Immunisation against M. tuberculosis with current available BCG vaccine lacks efficacy in preventing adult pulmonary tuberculosis. Targeting nasal mucosa is an attractive option for a more effective immunization. The delivery of BCG via the intranasal route involves overcoming barriers such as crossing the physical barrier imposed by the mucus layer and ciliar remotion, cellular uptake and intracellular trafficking by antigen presenting cells. Due to its biodegradable, immunogenic and mucoadhesive properties, chitosan particulate delivery systems can act both as vaccine carrier and adjuvant, improving the elicited immune response. In this study, different combinations of Chitosan/Alginate/TPP microparticles with BCG were produced as vaccine systems. The developed microparticle system successfully modulates BCG surface physicochemical properties and promotes effective intracellular uptake by human macrophage cell lines Preliminary immune responses were evaluated after s.c. and intranasal immunisation of BALB/c mice. BCG vaccination successfully stimulated the segregation of IgG2a and IgG1, where intranasal immunisation with chitosan/alginate particulate system efficiently elicited a more equilibrated cellular/humoral immune response.

Farmers are occupationally exposed to many respiratory hazards at work and display higher rates of asthma and respiratory symptoms than other workers. Dust is one of the components present in poultry production that increases risk of adverse respiratory disease occurrence. Dust originates from poultry residues, molds, and feathers and is biologically active as it contains microorganisms. Exposure to dust is known to produce a variety of clinical responses, including asthma, chronic bronchitis, chronic airways obstructive disease (COPD), allergic alveolitis, and organic dust toxic syndrome (ODTS). A study was developed to determine particle contamination in seven poultry farms and correlate this with prevalence rate of respiratory defects and record by means of a questionnaire the presence of clinical symptoms associated with asthma and other allergy diseases by European Community Respiratory Health Survey. Poultry farm dust contamination was found to contain higher concentrations of particulate matter (PM) PM5 and PM10. Prevalence rate of obstructive pulmonary disorders was higher in individuals with longer exposure regardless of smoking status. In addition, a high prevalence for asthmatic (42.5%) and nasal (51.1%) symptoms was noted in poultry workers. Data thus show that poultry farm workers are more prone to suffer from respiratory ailments and this may be attributed to higher concentrations of PM found in the dust. Intervention programs aimed at reducing exposure to dust will ameliorate occupational working conditions and enhance the health of workers.

Attenuated Mycobacterium bovis bacillus Calmette-Guérin (BCG) is the only currently available vaccine against tuberculosis. It is highly effective in pre-exposure immunisation against TB in children when administered by subcutaneous route to newborns. However, it does not provide permanent protection in adults. In this work, polymeric chitosan-alginate microparticles have been evaluated as potential nasal delivery systems and mucosal adjuvants for live attenuated BCG. Chitosan (CS) has been employed as adjuvant and mucosal permeation-enhancer, and, together with alginate (ALG), as additive to enhance BCG-loaded microparticles (MPs) cellular uptake in a human monocyte cell line, by particle surface modification. The most suitable particles were used for vaccine formulation and evaluation of immune response following intranasal immunisation of BALB/c mice.

This paper presents work in progress, to develop an efficient and economic way to directly produce Technetium 99metastable (99mTc) using low-energy cyclotrons. Its importance is well established and relates with the increased global trouble in delivering 99mTc to Nuclear Medicine Departments relying on this radioisotope. Since the present delivery strategy has clearly demonstrated its intrinsic limits, our group decided to follow a distinct approach that uses the broad distribution of the low energy cyclotrons and the accessibility of Molybdenum 100 (100Mo) as the Target material. This is indeed an important issue to consider, since the system here presented, named CYCLOTECH, it is not based on the use of Highly Enriched (or even Low Enriched) Uranium 235 (235U), so entirely complying with the actual international trends and directives concerning the use of this potential highly critical material. The production technique is based on the nuclear reaction 100Mo (p,2n) 99mTc whose production yields have already been documented. Until this moment two Patent requests have already been submitted (the first at the INPI, in Portugal, and the second at the USPTO, in the USA); others are being prepared for submission on a near future. The object of the CYCLOTECH system is to present 99mTc to Nuclear Medicine radiopharmacists in a routine, reliable and efficient manner that, remaining always flexible, entirely blends with established protocols. To facilitate workflow and Radiation Protection measures, it has been developed a Target Station that can be installed on most of the existing PET cyclotrons and that will tolerate up to 400 μA of beam by allowing the beam to strike the Target material at an adequately oblique angle. The Target Station permits the remote and automatic loading and discharge of the Targets from a carriage of 10 Target bodies. On other hand, several methods of Target material deposition and Target substrates are presented. The object was to create a cost effective means of depositing and intermediate the target material thickness (25 - 100μm) with a minimum of loss on a substrate that is able to easily transport the heat associated with high beam currents. Finally, the separation techniques presented are a combination of both physical and column chemistry. The object was to extract and deliver 99mTc in the identical form now in use in radiopharmacies worldwide. In addition, the Target material is recovered and can be recycled.

Background: The aim was to evaluate the presence of metabolic bone disease (MBD) in patients with Crohn’s disease (CD) and to identify potential etiologic factors. Methods: The case–control study included 99 patients with CD and 56 controls with a similar age and gender distribution. Both groups had dual-energy x-ray absorptionmetry and a nutritional evaluation. Single nucleotide polymorphisms at the IL1, TNF-a, LTa, and IL-6 genes were analyzed in patients only. Statistical analysis was performed using SPSS software. Results: The prevalence of MBD was significantly higher in patients (P ¼ 0.006). CD patients with osteoporosis were older (P < 0.005), small bowel involvement and surgical resections were more frequent (P < 0.005), they more often exhibited a penetrating or stricturing phenotype (P < 0.05), duration of disease over 15 years (P < 0.005), and body mass index (BMI) under 18.5 kg/m2 (P < 0.01) were more often found. No association was found with steroid use. Patients with a Z-score < 2.0 more frequently had chronic active disease (P < 0.05). With regard to diet, low vitamin K intake was more frequent (P ¼ 0.03) and intake of total, monounsaturated, and polyunsaturated fat was higher in patients with Z-score < 2.0 (P < 0.05). With respect to genetics, carriage of the polymorphic allele for LTa252 A/G was associated with a higher risk of osteoporosis (P ¼ 0.02). Regression analysis showed that age over 40 years, chronic active disease, and previous colonic resections were independently associated with the risk of developing MBD. Conclusions: The prevalence of MBD was significantly higher in CD patients. Besides the usual risk factors, we observed that factors related to chronic active and long-lasting disease increased the risk of MBD.

This study compared the ground reaction forces (GRF) and plantar pressures between unloaded and occasional loaded gait. The GRF and plantar pressures of 60 participants were recorded during unloaded gait and occasional loaded gait (wearing a backpack that raised their body mass index to 30); this load criterion was adopted because is considered potentially harmful in permanent loaded gait (obese people). The results indicate an overall increase (absolute values) of GRF and plantar pressures during occasional loaded gait (p < 0.05); also, higher normalized (by total weight) values in the medial midfoot and toes, and lower values in the lateral rearfoot region were observed. During loaded gait the magnitude of the vertical GRF (impact and thrust maximum) decreased and the shear forces increased more than did the proportion of the load (normalized values). These data suggest a different pattern of GRF and plantar pressure distribution during occasional loaded compared to unloaded gait.

Foram realizadas culturas bacteriológicas a partir de material colhido na boca e nariz entre 38 pacientes indígenas Suruí e 58 Karitiana, no Estado de Rondônia. Entre os Suruí, foi isolado S. aureus em 10 (26,3%) indivíduos e S. epidermidis em 25 (65,8%). Na população Karitiana, S. aureus foi isolado em 21 (36,2%) pacientes e S. epidermidis em 42 (72,4%). Testes de sensibilidade as drogas foram realizados com todas as cepas de S. aureus isoladas. É discutido o papel de portadores inaparentes desta bactéria nas cavidades oral e nasal, assim como alguns aspectos relacionados à sua transmissão e prevalência de piodermites entre populações indígenas.

Next-generation vaccines for tuberculosis should be designed to prevent the infection and to achieve sterile eradication of Mycobacterium tuberculosis. Mucosal vaccination is a needle-free vaccine strategy that provides protective immunity against pathogenic bacteria and viruses in both mucosal and systemic compartments, being a promising alternative to current tuberculosis vaccines. Micro and nanoparticles have shown great potential as delivery systems for mucosal vaccines. In this review, the immunological principles underlying mucosal vaccine development will be discussed, and the application of mucosal adjuvants and delivery systems to the enhancement of protective immune responses at mucosal surfaces will be reviewed, in particular those envisioned for oral and nasal routes of administration. An overview of the essential vaccine candidates for tuberculosis in clinical trials will be provided, with special emphasis on the potential different antigens and immunization regimens.

O objectivo principal dos estudos descritos nesta dissertação foi descrever a anatomia cirúrgica da região olfactiva, determinar a área de distribuição da mucosa olfactiva nas fossas nasais e estudar as células estaminais nela presentes para fundamentar e desenvolver uma técnica cirúrgica destinada à sua colheita por via transnasal endoscópica. O objectivo secundário foi avaliar a exequibilidade, segurança e eficácia da utilização da mucosa olfactiva na reparação das lesões traumáticas crónicas e severas da medula espinal. As investigações incluíram a dissecção endoscópica e estudos morfométricos da região olfactiva de cadáveres recentes, o exame histológico de especímenes de um banco de peças anatómicas da região olfactiva e a cultura de células estaminais olfactivas obtidas a partir de amostras de indivíduos sem patologia naso-sinusal. Realizaram-se ainda estudos clínicos experimentais nos quais se transplantou a mucosa olfactiva colhida das fossas nasais na medula espinal de doentes com lesões traumáticas da medula espinal. Os estudos foram realizados num hospital de nível terciário com afiliação universitária, o Hospital de Egas Moniz do Centro Hospitalar de Lisboa Ocidental: em serviços clínicos (Serviço de Otorrinolaringologia) ou em unidades vocacionadas para a investigação (Unidade de Microcirurgia, Unidade de Neuropatologia). Parte dos estudos foram ainda realizados em locais externos à instituição, como o Serviço de Patologia Forense da Delegação de Lisboa do Instituto de Medicina Legal e a ECBio, I&D em Biotecnologia, S.A. Demonstrou-se que a região olfactiva pode ser abordada sistematicamente por técnica endoscópica. Demonstrou-se a área de distribuição da mucosa olfactiva nas fossas nasais e concebeu-se uma técnica para sua a colheita. Isolaram-se e quantificaram-se as células estaminais da mucosa olfactiva e colaborou-se na invenção de um método destinado à sua cultura e proliferação, que poderá ter valor na utilização das células cultivadas em outras patologias. Demonstrou-se ainda que, no tratamento dos doentes com lesões traumáticas crónicas da medula espinal, a transplantação de mucosa olfactiva autóloga é realizável, razoavelmente segura e potencialmente benéfica

RESUMO Introdução: a bronquiolite aguda afecta 30 a 60% das crianças saudáveis até ao terceiro aniversário. Destas, 50% evoluem para sibilância recorrente, com consequências pessoais, familiares e sociais relevantes. Objectivos: identificação de parâmetros clínicos e laboratoriais associados à sibilância recorrente em crianças saudáveis. A detecção da actividade local de quatro citocinas intervenientes nas vias imunológicas Th1 ou Th2 permitiu, gualmente, o estudo do respectivo comportamento em função dos referidos parâmetros. Material e Métodos: estudo prospectivo de coorte, efectuado no serviço de urgência pediátrica do HSFX no Inverno 2004/2005. Recrutada amostra de conveniência de 188 lactentes de idade inferior a 24 meses e diagnóstico de bronquiolite aguda. Excluídos portadores de doença crónica, ex-prematuros e os doentes sujeitos a corticoterapia nas duas semanas anteriores. A todos foi feito questionário padronizado, exame clínico e estratificação da gravidade através de índice validado. Na mucosa nasal, foram pesquisados Ag víricos e estudada a expressão génica de quatro citocinas - representativas da via Th1 (IFN-g e IL-12) ou Th2 (IL-4 e IL-13). Esta determinação foi efectuada por RT-PCR, no Laboratório de Biologia Molecular do Centro de Histocompatibilidade do Sul. Durante o seguimento, em consulta de pneumologia pediátrica até à idade de 36 meses, foram excluídos os seguintes casos: patologia anatómica ou funcional que justificasse sibilância recorrente, valores totais de IgE elevados, Phadiatop® positivo ou défice de IgG, IgA ou IgM. Os resultados foram estudados por estatística descritiva, análise univariada e multivariada. Foi considerado com significado estatístico, em todas as análises, o valor de p < 0,05. Resultados: das 188 crianças recrutadas, foram excluídas 36 (principalmente por identificação de outras causas de sibilância recorrente). Da amostra final de 152 doentes, 76 (50%) foram considerados sibilantes e 76 não sibilantes. Os seguintes parâmetros à entrada relacioram-se com a evolução para sibilância recorrente: idade inferior a 6 meses (OR: 3,8; IC95% 1,8-9,1); exposição ao fumo de tabaco na vida intrauterina (OR: 2,5; IC95% 1,2-5,2) ou após o nascimento (OR: 2,4; IC95% 1,0-5,8); atopia materna (OR: 4,0; IC95% 1,5-10,3); maior gravidade (OR: 1,2; IC95% 1,1-1,4); apirexia (OR: 4,7; IC95% 1,8-11,8); identificação de vírus (OR: 3,5; IC95% 1,4-8,7); predomínio imunitário de tipo Th2 em lactentes com idade superior a seis meses (OR:1,3; IC95% 1,0-1,7). Verificou-se predomínio Th1 na mucosa nasal das crianças com irmãos ou que frequentavam o infantário (OR: 3,1; IC95% 1,4-6,5); nas amamentadas (OR: 1,2; IC95% 1,1-1,4) e naquelas em que se identificaram Ag víricos (OR: 4,0; IC95% 1,3-11,9). O contacto com fumo de tabaco no domicílio foi o determinante mais importante de predomínio Th2 (OR: 2,3; IC95% 1,1-4,7). Discussão e Conclusões: Foram considerados factores de risco para sibilância recorrente em lactentes saudáveis: idade inferior no momento do primeiro episódio de bronquiolite; exposição passiva (antenatal ou pós-natal) ao fumo de tabaco;antecedentes maternos de atopia; primeiro episódio com maior gravidade, sem febre e com dentificação de Ag víricos nas secreções nasais. Os resultados do presente estudo reforçam as recomendações usuais de aleitamento materno, limitação de contactos sociais em lactentes e evicção tabágica durante a gravidez e após o nascimento.
