Chronic lead exposure induces hypertension in humans and animals, affecting endothelial function. However, studies concerning acute cardiovascular effects are lacking. We investigated the effects of acute administration of a high concentration of lead acetate (100 µΜ) on the pressor response to phenylephrine (PHE) in the tail vascular bed of male Wistar rats. Animals were anesthetized with sodium pentobarbital and heparinized. The tail artery was dissected and cannulated for drug infusion and mean perfusion pressure measurements. Endothelium and vascular smooth muscle relaxation were tested with acetylcholine (5 µg/100 µL) and sodium nitroprusside (0.1 µg/100 µL), respectively, in arteries precontracted with 0.1 µM PHE. Concentration-response curves to PHE (0.001-300 µg/100 µL) were constructed before and after perfusion for 1 h with 100 µΜ lead acetate. In the presence of endothelium (E+), lead acetate increased maximal response (Emax) (control: 364.4 ± 36, Pb2+: 480.0 ± 27 mmHg; P < 0.05) and the sensitivity (pD2; control: 1.98 ± 0.07, 2.38 ± 0.14 log mM) to PHE. In the absence of endothelium (E-) lead had no effect but increased baseline perfusion pressure (E+: 79.5 ± 2.4, E-: 118 ± 2.2 mmHg; P < 0.05). To investigate the underlying mechanisms, this protocol was repeated after treatment with 100 µM L-NAME, 10 µM indomethacin and 1 µM tempol in the presence of lead. Lead actions on Emax and pD2 were abolished in the presence of indomethacin, and partially abolished with L-NAME and tempol. Results suggest that acute lead administration affects the endothelium, releasing cyclooxygenase-derived vasoconstrictors and involving reactive oxygen species.

Isolated segments of the perfused rat tail artery display a high basal tone when compared to other isolated arteries such as the mesenteric and are suitable for the assay of vasopressor agents. However, the perfusion of this artery in the entire tail has not yet been used for functional studies. The main purpose of the present study was to identify some aspects of the vascular reactivity of the rat tail vascular bed and validate this method to measure vascular reactivity. The tail severed from the body was perfused with Krebs solution containing different Ca2+ concentrations at different flow rates. Rats were anesthetized with sodium pentobarbital (65 mg/kg) and heparinized (500 U). The tail artery was dissected near the tail insertion, cannulated and perfused with Krebs solution plus 30 µM EDTA at 36oC and 2.5 ml/min and the procedures were started after equilibration of the perfusion pressure. In the first group a dose-response curve to phenylephrine (PE) (0.5, 1, 2 and 5 µg, bolus injection) was obtained at different flow rates (1.5, 2.5 and 3.5 ml/min). The mean perfusion pressure increased with flow as well as PE vasopressor responses. In a second group the flow was changed (1.5, 2, 2.5, 3 and 3.5 ml/min) at different Ca2+ concentrations (0.62, 1.25, 2.5 and 3.75 mM) in the Krebs solution. Increasing Ca2+ concentrations did not alter the flow-pressure relationship. In the third group a similar protocol was performed but the rat tail vascular bed was perfused with Krebs solution containing PE (0.1 µg/ml). There was an enhancement of the effect of PE with increasing external Ca2+ and flow. PE vasopressor responses increased after endothelial damage with air and CHAPS, suggesting an endothelial modulation of the tone of the rat tail vascular bed. These experiments validate the perfusion of the rat tail vascular bed as a method to investigate vascular reactivity.

Ouabain is an endogenous substance occurring in the plasma in the nanomolar range, that has been proposed to increase vascular resistance and induce hypertension. This substance acts on the a-subunit of Na+,K+-ATPase inhibiting the Na+-pump activity. In the vascular smooth muscle this effect leads to intracellular Na+ accumulation that reduces the activity of the Na+/Ca2+ exchanger and to an increased vascular tone. It was also suggested that circulating ouabain, even in the nanomolar range, sensitizes the vascular smooth muscle to vasopressor substances. We tested the latter hypothesis by studying the effects of ouabain in the micromolar and nanomolar range on phenylephrine (PE)-evoked pressor responses. The experiments were performed in normotensive and hypertensive rats in vivo, under anesthesia, and in perfused rat tail vascular beds. The results showed that ouabain pretreatment increased the vasopressor responses to PE in vitro and in vivo. This sensitization after ouabain treatment was also observed in hypertensive animals which presented an enhanced vasopressor response to PE in comparison to normotensive animals. It is suggested that ouabain at nanomolar concentrations can sensitize vascular smooth muscle to vasopressor stimuli possibly contributing to increased tone in hypertension.

A transitory increase in blood pressure (BP) is observed following upper airway surgery for obstructive sleep apnea syndrome but the mechanisms implicated are not yet well understood. The objective of the present study was to evaluate changes in BP and heart rate (HR) and putative factors after uvulopalatopharyngoplasty and septoplasty in normotensive snorers. Patients (N = 10) were instrumented for 24-h ambulatory BP monitoring, nocturnal respiratory monitoring and urinary catecholamine level evaluation one day before surgery and on the day of surgery. The influence of postsurgery pain was prevented by analgesic therapy as confirmed using a visual analog scale of pain. Compared with preoperative values, there was a significant (P < 0.05) increase in nighttime but not daytime systolic BP (119 ± 5 vs 107 ± 3 mmHg), diastolic BP (72 ± 4 vs 67 ± 2 mmHg), HR (67 ± 4 vs 57 ± 2 bpm), respiratory disturbance index (RDI) characterized by apnea-hypopnea (30 ± 10 vs 13 ± 4 events/h of sleep) and norepinephrine levels (22.0 ± 4.7 vs 11.0 ± 1.3 µg l-1 12 h-1) after surgery. A positive correlation was found between individual variations of BP and individual variations of RDI (r = 0.81, P < 0.01) but not between BP or RDI and catecholamines. The visual analog scale of pain showed similar stress levels on the day before and after surgery (6.0 ± 0.8 vs 5.0 ± 0.9 cm, respectively). These data strongly suggest that the cardiovascular changes observed in patients who underwent uvulopalatopharyngoplasty and septoplasty were due to the increased postoperative RDI.

Introdução: Muito embora os estudos apontem para um efeito positivo do exercício físico, em especial o treinamento com exercício aeróbio, sobre a pressão arterial e a distensibilidade arterial, pouco se sabe sobre os efeitos do treinamento com exercício de resistência aeróbia sobre a complacência vascular de indivíduos jovens saudáveis. Objetivos: Avaliar o efeito de 16 semanas de treinamento de resistência aeróbia sobre a função vascular e a pressão arterial de indivíduos jovens sedentários. Métodos: Foram avaliados 56 voluntários (de ambos os sexos, na faixa etária de 18 à 29 anos) antes e após 16 semanas de treinamento com corrida 3 vezes por semana. As medidas de pressão arterial foram realizadas de acordo com a VI Diretrizes Brasileiras de Hipertensão e a velocidade de onda de pulso (VOP) foi realizada com a utilização de um gravador automático computadorizado e os resultados foram analisados pelo programa Complior®. Resultados: Dos 56 indivíduos que participaram do presente estudo, 44 eram do sexo masculino (78,5%) e 12 do sexo feminino (21,5 %). Eles apresentaram idade de 22 ± 3 anos, estatura de 1,75 ± 0,07 metros, circunferência de cintura de 79,6 ± 7,8 cm e PAM de 79 ± 6,4 mmHg. O treinamento promoveu redução da FC repouso (69 ± 7,0 vs. 61 ± 7,1; p<0,05) e aumento do VO2pico (43,3 ± 7,3 vs. 50,1 ± 7,2; p<0,05). Entretanto, pressão arterial sistólica (107 ± 9,4 vs. 110 ± 10), pressão arterial diastólica (63 ± 5,7 vs. 62 ± 5,5), pressão de pulso (44 ± 7,0 vs. 48 ± 7,0) e VOP (6,5 ± 1,1 vs. 6,5 ± 1,1) não apresentaram alteração após o treinamento físico (p>0,05). Conclusões: Podemos concluir que 16 semanas de treinamento de resistência aeróbia foram capazes de aumentar a aptidão cardiorrespiratória, porém não provocaram alterações sobre a velocidade de onda de pulso e pressão arterial em voluntários saudáveis e sedentários. Sugere-se que a ausência de adaptações vasculares após o treinamento seja devido às características da amostra – indivíduos jovens e saudáveis.

Zumbido é uma das queixas otológicas mais comuns com que o otorrinolaringologista se depara. O "hum" venoso é descrito como uma causa pouco comum de zumbido vascular, pouco lembrado ou reconhecido como entidade clínica. OBJETIVO: O objetivo do estudo é identificar os casos de "hum" venoso dentre os pacientes com zumbido pulsátil atendidos no Ambulatório de Zumbido da Disciplina de Otorrinolaringologia da UNIFESP-EPM e compará-los com a literatura. MATERIAL E MÉTODO: Estudo retrospectivo dos pacientes com "hum" venoso realizado na UNIFESP-EPM de abril de 1997 a abril de 2003, analisando-se os parâmetros: idade de aparecimento, freqüência, lado acometido, presença de perda auditiva e tontura associadas, fatores de piora e melhora, resultados de audiometria, exame vestibular e tomografia computadorizada de ossos temporais, evolução e tratamento realizados. Foi utilizado um protocolo de exames e tratamento e os resultados foram comparados com os da literatura. RESULTADOS: O zumbido pulsátil ocorreu em 7,5% e o "hum" venoso em 3% do total de pacientes com zumbido, todos no sexo feminino, sem prevalência por época de aparecimento, acometendo mais a orelha esquerda. Em todos os pacientes houve melhora com tratamento clínico, não sendo necessária intervenção cirúrgica em nenhum caso. CONCLUSÃO: O "hum" venoso não é uma causa incomum de zumbido (39% dos zumbidos pulsáteis) como citado na literatura. O tratamento deve ser realizado atuando-se sobre os fatores responsáveis pelo zumbido e decorrentes do mesmo. Em grande número de casos o mesmo desaparece espontaneamente, não necessitando de tratamento. O tratamento cirúrgico raramente é necessário, devendo ser reservado apenas aos casos em que não haja melhora com o tratamento clínico.

Pectus excavatum is the most common deformity of the thorax. A minimally invasive surgical correction is commonly carried out to remodel the anterior chest wall by using an intrathoracic convex prosthesis in the substernal position. The process of prosthesis modeling and bending still remains an area of improvement. The authors developed a new system, i3DExcavatum, which can automatically model and bend the bar preoperatively based on a thoracic CT scan. This article presents a comparison between automatic and manual bending. The i3DExcavatum was used to personalize prostheses for 41 patients who underwent pectus excavatum surgical correction between 2007 and 2012. Regarding the anatomical variations, the soft-tissue thicknesses external to the ribs show that both symmetric and asymmetric patients always have asymmetric variations, by comparing the patients’ sides. It highlighted that the prosthesis bar should be modeled according to each patient’s rib positions and dimensions. The average differences between the skin and costal line curvature lengths were 84 ± 4 mm and 96 ± 11 mm, for male and female patients, respectively. On the other hand, the i3DExcavatum ensured a smooth curvature of the surgical prosthesis and was capable of predicting and simulating a virtual shape and size of the bar for asymmetric and symmetric patients. In conclusion, the i3DExcavatum allows preoperative personalization according to the thoracic morphology of each patient. It reduces surgery time and minimizes the margin error introduced by the manually bent bar, which only uses a template that copies the chest wall curvature.

Laparoscopy is a surgical procedure on which operations in the abdomen are performed through small incisions using several specialized instruments. The laparoscopic surgery success greatly depends on surgeon skills and training. To achieve these technical high-standards, different apprenticeship methods have been developed, many based on in vivo training, an approach that involves high costs and complex setup procedures. This paper explores Virtual Reality (VR) simulation as an alternative for novice surgeons training. Even though several simulators are available on the market claiming successful training experiences, their use is extremely limited due to the economic costs involved. In this work, we present a low-cost laparoscopy simulator able to monitor and assist the trainee’s surgical movements. The developed prototype consists of a set of inexpensive sensors, namely an accelerometer, a gyroscope, a magnetometer and a flex sensor, attached to specific laparoscopic instruments. Our approach allows repeated assisted training of an exercise, without time constraints or additional costs, since no human artificial model is needed. A case study of our simulator applied to instrument manipulation practice (hand-eye coordination) is also presented.

Laparoscopic surgery (LS) has revolutionized traditional surgical techniques introducing minimally invasive procedures for diagnosis and local therapies. LSs have undeniable advantages, such as small patient incisions, reduced postoperative pain and faster recovery. On the other hand, restricted vision of the anatomical target, difficult handling of the surgical instruments, restricted mobility inside the human body, need of dexterity to hand-eye coordination and inadequate and non-ergonomic surgical instruments may restrict LS only to more specialized surgeons. To overcome the referred limitations, this work presents a new robotic surgical handheld system – the EndoRobot. The EndoRobot was designed to be used in clinical practice or even as a surgical simulator. It integrates an electromechanical system with 3 degrees of freedom. Each degree can be manipulated independently and combined with different levels of sensitivity allowing fast and slow movements. As other features, the EndoRobot has battery power or external power supply, enables the use of bipolar radiofrequency to prevent bleeding while cutting and allows plug-and-play of the laparoscopic forceps for rapid exchange. As a surgical simulator, the system was also instrumented to measure and transmit, in real time, its position and orientation for a training software able to monitor and assist the trainee’s surgical movements.

O estudo da função vascular periférica tem beneficiado do desenvolvimento de diversas técnicas e metodologias de avaliação o as quais permitem caracterizar a função com um mínimo de invasibilidade. O estudo do significado de diversas variáveis transcutâneas tem ajudado a melhor compreender as complexas relações que regem a perfusão dos tecidos in vivo. No presente estudo procurámos avaliar o perfil de resposta de um grupo de indivíduos saudáveis jovens de ambos os sexos (n=23, 22,4 ± 2,2 anos), a uma manobra dinâmica (provocação) envolvendo a variação postural passiva do membro inferior (A: elevação da perna e B: decúbito dorsal) de modo a caracterizar a função vascular periférica por meio de variáveis transcutâneas (tc). A perfusão local medida por Fluxometria de Laser Doppler (LDF) , a Perda Trans-epidérmica de água (PTEA) as pressões parciais de oxigénio e dióxido de carbono transcutâneos e a oximetria de pulso foram escolhidas e, um intervalo de confiança de 95% adoptado. LDF, PTEA e tcpO 2 são, manifestamente, as variáveis que melhor permitem seguir o processo em cada fase experimental. Em A foram registadas, durante a provocação, diferenças significativas na perfusão sanguínea e na pressão transcutânea de oxigénio e PTEA, especialmente nas mulheres, enquanto que o perfil de resposta obtido em B foi idêntico em ambos os grupos e comparável B hiperémia reactiva. Qualquer dos protocolos permitiu evidenciar uma relação inversamente recíproca entre a PTEA e o LD. Os resultados obtidos confirmam a utilidade destas metodologias na avaliação da função vascular periférica, confirmando as relações entre PTEA e LDF as quais devem ser adequadamente aprofundadas.

As doenças vasculares cerebrais constituem a principal causa de incapacidade permanente em Portugal, reflectindo-se na funcionalidade dos indivíduos. Após um Acidente Vascular Cerebral (AVC) são frequentes as alterações posturais, não apenas no que respeita a capacidade para manter uma postura estática mas também a capacidade para mover dentro da base de sustentação sem ocorrência de quedas. O presente trabalho apresenta alguns dados preliminares relacionados com o equilíbrio e a funcionalidade de indivíduos utentes do Hospital Fernando da Fonseca, que no período de Setembro de 2008 a Março de 2009, sofreram AVC.
