Dados faltantes são um problema comum em estudos epidemiológicos e, dependendo da forma como ocorrem, as estimativas dos parâmetros de interesse podem estar enviesadas. A literatura aponta algumas técnicas para se lidar com a questão, e, a imputação múltipla vem recebendo destaque nos últimos anos. Esta dissertação apresenta os resultados da utilização da imputação múltipla de dados no contexto do Estudo Pró-Saúde, um estudo longitudinal entre funcionários técnico-administrativos de uma universidade no Rio de Janeiro. No primeiro estudo, após simulação da ocorrência de dados faltantes, imputou-se a variável cor/raça das participantes, e aplicou-se um modelo de análise de sobrevivência previamente estabelecido, tendo como desfecho a história auto-relatada de miomas uterinos. Houve replicação do procedimento (100 vezes) para se determinar a distribuição dos coeficientes e erros-padrão das estimativas da variável de interesse. Apesar da natureza transversal dos dados aqui utilizados (informações da linha de base do Estudo Pró-Saúde, coletadas em 1999 e 2001), buscou-se resgatar a história do seguimento das participantes por meio de seus relatos, criando uma situação na qual a utilização do modelo de riscos proporcionais de Cox era possível. Nos cenários avaliados, a imputação demonstrou resultados satisfatórios, inclusive quando da avaliação de performance realizada. A técnica demonstrou um bom desempenho quando o mecanismo de ocorrência dos dados faltantes era do tipo MAR (Missing At Random) e o percentual de não-resposta era de 10%. Ao se imputar os dados e combinar as estimativas obtidas nos 10 bancos (m=10) gerados, o viés das estimativas era de 0,0011 para a categoria preta e 0,0015 para pardas, corroborando a eficiência da imputação neste cenário. Demais configurações também apresentaram resultados semelhantes. No segundo artigo, desenvolve-se um tutorial para aplicação da imputação múltipla em estudos epidemiológicos, que deverá facilitar a utilização da técnica por pesquisadores brasileiros ainda não familiarizados com o procedimento. São apresentados os passos básicos e decisões necessárias para se imputar um banco de dados, e um dos cenários utilizados no primeiro estudo é apresentado como exemplo de aplicação da técnica. Todas as análises foram conduzidas no programa estatístico R, versão 2.15 e os scripts utilizados são apresentados ao final do texto.

Os miomas uterinos (MU) são considerados os tumores mais comumente encontrados no sistema reprodutor feminino. Este estudo buscou investigar associação entre o ganho de peso na idade adulta e a ocorrência de miomas. Esta relação foi explorada em estudos internacionais na maior parte americanos. No Brasil nenhum estudo havia abordado este tema até o término desta dissertação. Para investigar como a associação entre o ganho de peso na idade adulta afeta o risco de miomas, foi conduzido um estudo transversal referente aos dados da linha de base (1999-2001) do estudo longitudinal Pró-Saúde onde 2594 servidores técnico-administrativos de uma universidade pública do Rio de Janeiro são participantes, entre eles foram selecionadas 1560 mulheres com diagnóstico médico autorrelatado de MU e que tiveram seus pesos aos vinte anos de idade autorrelatados em questionário autopreenchível cujas idades variaram entre 22 a 67 anos. Foi avaliada a mudança de peso desde os 20 anos até o momento da entrada no estudo. As prevalências de miomas encontradas neste grupo foram23.3% (IC95% - 21,2; 25,4), situando-se em posição intermediária quando comparada as prevalências encontradas nos estudos norte-americanos. A maior prevalência de ganho de peso foi encontrada no 4 quintil cujos valores variaram entre ganho maior que 0,7 Kg/ano e menor que 1,1 Kg/ano. A faixa etária com maiores prevalências de diagnóstico médico auto-relatado de miomas ficou compreendida entre 45 a 54 anos (39,1%).Os valores de peso aos 20 anos informados pelas respondentes variaram entre 30,0 e 96,0 kg. O IMC aos 20 anos de idade se situou está dentro da categoria de normalidade entre a maior parte das participantes. Os resultados não sugerem associação entre o ganho de peso na idade adulta e miomas nesta população de estudo

Alguns estudos vêm apontando o hipotireoidismo como um fator de risco para depressão; entretanto, esta associação é ainda controversa. Como objetivo o presente estudo estimou a prevalência de sintomas depressivos numa amostra probabilística de mulheres e investigou se existe uma associação entre níveis de TSH e a presença de sintomas depressivos. Conduzimos um estudo transversal de base populacional onde avaliou-se uma amostra de mulheres com 35 anos ou mais anos de idade, residentes no município do Rio de Janeiro (RJ), que foram entrevistadas e tiveram amostras de sangue coletadas. O desenho de amostra proposto para a pesquisa seguiu um modelo de amostragem probabilística por conglomerado, em três estágios de seleção, tendo por base os setores censitários do município. A função tireoidiana foi medida pelo TSH sérico, a partir das amostras coletadas no domicílio. Para avaliação da presença/ausência de sintomas depressivos utilizou-se um instrumento padronizado e validado (PRIME-MD). Foram também coletados dados sócio demográficos e outras informações de saúde da participante. A prevalência de sintomas depressivos, frequências e associações foram calculadas levando-se em conta o desenho de amostra complexo, utilizando procedimentos específicos do pacote SAS (versão 9.1). Da amostra de 1500 mulheres, 1298 foram entrevistadas, sendo o percentual de não-resposta igual a 13,5%. A amostra final foi composta por 1249 participantes, onde foi encontrada uma prevalência de sintomas depressivos igual a 45,9%. Em relação aos níveis de TSH, 4,8% da amostra apresentaram-se acima do ponto de corte adotado (≥6,0 mUl/ml). Destas, 61,9% apresentaram sintomas depressivos, contra 44,9% entre as mulheres com TSH<6,0 (p=0,04). A análise ajustada mostrou que mulheres com nível de TSH≥6,0 tiveram uma chance duas vezes maior de apresentarem sintomas depressivos do que aquelas com TSH normal (OR=2,04). Ao excluir da análise as participantes que auto-referiram diagnóstico prévio de doença tireoidiana, a associação entre níveis de TSH e sintomas depressivos perdeu a significância. Concluindo, foi observada alta prevalência de sintomas depressivos na amostra, especialmente entre o grupo com níveis elevados de TSH. Os resultados chamam atenção para a importância de se investigar em pacientes deprimidos a comorbidade tireoidiana.

Purpose Encouraging office workers to 'sit less and move more' encompasses two public health priorities. However, there is little evidence on the effectiveness of workplace interventions for reducing sitting, even less about the longer term effects of such interventions and still less on dual-focused interventions. This study assessed the short and mid-term impacts of a workplace web-based intervention (Walk@WorkSpain, W@WS; 2010-11) on self-reported sitting time, step counts and physical risk factors (waist circumference, BMI, blood pressure) for chronic disease. Methods Employees at six Spanish university campuses (n=264; 42 +/- 10 years; 171 female) were randomly assigned by worksite and campus to an Intervention (used W@WS; n=129; 87 female) or a Comparison group (maintained normal behavior; n=135; 84 female). This phased, 19-week program aimed to decrease occupational sitting time through increased incidental movement and short walks. A linear mixed model assessed changes in outcome measures between the baseline, ramping (8 weeks), maintenance (11 weeks) and follow-up (two months) phases for Intervention versus Comparison groups. Results A significant 2 (group) x 2 (program phases) interaction was found for self-reported occupational sitting (F[3]=7.97, p=0.046), daily step counts (F[3]=15.68, p=0.0013) and waist circumference (F[3]=11.67, p=0.0086). The Intervention group decreased minutes of daily occupational sitting while also increasing step counts from baseline (446 +/- 126; 8,862 +/- 2,475) through ramping (+425 +/- 120; 9,345 +/- 2,435), maintenance (+422 +/- 123; 9,638 +/- 3,131) and follow-up (+414 +/- 129; 9,786 +/- 3,205). In the Comparison group, compared to baseline (404 +/- 106), sitting time remained unchanged through ramping and maintenance, but decreased at follow-up (-388 +/- 120), while step counts diminished across all phases. The Intervention group significantly reduced waist circumference by 2.1cms from baseline to follow-up while the Comparison group reduced waist circumference by 1.3cms over the same period. Conclusions W@WSis a feasible and effective evidence-based intervention that can be successfully deployed with sedentary employees to elicit sustained changes on "sitting less and moving more".

This paper summarises the findings of investigations to date in understanding what luxury and premiumness mean to the high-end automotive consumer. Existing writings on luxury and premiumness are considered. An exploratory study was carried out in two countries using 309 respondents and 18 prestige cars. A "stream of consciousness" approach was used to capture respondent's views on a selection of vehicles. The codified transcripts were used to identify key differences between the top and bottom rated vehicles, in terms of the nature and quantity of emotional responses elicited. This paper describes some of these key product differences that were self-reported to impact upon a luxury response. Finally, suggestions are made as to the next steps required for this research. © 2007 ACM.

Designers often assume that their users will have some digital technological prior experience. We examined these levels of prior experience by surveying frequency and ease of technology use with a range of technology products. 362 people participated as part of a UK nationwide larger survey of people's capabilities and characteristics to inform product design. We found that frequency and self-reported ease of use are indeed correlated for all of the products. Furthermore, both frequency and ease of use declined significantly with age for most of the products. In fact, 29% of the over 65s had never or rarely used any of the products, except for digital TV. We conclude that interfaces need to be designed carefully to avoid implicit assumptions about users' previous technology use.

Mammalian studies show that frustration is experienced when goal-directed activity is blocked. Despite frustration's strongly negative role in health, aggression and social relationships, the neural mechanisms are not well understood. To address this we developed a task in which participants were blocked from obtaining a reward, an established method of producing frustration. Levels of experienced frustration were parametrically varied by manipulating the participants' motivation to obtain the reward prior to blocking. This was achieved by varying the participants' proximity to a reward and the amount of effort expended in attempting to acquire it. In experiment 1, we confirmed that proximity and expended effort independently enhanced participants' self-reported desire to obtain the reward, and their self-reported frustration and response vigor (key-press force) following blocking. In experiment 2, we used functional magnetic resonance imaging (fMRI) to show that both proximity and expended effort modulated brain responses to blocked reward in regions implicated in animal models of reactive aggression, including the amygdala, midbrain periaqueductal grey (PAG), insula and prefrontal cortex. Our findings suggest that frustration may serve an energizing function, translating unfulfilled motivation into aggressive-like surges via a cortical, amygdala and PAG network.

To investigate the symptom burden experiences of individuals with inflammatory bowel disease (IBD). An explanatory sequential mixed methods study was conducted. A cross-sectional, correlational survey was first undertaken. Symptom burden was measured using a modified disease specific version of the Memorial Symptom Assessment Scale, which was administered to a consecutive sample of individuals with IBD (n = 247) at an IBD Outpatients department in one urban teaching hospital in Ireland. Disease activity was determined using clinical disease activity indices, which were completed by the consulting physician. A sequential qualitative, descriptive study was then conducted aimed at explaining noteworthy quantitative findings. A criterion-related purposeful sample of seven participants from the quantitative study was recruited. Semi-structured face to face interviews were conducted using an interview guide and data were analysed using content analysis. Findings revealed that participants experienced a median of 10 symptoms during the last week, however as many as 16 symptoms were experienced during active disease. The most burdensome symptoms were lack of energy, bowel urgency, diarrhoea, feeling bloated, flatulence and worry. Total symptom burden was found to be low with a mean score of 0.56 identified out of a possible range from 0 to 4. Participants with active disease (M = 0.81, SD = 0.48; n = 68) had almost double mean total symptom burden scores than participants with inactive disease (M = 0.46, SD = 0.43; n = 166) (p < 0.001). Mean total psychological symptom burden was found to be significantly greater than mean total physical symptom burden (rho = 0.73, n = 247, p < 0.001). Self-reported disease control, gender, number of flare ups in the last two years, and smoking status was found to be significant predictors of total symptom burden, with self-reported disease control identified as the strongest predictor. Qualitative data revealed tiredness, pain, bowel symptoms, worry and fear as being burdensome. Furthermore, symptom burden experiences were described in terms of its impact on restricting aspects of daily activities, which accumulated into restrictions on general life events. Psychological symptom burden was revealed as more problematic than physical symptom burden due to its constant nature, with physical and psychological symptoms described to occur in a cyclical manner. Participants revealed that disease control was evaluated not only in terms of symptoms, but also in terms of their abilities to control the impact of symptoms on their lives. This study highlights the considerable number of symptoms and the most burdensome symptoms experienced by individuals with IBD, both during active and inactive disease. This study has important implications on symptom assessment in terms of the need to encompass both physical and psychological symptoms. In addition, greater attention needs to be placed on psychological aspects of IBD care.

Process guidance supports users to increase their process model understanding, process execution effectiveness as well as efficiency, and process compliance performance. This paper presents a research in progress encompassing our ongoing DSR project on Process Guidance Systems and a field evaluation of the resulting artifact in cooperation with a company. Building on three theory-grounded design principles, a Process Guidance System artifact for the company’s IT service ticketing process is developed, deployed and used. Fol-lowing a multi-method approach, we plan to evaluate the artifact in a longitudinal field study. Thereby, we will not only gather self-reported but also real usage data. This article describes the development of the artifact and discusses an innovative evaluation approach.

This thesis investigated the relationship of explicit (self-report), implicit (IAT) and physiological variables to the placebo effect. The thesis consisted of three main parts. The first collected background data and developed models for two constructs (Optimism and Mindfulness) associated with the placebo effect and implicit attitudes, respectively. The second part of the thesis consisted of the development of an explicit measure of treatment expectancies, and the development of two IATs, one for Optimism and the other for Treatment Credibility. The final portion of the thesis was an experimental study (N=111) which tested these new measures in a sample of healthy volunteers. The primary hypothesis of the thesis, that there would be a relationship between the placebo effect and implicit measures, was not supported. Major findings include an effect of semantic priming on placebo response mediated by condition (Deceptive versus Open Placebo), an unexpected negative relationship between Optimism and self-reported Health, and a physiological relationship between pain ratings and GSR data, which was also mediated by Condition in the experiment. OBJECTIVE: This study compared self-reported subjective life expectancy (i.e., probability of living to age 75) for normal-weight, overweight, and obese weight groups to examine whether individuals are internalizing information about the health risks due to excessive weight. RESEARCH METHODS AND PROCEDURES: Using data from the Health and Retirement Study, a total of 9035 individuals 51 to 61 years old were analyzed by BMI category. The primary outcome measure was individuals' reports about their own expectations of survival to age 75. Absolute and relative risks of survival were compared with published estimates of survival to age 75. RESULTS: Consistently, higher levels of BMI were associated with lower self-estimated survival probabilities. Differences relative to normal weight ranged from 4.9% (p < 0.01) for male nonsmokers to 8.8% (p < 0.001) for female nonsmokers. However, these differences were substantially less than those obtained from published survival curve estimates, suggesting that obese individuals tended to underestimate mortality risks. DISCUSSION: Individuals appeared to underestimate the mortality risks of excessive weight; thus, knowledge campaigns about the risks of obesity should remain a top priority.
