The objective of this study was to identify vulnerability to tuberculosis (TB) related to knowledge about the disease among 76 nursing students and professionals. A quantitative descriptive study was conducted using a closed questionnaire for the collection of data regarding transmission, preventive and biosafety measures, diagnosis, and prejudice regarding the disease. The SAS software version 9.1.3 was used for data analysis, with the level of significance set at 5% (p < 0.05). Nursing students and professionals showed a vulnerability to TB related to knowledge about transmission, preventive and biosafety measures, and diagnosis of the disease. With respect to transmission, vulnerability was higher among nursing professionals. The results indicate the need for investment by healthcare institutions surrounding this topic in view of the important role of nursing in the establishment of strategies for prevention and control of the disease.

Structural definition of the receptors for neurotropic and angiogenic modulators such as fibroblast growth factors and related polypeptides will yield insight into the mechanisms that control early development, embryogenesis, organogenesis, wound repair and neovessel formation. We isolated 3 murine cDNAs encoding different binding domains of these receptors (flg). Comparison of these ectoplasmic portions showed that two of the forms corresponded to previously described murine molecules whereas the third one had a different ectoplasmic portion generated by specific changes in two regions. Interestingly, expression of this third form seems to be restricted in its tissue distribution. Such modifications could influence the ligand specificity of the different receptors and/or their binding affinity.

Els queviures són petits comerços d’alimentació on el tractament personal amb els clients es la clau de l’èxit ja que això provoca una conscienciació per part d’aquests i així es mantenen fidels a aquest establiment. En canvi, el supermercats tracten de explotar al màxim la seva reducció dels costos sense tenir tant en compte el tracte amb el client i donant-li més importància al preu de producte. Com podem observar, són dos tipus de comerços molt diferenciats tot i que al cap i a la fi, són competidors directes.La idea d’aquest treball es veure com afecta la crisi actual en aquests petits comerços i comparar-los amb el grans supermercats per arribar a una sèrie de conclusions.Hem escollit aquest tema ja que ens ha semblat interessant tractar de demostrar com el queviures tradicionals segueixen sent un negoci de profit, tot i la forta expansió que han patit les grans superfícies comercials. Un exemple d’aquest fort creixement podrien ser les dades que ens ha facilitat l’Ajuntament de Barcelona on es reflexa que el consum d’aliments es solen donar en primer lloc al supermercat (47,3%), seguit pel mercat municipal (29,3%) i en tercer lloc en la botiga de barri (16,2%).Un dels grans problemes amb que es troben els queviures és el canvi en el ritme de vida de la societat actual, sempre amb presses i estressada, que busca més, la comoditat i l’estalvi, tant de temps com de diners, a l’hora d’anar a comprar. Peraquest motiu, hem volgut investigar sobre quin es el factor que facilita la supervivència dels queviures.Degut a que el tema dels comerços estrangers va ser un tema que va provocar molta controvèrsia a la presentació del nostre treball, hem cregut convenient estudiar-lo una mica més a fons, únicament a Poble Sec. Per fer-ho hem estudiat el punt de vista que tenen els comerços estrangers sobre els comerços nacionals i també la situació actual del nombre de queviures regentats per uns i altres a través de informació extreta de la associació de comerciants de Poble Sec.Per crear el nostre treball hem realitzar diferents enquestes i entrevistes, tant a consumidors com a propietaris, per poder extreure informació amb la qual poder treballar. En total hem realitzar 100 enquestes a clients, una enquesta a cadapropietari i dues entrevistes: una a un propietari estranger i una altra a Pedro de Diego ( fundador de la associació de comerciants del Poble Sec i primer tresorer del barri)Cal destacar que ens hem trobat amb moltes dificultats per trobar o aconseguir dades econòmiques i financeres sobre aquests establiments i per això hem realitzat una aproximació a la proporció de beneficis sobre ingressos per veure quina proporció dels ingressos es poden considerar beneficis.També hem recopilat diverses notícies de diaris que tracten sobre temes relacionats amb la crisi del petit comerç i sobre l’augment en el nombre de nous queviures regentats per gent estrangera, i a partir d’aquestes hem realitzat una breu crítica sobre la situació actual.

Aquest treball tracta, des d’una perspectiva general, sobre quin impacte té el virus del VIH sobre el creixement econòmic dels països africans. Per a veure com evoluciona una economia, s’ha pres com a mesura el PIB per càpita, ja que es tracta d’una mesura que no té en compte el tamany dels estats, sinó que permet veure de manera més directa com pateix la malaltia la població.Partint d’una hipòtesi en forma de pregunta, i després d’una introducció a la malaltia i la seva situació en l’actualitat, se separa l’anàlisi dels efectes microeconòmics de la dels macroeconòmics, per acabar enllaçant-les i il·lustrant-les amb una senzilla explicació seguint el model de creixement de Solow. La conclusió a la qual s’arriba és que, a llarg termini, la SIDA comporta unes conseqüències clarament negatives sobre l’economia de l’Àfrica i que, a més a més, requereixen d’una resposta urgent a nivell global per evitar-les.

Un dels principals motius que ens va impulsar en l’elecció del tema és que es tracta d’untema que pot despertar curiositat entre la població.Un altre motiu, es que varem trobar que està íntimament relacionat amb els estudis queestem cursant, donat que afecta als pressupostos de l’estat i a la seva restricciópressupostària, i per tant, està directament relacionat amb la macroeconomia. En el nostrecas, reduirem l’àmbit d’estudi al territori català, de manera que estudiarem aquestes duesmalalties dins la despesa en sanitat pública catalana. A demés, estan finançades amb elsnostres impostos, i per tant la seva despesa afecta a la restricció pressupostària delsciutadans.L’elecció d’aquestes malalties no ha estat feta a l’atzar. Inicialment, varem pensar enestudiar els costos dels interns penitenciaris que patien aquestes malalties. Com que laSIDA i d’hepatitis C són les malalties més freqüents dins la presó, i les que tenen unscostos més característics donada la complexitat dels seus tractaments, varem pensar queserien prou representatives.No obstant, a mesura que ens anàvem endinsant en el tema, ens varem adonar que tambéseria molt interessant comparar el cost de les malalties amb el de les persones no recluses, iesbrinar si hi havia algun tipus de cost diferencial. És per això que varem decidir analitzaraquestes dues malalties tant dins com fora.Un altre factor que ens ha impulsat en l’elecció del tema és el fet que el nombre d’interns ales presons té un ritme de creixement constant que s’ha accelerat en els últims anys,sobretot degut a l’augment de la immigració. Això implica un augment progressiu de ladespesa, que es tradueix en una necessitat d’ingressos majors per tal de poder equilibrar larestricció de la qual parlàvem abans.També varem voler anar una mica més lluny i analitzar el pes d’aquestes malalties dins dela despesa que la generalitat ha establert per a la sanitat pública. Com les dues son MDO (malalties de declaració obligatòria ) estan finançades completament pel sector públic.L’objectiu era veure si representaven un cost tant elevat com pensàvem.OBJECTIUS DEL TREBALL:· Demostrar l’elevat cost que suposen certes malalties per l’estat.· Manifestar els canvis en el cost de les malalties amb l’evolució delstractaments.· Analitzar els costos sanitaris extres que es produeixen a les presons.· Destacar l’augment accelerat del nombre d’interns i l’augment del cost sanitarique això suposa. METODOLOGIA: Per tal de poder realitzar l’estudi comparatiu, hem hagut de calcular manualment els costosde les malalties, tot informant-nos del preu dels medicament, les dosis, el cost de lesconsultes externes,etc. A més, per a calcular el cost del tractament dins la presó, ens hemhagut d’informar dels aspectes més generals que envolten a un pres, per poder veure sirealment existeix un cost diferencial respecte la malaltia a l’exterior. Per obtenir aquestesdiverses informacions, ens hem hagut de posar en contacte amb el personal que treballa ala presó que hem pres com a model d’estudi.Així, podem dividir les nostres fonts d’informació en 3 categories:• Obtenció d’informació directament amb el personal de la presó:– Entrevista amb la directora d’infermeria de la Secretaria de ServeisPenitenciaris, Rehabilitació i Justícia Juvenil– Entrevista amb la Cap d’infermeria del Centre Quatre Camins.• Informació a partir de mostres facilitades pels propis funcionaris de la presó• Informació a partir d’estudis sobre el tema i de dades oficials, concretament lesdades oficials sobre els Pressupostos de la Generalitat.

Purpose. To investigate the role of the myocyte enhancer factor 2 (Mef2) transcription factor family in retinal diseases, Mef2c expression was assessed during retinal degeneration in the Rpe65(-/-) mouse model of Leber's congenital amaurosis (LCA). Mef2c-dependent expression of photoreceptor-specific genes was further addressed. Methods. Expression of Mef2 members was analyzed by oligonucleotide microarray, quantitative PCR (qPCR) and in situ hybridization. Mef2c-dependent transcriptional activity was assayed by luciferase assay in HEK293T cells. Results. Mef2c was the only Mef2 member markedly downregulated during retinal degeneration in Rpe65(-/-) mice. Mef2c mRNA level was decreased by more than 2 fold at 2 and 4 months and by 3.5 fold at 6 months in retinas of Rpe65(-/-) mice. Downregulation of Mef2c at the protein level was confirmed in Rpe65(-/-) retinas. The decrease in Mef2c mRNA levels in the developing Rpe65(-/-) retinas, from post-natal day (P)13 onward, was concomitant with the decreased expression of the rod-specific transcription factors Nrl and Nr2e3. Nrl was further shown to drive Mef2c transcriptional activity, supporting a physiological role for Mef2c in the retina. In addition, Mef2c appeared to act as a transcriptional repressor of its own expression, as well as those of the retina-specific retinal G-protein coupled receptor (Rgr), rhodopsin and M-opsin genes. Conclusions. These findings highlight the early altered regulation of the rod-specific transcriptional network in Rpe65-related disease. They further indicate that Mef2c may act as a novel transcription factor involved in the development and the maintenance of photoreceptor cells.

The cytokine macrophage migration inhibitory factor plays a central role in inflammation, cell proliferation and tumorigenesis. Moreover, macrophage migration inhibitory factor levels correlate with tumor aggressiveness and metastatic potential. Histone deacetylase inhibitors are potent antitumor agents recently introduced in the clinic. Therefore, we hypothesized that macrophage migration inhibitory factor would represent a target of histone deacetylase inhibitors. Confirming our hypothesis, we report that histone deacetylase inhibitors of various chemical classes strongly inhibited macrophage migration inhibitory factor expression in a broad range of cell lines, in primary cells and in vivo. Nuclear run on, transient transfection with macrophage migration inhibitory factor promoter reporter constructs and transduction with macrophage migration inhibitory factor expressing adenovirus demonstrated that trichostatin A (a prototypical histone deacetylase inhibitor) inhibited endogenous, but not episomal, MIF gene transcription. Interestingly, trichostatin A induced a local and specific deacetylation of macrophage migration inhibitory factor promoter-associated H3 and H4 histones which did not affect chromatin accessibility but was associated with an impaired recruitment of RNA polymerase II and Sp1 and CREB transcription factors required for basal MIF gene transcription. Altogether, this study describes a new molecular mechanism by which histone deacetylase inhibitors inhibit MIF gene expression, and suggests that macrophage migration inhibitory factor inhibition by histone deacetylase inhibitors may contribute to the antitumorigenic effects of histone deacetylase inhibitors.

The production of extracellular soluble proteins was studied in serum-free aggregating cell cultures of fetal rat telencephalon labeled on culture day 7 with a mixture of radioactive amino acid precursors. Cultures treated continuously with epidermal growth factor (EGF; 20 ng/ml) showed a generally increased protein secretion and a particularly enhanced production of a few distinct extracellular proteins. The time lag of this response after an initial dose of EGF (25 ng/ml) on day 7 was 48 h. The total macromolecular radioactivity that accumulated within 96 h of labeling in the media of EGF-treated cultures was 175% of untreated controls, whereas no difference was found in the proportions of intracellular amino acid incorporation. Cultures which received a single dose of EGF (25 ng/ml) on day 1 showed still a greatly increased protein secretion on day 7. Prevention of extracellular protein accumulation by reducing the initial cell number and increasing the rate of media changes did not affect the EGF-induced stimulation of the two glial enzymes, glutamine synthetase and 2',3'-cyclic nucleotide 3'-phosphohydrolase. The results suggest that both the increased production of extracellular proteins and the enhanced development of glial enzymatic activities reflect the stimulated phenotypic expression of EGF-sensitive brain cells.

In gram-negative bacteria, the outer membrane lipopolysaccharide is the main component triggering cytokine release from peripheral blood mononuclear cells (PBMCs). In gram-positive bacteria, purified walls also induce cytokine release, but stimulation requires 100 times more material. Gram-positive walls are complex megamolecules reassembling distinct structures. Only some of them might be inflammatory, whereas others are not. Teichoic acids (TA) are an important portion (> or =50%) of gram-positive walls. TA directly interact with C3b of complement and the cellular receptor for platelet-activating factor. However, their contribution to wall-induced cytokine-release by PBMCs has not been studied in much detail. In contrast, their membrane-bound lipoteichoic acids (LTA) counterparts were shown to trigger inflammation and synergize with peptidoglycan (PGN) for releasing nitric oxide (NO). This raised the question as to whether TA are also inflammatory. We determined the release of tumor necrosis factor (TNF) by PBMCs exposed to a variety of TA-rich and TA-free wall fragments from Streptococcus pneumoniae and Staphylococcus aureus. TA-rich walls from both organisms induced measurable TNF release at concentrations of 1 microg/ml. Removal of wall-attached TA did not alter this activity. Moreover, purified pneumococcal and staphylococcal TA did not trigger TNF release at concentrations as high as > or =100 microg/ml. In contrast, purified LTA triggered TNF release at 1 microg/ml. PGN-stem peptide oligomers lacking TA or amino-sugars were highly active and triggered TNF release at concentrations as low as 0.01 microg/ml (P. A. Majcherczyk, H. Langen, et al., J. Biol. Chem. 274:12537-12543,1999). Thus, although TA is an important part of gram-positive walls, it did not participate to the TNF-releasing activity of PGN.

Necrotizing enterocolitis (NEC) is a severe neonatal disease affecting particularly preterm infants. Its exact pathogenesis still remains unknown. In this study, we have compared the prevalence of vascular obstructive lesions in placentae of premature newborns which developed NEC and of a control group. We further compared separately the findings of placentae of infants of less than 30 weeks of gestation, the age group in which NEC occurs most frequently. We found signs of fetal vascular obstructive lesions in 65% of the placentae of preterm patients developing NEC, compared to only 17% of the placentae of preterm patients in the control group. In the age groups below 30 weeks of gestation, 58.5% of placentae of later NEC patients presented such lesions compared to 24.5% in the control group. The significant difference between NEC and control group suggests a strong association between fetal vascular obstructive lesions and NEC. Therefore, we propose that fetal vascular obstructive lesions might be considered as a risk factor for the development of NEC in premature infants.

As part of an ongoing effort to improve the technique of immunoscintigraphy for the detection of human carcinomas with radiolabeled monoclonal antibodies (MABs) to carcinoembryonic antigen (CEA), we have developed a series of MABs to CEA and have studied the effects of low- and physiological molarity buffers on their CEA binding and affinity, as well as their cross-reactivity with granulocyte glycoprotein(s). These in vitro results in different buffer systems were then correlated with the use of these MABs to CEA in the detection of human colon carcinoma grafts in nude mice. Our results show that the binding of CEA by some MABs is influenced by ionic strength and that this may be an important factor in their successful use for the immunolocalization of carcinomas in vivo.
Enhanced visuospatial memory following intracerebroventricular administration of nerve growth factor

The present work assessed the effects of intracerebroventricular injections of rh recombined human nerve growth factor (rh NGF) (5 micrograms/2.5 microl) at postnatal days 12 and 13 upon the development of spatial learning capacities. The treated rats were trained at the age of 22 days to escape onto an invisible platform at a fixed position in space in a Morris navigation task. For half of the subjects, the training position was also cued, a procedure aimed at facilitating escape and at reducing attention to the distant spatial cues. Later, at the age of 6 months, all the rats were trained in a radial-arm maze task. Treatment effects were found in both immature and adult rats. The injection of NGF improved the performance in the Morris navigation task in both training conditions. There was a significant reduction in the escape latency and an increased bias toward the training platform quadrant during probe trials. The most consistent effect was the precocious development of an adult-like spatial memory. In the radial-arm maze, the NGF-treated rats made significantly fewer reentries than vehicle rats and this effect was particularly marked in the treated female rats. Taken together, these experiments reveal that the development and the maintenance of an accurate spatial representation are tightly related to the development of brain structures facilitated by the action of NGF. Moreover, these experiments demonstrate that an acute pharmacological treatment that leads to a transient modification in the choline acetyltransferase activity can induce a behavioral change long after the treatment.

Summary The proinflammatory cytokine macrophage migration inhibitory factor (MIF) has emerged as a central mediator of inflammation and innate immune defense against infections. MIF has been shown to play an important role in the pathogenesis of infectious diseases like sepsis, tuberculosis and autoimmune inflammatory diseases, such as arthritis, inflammatory bowel disease and asthma. Two functional polymorphisms of the MIF gene promoter, a five to eight CATT repeat microsatellite at position -794 and a G/C SNP at position -173, have been associated with increased susceptibility to or severity of autoimmune inflammatory diseases like arthritis, colitis and atopy. The aim of this thesis was to define whether, and if so by which mechanisms, MIF gene polymorphisms influence the susceptibility to or the outcome of one of the most severe and one of the most prevalent infectious diseases: meningococcal sepsis and tuberculosis, respectively. The results of the comparison between 1106 patients suffering from severe meningococcal infections and 434 healthy volunteers showed that carriers of the CATT5-5 genotype were protected from meningococcemia. A transmission disequilibrium test involving 106 families confirmed this association. At baseline and after stimulation with Neisseria meningitidis, the CATT5 MIF promoter drove lower transcriptional activity than the CATT6 or CATT7 alleles in human monocytic cells and whole blood of CATT5-5 healthy individuals tended to produce less MIF than whole blood of CATT6-6 individuals. Beyond, we describe several new MIF gene polymorphisms in Africans. Genotyping the CATT microsatellite and the -173*G/C SNP revealed great genetic diversity in six African ethnic groups. Comparing 471 African tuberculosis cases and 932 matched healthy controls, we observed ethnicity dependent associations of the -173*G/C and the CATT5-8 with susceptibility to or severity of tuberculosis, but confirmation in larger cohorts ìs needed. In conclusion, we report that homozygous carriage of a low expression allele of the MIF gene protects from meningococcal disease. These results support the concept that analyses of MIF genotypes in patients with sepsis may help to classify patients into risk categories and to identify those patients who may benefit from anti-MIF therapeutic strategies.

Fibroblast growth factor 23 (FGF23) is a circulating factor secreted by osteocytes that is essential for phosphate homeostasis. In kidney proximal tubular cells FGF23 inhibits phosphate reabsorption and leads to decreased synthesis and enhanced catabolism of 1,25-dihydroxyvitamin D3 (1,25[OH]2 D3 ). Excess levels of FGF23 cause renal phosphate wasting and suppression of circulating 1,25(OH)2 D3 levels and are associated with several hereditary hypophosphatemic disorders with skeletal abnormalities, including X-linked hypophosphatemic rickets (XLH) and autosomal recessive hypophosphatemic rickets (ARHR). Currently, therapeutic approaches to these diseases are limited to treatment with activated vitamin D analogues and phosphate supplementation, often merely resulting in partial correction of the skeletal aberrations. In this study, we evaluate the use of FGFR inhibitors for the treatment of FGF23-mediated hypophosphatemic disorders using NVP-BGJ398, a novel selective, pan-specific FGFR inhibitor currently in Phase I clinical trials for cancer therapy. In two different hypophosphatemic mouse models, Hyp and Dmp1-null mice, resembling the human diseases XLH and ARHR, we find that pharmacological inhibition of FGFRs efficiently abrogates aberrant FGF23 signaling and normalizes the hypophosphatemic and hypocalcemic conditions of these mice. Correspondingly, long-term FGFR inhibition in Hyp mice leads to enhanced bone growth, increased mineralization, and reorganization of the disturbed growth plate structure. We therefore propose NVP-BGJ398 treatment as a novel approach for the therapy of FGF23-mediated hypophosphatemic diseases.
