Four years after the first visit seventeen public health units were visited again and evaluated as to standards of storage recommended by the Brazilian Immunization Programme. In 100% of the units, refrigerators and proper inside location of vaccines in the refrigerator were adequatety or regularfy maintained and checked, respectively. However, when control of temperature was checked, onfy 64.7% presented adequate storage conditions. In 94.1 % of the units, health workers complained of lack of immediate technical support in emergency situations. In 55.2 % the titers vaccine samples of were under the minimal recommended potency. It is necessary that the factors concerning the cold chain be continualfy evaluated so that the quality of the vaccines that will be used is not affected.

RESUMO - Introdução: A despesa em saúde aumentou consideravelmente nas últimas décadas na maioria dos países industrializados. Por outro lado, os indicadores de saúde melhoraram. A evidência empírica sobre a relação entre as despesas em saúde e a saúde das populações tem sido inconclusiva. Este estudo aborda a relação entre as despesas em saúde e a saúde das populações através de dados agregados para 34 países para o período 1980-2010. Metodologia: Utilizou-se o coeficiente de correlação de Pearson para avaliar a correlação entre as variáveis explicativas e os indicadores de saúde. Procedeuse ainda à realização de uma regressão multivariada com dados em painel para cada indicador de saúde utilizado como variável dependente: esperança de vida à nascença e aos 65 anos para mulheres e homens, anos de vida potencialmente perdidos para mulheres e homens e mortalidade infantil. A principal variável explicativa utilizada foi a despesa em saúde, mas consideraram-se também vários fatores de confundimento, nomeadamente a riqueza, fatores estilo de vida, e oferta de cuidados. Resultados: A despesa per capita tem impacto nos indicadores de saúde mas ao adicionarmos a variável PIB per capita deixa de ser estatisticamente significativa. Outros fatores têm um impacto significativo para quase todos os indicadores de saúde utilizados: consumo de álcool e tabaco, gordura, o número de médicos e a imunização, confirmando vários resultados da literatura. Conclusão: Os resultados vão ao encontro de alguns estudos que afirmam o impacto marginal das despesas em saúde e do progresso da medicina nos resultados em saúde desde os anos 80 nos países industrializados.

The Fucose-Mannose Ligand (FML) of Leishmania donovani is a complex glycoproteic fraction. Its potential use as a tool for diagnosis of human visceral leishmaniasis was tested with human sera from Natal, Rio Grande do Norte, Brazil. The FML-ELISA test, showed 100% sensitivity and 96% specificity, identifying patients with overt kala-azar (p < 0.001, when compared to normal sera), and subjects with subclinical infection. More than 20% apparently healthy subjects with positive reaction to FML developed overt kala-azar during the following 10 months. In the screening of human blood donnors, a prevalence of 5% of sororeactive subjects was detected, attaining 17% in a single day. The GP36 glycoprotein of FHL is specifically reconized by human kala-azar sera. The immunoprotective effect of FML on experimental L. donovanii infection was tested in swiss albino mice. The protection scheemes included three weekly doses of FML, supplemented or not with saponin by the subcutaneous or intraperitoneal routes and challenge with 2x 10(7) amastigotes of Leishmania donovani. An enhancement of 80.0 % in antibody response (p<0.001) and reduction of 85.5 % parasite liver burden (p<0.001) was detected in animals immunized with FML saponin, unrespectivety of the immunization route.

RESUMO - O cancro do colo útero representa um importante problema de saúde pública em Portugal: é o terceiro cancro mais frequente nas mulheres entre os 15 e os 44 anos, originando a morte de 346 mulheres anualmente. Contudo, esta patologia é altamente evitável, nomeadamente, através da imunização contra a infeção HPV, que é a causa necessária para o desenvolvimento do cancro. A elevada prevalência da infeção em mulheres mais velhas sugere que a vacinação poderá ser uma estratégia custo-efetiva mesmo numa faixa etária superior. Para que seja racionalmente ponderada a comparticipação da vacina nestas mulheres é necessária a realização de um estudo fármaco-económico que comprove o custo-efetividade desta intervenção, já que o seu financiamento atual prevê apenas as mulheres não abrangidas pelo Programa Nacional de Vacinação, dos 18 aos 25 anos. Os objetivos do trabalho são realizar uma revisão da literatura sobre estudos de avaliação económica relativos à prevenção do CCU e avaliar a relação de custo-efetividade de vacinar mulheres contra o HPV entre os 26 e os 55 anos em comparação com a prática clínica corrente, em Portugal. É utilizado o Modelo Global Cervarix® e realiza-se uma análise de custo-utilidade e de custo-efetividade. Os resultados demonstraram que a vacinação em mulheres dos 26 aos 45 anos poderá ser uma opção custo-efetiva, permitindo um aumento de anos de vida, uma diminuição dos casos e mortes por CCU e um incremento de QALYs. O RCEI variou entre 7.914€/QALY e 29.049€/QALY com a vacinação aos 26 e aos 45 anos, respetivamente, para a alternativa de vacinação mais rastreio versus a situação atual de rastreio organizado e oportunístico, em Portugal.

Humoral and cellular immune responses were evaluated in 44 C57BL/6 mice immunized with the Trypanosoma cruzi recombinant antigens CRA and FRA. Both antigens induced cutaneous immediate-type hypersensitivity response. The levels of IgG1, IgG2a, IgG2b and IgG3 were high in CRA immunized mice. IgG3 was the predominant isotype. Although no difference in antibody levels was observed in FRA-immunized mice when compared to control mice, both antigens were able to induce lymphoproliferation in immunized mice. Significant differences were observed between incorporation of [³H]- thymidine by spleen cell stimulated in vitro with CRA or FRA and the control group. These results suggest that CRA and FRA could be involved in mechanisms of resistance to Trypanosoma cruzi infection.

The Graphics Processing Unit (GPU) is present in almost every modern day personal computer. Despite its specific purpose design, they have been increasingly used for general computations with very good results. Hence, there is a growing effort from the community to seamlessly integrate this kind of devices in everyday computing. However, to fully exploit the potential of a system comprising GPUs and CPUs, these devices should be presented to the programmer as a single platform. The efficient combination of the power of CPU and GPU devices is highly dependent on each device’s characteristics, resulting in platform specific applications that cannot be ported to different systems. Also, the most efficient work balance among devices is highly dependable on the computations to be performed and respective data sizes. In this work, we propose a solution for heterogeneous environments based on the abstraction level provided by algorithmic skeletons. Our goal is to take full advantage of the power of all CPU and GPU devices present in a system, without the need for different kernel implementations nor explicit work-distribution.To that end, we extended Marrow, an algorithmic skeleton framework for multi-GPUs, to support CPU computations and efficiently balance the work-load between devices. Our approach is based on an offline training execution that identifies the ideal work balance and platform configurations for a given application and input data size. The evaluation of this work shows that the combination of CPU and GPU devices can significantly boost the performance of our benchmarks in the tested environments, when compared to GPU-only executions.
