The goals of this program of research were to examine the link between self-reported vulvar pain and clinical diagnoses, and to create a user-friendly assessment tool to aid in that process. These goals were undertaken through a series of four empirical studies (Chapters 2-6): one archival study, two online studies, and one study conducted in a Women’s Health clinic. In Chapter 2, the link between self-report and clinical diagnosis was confirmed by extracting data from multiple studies conducted in the Sexual Health Research Laboratory over the course of several years. We demonstrated the accuracy of diagnosis based on multiple factors, and explored the varied gynecological presentation of different diagnostic groups. Chapter 3 was based on an online study designed to create the Vulvar Pain Assessment Questionnaire (VPAQ) inventory. Following the construct validation approach, a large pool of potential items was created to capture a broad selection of vulvar pain symptoms. Nearly 300 participants completed the entire item pool, and a series of factor analyses were utilized to narrow down the items and create scales/subscales. Relationships were computed among subscales and validated scales to establish convergent and discriminant validity. Chapters 4 and 5 were conducted in the Department of Obstetrics & Gynecology at Oregon Health & Science University. The brief screening version of the VPAQ was employed with patients of the Program in Vulvar Health at the Center for Women’s Health. The accuracy and usefulness of the VPAQscreen was determined from the perspective of patients as well as their health care providers, and the treatment-seeking experiences of patients was explored. Finally, a second online study was conducted to confirm the factor structure, internal consistency, and test-retest reliability of the VPAQ inventory. The results presented in these chapters confirm the link between targeted questions and accurate diagnoses, and provide a guideline that is useful and accessible for providers and patients.

Collecting data via a questionnaire and analyzing them while preserving respondents’ privacy may increase the number of respondents and the truthfulness of their responses. It may also reduce the systematic differences between respondents and non-respondents. In this paper, we propose a privacy-preserving method for collecting and analyzing survey responses using secure multi-party computation (SMC). The method is secure under the semi-honest adversarial model. The proposed method computes a wide variety of statistics. Total and stratified statistical counts are computed using the secure protocols developed in this paper. Then, additional statistics, such as a contingency table, a chi-square test, an odds ratio, and logistic regression, are computed within the R statistical environment using the statistical counts as building blocks. The method was evaluated on a questionnaire dataset of 3,158 respondents sampled for a medical study and simulated questionnaire datasets of up to 50,000 respondents. The computation time for the statistical analyses linearly scales as the number of respondents increases. The results show that the method is efficient and scalable for practical use. It can also be used for other applications in which categorical data are collected.

Background
There is a growing impetus across the research, policy and practice communities for children and young people to participate in decisions that affect their lives. Furthermore, there is a dearth of general instruments that measure children and young people’s views on their participation in decision making. This paper presents the reliability and validity of the Child and Adolescent Participation in Decision Making Questionnaire (CAP-DMQ) and specifically looks at a population of looked-after children where a lack of participation in decision making is an acute issue.
Methods
The participants were 151 looked after children and adolescents between 10-23 years of age who completed the 10 item CAP-DMQ. Of the participants 113 were in receipt of an advocacy service that had an aim of increasing participation in decision-making with the remaining participants not having received this service.
Results
The results showed that the CAP-DMQ had good reliability (Cronbach’s alpha = .94) and showed promising uni-dimensional construct validity through an exploratory factor analysis. The items in the CAP-DMQ also demonstrated good content validity by overlapping with prominent models of child and adolescent participation (Lundy 2007) and decision making (Halpern 2014). A regression analysis showed that age and gender were not significant predictors of CAP-DMQ scores but receipt of advocacy was a significant predictor of scores (effect size d=.88), thus showing appropriate discriminant criterion validity.
Conclusion
Overall, the CAP-DMQ showed good reliability and validity. Therefore, the measure has excellent promise for theoretical investigation in the area of child and adolescent participation in decision making and equally shows empirical promise for use as a measure in evaluating services which have increasing the participation of children and adolescents in decision making as an intended outcome.

Abstract The number of students engaged in Massive Open Online Courses (MOOCs) is increasing rapidly. Due to the autonomy of students in this type of education, students in MOOCs are required to regulate their learning to a greater extent than students in traditional, face-to-face education. However, there is no questionnaire available suited for this online context that measures all aspects of self-regulated learning (SRL). In this study, such a questionnaire is developed based on existing SRL questionnaires. This is the self-regulated online learning ques- tionnaire. Exploratory factor analysis (EFA) on the first dataset led to a set of scales differing from those theoretically defined beforehand. Confirmatory factor analysis (CFA) was conducted on a second dataset to compare the fit of the theoretical model and the exploratively obtained model. The exploratively obtained model provided much better fit to the data than the theoretical model. All models under investigation provided better fit when excluding the task strategies scale and when merging the scales measuring metacognitive activities. From the results of the EFA and the CFA it can be concluded that further development of the questionnaire is necessary.

Le présent mémoire a pour objectif de développer un questionnaire qui permettra éventuellement de comparer l’influence respective des pensées négatives, positives et réalistes sur l’adaptation psychologique au cancer du sein, et ce, durant et après les traitements de chimiothérapie. Ce questionnaire a été développé de manière à avoir une structure similaire à celle du Orientation to Motherhood scale (OM; Churchill & Davis, 2010), lequel quantifie la fréquence d’occurrence de pensées anticipatoires à valence positive et négative afin de déterminer si l’orientation de pensée des répondants est davantage négative, positive ou réaliste. Ce mémoire vise également à récolter de l’information sur la nature des pensées des patientes avant la chimiothérapie. Deux étapes ont été nécessaires afin de rencontrer ces objectifs. Dans un premier temps, vingt-cinq femmes atteintes d’un cancer du sein ont pris part à une première série d’entrevues individuelles visant à générer une banque d’items potentiels pour le questionnaire (N = 15). Par la suite, une deuxième série d’entrevues a été menée afin de permettre la tenue d’une étude pilote (N = 10) et de raffiner la forme et le contenu du questionnaire. Le Questionnaire des pensées et anticipations au sujet du cancer (PAC) est un questionnaire auto-rapporté de 51 items permettant de documenter la fréquence de diverses pensées anticipatoires à valence positive et négative chez les femmes s’apprêtant à commencer des traitements de chimiothérapie pour un cancer du sein. Les résultats de l’étude pilote sont encourageants quant à la capacité du PAC à discriminer les participantes entre elles sur la fréquence de leurs pensées à valence positive et négative. De plus, les différents profils observés chez les participantes de l’étude pilote supportent l’existence d’une relation entre les réponses au PAC et la détresse psychologique. Les données préliminaires mettent également en lumière la plus forte fréquence de pensées à valence positive que négative.

In dieser Arbeit wird die deutschsprachige Version des bewährten Home Situation Questionnaire einer psychometrischen Neunormierung unterzogen. Ziel des hier untersuchten Inventars ist es, die familiären Verhaltensprobleme von hyperkinetisch auffälligen Kindern situationsspezifisch aufzuschlüsseln. Obwohl die Situationsabhängigkeit der hyperkinetischen Symptome weithin anerkannt wird, sind messtheoretisch abgesicherte Inventare zur bedingungsanalytischen Verhaltensdiagnostik bislang nur eingeschränkt verfügbar. Als eine von wenigen Ausnahmen stellt sich der HSQ dar, der zur bedingungsanalytischen Verhaltensdiagnostik bei AD(H)S eingesetzt wird. Zwar wurde die englischsprachige Originalfassung adaptiert und als „Fragebogen für familiäre Belastungssituationen“ (FFB) ins Deutsche übersetzt, bislang aber nur von einer schmalen Datenbasis getragen und noch kaum messtheoretisch abgesichert. Insofern steht eine psychometrische Prüfung und Neunormierung des deutschen Home Situation Questionnaire noch aus. Das Inventar wurde an einer Stichprobe von insgesamt 237 hyperkinetisch auffälligen und 361 klinisch unauffälligen Kindern neu normiert. Hierfür wurden Normwerte geschlechterspezifisch ermittelt, Staninewerte berechnet und Prozentränge ausgewiesen. Zur Bestimmung der Faktorenstruktur wurde eine Hauptkomponentenanalyse errechnet, die eine drei- bzw. vierfaktorielle Lösung nahelegt und konform mit früheren Befunden geht. Demnach können die berichteten Auffälligkeiten und Belastungen anhand der folgenden Situationsprofile mit jeweils unterschiedlichem Anforderungsgehalt interpretiert werden: Regulation in sozialen Situationen, Familiäre Routinen, Innerfamiliäre Interaktionen sowie Selbstregulierte Tätigkeiten. Auch trennt der FFB deutlich und mit hoher Effektstärke (d=2.03) AHDS zwischen betroffenen und nicht-betroffenen Kindern. Somit lassen die Befunde auf zufriedenstellende psychometrische Kennwerte und Erklärungsgüte schließen. Familiäre Anforderungs- und Funktionsbereiche können mit dem FFB abgebildet und symptomkritische Situationen bedingungsanalytisch untersucht werden. In der Diskussion werden die methodischen Einschränkungen problematisiert und weiterführende Forschungsdesiderata benannt. Dazu gehört vor allem die Replikation an größeren Stichproben, die sich auch bezüglich Altersstufen repräsentativ zeigen; auch die Items des FFB bedürfen einer methodenkritischen Betrachtung und Anpassung an aktuelle gesellschaftliche Lebensformen. Die Entwicklung eines sprachfreien Fragebogens sowie eine computergestützte Version wären hilfreiche und an heutige Standards angepasste Varianten.

Croyant que la connaissance approfondie du style attributionnel des élèves inscrits au cégep pourrait être un pas de plus vers la connaissance des processus psycho-cognitifs de l'élève en situations d'apprentissage scolaire et également une piste d'intervention pour réduire les abandons et les échecs au collégial, nous avons élaboré et validé un questionnaire étudiant les attributions causales reliées à des situations scolaires, le QACSS. La présente recherche correspond à la première étape d'une étude portant sur le style attributionnel des cégépiens. Nous basant sur la théorie des attributions de Weiner et sur l'étude expérimentale de l'ASQ (Attributionnal Style Questionnaire) de Peterson et al. (1982), nous avons élaboré et procédé à l'expérimentation du QACSS auprès de 317 élèves de niveau collégial, inscrits dans divers programmes collégiaux, au secteur régulier. Nous présentons et analysons, dans ce rapport, les résultats obtenus lors de notre étude de validité et de fidélité et nous discutons des principales questions surgies lors de cette analyse. En conclusion, nous affirmons que le QACSS peut être considéré comme un outil fiable de la mesure des attributions causales reliées aux situations scolaires.
