Treatment of posttraumatic cubitus varus in children and adolescents. Supracondylar humeral osteotomy using radial external fixation

OBJECTIVE Precise adaptable fixation of a supracondylar humerus osteotomy with a radial/lateral external fixator to correct posttraumatic cubitus varus. INDICATIONS Acquired, posttraumatic cubitus varus as a result of a malhealed and unsatisfactorily treated supracondylar humerus fracture. Idiopathic, congenital cubitus varus (very seldom) if the child (independent of age and after complete healing) is cosmetically impaired; stability of the elbow is reduced due to malalignment (hyperextension); secondary problems and pain (e. g., irritation of the ulnar nerve) are expected or already exist; or there is an explicit wish of the child/parents (relative indication). CONTRAINDICATIONS In principle there are no contraindications provided that the indication criteria are filled. The common argument of age does not represent a contraindication in our opinion, since angular remodeling at the distal end of the humerus is practically nonexistent. SURGICAL TECHNIQUE Basically, the surgical technique of the radial external fixator is used as previously described for stabilization of complex supracondylar humeral fractures. With the patient in supine position, the arm is placed freely on an arm table. Using a 4-5 cm long skin incision along the radial, supracondylar, the extracapsular part of the distal humerus is prepared, whereby great caution regarding the radial nerve is advised. In contrast to the procedure used in radial external fixation for supracondylar humeral fracture treatment, two Schanz screws are always fixed in each fragment at a distance of 1.5-2 cm. The osteotomy must allow the fragment to freely move in all directions. The proximal and distal two Schanz screws are then connected with short 4 mm carbon or stainless steel rods. These two rods are connected with each other over another rod using the tub-to-tub technique. Now the preliminary correction according the clinical situation can be performed and the clamps are tightened. Anatomical axis and function are checked. If these are radiologically and clinically perfect, all clamps are definitively tightened; if the alignment or the function is not perfect, then further adjustments can be made. POSTOPERATIVE MANAGEMENT Due to the excellent stability, further immobilization not necessary. Immediate functional follow-up treatment performed according to pain. RESULTS Adequate healing is usually expected within 6 weeks. At this time the external fixator can be removed in the fracture clinic. Because the whole operation is performed in an extraarticular manner and the mobility of the elbow is not affected, deterioration of function has never been observed. Also regarding the cosmetic/anatomical situation, good results are expected because they were already achieved intraoperatively.

Advances and challenges in skeletal muscle angiogenesis.

The role of capillaries is to serve as the interface for delivery of oxygen and removal of metabolites to/from tissues. During the past decade there has been a proliferation of studies that have advanced our understanding of angiogenesis demonstrating tissue capillary supply is under strict control during health, but poorly controlled in disease - resulting in either excessive capillary growth (pathological angiogenesis) or losses in capillarity (rarefaction). Given that skeletal muscle comprises nearly 40% of body mass in humans, skeletal muscle capillary density has a significant impact on metabolism, endocrine function, and locomotion, and is tightly regulated at many different levels. Skeletal muscle is also high adaptable, and thus one of the few organ systems which can be experimentally manipulated (e.g. by exercise) to study physiologic regulation of angiogenesis. This review will focus on 1) the methodological concerns that have arisen in determining skeletal muscle capillarity, and 2) highlight the concepts that are reshaping our understanding of the angio-adaptation process. We also summarize selected new findings (physical influences, molecular changes and ultrastructural rearrangement of capillaries) that identify areas of future research with the greatest potential to expand our understanding of how angiogenesis is normally regulated, and that may also help to better understand conditions of uncontrolled (pathologic) angiogenesis.

IN VITRO INDUCTION OF XENOIMMUNE RESPONSES TO LIPOSOMES CONTAINING HUMAN COLON TUMOR ANTIGENS

Liposomes prepared with human LS174T colon tumor cell membranes induce specific primary and secondary xenogeneic immune responses in BALB/c splenocytes in vitro. The multilamellar vesicular liposomes were prepared by adding sonicated membrane fragments in 8 mM CaCl(,2) to a dried lipid film. Cytoxic splenocytes generated in vivo exhibited specificity for the LS174T cell; liposomes elicited higher levels of cytotoxicity than did membranes (P < 0.01). Secondary blastogenic responses elicited in in vivo-primed spleen cells by liposomes also produced a significantly greater (P < 0.005) response than membranes. Subsequently, in vitro induction of primary blastogenic and cytotoxic responses by liposomes were accomplished and revealed similar kinetics to that of whole LS174T cell immunogens. Specificity of the in vitro-primed spleen cells was clearly demonstrated (P < 0.01) on a variety of human tumor cells using both the primed lymphocyte and cell-mediated cytotoxicity assays. The results of competitive inhibition tests with autologous lymphoblasts demonstrated that 30% of the cytotoxic activity was directed against lymphocyte antigens.^ The adjuvant effect of liposomes was shown to be mediated primarily by tumor antigens exposed on the outer surface of liposomes. Trypsinization of the liposomes which eliminated 96% of the surface protein reduced the ability of liposomes to induce cytotoxic splenocytes. The generation of cytolytic activity with liposomes of increasing protein concentration showed that while 10 (mu)g protein was threshold, 100 (mu)g protein induced maximal responses. In addition, membrane fluidity studies revealed that rigid liposomes were significantly (P < 0.05) more efficacious than fluid liposomes in inducing cytotoxicity.^ The induction of the primary response required the presence of nonadherent splenocytes bearing the Thy-1, Lyt-1, and Lyt-2 surface markers. The role of a Lyt-123 subpopulation was suggested by the inability of both the Lyt-1 and Lyt-2 depleted populations to completely restore the cytolytic levels to normal. In addition, the interaction of I-A('+) spleen adherent cells with liposomes for at least 8 hours was required to generate maximal cytotoxic activity. The phenotype of the cytotoxic effector was Thy-1('+), Lyt-2('+), and I-A('d-).^ Incorporation of tumor antigens into liposomes has thus enabled primary immunization in vitro to human colon cancer antigens and may afford an adaptable means to evaluate and to select specific immune responses, as well as to identify colon tumor-specific determinants.^

A Su Salud Injury Prevention expansion: Analysis of injury prevention perceptions in a Hispanic community

Unintentional injury is the leading cause of death for American ages one to 44 and is ranked in the top ten causes of death for all age groups (CDC, 2006a). A Su Salud Injury Prevention was developed to address injury prevention awareness and education. The program is a mass media education campaign that uses role models, mass media, and community outreach to prevent injury. In 2009, University Health System (UHS) expanded the program. Baseline data were collected from 426 residents in targeted neighborhoods northwest of downtown San Antonio to support the expansion. The purpose of this study was to explore injury perceptions, knowledge, and behaviors of adults living in the expansion area, and define the predominant factors associated with these perceptions. A secondary aim was to assess community awareness and willingness to participate in the program.^ Survey results showed motor vehicle crashes (MVC), falls, drinking and driving, and guns and assaults were considered the most serious injures for adults. The most serious child injuries were MVC, abuse and neglect, falls, and head injuries. Residents were knowledgeable of state seatbelt policy, and over 90% responded as compliant for seatbelt and child car seat use. Most were knowledgeable about drinking and driving state policy and negative outcomes. However, 70% of those reporting driving under the influence of alcohol within the last year engaged in repeat high risk behavior. Men and residents under the age of 55 were more likely to engage in repeat drinking and driving (OR= 3.6, 7.0 respectively). Residents consider injury prevention an important issue, and have interest in a local injury prevention program. Younger women are the most likely to participate in a local program as potential role models and volunteers.^ Results from the study are summarized into an injury prevention and demographic profile of the community that will be used to develop tailored injury prevention messages to create a more effective program, and support program coordinators in effective community engagement. Results will also be used as a comparative basis for future evaluation of a behavioral injury prevention program focused on a predominantly Mexican-American community.^

Strain-gauge, brushless torque meter

A torque meter comprising hollow-keyed, input and output female shafts adapted to receive the male shafts of the power source and machine respectively. Each shaft has a circular flange whose face is perpendicular to the center line of the shafts. Each flange has a plurality of equally spaced cylindrical recesses machined into the inside face thereto adapted to receive conical inserts therein. Balls are contained by the conical inserts and transmit the rotational movement from the input to the output shaft. A stationary housing extends around the input and output shaft and has a transducer shell secured thereto. When force is applied to the input shaft to cause movement, the balls encounter torsional resistance which causes the balls to roll up the ramps of the conical seat inserts to separate the two torque flanges. The force transmitted through the balls causes rotation to the output shaft and produces tension to the stationary transducer shell. The stationary transducer shell is instrumented with semi-conductor strain gauges.

Estimación de la evapotranspiración real regional a partir de datos satelitales

El objetivo del presente trabajo fue determinar la Evapotranspiración real (ETR) a nivel regional utilizando la información del satélite meteorológico NOAA-AVHRR y comparar los resultados obtenidos con los calculados a partir de un modelo de simulación de balance hídrico. Para la estimación de la ETR se analizaron 30 imágenes que abarcan el oasis Norte de Mendoza. Con la información de los canales C1 (Visible) y C2 (IRC) se obtuvo el índice verde normalizado (NDVI), a través del cual se siguió la evolución anual de la vegetación y con la correspondiente al Infrarrojo térmico (C4 y C5) se calculó la Temperatura de superficie (Ts) por el método Split - Windows Luego se vinculó la Ts calculada por teledetección con la temperatura del aire (Ta), para finalmente calcular la suma acumulada de las diferencias entre Ts y Ta, conocida como SDD (stress degree day) que permite estimar globalmente las características de stress hídrico a nivel regional. Conociendo (Ts-Ta) se estimó la ETR a partir de la radiación neta y de los coeficientes A y B que se estimaron según las características de la cobertura vegetal, aplicando una relación simplificada a partir del balance de energía, desarrollado por Jackson (1977) y Seguin (1983) según la ecuación: ETR = Rn + A -B ( Ts - Ta ) Posteriormente, se incluyó en los cálculos los valores de Emisividad y se hizo variar el coeficiente B de acuerdo a la ocupación del suelo en cada uno de los polígonos en que fue dividida el área de estudio. En la etapa final se compararon estadísticamente los datos de ETR estimados por los distintos métodos con los simulados por el modelo y se obtuvo como conclusión final que: la estimación de la ETR a nivel regional mediante datos satelitales, se adapta muy bien a la mayoría de los casos y es sencilla de calcular, por lo que la metodología desarrollada es fácilmente extrapolable a otros oasis de la región.

Visibilidad de los repositorios institucionales argentinos en la Web : Indicadores y buenas prácticas

Se aborda el estudio de la evaluación de los repositorios institucionales desde una perspectiva cibermétrica. En primer lugar se presenta la sede online universitaria como un sistema complejo y rico documentalmente, analizable desde un punto de vista tanto sistémico como cuantitativo. Tras ello, se sitúa el repositorio institucional como una sede independiente alojada dentro de la sede académica, con unos objetivos y características propios, y que pueden ser igualmente analizados desde un punto de vista cibermétrico. A continuación se exponen los indicadores cibermétricos más importantes utilizados en el análisis de repositorios institucionales (aplicando su uso a SeDICI, repositorio institucional de la Universidad Nacional de la Plata). Se finaliza con la presentación del Ranking Web de Repositorios del Mundo, en el que se aplican diversos indicadores cibermétricos, analizando la presencia de los repositorios institucionales argentinos en el mismo.

Educación culturalmente responsable : Implicancias teóricas y aplicaciones prácticas

En el marco de la globalización, la tendencia creciente hacia el aula multilingual y multicultural revela la importancia que han adquirido los factores culturales en la educación en lengua extranjera y la educación en general. Más específicamente, la presencia abrumadora de la diversidad cultural en distintos contextos educativos indica la necesidad de comprender qué significa la noción de 'educación culturalmente responsable'. Los educadores de hoy necesitamos el conocimiento, las estrategias y los recursos para atender la complejidad que la integración de la lengua y la cultura requiere en la realidad cotidiana del aula. Este trabajo está dividido en dos partes. La primera parte se centrará en la noción de 'educación culturalmente responsable'. A través de distintas actividades de concientización, se explorará qué significa enseñar una lengua extranjera desde esta perspectiva. La segunda parte ofrecerá una aplicación práctica del concepto, adaptable a distintos contextos1 , utilizando literatura multicultural como punto de partida

Este trabajo se concentra en el papel desempeñado por las autoridades militares desde que se fundó Chascomús, en 1779, y mientras fue asiento de las tropas que defendían la frontera (c. 1815), incluyendo también el contrapunto entre los intereses locales y regionales en la transición del Antiguo Régimen a la independencia
