The physical education curriculum stresses the importance of improving students’ key motor skills. This idea agrees with the contributions of research into motor development concerning the important role these skills play in child development. In order to facilitate this improvement is essential to know the different basic aspects of how these key motor skills evolve as well as the essential factors related to the characteristics and conditions of educational practice

From the areas of motor learning and the teaching of Physical Education we promote coherent methodological orientationswith the present curricular guide that are focused in such a way so that the student is able to act independently. In thissense we may consider the problem situations as a good example for methodological proposals. So as to determine its useand adequateness for Physical Education it is conveniente to analyse the characteristics of the motor tasks that areproposed as well as the features of the teaching intervention in such a way as to ease the adquisition and development ofthe motor competence of the students

In front of a moment of change in the education, with the entry into force of the LOE (Ley Orgánica de Educación, 2006), the aim of thisarticle is to face the challenge of trying to answer to the need to make concrete learning agreements with the new curriculum based on competences.The present article arises from the interest of being able to offer an approach that facilitates the exposition to show the didactic units from the areaof physical education in relation with the pedagogic principles of the LOE. Departing from this commitment, let’s sense beforehand an offer of thecontributions of the Physical Education to the Basic Competence to learn to learn which entails to develop some capacities in fuction of knowing todo skills

There is a continuous need for antibiotics, mainly with new mechanisms of action, since infectious diseases represent the second major cause of death in the world and bacteria resistance levels are high. This review describes the contribution of microbial natural products for the development of the major antibiotic classes, the mechanisms of action of current antibiotics, some modern approaches involving genetic tools for the discovery and development of new antibiotics from microbial products and antibiotics in clinical trials.

Cancer is a multi-factorial disease linked with different initiating causes, cofactors and promoters, and several types of cellular damage. Advancing knowledge on the cellular and molecular biology of the processes that regulate cell proliferation, cell differentiation and cellular responses to external signals, has provided a wealth of information about the cancer cell and how it differs from a normal one. These findings make available a number of potential targets for new therapeutic approaches. The Medicinal Chemistry artwork performed so far in the development of selective and potent adenosine receptor A3 ligands, a current cancer target, will be highlighted in this work.

Adenocarcinomas of the cardia and gastroesophageal junction are peculiar entities with three different origins, which differ somewhat from other adenocarcinomas of the stomach in their clinical presentation and pathogenesis, and have a poorer prognosis. In this article the authors reviewed definitions, incidence and epidemiology, etiologic factors, genetic implications, clinical presentation, diagnosis, staging and treatment, with emphasis on the surgical approach, discussing the current management of these cancers. The prognostic factors related specifically to the cardia cancers are: esophageal invasion greater than 3cm, microscopic residual tumor and wall penetration (>T2). Preoperative workup should include computed tomography, and endoscopic ultrasonography and laparoscopy when available. Preoperative recognition of T3/ T4/N2 lesions should indicate inclusion in neo-adjuvant protocols whenever possible. The authors present the results of 46 resected cases of adenocarcinomas of the cardia and GE junction of the Instituto Nacional do Câncer- Brazil (1981-1995). Cure was intended in 29 and palliation in 17 patients. The most common type of resection was total gastrectomy with abdominal esophagectomy (28 cases). Morbidity (major and minor) occurred in 50% of the patients. The main causes were of respiratory origin and fistulas (19.6% each). Death occurred in 44% of the patients with fistula. Postoperative death until the 30th day occurred in 17.24% of the curative cases and in 23.52% of the palliative ones. The median survival time was 68.5 months for stage I, 25 months for stage II, 31 months for stage III and 12.5 months for stage IV diseases. The median survival time was 8 months for palliation and 28.5 months for cure. No long-term survival was obtained with the palliative group, whereas 25% survived five years of more in the curative group. The authors conclude that the surgical approach should be the one the surgeon feels more comfortable with. Complete removal of the disease proved by frozen section, splenectomy and D2 lymphadenectomy should be the standard therapy with curative intent.

The papiloduodenectomy is a procedure that, although accomplished rarely, has its validity in selected cases of papillary tumors. It is included in this group the patients with important diseases that doesn't have conditions of supporting the surgery of Whipple, the standard procedure for these pathologies. The authors review its experience with four patient carriers of malign neoplasia of the duodenal papila submitted to papiloduodenectomy. All the patients came jaundiced to the admission. The age varied of 62 to 82 years. The ultrassonography, the computed tomography and the endoscopy with biopsy were used for diagnosis and staging, and all the patients presented with initial stages (TI or T2). Two patients were classified as ASA3 and the other two as ASA4, according to the American Society of Anesthesiology. All the diagnoses were proven by the pathological exam of the surgical piece. It is ended that this procedure facilitates to obtain appropriate margins and satisfactory results when indicated of appropriate way.

The pectus excavatum treatment has two different approaches: non-surgical techniques (modified dynamic thoracic compressor, exercises and the vacuum bell) or surgical techniques (silastic or solid silicone implant, open surgical repair like sternochondroplasty and minimally invasive repair). The introduction of Nuss procedure improved the pectus excavatum treatment, but its low acceptance was due to the high complication rate (e.g. cardiac perfuration). The thoracoscopy use for bar mediastinal passage reduced the complication rate. In comparison with sternochondroplasty, the Nuss procedure has smaller incision, less blood loss and less operative time. However, it has more reoperations, complications, longer hospital stay and more readmission rates, more time of thoracic epidural catheter for postoperative analgesia and more need for analgesic after being discharged. Although Nuss procedure has been used in children, patients under ten years must be only observed. The Nuss procedure is applicable to moderate or light symmetrical pectus excavatum, without costal protrusion, in young and adolescents patients. Furthermore, the sternochondroplasty is applicable to severe or asymmetric pectus excavatum, with or without inferior costal protrusion. Therefore, Nuss procedure and sternocondroplasty are not antagonistic procedures, and they must be used in accordance with a treatment organogram and the technique choice must be by functional and aesthetic outcome.

OBJETIVO: Avaliar a sobrevida livre de doença nos tumores de hipofaringe submetidos ao tratamento operatório e à radioterapia pós-operatória. MÉTODOS: Estudo retrospectivo de 174 pacientes com distribuição, de acordo com o estádio clínico em: dois casos de estádio clínico I; quatro II; 46 III e 122 IV. Quanto ao gênero, 163 eram masculinos e 11 femininos, com idade média de 56 anos. Todos os casos foram submetidos à faringolaringectomia e realizados 206 esvaziamentos cervicais radicais e 16 seletivos. Cento seis pacientes foram submetidos à radioterapia pós-operatória, com dose média de 58,2 Gy. RESULTADOS: O exame histológico demonstrou dois casos de estádio clínico pT1, 15 pT2, 100 pT3 e 57 pT4. Quanto aos linfonodos, 25 pacientes apresentavam ausência de linfonodos comprometidos pela neoplasia (pN0) e 149 com linfonodos comprometidos pela neoplasia (pN+). A sobrevida livre de doença há cinco anos foi de 40% e a global de 28%. A sobrevida livre de doença há cinco anos foi de 75% no estádio clínico III versus 28% no IV, de acordo com o resultado do exame histológico. CONCLUSÃO: A manifestação inicial do carcinoma epidermóide de hipofaringe ocorre na fase avançada (estádios III e IV), com sobrevida livre de doença a cinco anos superior no estádio clínico III.

OBJETIVO: avaliar por meio de um estudo retrospectivo não randomizado as respostas tumorais à terapêutica neoadjuvante, conforme os achados histopatológicos das peças cirúrgicas dos pacientes operados e tratados por carcinoma espinocelular do terço médio e distal do esôfago. MÉTODOS: Foram incluídos no estudo 97 pacientes assim distribuídos: grupo I 81 (83,5%) submetidos à radioterapia neoadjuvante; e grupo II 16 (16,5%) submetidos à radioterapia e quimioterapia neoadjuvantes. Um terceiro grupo de 26 pacientes submetidos à esofagectomia exclusiva foi utilizado na comparação das complicações pós-operatórias. As características de cada paciente (idade, sexo e raça), o local do tumor, o estadiamento, e a avaliação histológica das modalidades de tratamento foram revisadas e analisadas. A resposta tumoral à terapêutica neoadjuvante foi avaliada com estudos histopatológicos da peça cirúrgica. RESULTADOS: Não houve diferenças estatisticas significativas quanto à cor, sexo, idade, estadiamento e complicações pós-operatórias nos pacientes dos três grupos analisados. Os pacientes submetidos à radioterapia e quimioterapia neoadjuvante apresentaram redução tumoral mais satisfatória, com melhor eficácia local, quando comparado ao grupo submetido apenas a radioterapia neoadjuvante. CONCLUSÃO: o estudo sugere que a radioterapia associada à quimioterapia apresentou maior eficácia local na redução tumoral em comparação com o grupo tratado com radioterapia; além disso, a terapêutica neoadjuvante não elevou as complicações pós-operatórias em comparação aos pacientes submetidos à cirurgia exclusiva.

Gestação gemelar com gêmeo acárdio é um evento raro que se caracteriza pela presença de uma anastomose placentária entre o gêmeo doador, morfologicamente normal, e o gêmeo acárdio, levando à perfusão contra-corrente de um dos gêmeos. As principais complicações encontram-se associadas à prematuridade e insuficiência cardíaca do gêmeo normal secundária à sobrecarga circulatória. Várias modalidades terapêuticas têm sido propostas, mas não há consenso em relação aos resultados observados. A oclusão intra-fetal da artéria umbilical do gêmeo acárdio tem se mostrado um método de baixo custo, fácil execução e alta eficácia no controle da sobrecarga circulatória. Apresentamos dois casos com ablação intra-fetal e boa evolução. Uma gestação trigemelar na qual se realizou a ablação intra-fetal com 29 semanas, evoluindo com parto prematuro e nascimento de um recém-nascido em boas condições. Uma gestação gemelar cuja ablação foi realizada com 31 semanas e que evoluiu com rotura prematura de membranas e parto prematuro com nascimento de uma criança saudável.
