We study the effects of strict conservation laws and the problem of negative contributions to final momentum distribution during the freeze-out through 3-dimensional hypersurfaces with spacelike normal. We study some suggested solutions for this problem, and demonstrate it in one example.

The problem of freeze-out (FO) in relativistic heavy-ion reactions is addressed. We develop and analyze an idealized one-dimensional model of FO in a finite layer, based on the covariant FO probability. The resulting post FO phase-space distributions are discussed for different FO probabilities and layer thicknesses.

The chiral antidepressant venlafaxine (VEN) is both a serotonin and a norepinephrine uptake inhibitor. CYP2D6 and CYP3A4 contribute to its metabolism, which has been shown to be stereoselective. Ten CYP2D6 genotyped and depressive (F32x and F33x, ICD-10) patients participated in an open study on the pharmacokinetic and pharmacodynamic consequences of a carbamazepine augmentation in VEN non-responders. After an initial 4-week treatment with VEN (195 +/- 52 mg/day), the only poor metabolizer out of 10 depressive patients had the highest plasma concentrations of S-VEN and R-VEN, respectively, whereas those of R-O-demethyl-VEN were lowest. Five non-responders completed the second 4-week study period, during which they were submitted to a combined VEN-carbamazepine treatment. In the only non-responder to this combined treatment, there was a dramatic decrease of both enantiomers of VEN, O-demethylvenlafaxine, N-desmethylvenlafaxine and N, O-didesmethylvenlafaxine in plasma, which suggests non-compliance, although metabolic induction by carbamazepine cannot entirely be excluded. The administration of carbamazepine [mean +/- SD, range: 360 +/- 89 (200-400) mg/day] over 4 weeks did not result in a significant modification of the plasma concentrations of the enantiomers of VEN and its O- and N-demethylated metabolites in the other patients. In conclusion, these preliminary observations suggest that the combination of VEN and carbamazepine represents an interesting augmentation strategy by its efficacy, tolerance and absence of pharmacokinetic modifications. However, these findings should be verified in a more comprehensive study.

Les laves torrentielles sont l'un des vecteurs majeurs de sédiments en milieu montagneux. Leur comportement hydrogéomorphologique est contrôlé par des facteurs géologique, géomorphologique, topographique, hydrologique, climatique et anthropique. Si, en Europe, la recherche s'est plus focalisée sur les aspects hydrologiques que géomorphologiques de ces phénomènes, l'identification des volumes de sédiments potentiellement mobilisables au sein de petits systèmes torrentiels et des processus responsables de leur transfert est d'une importance très grande en termes d'aménagement du territoire et de gestion des dangers naturels. De plus, une corrélation entre des événements pluviométriques et l'occurrence de laves torrentielles n'est pas toujours établie et de nombreux événements torrentiels semblent se déclencher lorsqu'un seuil géomorphologique intrinsèque (degré de remplissage du chenal) au cours d'eau est atteint.Une méthodologie pragmatique a été développée pour cartographier les stocks sédimentaires constituant une source de matériaux pour les laves torrentielles, comme outil préliminaire à la quantification des volumes transportés par ces phénomènes. La méthode s'appuie sur des données dérivées directement d'analyses en environnement SIG réalisées sur des modèles numériques d'altitude de haute précision, de mesures de terrain et d'interprétation de photographies aériennes. La méthode a été conçue pour évaluer la dynamique des transferts sédimentaires, en prenant en compte le rôle des différents réservoirs sédimentaires, par l'application du concept de cascade sédimentaire sous un angle cartographique.Les processus de transferts sédimentaires ont été étudiés dans deux bassins versants des Alpes suisses (torrent du Bruchi, à Blatten beiNaters et torrent du Meretschibach, à Agarn). La cartographie géomorphologique a été couplée avec des mesures complémentaires permettant d'estimer les flux sédimentaires et les taux d'érosion (traçages de peinture, piquets de dénudation et utilisation du LiDAR terrestre). La méthode proposée se révèle innovatrice en comparaison avec la plupart des systèmes de légendes géomorphologiques existants, qui ne sont souvent pas adaptés pour cartographier de manière satisfaisante les systèmes géomorphologiques complexes et actifs que sont les bassins torrentiels. L'intérêt de cette méthode est qu'elle permet l'établissement d'une cascade sédimentaire, mais uniquement pour des systèmes où l'occurrence d'une lave torrentielle est contrôlé par le degré de remplissage en matériaux du chenal. Par ailleurs, le produit cartographique ne peut être directement utilisé pour la création de cartes de dangers - axées sur les zones de dépôt - mais revêt un intérêt pour la mise en place de mesures de correction et pour l'installation de systèmes de monitoring ou d'alerte.La deuxième partie de ce travail de recherche est consacrée à la cartographie géomorphologique. Une analyse a porté sur un échantillon de 146 cartes ou systèmes de légende datant des années 1950 à 2009 et réalisés dans plus de 40 pays. Cette analyse a permis de mettre en évidence la diversité des applications et des techniques d'élaboration des cartes géomorphologiques. - Debris flows are one of the most important vectors of sediment transfer in mountainous areas. Their hydro-geomorphological behaviour is conditioned by geological, geomorphological, topographical, hydrological, climatic and anthropic factors. European research in torrential systems has focused more on hydrological processes than on geomorphological processes acting as debris flow triggers. Nevertheless, the identification of sediment volumes that have the potential to be mobilised in small torrential systems, as well as the recognition of processes responsible for their mobilisation and transfer within the torrential system, are important in terms of land-use planning and natural hazard management. Moreover, a correlation between rainfall and debris flow occurrence is not always established and a number of debris flows seems to occur when a poorly understood geomorphological threshold is reached.A pragmatic methodology has been developed for mapping sediment storages that may constitute source zone of bed load transport and debris flows as a preliminary tool before quantifying their volumes. It is based on data directly derived from GIS analysis using high resolution DEM's, field measurements and aerial photograph interpretations. It has been conceived to estimate sediment transfer dynamics, taking into account the role of different sediment stores in the torrential system applying the concept of "sediment cascade" in a cartographic point of view.Sediment transfer processes were investigated in two small catchments in the Swiss Alps (Bruchi torrent, Blatten bei Naters and Meretschibach torrent, Agarn). Thorough field geomorphological mapping coupled with complementary measurements were conducted to estimate sediment fluxes and denudation rates, using various methods (reference coloured lines, wooden markers and terrestrial LiDAR). The proposed geomorphological mapping methodology is quite innovative in comparison with most legend systems that are not adequate for mapping active and complex geomorphological systems such as debris flow catchments. The interest of this mapping method is that it allows the concept of sediment cascade to be spatially implemented but only for supply-limited systems. The map cannot be used directly for the creation of hazard maps, focused on the deposition areas, but for the design of correction measures and the implementation of monitoring and warning systems.The second part of this work focuses on geomorphological mapping. An analysis of a sample of 146 (extracts of) maps or legend systems dating from the middle of the 20th century to 2009 - realised in more than 40 different countries - was carried out. Even if this study is not exhaustive, it shows a clear renewed interest for the discipline worldwide. It highlights the diversity of applications, techniques (scale, colours and symbology) used for their conception.

A pacemaker, regularly emitting chemical waves, is created out of noise when an excitable photosensitive Belousov-Zhabotinsky medium, strictly unable to autonomously initiate autowaves, is forced with a spatiotemporal patterned random illumination. These experimental observations are also reproduced numerically by using a set of reaction-diffusion equations for an activator-inhibitor model, and further analytically interpreted in terms of genuine coupling effects arising from parametric fluctuations. Within the same framework we also address situations of noise-sustained propagation in subexcitable media.

Chile is one of the countries with the highest level of income inequality worldwide. Given the association between economic inequality and patterns of social stratification, in such context it would be expected to find a large dispersion of subjective social status' indicators in the population. Nevertheless, analyses from international surveys reveal a strong tendency to the mean of the subjective status i.e. there would be downward and upward biases of the subjective status regarding the objective status. The present research attempts to tackle this phenomenon for the Chilean case, with a focus on the influence of status and class variables on the subjective status. For the analysis we use Chilean data from the social inequality module of the International Social Survey Programme 2009. The results indicate a strong tendency to the mean of the subjective status in the population, particularly from the side of those with higher objective status.

Recent theory of physiology of language suggests a dual stream dorsal/ventral organization of speech perception. Using intra-cerebral Event-related potentials (ERPs) during pre-surgical assessment of twelve drug-resistant epileptic patients, we aimed to single out electrophysiological patterns during both lexical-semantic and phonological monitoring tasks involving ventral and dorsal regions respectively. Phonological information processing predominantly occurred in the left supra-marginal gyrus (dorsal stream) and lexico-semantic information occurred in anterior/middle temporal and fusiform gyri (ventral stream). Similar latencies were identified in response to phonological and lexico-semantic tasks, suggesting parallel processing. Typical ERP components were strongly left lateralized since no evoked responses were recorded in homologous right structures. Finally, ERP patterns suggested the inferior frontal gyrus as the likely final common pathway of both dorsal and ventral streams. These results brought out detailed evidence of the spatial-temporal information processing in the dual pathways involved in speech perception.

STUDY OBJECTIVES: To review the etiology and pathophysiology of pneumomediastinum in severe blunt trauma, with a special interest in one of its possible origins, the Macklin effect. The Macklin effect relates to a three-step pathophysiologic process: blunt traumatic alveolar ruptures, air dissection along bronchovascular sheaths, and spreading of this blunt pulmonary interstitial emphysema into the mediastinum. The clinical relevance of the Macklin effect was also evaluated. SETTING: A university hospital serving as a reference trauma center. PATIENTS: A selection of 51 patients with severe blunt trauma between 1995 and 2000. Inclusion criteria: Severe trauma or high-speed deceleration justifying chest CT; if chest CT demonstrated a pneumomediastinum, bronchoscopy and esophagoscopy were performed to rule out tracheobronchial or esophageal injury. DESIGN: Retrospective analysis of patients' clinical files, chest CT, and bronchoscopy and esophagoscopy reports. The Macklin effect was diagnosed when an air collection adjacent to a bronchus and a pulmonary vessel could be clearly identified on the chest CT. Clinical relevance of the Macklin effect was statistically evaluated regarding its repercussions on the pulmonary gas exchange function, the respective durations of intensive care and total hospital stay, and the associated injuries. RESULTS: Twenty (39%) Macklin effects and 5 tracheobronchial injuries (10%) were identified. One tracheobronchial injury occurred simultaneously with the Macklin effect. The presence of the Macklin effect affected neither the clinical profile nor the result of pulmonary gas analysis on hospital admission, but was associated with a significant (p < 0.001) lengthening of the intensive care stay. CONCLUSIONS: The Macklin effect is present in 39% of severe blunt traumatic pneumomediastinum detected by CT. Its identification does not rule out a tracheobronchial injury. The Macklin effect reflects severe trauma, since it is associated with significantly prolonged intensive care stay.

A better prediction of the outcome after ischemia and estimation of onset time at early time points would greatly facilitate clinical decisions. Therefore, the aim of the present study was to use magnetic resonance spectroscopy to identify neurochemical markers for outcome prediction at early time points after ischemia.ICR-CD1 mice were subjected to 10-minute, 30-minute or permanent middle cerebral artery occlusion (MCAO). The regional cerebral blood flow (CBF) was monitored in all animals by laser-Doppler flowmetry. All MR studies were carried out in a horizontal 14.1T magnet. Fast spin echo images with T2-weighted parameters were Bacquired to localize the volume of interest and evaluate the lesion size. Immediately after adjustment of field inhomogeneities, localized 1H MRS was applied to obtain the neurochemical profile from the striatum (6-8 μl) or the cortex (2.2-2.5 μl). Six animals (sham group) underwent nearly identical procedures without MCAO.By comparing the evolution of several metabolites in ischemia of varying severity, we observed that glutamine increases early after transient ischemia independently of severity, but decreases in permanent ischemia. On the opposite, GABA increased in permanent ischemia and decreased in transient. We also observed a decrease in the sum of N-acetyl aspartate + glutamate + taurine in all irreversibly damaged tissues, independently of reperfusion and severity. Finally, we have observed that some metabolites decrease exponentially after ischemia. This exponential decrease could be used to determine the time of ischemia onset in permanent ischemia.In Conclusion, magnetic resonance spectroscopy can be used as a prognostic and diagnostic tool to monitor reperfusion, identify reversibly and irreversibly damaged tissue and evaluate the time of ischemia onset. If these Results can be translated to stroke patients, this technique would greatly improve the diagnosis and help with clinical decisions.

BACKGROUND: Post-transplant lymphoproliferative disease (PTLD) is a life-threatening complication of immunosuppression following transplantation. Epstein-Barr virus (EBV) and gammopathy in serum are associated with PTLD, but these two parameters have not been evaluated in parallel for their association with PTLD. METHODS: We evaluated the incidence of EBV load positivity, gammopathy, and protein expression in sera from all PTLD patients diagnosed at our hospital during the past seven yr. Results were compared with those of a control group including matched transplanted patients who did not develop PTLD. RESULTS: Seven of 10 PTLD patients presented EBV(+) PTLD, for which five patients had detectable serum EBV DNA levels compared with none of 38 controls (RR between two groups =121, p &lt; 0.0001). Five out of 10 patients had gammopathy at PTLD diagnosis compared with 5/38 controls (RR between two groups = 6.6, p = 0.022). Additionally, protein serum analysis by high-resolution two-dimensional gel electrophoresis and image examination failed to evidence specific abnormality in patients with PTLD compared with controls. CONCLUSIONS: Our results confirm an association between EBV in sera and gammopathy with PTLD, and highlight the high specificity of the former analysis. Whether a combination of both analyses will improve the clinical detection of PTLD remains to be evaluated in a larger prospective cohort study.

Background: A patient's chest pain raises concern for the possibility of coronary heart disease (CHD). An easy to use clinical prediction rule has been derived from the TOPIC study in Lausanne. Our objective is to validate this clinical score for ruling out CHD in primary care patients with chest pain. Methods: This secondary analysis used data collected from a oneyear follow-up cohort study attending 76 GPs in Germany. Patients attending their GP with chest pain were questioned on their age, gender, duration of chest pain (1-60 min), sternal pain location, pain increases with exertion, absence of tenderness point at palpation, cardiovascular risks factors, and personal history of cardiovascular disease. Area under the curve (ROC), sensitivity and specificity of the Lausanne CHD score were calculated for patients with full data. Results: 1190 patients were included. Full data was available for 509 patients (42.8%). Missing data was not related to having CHD (p = 0.397) or having a cardiovascular risk factor (p = 0.275). 76 (14.9%) were diagnosed with a CHD. Prevalence of CHD were respectively of 68/344 (19.8%), 2/62 (3.2%), 6/103 (5.8%) in the high, intermediate and low risk category. ROC was of 72.9 (CI95% 66.8; 78.9). Ruling out patients with low risk has a sensitivity of 92.1% (CI95% 83.0; 96.7) and a specificity of 22.4% (CI95% 18.6%; 26.7%). Conclusion: The Lausanne CHD score shows reasonably good sensitivity and can be used to rule out coronary events in patients with chest pain. Patients at risk of CHD for other rarer reasons should nevertheless also be investigated.

BACKGROUND: Detailed comparison of effectiveness between organised and opportunistic mammography screening operating in the same country has seldom been carried out. PATIENTS AND METHODS: Prognostic indicators, as defined in the European Guidelines, were used to evaluate screening effectiveness in Switzerland. Matching of screening programmes' records with population-based cancer registries enabled to compare indicators of effectiveness by screening and detection modality (organised versus opportunistic screening, unscreened, interval cancers). Comparisons of prognostic profile were also drawn with two Swiss regions uncovered by service screening of low and high prevalence of opportunistic screening, respectively. RESULTS: Opportunistic and organised screening yielded overall little difference in prognostic profile. Both screening types led to substantial stage shifting. Breast cancer prognostic indicators were systematically more favourable in Swiss regions covered by a programme. In regions without a screening programme, the higher the prevalence of opportunistic screening, the better was the prognostic profile. CONCLUSIONS: Organised screening appeared as effective as opportunistic screening. Mammography screening has strongly influenced the stage distribution of breast cancer in Switzerland, and a favourable impact on mortality is anticipated. Extension of organised mammography screening to the whole of Switzerland can be expected to further improve breast cancer prognosis in a cost-effective way.
