This case report discusses an unusual presentation of ST-segment elevation myocardial infarction (STEMI) with normal coronary arteries and severe mechanical complications successfully treated with surgery. An 82-year-old man presented STEMI with angiographically normal coronary arteries and no major echocardiographic alterations at discharge. At the first month follow-up, he complained of fatigue and dyspnea, and contrast echocardiography complemented by cardiac magnetic resonance imaging revealed a large left ventricular apical aneurysm with a thrombus communicating by two jets of a turbulent flow to an aneurysmatic formation of the right ventricular apex. The patient underwent a Dor procedure, which was successful. Ventricular septal defects and ventricular aneurysms are rare but devastating complications of STEMI, with almost all patients presenting multivessel coronary artery disease. Interestingly in this case, the angiographic pattern was normal.

BACKGROUND: Screening and treatment of latent tuberculosis infection (LTBI) in asylum seekers (AS) may prevent future cases of tuberculosis. As the screening with Interferon Gamma Release Assay (IGRA) is costly, the objective of this study was to assess which factors were associated with LTBI and to define a score allowing the selection of AS with the highest risk of LTBI. METHODS: In across-sectional study, AS seekers recently arrived in Vaud County, after screening for tuberculosis at the border were offered screening for LTBI with T-SPOT.TB and questionnaire on potentially risk factors. The factors associated with LTBI were analyzed by univariate and multivariate regression. RESULTS: Among 393 adult AS, 98 (24.93%) had a positive IGRA response, five of them with active tuberculosis previously undetected. Six factors associated with LTBI were identified in multivariate analysis: origin, travel conditions, marital status, cough, age and prior TB exposure. Their combination leads to a robust LTBI predictive score. CONCLUSIONS: The prevalence of LTBI and active tuberculosis in AS is high. A predictive score integrating six factors could identify the asylum seekers with the highest risk for LTBI.

A resident within one of the counties in your region has expressed some concern regarding potential adverse health effects from dust and material that may be found within foundry slag that has been used as a replacement for road rock. This consultation includes a comparison of nuisance issues from dust generated from a road constructed of foundry slag and dust generated from a road constructed from typical road rock. This consultation also includes a discussion of the potential health effects of exposure of heavy metals from incidental ingestion of foundry slag.

The Railroad Avenue groundwater contamination site (the site) is in West Des Moines, Polk County, Iowa. Located on approximately 120 acres. The site comprises mixed residential, industrial and commercial properties. Underneath the site, chlorinated volatile organic compounds (VOCs) have contaminatcd the shallow (i.e., 30-50 feet deep) groundwater. These compounds have compromised several shallow wells within the West Des Moines water works system. A contamination source, however, has not yet been identified. In 1993, routine water analysis by the City of West Des Moines identified 1, 2 cis-dichlorocthylcne (1, 2 cis-DCE) at a concentration of 1.2 μg/L (micrograms) per liter of water) in the water supply. Subsequently. several shallow municipal wells were found to be contaminated by VOCs, including 1. 2 cis-DCE, trichloroethylene (TCE), tetrachloroethylene (PCE) and benzene. Five of these wells have been taken out of service. Because of the impact on the West Des Moines water supply, the U.S. Environmental Protection Agency (USEPA) has assigned the site to the National Priorities List. Surface water und sediment at the site have not been impacted by the VOCs. Testing for VOCs in surface soils has not revealed any significant VOC contamination. Subsurface soils -- generally 8 feet or greater in depth -- are contaminated with VOCs, but at levels which should not present a health hazard. The past, present, and future health hazard category chosen for this site is no apparent public health hazard. This category is used when exposure to toxins might be occurring or might have occurrcd in the past, but at levels below any known health hazard. Analysis of available environmental data has not revealed that residental or commercial water customers are or have been exposed to VOCs at concentrations that might cause any adverse health effects.

Claes Wilhelm Gyldén (1802-1872) oli maanmittauksen ja metsänhoidon ylihallituksen ylitirehtööri vuosina 1854-72. Ylitirehtööri omisti paljon aikaansa metsänhoidon ja maanjakotoiminnan lisäksi myös maamme kartastotoiminnalle. Hän julkaisi vuonna 1853 Suomenmaan korko-kartan, joka on maailman ensimmäisiä korkeusvyöhykekarttoja. Myös Suomen yleiskartan 1:400 000 (Karta över Finland) ensimmäisen painoksen karttalehdet painettiin vuosina 1864-1872 hänen johtajakaudellaan. Vuosina 1837-1843 toimiessaan maanmittausinsinöörinä maanmittaushallituksessa C. W. Gyldén julkaisi Suomen kaikkien silloisten kaupunkien kaupunkikartat, yhteensä 31 karttalehteä. Nämä asemakaavakartat painettiin kaikki samassa koossa 50,8 x 65,9 cm. Täst johtuen karttojen mittakaavat vaihtelivat asteikkojen 1:3200 – 1:10000 välillä. Kaupungin asemakaavan lisäksi jokaisessa kartassa on yleissilmäyskartta, julkisten rakennusten luettelo sekä niiden sijainti. Lisäksi hän julkaisi vuonna 1844 näiden kaupunkien historiaa ja tilastotietoja kuvaavan selityskirjan.
