The integration of growing amounts of distributed generation in power systems, namely at distribution networks level, has been fostered by energy policies in several countries around the world, including in Europe. This intensive integration of distributed, non-dispatchable, and natural sources based generation (including wind power) has caused several changes in the operation and planning of power systems and of electricity markets. Sometimes the available non-dispatchable generation is higher than the demand. This generation must be used; otherwise it is wasted if not stored or used to supply additional demand. New policies and market rules, as well as new players, are needed in order to competitively integrate all the resources. The methodology proposed in this paper aims at the maximization of the social welfare in a distribution network operated by a virtual power player that aggregates and manages the available energy resources. When facing a situation of excessive non-dispatchable generation, including wind power, real time pricing is applied in order to induce the increase of consumption so that wind curtailment is minimized. This method is especially useful when actual and day-ahead resources forecast differ significantly. The distribution network characteristics and concerns are addressed by including the network constraints in the optimization model. The proposed methodology has been implemented in GAMS optimization tool and its application is illustrated in this paper using a real 937-bus distribution network with 20.310 consumers and 548 distributed generators, some of them non-dispatchable and with must take contracts. The implemented scenario corresponds to a real day in Portuguese power system.

The non-technical loss is not a problem with trivial solution or regional character and its minimization represents the guarantee of investments in product quality and maintenance of power systems, introduced by a competitive environment after the period of privatization in the national scene. In this paper, we show how to improve the training phase of a neural network-based classifier using a recently proposed meta-heuristic technique called Charged System Search, which is based on the interactions between electrically charged particles. The experiments were carried out in the context of non-technical loss in power distribution systems in a dataset obtained from a Brazilian electrical power company, and have demonstrated the robustness of the proposed technique against with several others natureinspired optimization techniques for training neural networks. Thus, it is possible to improve some applications on Smart Grids.

The smart grid concept is a key issue in the future power systems, namely at the distribution level, with deep concerns in the operation and planning of these systems. Several advantages and benefits for both technical and economic operation of the power system and of the electricity markets are recognized. The increasing integration of demand response and distributed generation resources, all of them mostly with small scale distributed characteristics, leads to the need of aggregating entities such as Virtual Power Players. The operation business models become more complex in the context of smart grid operation. Computational intelligence methods can be used to give a suitable solution for the resources scheduling problem considering the time constraints. This paper proposes a methodology for a joint dispatch of demand response and distributed generation to provide energy and reserve by a virtual power player that operates a distribution network. The optimal schedule minimizes the operation costs and it is obtained using a particle swarm optimization approach, which is compared with a deterministic approach used as reference methodology. The proposed method is applied to a 33-bus distribution network with 32 medium voltage consumers and 66 distributed generation units.

RESUMO - Este trabalho consiste numa análise sobre a viabilidade económico-financeira do projecto de construção de uma Unidade de Cuidados Continuados Integrados de Longa Duração e Manutenção pela Fundação COI, que pretende ser uma resposta adequada à diversidade de fenómenos que têm origem no envelhecimento populacional, inserindo-se no modelo de prestação de cuidados de saúde e apoio social preconizado pela Rede Nacional de Cuidados Continuados Integrados. Em termos estruturais, numa primeira fase, o trabalho faz uma análise sobre o estado da arte dos cuidados continuado, nomeadamente através da sua conceptualização e enquadramento, focando os aspectos e características da RNCCI, nomeadamente os aspectos relacionados com a sustentabilidade financeira e os desafios futuros que este sector da saúde se confronta. Numa segunda fase, o trabalho apresenta o projecto de investimento alvo do estudo de caso, mediante o enquadramento histórico e caracterização da Fundação COI, sendo descrita especificamente a tipologia de longa duração e manutenção e o projecto em termos da sua estrutura física e humana. Numa terceira fase, o trabalho estuda a racionalidade do projecto de investimento, através da análise económico-financeira. Tendo-se concluído, com a utilização de indicadores de avaliação e decisão de investimentos, que a ULDM, é viável. Deve-se contudo ter em conta que a sua viabilidade é sensível a hipotéticas alterações de cenários, nomeadamente perante diferentes taxas de ocupação ou alteração do modelo de pagamento. ----------------------- ABSTRACT - This paper consists on analyzing the economical and financial viability of a project of Integrated Continuous Cares on Long-Term and Maintenance Unit (LTMU), managed by COI Foundation, that can create a correct social answer on the various aspects and phenomenon’s that have their origin in an aged population. This kind of project belongs in the role model of social and health cares of the National Network of Integrated Continuous Cares (NNICC). In a first stage the paper is focused on the analysis of the state of the present services of the continuous cares in Portugal, more concretely of his conceptualization and social frame, putting under the lights the aspects and characteristics of the NNICC, more closely the parts related to their financial sustainability and the challenges that this health sector faces nowadays. The next stage is about the investment project applied to this case-study, given his social and historical frame and attending to the specific characteristics of the COI Foundation. It’s also describing its specific LTMU typology under the involved physic and human structure. Finally this paper study the investment project rationality trough scientific methods like its economic–financial analysis. The conclusion, using decision and evaluation procedures that lead to financial indicators, is that the LTMU, is viable. However, the viability of the project is sensible to an hypothetical change of scenario, namely with different rates of change of occupation or type of payment.

Paracoccidioidomycosis is a chronic granulomatous disease that induces a specific inflammatory and immune response. The participation of nitric oxide (NO), a product of the inducible nitric oxide synthase enzyme (iNOS), as an important fungicidal molecule against Paracoccidioides brasiliensis has been demonstrated. In order to further characterize the Oral Paracoccidioidomycosis (OP), we undertook an immunohistochemical study of iNOS+, CD45RO+, CD3+, CD8+, CD20+, CD68+ cells and mast cells. The samples were distributed in groups according to the number of viable fungi per mm². Our results showed weak immunolabeling for iNOS in the multinucleated giant cells (MNGC) and in most of the mononuclear (MN) cells, and the proportion of iNOS+ MN/MNGC cells in the OP were comparable to Control (clinically healthy oral tissues). Additionally, our analysis revealed a similarity in the number of CD4+ cells between the Control and the OP groups with higher numbers of fungi. These findings suggest that a low expression of iNOS and a decrease in the CD4+ T cells in OP may represent possible mechanisms that permit the local fungal multiplication and maintenance of active oral lesions.

Slowed atrial conduction may contribute to reentry circuits and vulnerability for atrial fibrillation (AF). The autonomic nervous system (ANS) has modulating effects on electrophysiological properties. However, complex interactions of the ANS with the arrhythmogenic substrate make it difficult to understand the mechanisms underlying induction and maintenance of AF. AIM: To determine the effect of acute ANS modulation in atrial activation times in patients (P) with paroxysmal AF (PAF). METHODS AND RESULTS: 16P (9 men; 59±14years) with PAF, who underwent electrophysiological study before AF ablation, and 15P (7 men; 58±11years) with atrioventricular nodal reentry tachycardia, without documentation or induction of AF (control group). Each group included 7P with arterial hypertension but without underlying structural heart disease. The study was performed while off drugs. Multipolar catheters were placed at the high right atrium (HRA), right atrial appendage (RAA), coronary sinus (CS) and His bundle area (His). At baseline and with HRA pacing (600ms, shortest propagated S2) we measured: i) intra-atrial conduction time (IACT, between RAA and atrial deflection in the distal His), ii) inter-atrial conduction time (interACT, between RAA and distal CS), iii) left atrial activation time (LAAT, between atrial deflection in the distal His and distal CS), iv) bipolar electrogram duration at four atrial sites (RAA, His, proximal and distal CS). In the PAF group, measurements were also determined during handgrip and carotid sinus massage (CSM), and after pharmacological blockade of the ANS (ANSB). AF was induced by HRA programmed stimulation in 56% (self-limited - 6; sustained - 3), 68.8% (self-limited - 6; sustained - 5), and 50% (self-limited - 5; sustained - 3) of the P, in basal, during ANS maneuvers, and after ANSB, respectively (p=NS). IACT, interACT and LAAT significantly lengthened during HRA pacing in both groups (600ms, S2). P with PAF have longer IACT (p<0.05), a higher increase in both IACT, interACT (p<0.01) and electrograms duration (p<0.05) with S2, and more fragmented activity, compared with the control group. Atrial conduction times and electrograms duration were not significantly changed during ANS stimulation. Nevertheless, ANS maneuvers increased heterogeneity of the local electrograms duration. Also, P with sustained AF showed longer interACT and LAAT during CSM. CONCLUSION: Atrial conduction times, electrograms duration and fractionated activity are increased in PAF, suggesting a role for conduction delays in the arrhythmogenic substrate. Acute vagal stimulation is associated with prolonged interACT and LAAT in P with inducible sustained AF and ANS modulation may influence the heterogeneity of atrial electrograms duration.

Smart Grids (SGs) have emerged as the new paradigm for power system operation and management, being designed to include large amounts of distributed energy resources. This new paradigm requires new Energy Resource Management (ERM) methodologies considering different operation strategies and the existence of new management players such as several types of aggregators. This paper proposes a methodology to facilitate the coalition between distributed generation units originating Virtual Power Players (VPP) considering a game theory approach. The proposed approach consists in the analysis of the classifications that were attributed by each VPP to the distributed generation units, as well as in the analysis of the previous established contracts by each player. The proposed classification model is based in fourteen parameters including technical, economical and behavioural ones. Depending of the VPP strategies, size and goals, each parameter has different importance. VPP can also manage other type of energy resources, like storage units, electric vehicles, demand response programs or even parts of the MV and LV distribution network. A case study with twelve VPPs with different characteristics and one hundred and fifty real distributed generation units is included in the paper.

O setor da reabilitação em Portugal tem vindo a crescer consideravelmente ao longo dos últimos anos, demonstrando uma consciencialização da sociedade e das entidades competentes para definição de estratégias e planeamento, para uma manutenção das estruturas atempada de modo a prevenir patologias que provocam a sua degradação. O presente trabalho pretende fazer uma síntese das técnicas de proteção e reparação das estruturas de betão armado, efetuando também ensaios para determinação das causas das anomalias que levaram à sua reparação. O projeto de reabilitação deve seguir as recomendações indicadas nos Eurocódigos e norma EN1504 sobre produtos e sistemas para a proteção e reparação das estruturas de betão. Segundo esta norma explicam – se as principais causas de deterioração das estruturas de betão armado e os sintomas que estão na base dessa deterioração. Desta forma são descritos os métodos de intervenção, explicando o seu funcionamento e modo mais adequado de utilização com indicação dos produtos adequados para cada intervenção. Para aplicação do referido nos parágrafos anteriores desenvolveu-se uma proposta de fichas de inspeção, através de levantamento fotográfico e registo das anomalias e suas causas e ensaios não destrutivos de modo a perceber o estado atual da estrutura para uma melhor compreensão da metodologia de reparação a seguir

Os Sistemas de Gestão Técnica Centralizada (SGTC) assumem-se como essenciais nos grandes edifícios, já que permitem monitorizar, controlar, comandar e gerir, de forma facilitada, integrada e otimizada, as várias instalações existentes no edifício. O estado da arte de um SGTC baseia-se numa arquitetura distribuída, com recurso a Quadros de Gestão Técnica (QGT) que incluem Automation Servers - equipamentos nativos nos protocolos de comunicação mais comummente utilizados neste âmbito, incorporadores de funcionalidades e programações pré-definidas, e que ficarão responsáveis por integrar na sua área de influência, um conjunto de pontos de SGTC, definidos em projeto. Numa nova filosofia de instalação, integração e comunicação facilitada entre dispositivos que nos quadros elétricos geram dados relevantes para o utilizador e desencadeiam ações úteis na gestão de uma instalação, surge o novo conceito no mercado de Smart Panels, da Schneider Electric. Este sistema baseia-se numa ampla e diversa gama de possibilidades de medição e monitorização energética e da própria aparelhagem, com um sistema de comunicação com o sistema de gestão e controlo da instalação integrado no próprio quadro, dispensando assim a necessidade de um sistema externo (QGT), de recolha, comunicação e processamento de informação. Após o estudo descritivo teórico dos vários tópicos, questões e considerações relacionadas com os SGTC, os Smart Panels e a sua integração, o projeto e estudo comparativo do SGTC sem e com a integração de Smart Panels num grande centro comercial, permitiu concluir que a integração de Smart Panels num SGTC pode conferir vantagens no que diz respeito à implificação do projeto, da instalação, do comissionamento, programação, e da própria exploração da instalação elétrica, traduzindo-se numa redução dos custos normalmente elevados inerentes à mão de obra associada a todos estes processos.

Aim: The objective was to describe an outbreak of bloodstream infections by Burkholderia cepacia complex (Bcc) in bone marrow transplant and hematology outpatients.Methods: On February 15, 2008 a Bcc outbreak was suspected. 24 cases were identified. Demographic and clinical data were evaluated. Environment and healthcare workers' (HCW) hands were cultured. Species were determined and typed. Reinforcement of hand hygiene, central venous catheter (CVC) care, infusion therapy, and maintenance of laminar flow cabinet were undertaken. 16 different HCWs had cared for the CVCs. Multi-dose heparin and saline were prepared on counter common to both units.Findings: 14 patients had B. multivorans(one patient had also B. cenopacia), six non-multivorans Bcc and one did not belong to Bcc. Clone A B. multivorans occurred in 12 patients (from Hematology); in 10 their CVC had been used on February 11/12. Environmental and HCW cultures were negative. All patients were treated with meropenem, and ceftazidime lock-therapy. Eight patients (30%) were hospitalized. No deaths occurred. After control measures (multidose vial for single patient; CVC lock with ceftazidime; cleaning of laminar flow cabinet; hand hygiene improvement; use of cabinet to store prepared medication), no new cases occurred.Conclusions: This polyclonal outbreak may be explained by a common source containing multiple species of Bcc, maybe the laminar flow cabinet common to both units. There may have been contamination by B. multivorans (clone A) of multi-dose vials.

The reactivation of Chagas disease in HIV infected patients presents high mortality and morbidity. We present the case of a female patient with confirmed Chagasic meningoencephalitis as AIDS-defining illness. Interestingly, her TCD4+ lymphocyte cell count was 318 cells/mm3. After two months of induction therapy, one year of maintenance with benznidazol, and early introduction of highly active antiretroviral therapy (HAART), the patient had good clinical, parasitological and radiological evolution. We used a qualitative polymerase chain reaction for the monitoring of T. cruzi parasitemia during and after the treatment. We emphasize the potential value of molecular techniques along with clinical and radiological parameters in the follow-up of patients with Chagas disease and HIV infection. Early introduction of HAART, prolonged induction and maintenance of antiparasitic therapy, and its discontinuation are feasible, in the current management of reactivation of Chagas disease.

RESUMO - Enquadramento: Com o aumento da Esperança Média de Vida, e o consequente envelhecimento generalizado da população portuguesa, envelhecer com boa saúde e com elevada qualidade de vida, onde as pessoas com mais de 65 anos tenham a possibilidade de expressar todas as suas potencialidades e manterem um papel ativo na sua vida e na sociedade, é vincadamente um dos maiores desafios da sociedade contemporânea. Desta forma, foi imposto à sociedade e aos sistemas de saúde, a criação de novas estratégias, que promovessem a reabilitação, a autonomia e a qualidade de vida dos idosos. Nesta linha, surgiu em Portugal a criação da Rede Nacional de Cuidados Continuados Integrados, combinando os cuidados de saúde com o apoio social adequado a esta população. Objetivos: Num primeiro objetivo, pretende-se avaliar os diferentes domínios da autoperceção de qualidade de vida de indivíduos com idade igual ou superior a 65 anos, que se encontrem em período de internamento nas diferentes tipologias de Unidades de internamento da RNCCI (Unidade de Convalescença, Unidade de Média Duração e Reabilitação e Unidade de Longa Duração e Manutenção), bem como o seu grau de (in) dependência, de risco de falha de auto-cuidado, e de risco de quedas. Como segundo objetivo propomo-nos a avaliar o grau de satisfação desses mesmos utentes, relativamente à equipa de profissionais de saúde e aos aspetos organizacionais e serviços prestados pela Unidade da RNCCI, onde se encontram internados. Por último, pretendemos averiguar a existência ou inexistência de relação entre as demais variáveis em estudo com a tipologia de Unidade de internamento onde o utente se encontra a receber cuidados. Métodos: O presente estudo é caraterizado como um estudo quantitativo, de caráter exploratório, e de índole descritivo-correlacional, que visa descrever fenómenos e, posteriormente, identificar e explorar possíveis relações entre variáveis. O estudo centrou-se em indivíduos, com idade igual ou superior a 65 anos, internados em Unidades de Cuidados Continuados da Rede Nacional de Cuidados Continuados Integrados, tendo sido efetuados dois questionários distintos: um de satisfação, e um segundo instrumento de perceção da qualidade de vida, denominado de EasyCare, estando este já cientificamente validado a nível internacional e nacional. Foi obtida uma amostra de 35 utentes.
