Taxes de reincidència 2010 de justícia juvenil: actualització de la taxa de reincidència dels joves sotmesos a mesures de llibertat vigilada i internament en centre

L’estudi actualitza les taxes de reincidència dels menors sotmesos a una mesura d’internament o de llibertat vigilada que van ser publicades a la recerca “La reincidència en el delicte en la justícia de menors” finalitzada l’any 2005 i que van iniciar la sèrie. Aquest estudi ja és el sisè del mateix tipus i, en aquest cas, segueix els joves que van finalitzar una mesura de llibertat vigilada o d’internament l’any 2007, i els segueix fins el 31 de desembre de 2010 amb l’objectiu de saber si han comès un nou delicte que hagi estat detectat per la Xarxa d’execució penal, tant de joves com d’adults. S’ha estudiat tota la població de joves desinternats de centres, que per l’any 2007 foren 213 subjectes. En el cas de llibertat vigilada del total de joves que finalitzaren mesura l’any 2007 (N=886), s’ha fet una mostra de 493 subjectes (interval de confiança:95,5%; marge d’error ±3,04; p=q=50) Els resultats en llibertat vigilada apunten un lleuger augment en la taxa de reincidència (29,6%) respecte l’any anterior. Aquests darrers augments s’expliquen en part per la incorporació al Codi Penal dels delictes de trànsit, on els joves han fet un creixement espectacular que no hi era als anys anteriors. En internament la taxa de reincidència ha tornat a pujar fins el 58,7%. L’estudi permet comparar de forma seriada sis anys d’evolució de la taxa de reincidència juvenil després de la posada en marxa de la Llei Orgànica 5/2000, de 12 de gener, reguladora de la responsabilitat penal dels menors (LORPM).

Analyse des données du Dispositif Cantonal d'Indication et de Suivi pour les personnes Toxicodépendantes (DCIST) : 2010-2012

Le Dispositif Cantonal d'Indication et de Suivi pour les personnes Toxicodépendantes (DCIST) est un outil central de la politique de prévention et de lutte contre la toxicodépendance dans le canton de Vaud a. Ses principaux objectifs sont : Améliorer l'adéquation entre l'offre de prestations et les besoins des usagers; Améliorer la coordination des prestations sociales et médicales en développant le travail en réseau, aussi bien dans la prise en charge de l'usager que dans son suivi et son maintien dans le réseau social et médical. Le DCIST a donc pour but de renforcer le rôle du canton de Vaud dans le pilotage et la planification des prestations dans le domaine de l'aide aux personnes dépendantes et s'adresse à toute personne toxicodépendante, avec ou sans problématique d'alcool associée, ayant un projet de prise en charge dans un établissement socio-éducatif (ESE). Le passage par ce dispositif est également un préalable nécessaire à l'octroi d'une garantie financière par le SPAS dans le cadre des séjours résidentiels.

Neuroendocrine Tumors (NETs): A population-based study of incidence and survival in Girona province, 1994-2004

Els tumors neuroendocrins (TNEs) són un grup de neoplàsies poc freqüents i heterogènies i amb un ampli espectre d’agressivitat. Hi ha molt poca informació epidemiològica a nivell mundial, l’objectiu d’aquest estudi ha estat el de reportar-ne les dades a la província de Girona. Hem inclòs tots els codis de la ICD-O3 que codifiquen un TNE, període 1994-2002. Hem identificat 698 tumors. Es reporten les dades tant de la casuística com les taxes d’incidència i supervivència de cada TNE per separat. Els resultats són consistents amb les publicacions europees. El fet d’haver reportat la incidència i la supervivència dels TNEs a Girona contribueix a un millor coneixement d’aquestes neoplàsies.

Concomitant cisplatin and hyperfractionated radiotherapy in locally advanced head and neck cancer: 10-year follow-up of a randomized phase III trial (SAKK 10/94).

Purpose: To compare the long-term outcome of treatment with concomitant cisplatin and hyperfractionated radiotherapy versus treatment with hyperfractionated radiotherapy alone in patients with locally advanced head and neck cancer.Methods and Materials: From July 1994 to July 2000, a total of 224 patients with squamous cell carcinoma of the head and neck were randomized to receive either hyperfractionated radiotherapy alone (median total dose, 74.4 Gy; 1.2 Gy twice daily; 5 days per week) or the same radiotherapy combined with two cycles of cisplatin (20 mg/m(2) for 5 consecutive days during weeks 1 and 5). The primary endpoint was the time to any treatment failure; secondary endpoints were locoregional failure, metastatic failure, overall survival, and late toxicity assessed according to Radiation Therapy Oncology Group criteria.Results: Median follow-up was 9.5 years (range, 0.1-15.4 years). Median time to any treatment failure was not significantly different between treatment arms (hazard ratio [HR], 1.2 [95% confidence interval [CM 0.9-1.7; p = 0.17]). Rates of locoregional failure-free survival (HR, 1.5 [95% CI, 1.1-2.1;p = 0.021), distant metastasis-free survival (HR, 1.6 [95% CI, 1.1-2.5; p = 0.021), and cancer-specific survival (HR, 1.6 [95% CI, 1.0-2.5;p = 0.03]) were significantly improved in the combined-treatment arm, with no difference in major late toxicity between treatment arms. However, overall survival was not significantly different (HR, 1.3 [95% CI, 0.9-1.8; p = 0.11]).Conclusions: After long-term follow-up, combined-treatment with cisplatin and hyperfractionated radiotherapy maintained improved rates of locoregional control, distant metastasis-free survival, and cancer-specific survival compared to that of hyperfractionated radiotherapy alone, with no difference in major late toxicity. Estudio clínico, observacional, de una nueva modalidad ventilatoria; Ventilación Asistida Proporcional. Hospital de la Santa Creu i Sant Pau, Barcelona, 2010

La ventilació assistida proporcional és una modalitat en la que el respirador genera una assistència proporcional a l’esforç muscular del pacient. En aquest estudi s’han inclòs vuit malalts en “weaning” de la ventilació mecànica. Es descriu la tolerància clínica i es quantifica l’esforç muscular dels pacients. L’esforç muscular va estar sempre dintre de límits predefinits de confort. Concloem que la ventilació assistida proporcional dóna una ventilació mecànica similar a altres modalitats i és probablement més fisiològica.

Changing Cardiovascular Health: National Cardiovascular Health Policy 2010 - 2019

This new cardiovascular policy, Changing Cardiovascular Health covering the period 2010-2019, comes a decade after the first national cardiovascular health strategy, entitled Building Healthier Hearts, was published by the Department of Health in 1999. The new policy is timely given the pace of scientific discovery and related changes in medical practice, changes in health service structures, and patterns and influences on population health behaviours in Ireland over the decade. Cardiovascular health and its maintenance is a microcosm of health more generally. A policy that can improve cardiovascular health and cardiovascular disease management will have beneficial effects for the whole healthcare system and population.Download this document
