OBJECTIVE: To compare the outcome of balloon PTCA with final coronary stenosis diameter (SD) <=30%, with elective coronary stenting. METHODS: We performed a comparative analysis of the 6 month outcomes in patients treated with primary stenting and those who obtained an optimal balloon PTCA result treated during the first 12 hours of AMI onset included in the STENT PAMI randomized trial. RESULTS: The results were analysed into 3 groups: primary stenting (441 patients, SD=22±6%), optimal PTCA (245 patients), and nonoptimal PTCA (182 patients, SD= 37±5%). At the end of the 6 months primary stent group presented with the lowest restenosis(23 vs. 31 vs. 45%, p=0.001, respectively). Ischemia-driven target vessel revascularization rate (TVR) (7 vs. 15.5 vs. 19%, p=0.001, respectively). CONCLUSION: At the 6 month follow-up, primary stenting offered the lowest restenosis and ischemia-driven TVR rates. Compared to optimal balloon PTCA. Nonoptimal primary balloon PTCA pts (SD=31-50%), had the worst late angiographic outcomes and should be treated more actively with coronary stent implantation.

OBJECTIVE: We conducted a comparative analysis of the in-hospital outcomes of patients who underwent primary percutaneous transluminal angioplasty (PTCA) or stent implantation because of an acute myocardial infarction (AMI) related to an acute vein graft occlusion. METHODS: Since 1991 the Brazilian Society of Hemodynamic and Interventional Cardiology has maintained a large database (CENIC). From these, we selected all consecutive patients, who underwent primary PTCA or stenting in the first 24 hours of AMI, with the target vessel being an occluded vein graft. Immediate results and major coronary events occurring up until hospital discharge were analyzed. RESULTS: During this period, 5,932 patients underwent primary PTCA or stenting; 158 (3%) of the procedures were performed because of an acute vein graft occlusion. Stenting was performed in 74 (47%) patients. Patients treated with stents had a higher success rate and lower mean residual stenosis compared with those who underwent primary balloon PTCA. The incidence of reinfarction and death were similar for stenting and balloon PTCA. CONCLUSION: Primary percutaneous treatment of AMI related to acute vein graft occlusion is still an uncommon practice. Primary stenting improved luminal diameter and offered higher rates of success; however, this strategy did not reduce the in-hospital reinfarction and death rate, compared with that occurring with PTCA treatment.

OBJECTIVE: To demonstrate the feasibility and safety of simultaneous catheterization and mapping of the 4 pulmonary veins for ablation of atrial fibrillation. METHODS: Ten patients, 8 with paroxysmal atrial fibrillation and 2 with persistent atrial fibrillation, refractory to at least 2 antiarrhythmic drugs and without structural cardiopathy, were consecutively studied. Through the transseptal insertion of 2 long sheaths, 4 pulmonary veins were simultaneously catheterized with octapolar microcatheters. After identification of arrhythmogenic foci radiofrequency was applied under angiographic or ultrasonographic control. RESULTS: During 17 procedures, 40 pulmonary veins were mapped, 16 of which had local ectopic activity, related or not with the triggering of atrial fibrillation paroxysms. At the end of each procedure, suppression of arrhythmias was obtained in 8 patients, and elimination of pulmonary vein potentials was accomplished in 4. During the clinical follow-up of 9.6±3 months, 7 patients remained in sinus rhythm, 5 of whom were using antiarrhythmic drugs that had previously been ineffective. None of the patients had pulmonary hypertension or evidence of stenosis in the pulmonary veins. CONCLUSION: Selective and simultaneous catheterization of the 4 pulmonary veins with microcatheters for simultaneous recording of their electrical activity is a feasible and safe procedure that may help ablation of atrial fibrillation.

OBJECTIVE - To report the results of percutaneous occlusion of persistent ductus arteriosus with the Amplatzer prosthesis in 2 Brazilian cardiological centers. METHODS - From May 1998 to July 2000, 33 patients with clinical and laboratory diagnosis of persistent ductus arteriosus underwent attempts at percutaneous implantation of the Amplatzer prosthesis. The median age was 36 months (from 6 months to 38 years), and the median weight was 14kg (from 6 to 92kg). Sixteen patients (48.5%) were under 2 years of age at the time of the procedure. All patients were followed up with periodical clinical and echocardiographic evaluations to assess the presence and degree of residual shunt and possible complications, such as pseudocoarctation of the aorta and left pulmonary artery stenosis. RESULTS - The minimum diameter of the arterial ducts ranged from 2.5 to 7.0mm (mean of 4.0±1.0, median of 3.9). The rate of success for implantation of the prosthesis was 100%. Femoral pulse was lost in 1 patient. The echocardiogram revealed total closure prior to hospital discharge in 30 patients, and in the follow-up visit 3 months later in the 3 remaining patients. The mean follow-up duration was 6.4±3.4 months. All patients were clinically well, asymptomatic, and did not need medication. No patient had narrowing of the left pulmonary artery or of the aorta. No early or late embolic events occurred, nor did infectious endarteritis. A new hospital admission was not required for any patient. CONCLUSION - The Amplatzer prosthesis for persistent ductus arteriosus is safe and highly effective for occlusion of ductus arteriosus of varied diameters, including large ones in small symptomatic infants.

A 59-year-old female patient with mitral valve prolapse and a previous history of lumbosacral spondyloarthrosis and lumbar disk hernia had an episode of infective endocarditis due to Streptococcus viridans, which evolved with peripheral embolism to the left kidney, spleen, and left iliac artery, and intraventricular cerebral hemorrhage. Her clinical manifestations were low back pain and hematuria, which were initially attributed to an osteoarticular condition. Infective endocarditis is a severe polymorphic disease with multiple clinical manifestations and it should always be included in the differential diagnosis by clinicians.

We report the case of a 7-year-old male child diagnosed with Williams-Beuren syndrome and arterial hypertension refractory to clinical treatment. The diagnosis was confirmed by genetic study. Narrowing of the descending aorta and stenosis of the renal arteries were also diagnosed. Systemic vascular alterations caused by deletion of the elastin gene may occur early in individuals with Williams-Beuren syndrome, leading to the clinical manifestation of systemic arterial hypertension refractory to drug treatment.

OBJECTIVE: To assess the occurrence of cardiac events in patients diagnosed with left main coronary artery disease on diagnostic cardiac catheterization and waiting for myocardial revascularization surgery. METHODS: All patients diagnosed with left main coronary artery disease (stenosis > or = 50%) consecutively identified on diagnostic cardiac catheterization during an 8-month period were selected for the study. The group comprised 56 patients (40 males and 16 females) with a mean age of 61±10 years. The cardiac events included death, nonfatal acute myocardial infarction, acute left ventricular failure, unstable angina, and emergency surgery. RESULTS: While waiting for surgery, patients experienced the following cardiac events: 7 acute myocardial infarctions and 1 death. All events occurred within the first 60 days after the diagnostic cardiac catheterization. More patients, whose indication for diagnostic cardiac catheterization was unstable angina, experienced events as compared with those with other indications [p=0.03, relative risk (RR) = 5.25, 95% confidence interval = 1.47 - 18.7]. In the multivariate analysis of logistic regression, unstable angina was also the only factor that independently contributed to a greater number of events (p = 0.02, OR = 8.43, 95% CI =1.37 - 51.7). CONCLUSION: Unstable angina in patients with left main coronary artery disease acts as a high risk factor for cardiac events, emergency surgery being recommended in these cases.

OBJECTIVE: To access the incidence of diagnostic errors in the initial evaluation of children with cardiac murmurs. METHODS: We evaluated our 7-years of experience in a public pediatric cardiology outpatient clinic. Of 3692 patients who were sent to the hospital, 2603 presented with a heart murmur and were investigated. Patients for whom a disagreement existed between the initial and final diagnoses were divided into the following 2 groups: G1 (n=17) with an initial diagnosis of an innocent murmur and a final diagnosis of cardiopathy, and G2 (n=161) with an initial diagnosis of cardiopathy and a final diagnosis of a normal heart. RESULTS: In G1, the great majority of patients had cardiac defects with mild hemodynamic repercussions, such as small ventricular septal defect and mild pulmonary stenosis. In G2, the great majority of structural defects were interventricular communication, atrial septal defect and pulmonary valve stenosis. CONCLUSION: A global analysis demonstrated that diagnostic error in the initial evaluation of children with cardiac murmurs is real, reaching approximately 6% of cases. The majority of these misdiagnoses were in patients with an initial diagnosis of cardiopathy, which was not confirmed through later complementary examinations. Clinical cardiovascular examination is an excellent resource in the evaluation of children suspected of having cardiopathy. Immediate outpatient discharge of children with an initial diagnosis of an innocent heart murmur seems to be a suitable approach.

OBJECTIVE: To assess the short- and long-term results of the use of streptokinase (SK) for the treatment of thromboses in cardiac valvular prostheses. METHODS: Seventeen patients with cardiac prosthetic thrombosis diagnosed by clinical, echocardiographic, and radioscopic findings underwent fibrinolytic treatment with a streptokinase bolus of 250,000 U followed by 100.000 U/hour. Short- and long-term results were assessed by radioscopy and echocardiography. RESULTS: Of the 17 patients, 12 had mechanical double-disk prostheses (4 aortic, 6 mitral, 2 tricuspid), 4 had single-disk prostheses (2 aortic, 1 mitral, and 1 tricuspid), and 1 had a tricuspid bioprosthesis. The success rate was 64.8%, the partial success rate was 17.6%, and the nonsuccess rate was 17.6%. All patients with a double-disk prosthesis responded, completely or partially, to the treatment. None of the patients with a single-disk prosthesis had complete resolution of the thrombosis. The time of streptokinase infusion ranged from 6 to 80 hours (mean of 56 h). The mortality rate due to the use of streptokinase was 5.8% and was secondary to cerebral bleeding. During streptokinase infusion, 3 (17.6%) embolic episodes occurred as follows: 1 cerebral, 1 peripheral, and 1 coronary. The rethrombosis index was 33% in a mean follow-up of 42 months. CONCLUSION: The use of fibrinolytic agents was effective and relatively safe in patients with primary thrombosis of a double-disk prosthesis. A fatal hemorrhagic complication occurred in 1 (5.8%) patient, and embolic complications occurred in 3 (17.6%) patients. In a mean 42-month follow-up, 67% of the patients were free from rethrombosis.

Paciente de 34 anos, com lúpus eritematoso sistêmico e síndrome de anticorpo antifosfolípide secundária, evoluiu com crises convulsivas, parcialmente controladas com anticonvulsivante, e ausculta de sopro cardíaco, cuja propedêutica mostrou a presença de vegetação em valva mitral. Diante do diagnóstico de endocardite de Libman-Sacks foi iniciada terapia com warfarina sódica, e após seis meses de anticoagulação oral, a paciente apresentava controle total das crises convulsivas e desaparecimento da vegetação valvar ao ecocardiograma. São discutidas a ocorrência da endocardite de Libman-Sacks no lúpus eritematoso sistêmico, sua associação com a síndrome de anticorpo antifosfolípide e a terapêutica anticoagulante, e feita uma revisão da literatura.

Quilopericárdio como complicação de cirurgia cardíaca é raro. Pode ser causado por lesão no ducto torácico, de tributárias do ducto ou pela trombose da confluência das veias jugular e subclávia esquerda, obstruindo a drenagem do ducto torácico. O tratamento pode ser conservador ou cirúrgico, dependendo da duração e do volume drenado. Apresentamos o caso de uma mulher de 24 anos que no pós-operatório tardio de troca da valva mitral (bioprótese) foi internada com quadro de tamponamento cardíaco devido à presença de quilopericárdio. São comentados os aspectos clínicos e o tratamento instituído.

OBJETIVO: Determinar os fatores preditores ecocardiográficos de morte em crianças com cardiomiopatia dilatada idiopática. MÉTODOS: Estudo retrospectivo de 148 crianças com cardiomiopatia dilatada idiopática diagnosticadas entre setembro/1979 a março/2003. Critérios para inclusão: insuficiência cardíaca e redução da contratilidade no ecocardiograma, na ausência de cardiopatia congênita ou secundária. Foram analisados 470 exames, durante 244,8 meses de evolução. Parâmetros analisados: dimensão do átrio esquerdo (DAE), relação átrio esquerdo/aorta (DAE/Ao), dimensão sistólica (DSVE) e diastólica (DDVE) do VE, massa do VE (massa), dimensão do ventrículo direito (DVD), fração de ejeção do VE (FE), percentagem de encurtamento do VE (%EC), gravidade da insuficiência das valvas atrioventriculares e da valva pulmonar e medida da pressão sistólica (PsVD) e diastólica (PdVD) do VD. Foi considerado significativo erro alfa <0,05. RESULTADOS: Idade média de 2,37 anos, ocorridos 35 óbitos (23,7% - IC95=17,1% a 31,2%). ANOVA demonstrou: DAE (p<0,0001), DAE/Ao (p<0,0001), DSVE (p=0,0061), DDVE (p=0,0086), massa (p<0,0001), FE (p=0,0074), %EC (p=0,0072) e DVD (p<0,0001). O agravamento da insuficiência mitral (IM) (p=0,0113) e tricúspide (IT) (p=0,0044) foram marcadores de óbito e a presença de IM, IT e insuficiência pulmonar moderada/grave foram deletérias à sobrevida. A análise de Cox demonstrou como preditores independentes da morte: DAE/Ao (p=0,0487), FE (<0,0001) e presença de IM moderada/grave (p=0,0419). CONCLUSÃO: Os pacientes com aumento progressivo da DAE/Ao, redução da FE e agravamento progressivo da IM, a despeito do tratamento clínico, devem ser considerados precocemente em programa de transplante cardíaco.

OBJETIVO: Testar a hipótese de que o índice de pulsatilidade do ducto venoso (IPDV) é maior nos fetos de mães diabéticas (FMD) com hipertrofia miocárdica (HM) do que em FMD sem HM e em fetos controles de mães não diabéticas (FMND) comparando os resultados com os picos de velocidade dos fluxos diastólicos nas valvas mitral e tricúspide. MÉTODOS: Estudo transversal incluindo fetos com idade gestacional entre 20 semanas até o termo, divididos em 3 grupos: 56 FMD com HM (grupo I), 36 FMD sem HM (grupo II) e 53 FMND (grupo III, controle). O Doppler-ecocardiograma avaliou o IPDV através da razão (velocidade sistólica - velocidade pré-sistólica)/velocidade média. As ondas E e A dos fluxos mitral e tricúspide foram também avaliadas. RESULTADOS: A média do IPDV no grupo I foi de 1,13 ± 0,64, no grupo II, de 0,84 ± 0,38 e no grupo III de 0,61±0,17. Aplicando-se a ANOVA e o teste de Tukey, houve diferença estatisticamente significativa entre os 3 grupos (p= 0,015 entre os grupos I e II, p < 0,001 entre os grupos I e III e p = 0,017 entre os grupos II e III). A média da onda E mitral foi significativamente maior no grupo I (0,39 ± 0,12 m/s) do que nos grupos II (0,32 ± 0,08 m/s) (p=0,024) e III (0,32 ± 0,08 m/s) (p=0,023). A média da onda E tricúspide foi também maior no grupo I (0,43 ± 0,1 m/s) do que no grupo III (0,35 ± 0,10 m/s) (p= 0,031). CONCLUSÃO: O IPDV é significativamente maior em FMD com HM do que em FMD sem HM e do que em FMND. Como o IPDV pode representar modificações na complacência ventricular, este índice pode ser um parâmetro mais sensível para a avaliação da função diastólica fetal.

OBJETIVO: Avaliar a incidência de trombo intracardíaco e de tromboembolismo nos três primeiros meses após a troca valvar por bioprótese e identificar fatores de risco para a formação de trombo. MÉTODO:Incluídos 184 pacientes, entre 15 e 75 anos de idade, submetidos a implante de bioprótese e realizados ecocardiogramas transtorácico e transesofágico (ETE) na fase hospitalar, média 8,4±3 dias, e três meses após, média 97,4±21,7 dias. RESULTADOS: Incidência de trombo foi significativamente mais elevada nos pacientes com prótese em posição mitral ou mitroaórtica (21,0%) do que em posição aórtica (2,8%), p<0,001. A análise de regressão logística multivariada identificou a prótese em posição mitral ou mitroaórtica como a única variável independente para a formação de trombo. No seguimento de três meses o ETE evidenciou trombo em 35 (20,7%) dos 169 pacientes com evolução ecocardiográfica, 31,7% nos mitrais e 3,1% nos aórticos, p<0,001. No 3º mês a análise de regressão logística multivariada também identificou a prótese em posição mitral ou mitroaórtica como a única variável independente para a formação de trombo. Durante os três meses de seguimento, três (1,6%) pacientes faleceram e oito (4,3%) apresentaram fenômeno embólico, todos para território cerebral. CONCLUSÃO: A incidência de trombo nos três primeiros meses, após o implante da bioprótese foi 14,1% nos 10 primeiros dias e 20,7% em três meses; a prótese em posição mitral ou mitroaórtica foi identificada como fator de risco para a formação de trombo; a incidência de fenômenos embólicos diagnosticados clinicamente foi inferior à proporção de trombo atrial documentado pelo ecocardiograma.

OBJETIVO: Relatar as manifestações clínicas e características demográficas de pacientes com febre reumática atendidos em serviço público no Estado do Acre. MÉTODOS: Estudo de corte transversal em pacientes atendidos consecutivamente no Ambulatório de Cardiologia da FUNDHACRE, avaliados através de questionário contendo dados demográficos, clínicos e laboratoriais. O diagnóstico de febre reumática foi realizado através da aplicação dos critérios de Jones, em associação com dados laboratoriais, eletrocardiograma, radiografia de tórax e ecocardiograma bidimensional. Excluídos portadores com outras cardiopatias, diabetes, obesidade, doenças inflamatórias, processos infecciosos, tabagismo, gestantes, uso de drogas anti-inflamatórias ou reposição hormonal. RESULTADOS: De julho/2003 a fevereiro/2004, foram avaliados 99 pacientes com febre reumática aguda (idade média de 11 anos, dp= ± 10,18) com predominância feminina (59,6%) e fenótipo racial mestiço de índio (60,6%). Excluídos 3 indivíduos, por não preencherem os critérios diagnósticos. A idade média de início foi de 9,1 anos, sendo que em 30,4% dos pacientes a doença foi diagnosticada no primeiro episódio de atividade reumática. As manifestações clínicas mais freqüentes foram cardite (69,7%), artrite (21,4%) e coréia (6,1%) e a regurgitação mitral (36,4%) a lesão mais comum seguida da associação de regurgitação mitral com aórtica (9,1%). CONCLUSÃO: Cardite reumática foi a manifestação mais freqüente de febre reumática, predominando no grupo racial mestiço de índio (60,6%), A baixa aderência à antibioticoprofilaxia contribuiu para recorrências e seqüelas cardíacas.
