processamento e precisão. Assim, a calibração e validação de modelos numéricos é essencial para os tornar ferramentas úteis para o projeto de obras marítimas. A presente dissertação tem como objetivo a calibração e validação do modelo numérico AMAZON, baseado na teoria da onda em condições de águas pouco profundas, para dois casos de estudo distintos com base em dados de ensaios realizados anteriormente. O primeiro caso consiste no estudo do espraiamento numa praia barra-fossa cujas condições de batimetria, nível da água e condições de agitação marítima inseridas no AMAZON correspondem às de ensaios em modelo físico realizados à escala 1:5 na Universidade Politécnica da Catalunha, Barcelona. Como o modelo utiliza parâmetros que devem ser calibrados, na sua aplicação e exploração realizou-se um estudo de sensibilidade a esses parâmetros. Com base nos resultados do AMAZON, utilizaram-se programas desenvolvidos em MatLab para o cálculo do espraiamento e do coeficiente de reflexão e compararam-se os resultados com os obtidos nos ensaios em modelo físico. Verificou-se que os resultados do AMAZON são bastante próximos dos valores obtidos pelos ensaios. O segundo caso consiste no estudo de uma estrutura conversora de energia das ondas do tipo coluna de água oscilante - OWC-WEC (Oscillating Water Column - Wave Energy Converter), para a qual o modelo AMAZON nunca tinha sido utilizado. Os dados introduzidos no modelo relativos ao perfil da estrutura, nível da água e condições de agitação correspondem aos dos ensaios realizados à escala 1:35 no LNEC (Laboratório Nacional de Engenharia Civil). Realizou-se um estudo de sensibilidade dos parâmetros do modelo no que respeita à elevação da superfície livre junto e no interior da câmara de água, ao fator de amplificação e ao desfasamento, e compararam-se os resultados com os obtidos nos ensaios em modelo físico. Os resultados do AMAZON constituem um incentivo para a realização e/ou análise de ensaios para diferentes profundidades, aberturas da parede vertical da câmara de água e períodos de onda incidente, que permitam calibração e validação adicionais dos parâmetros do modelo.

The cerebellum floccular complex lobes (FCLs) are housed in the FCL fossa of the periotic complex. There is experimental evidence indicating that the FCLs integrate visual and vestibular information, responsible for the vestibulo-ocular reflex, vestibulo-collic reflex, smooth pursuit and gaze holding. Thus, the behavior of extinct animals has been correlated with FCLs dimension in multiple paleoneuroanatomy studies. Here I analyzed braincase endocasts of a representative sample of Mammalia (48 species) and Aves (59 species) rendered using tomography and image segmentation and tested statistical correlations between the floccular complex volume, ecological and behavioral traits to assess various previously formulated paleobiological speculations. My results demonstrate: 1) there is no significant correlation between relative FCL volume and body mass; 2) there is no significant correlation between relative FCL and optic lobes size in birds; 3) average relative FCL size is larger in diurnal than in nocturnal birds but there is no statistically significant difference in mammals; 4) feeding strategies are related with different FCL size patterns in birds, but not in mammals; 5) locomotion type is not related with relative FCL size in mammals; 6) agility is not significantly correlated with FCL size in mammals. I conclude that, despite the apparent relation between FCL size and ecology in birds, the cerebellum of tetrapods is a highly plastic structure and may be adapted to control different functions across different taxonomic levels. For example, the european mole (Talpa europaea) which is fossorial and practically blind, has a FCL fossae relative size larger than those of bats, which are highly maneuverable. Therefore, variation in FCL size may be better explained by a combination of multiple factors with relation to anatomical and phylogenetic evolutionary constraints.

OBJECTIVE: It has been shown that the temporomandibular joint is frequently affected by juvenile idiopathic arthritis, and this degenerative disease, which may occur during facial growth, results in severe mandibular dysfunction. However, there are no studies that correlate oral health (tooth decay and gingival diseases) and temporomandibular joint dysfunction in patients with juvenile idiopathic arthritis. The aim of this study is to evaluate the oral and facial characteristics of the patients with juvenile idiopathic arthritis treated in a large teaching hospital. METHOD: Thirty-six patients with juvenile idiopathic arthritis (26 female and 10 male) underwent a systematic clinical evaluation of their dental, oral, and facial structures (DMFT index, plaque and gingival bleeding index, dental relationship, facial profile, and Helkimo's index). The control group was composed of 13 healthy children. RESULTS: The mean age of the patients with juvenile idiopathic arthritis was 10.8 years; convex facial profile was present in 12 juvenile idiopathic arthritis patients, and class II molar relation was present in 12 (P = .032). The indexes of plaque and gingival bleeding were significant in juvenile idiopathic arthritis patients with a higher number of superior limbs joints involved (P = .055). Anterior open bite (5) and temporomandibular joint noise (8) were present in the juvenile idiopathic arthritis group. Of the group in this sample, 94% (P = .017) had temporomandibular joint dysfunction, 80% had decreased mandibular opening (P = 0.0002), and mandibular mobility was severely impaired in 33% (P = .015). CONCLUSION: This study confirms that patients with juvenile idiopathic arthritis a) have a high incidence of mandibular dysfunction that can be attributed to the direct effect of the disease in the temporomandibular joint and b) have a higher incidence of gingival disease that can be considered a secondary effect of juvenile idiopathic arthritis on oral health.

Studies on development of female reproductive system insects have shown its utility in physiological age grading. The irreversible charocter of ovarian alterations correlated with mandibular wear and presence of yellow body, were applied to estimate the physiological age of Passalus convexus and Passalus latifrons females, giving results comparable to other studies and indicating the value of these methods in determination of physiological age.
