Folate is implicated in carcinogenesis via effects on DNA synthesis, repair, and methylation. Efficient folate metabolism requires other B vitamins and is adversely affected by smoking and alcohol. Esophageal adenocarcinoma (EAC) may develop through a process involving inflammation [reflux esophagitis (RE)] leading to metaplasia [Barrett’s esophagus (BE)] and carcinoma. Within a population-based, case-control study, we investigated associations between dietary folate and related factors and risks of EAC, BE, and RE. EAC and BE cases had histologically confirmed disease; RE cases had endoscopically visible inflammation. Controls, age-sex frequency matched to EAC cases, were selected through population and general practice registers. Participants underwent structured interviews and completed food-frequency questionnaires. Multivariate ORs and 95% CIs were computed using logistic regression. A total of 256 controls and 223 EAC, 220 BE, and 219 RE cases participated. EAC risk decreased with increasing folate intake (OR highest vs. lowest = 0.56; 95% CI: 0.31, 1.00; P-trend < 0.01). Similar trends were found for BE (P-trend < 0.01) and RE (P-trend = 0.01). Vitamin B-6 intake was significantly inversely related to risks of all 3 lesions. Riboflavin intake was inversely associated with RE. Vitamin B-12 intake was positively associated with EAC. For EAC, there was a borderline significant interaction between folate intake and smoking (P-interaction = 0.053); compared with nonsmokers with high (≥median) folate intake, current smokers with low intakes (<median) had an 8-fold increased risk (OR: 8.15; 95% CI: 3.61, 18.40). The same group had increased BE risk (OR: 2.93; 95% CI: 1.24, 6.92; P-interaction = 0.12). Folate and other dietary methyl-group factors are implicated in the etiology of EAC and its precursors.

Importance: Seriously ill hospitalized patients have identified communication and decision making about goals of care as high priorities for quality improvement in end-of-life care. Interventions to improve care are more likely to succeed if tailored to existing barriers.
Objective: To determine, from the perspective of hospital-based clinicians, (1) barriers impeding communication and decision making about goals of care with seriously ill hospitalized patients and their families and (2) their own willingness and the acceptability for other clinicians to engage in this process.
Design, Setting, and Participants: Multicenter survey of medical teaching units of nurses, internal medicine residents, and staff physicians from participating units at 13 university-based hospitals from 5 Canadian provinces.
Main Outcomes and Measures: Importance of 21 barriers to goals of care discussions rated on a 7-point scale (1 = extremely unimportant; 7 = extremely important).
Results: Between September 2012 and March 2013, questionnaires were returned by 1256 of 1617 eligible clinicians, for an overall response rate of 77.7% (512 of 646 nurses [79.3%], 484 of 634 residents [76.3%], 260 of 337 staff physicians [77.2%]). The following family member-related and patient-related factors were consistently identified by all 3 clinician groups as the most important barriers to goals of care discussions: family members' or patients' difficulty accepting a poor prognosis (mean [SD] score, 5.8 [1.2] and 5.6 [1.3], respectively), family members' or patients' difficulty understanding the limitations and complications of life-sustaining treatments (5.8 [1.2] for both groups), disagreement among family members about goals of care (5.8 [1.2]), and patients' incapacity to make goals of care decisions (5.6 [1.2]). Clinicians perceived their own skills and system factors as less important barriers. Participants viewed it as acceptable for all clinician groups to engage in goals of care discussions-including a role for advance practice nurses, nurses, and social workers to initiate goals of care discussions and be a decision coach.
Conclusions and Relevance: Hospital-based clinicians perceive family member-related and patient-related factors as the most important barriers to goals of care discussions. All health care professionals were viewed as playing important roles in addressing goals of care. These findings can inform the design of future interventions to improve communication and decision making about goals of care.

Colorectal cancer (CRC) is the second leading cause of cancer-related deaths in the Western world. It is becoming increasingly clear that CRC is a diverse disease, as exemplified by the identification of subgroups of CRC tumours that are driven by distinct biology. Recently, a number of studies have begun to define panels of diagnostically relevant markers to align patients into individual subgroups in an attempt to give information on prognosis and treatment response. We examined the immunohistochemical expression profile of 18 markers, each representing a putative role in cancer development, in 493 primary colorectal carcinomas using tissue microarrays. Through unsupervised clustering in stage II cancers, we identified two cluster groups that are broadly defined by inflammatory or immune-related factors (CD3, CD8, COX-2 and FOXP3) and stem-like factors (CD44, LGR5, SOX2, OCT4). The expression of the stem-like group markers was associated with a significantly worse prognosis compared to cases with lower expression. In addition, patients classified in the stem-like subgroup displayed a trend towards a benefit from adjuvant treatment. The biologically relevant and poor prognostic stem-like group could also be identified in early stage I cancers, suggesting a potential opportunity for the identification of aggressive tumors at a very early stage of the disease.

Fungi of the genus Aspergillus are widespread in the environment. Some Aspergillus species, most commonly Aspergillus fumigatus, may lead to a variety of allergic reactions and life-threatening systemic infections in humans. Invasive aspergillosis occurs primarily in patients with severe immunodeficiency, and has dramatically increased in recent years. There are several factors at play that contribute to aspergillosis, including both fungus and host-related factors such as strain virulence and host pulmonary structure/immune status, respectively. The environmental tenacity of Aspergilllus, its dominance in diverse microbial communities/habitats, and its ability to navigate the ecophysiological and biophysical challenges of host infection are attributable, in large part, to a robust stress-tolerance biology and exceptional capacity to generate cell-available energy. Aspects of its stress metabolism, ecology, interactions with diverse animal hosts, clinical presentations and treatment regimens have been well-studied over the past years. Here, we synthesize these findings in relation to the way in which some Aspergillus species have become successful opportunistic pathogens of human- and other animal hosts. We focus on the biophysical capabilities of Aspergillus pathogens, key aspects of their ecophysiology and the flexibility to undergo a sexual cycle or form cryptic species. Additionally, recent advances in diagnosis of the disease are discussed as well as implications in relation to questions that have yet to be resolved.

As alterações introduzidas pela actual reforma da Administração Pública preconizam uma gestão determinada por objectivos motivando os trabalhadores para um desempenho de qualidade reconhecendo o mérito e a excelência. A avaliação de desempenho dos enfermeiros regulamentada desde 1993, estabelece como princípios orientadores a objectividade, a continuidade, a flexibilidade e a periodicidade, em que a atribuição de uma menção qualitativa resulta da avaliação contínua centrada no conteúdo funcional de cada categoria profissional. A abordagem da justiça organizacional justifica-se, tendo em conta os efeitos das percepções de (in)justiça nas atitudes e comportamentos das pessoas. Essas reacções podem ter efeitos directos e indirectos no funcionamento dos grupos e da organização. Neste contexto, surge este estudo, não experimental, transversal e correlacional de tipo quantitativo, para o qual definimos como objectivos fundamentais: conhecer o nível de satisfação dos enfermeiros com o processo de avaliação de desempenho e variáveis relacionadas; avaliar a percepção de justiça organizacional dos enfermeiros e analisar as possíveis relações entre a satisfação com o processo de avaliação de desempenho e a percepção de justiça organizacional Os dados foram obtidos, através da aplicação de um questionário aos enfermeiros do Hospital em estudo e concluímos que, os enfermeiros avaliam positivamente o nível de satisfação com o processo de avaliação de desempenho, com valores superiores na dimensão comportamental, referente aos aspectos interaccionais do que na dimensão cognitiva, relacionada com os aspectos procedimentais. Os enfermeiros chefes são a categoria profissional com nível mais baixo de satisfação na dimensão referente aos aspectos interaccionais. A análise dos dados referentes ao nível de finalização das diferentes fases do processo de avaliação indicam uma clara descontinuidade dado que, apenas 23,8% dos enfermeiros participa na elaboração das normas e critérios e, inversamente, 97,6% elaboraram o relatório crítico de actividades sendo este o documento de suporte documental que permite a atribuição da menção qualitativa reforçando assim as críticas de burocratização do processo. Relativamente à percepção de justiça organizacional verificamos que a vertente distributiva é a que apresenta valores mais baixos sendo a vertente interaccional aquela em que se verifica valores médios mais elevados. Verificámos existirem correlações positivas e significativas entre as dimensões procedimental e interaccional da percepção de justiça, quer para a dimensão cognitiva quer para a dimensão comportamental do nível de satisfação com a avaliação de desempenho dos enfermeiros. O estudo reforça a convicção de que o processo de avaliação de desempenho deve ser mantido tal como regulamentado, devendo os esforços serem canalizados para corrigir os aspectos referentes à precisão com que o processo avalia o desempenho e o cumprimento de todas as etapas o que só se consegue com a participação reflectida na adesão a um sistema de valores que privilegie a qualidade e definição de indicadores de produtividade e qualidade dos cuidados de enfermagem.

A violência sexual contra mulheres e crianças é um fenómeno social e de saúde pública, transversal a diferentes culturas e épocas históricas. A consciencialização deste fenómeno ocorre porém num milénio em que a auto-determinação das mulheres e os direitos fundamentais das crianças passaram a ser considerados pelas principais estâncias políticas. Paralelamente, também a ciência tem dado o seu contributo para a compreensão e resolução destes crimes. Os estudos acerca dos mecanismos de predisposição para a agressão sexual visam a reabilitação dos indivíduos que cometeram ofensas sexuais, prevenindo a reincidência destes comportamentos. O objectivo é diminuir o número de vítimas, intervindo no agressor. O trabalho que apresentamos teve por finalidade investigar potenciais factores de predisposição para a agressão sexual em sujeitos do sexo masculino condenados por crime de violação e abuso sexual de menores, mas também em estudantes universitários com história de violência sexual. Pretendeu-se avaliar de que forma os esquemas precoces mal adaptativos, os mecanismos de inibição/excitação sexual e a personalidade intervêm nestas diferentes formas de violência sexual. Os resultados deste trabalho indicaram que as dimensões avaliadas poderão contribuir para a predisposição e/ou manutenção dos comportamentos de agressão sexual. Adicionalmente, verificou-se que as diferentes formas de violência sexual (violação, abuso sexual de menores e violência sexual em estudantes universitários) são caracterizadas por perfis distintos, e que esta especificidade poderá ser determinante na elaboração de modelos de conceptualização da agressão sexual.

The Miocene PX1 gabbro-pyroxenite pluton, Fuerteventura, Canary Islands, is a 3.5 x 5.5 km shallow-level intrusion (0.15-0.2 GPa and 1100-1120 degrees C), interpreted as the feeder-zone to an ocean-island volcano. It displays a vertical magmatic banding expressed in five 50 to 100 metre-wide NNE-SSW trending alkaline gabbro sequences alternating with pyroxenites. This emplacement geometry was controlled by brittle to ductile shear zones, generated by a regional E-W extensional tectonic setting that affected Fuerteventura during the Miocene. At a smaller scale, the PX1 gabbro and pyroxenite bands consist of metre-thick differentiation units, which suggest emplacement by periodic injection of magma pulses as vertical dykes that amalgamated, similarly to a sub-volcanic sheeted dyke complex. Individual dykes underwent internal differentiation following a solidification front parallel to the dyke edges. This solidification front may have been favoured by a significant lateral/horizontal thermal gradient, expressed by the vertical banding in the gabbros, the fractionation asymmetry within individual dykes and the migmatisation of the wall rocks. Pyroxenitic layers result from the fractionation and accumulation of clinopyroxene +/- olivine +/- plagioclase crystals from a mildly alkaline basaltic liquid. They are interpreted as truncated differentiation sequences, from which residual melts were extracted at various stages of their chemical evolution by subsequent dyke intrusions, either next to or within the crystallising unit. Compaction and squeezing of the crystal mush is ascribed to the incoming and inflating magma pulses. The expelled interstitial liquid was likely collected and erupted along with the magma flowing through the newly injected dykes. Clinopyroxene mineral orientation - as evidenced by EBSD and micro X-ray tomography investigations - displays a marked pure-shear component, supporting the interpretation of the role of compaction in the generation of the pyroxenites. Conversely, gabbro sequences underwent minor melt extraction and are believed to represent crystallised coalesced magma batches emplaced at lower rates at the end of eruptive cycles. Clinopyroxene orientations in gabbros record a simple shear component suggesting syn-magmatic deformation parallel to observed NNE-SSW trending shear zones induced by the regional tensional stress field. This emplacement model implies a crystallisation time of 1 to 5 years for individual dykes, consistent with PX1 emplacement over less than 0.5 My. A minimum amount of approximately 150 km(3) of magma is needed to generate the pluton, part of it having been erupted through the Central Volcanic Centre of Fuerteventura. If the regional extensional tectonic regime controls the PX1 feeder-zone initiation and overall geometry, rates and volumes of magma depend on other, source-related factors. High injection rates are likely to induce intrusion growth rates larger than could be accommodated by the regional extension. In this case, dyke intrusion by propagation of a weak tip, combined with the inability of magma to circulate through previously emplaced and crystallised dykes could result in an increase of non-lithostatic pressure on previously emplaced mushy dyke walls; thus generating strong pure-shear compaction within the pluton feeder-zone and interstitial melt expulsion. Sports-practicing youths are at an elevated risk for alcohol use and misuse. Although much attention has recently been given to depicting subgroups facing the greatest threats, little evidence exists on the contexts in which their drinking takes place. Using data from a cross-sectional study on youth sports participation and substance use in the French-speaking part of Switzerland, this study focused on the social contexts associated with hazardous drinking of 894 sports-practicing adolescents aged 16 to 20. Divided between those who had been drunk in the last month (hazardous drinkers, n = 315) and those who had not (n = 579), sports-practicing adolescents were compared on reported gatherings (sports-related, sports-unrelated, mixed) likely linked to their drinking behaviour. Mixed social contexts, followed by sports-unrelated ones, were reported as the most common context by both male and female youths who practiced sports. After controlling for several possible confounders, male hazardous drinkers were more than 3 times more likely to report sports-unrelated social contexts as the most common, compared to sport-related ones, while females were more than 7 times more likely to do so. Our findings seem to indicate that, rather than focusing only on sports-related factors, prevention of alcohol misuse among sports-practicing youths should also pay attention to the social contextualisation of their hazardous drinking.

This project was undertaken because of a need to analyse concepts in social science more specifically and sequence them more carefully in a social science program. Concepts have been identified vaguely on many curriculum documents or left in isolation from each other when they are specifically identified. The project's aim was to identify a method for analysing concepts and sequencing their teaching on some rational basis. Once the method for analysing concepts was identified a questionnaire was designed and administered to a random sample of students at the grade three, five and eight levels. The questionnaire attempted to measure their comprehension of specific social science concepts at several levels which became progressively more complex. The major hypothesis was that there would be a direct correlation between age and achievement on the questionnaire. The raw scores were seriated and correlated with the ages of the students using the rankdifference- squared method. For the majority of areas tested it was found that there was a significant correlation between age and achievement on the questionnaire. Variation in the correlation coefficients generated suggests that comprehension of social science concepts is not simply a function of age but is probably a function of several inter-related factors such as reading ability, skill in Basic Thinking Skills and age. Thirty students completed each test. There were three tests in the questionnaire.

Cette recherche étudie l'évolution de l'appui à la souveraineté du Québec entre 1976 et aujourd'hui, en considérant les diverses formulations de la question utilisées par les acteurs politiques et par les sondeurs. Cette question est abordée en faisant une analyse longitudinale multiniveaux de plus de 800 sondages. Une telle approche suggère que les caractéristiques des sondages sont nichées à l'intérieur d'unités contextuelles plus larges, des unités de temps. Cela permet d'une part d'observer quels sont les facteurs liés à la mesure qui influencent l'appui à la souveraineté du Québec et ensuite de voir comment ces mêmes effets varient à travers le temps et comment ils sont influencés par certains événements jugés importants. Il ressort de ces analyses que les propositions séparation, indépendance, souveraineté et souveraineté-association/partenariat entraînent généralement des appuis moyens significativement différents les uns des autres. Il apparaît aussi que certains événements ont eu une influence significative sur l'évolution de l'appui à ces diverses propositions. Enfin, il a été tenté de voir si les appuis propres à ces diverses propositions évoluaient de manière parallèle, s'ils réagissaient similairement aux différents événements considérés dans les analyses ou si au contraire ces appuis pouvaient parfois évoluer de manière différente. Les résultats à cet égard sont intéressants mais non concluants.

Cette thèse vise un apport conceptuel et empirique au sujet du rapport à l’alcool chez les hommes et les femmes âgés de 55 ans et plus provenant de la population générale du Canada. Les deux objectifs généraux sont de mieux comprendre l’influence du contexte social/environnemental sur la consommation d’alcool des adultes âgés et de mieux comprendre la relation entre la consommation d’alcool et la santé. La thèse est composée de deux articles empiriques associés à chacun des objectifs. Plus précisément, le premier article tente d’évaluer l’impact des changements associés à l’alcool depuis les années 1990 dans le contexte canadien sur la consommation d’alcool (effet période) en comparant des adultes âgés de 55 à 74 ans provenant de deux enquêtes populationnelles canadiennes : CADS de 1994 (Canada’s Alcohol and other Drugs Survey) et GENACIS de 2004 (GENder, Alcohol, and Culture: an International Study). Il vérifie également si le contexte a un effet significatif sur les différences observées en tenant compte de l’âge, de la perception subjective de la santé et de la composition sociodémographique des échantillons. Les résultats montrent que les taux de buveurs, de consommation excessive, de buveurs de bière et de vin sont plus élevés en 2004, dans un contexte social/environnemental plus favorable à l’alcool. De plus, les profils de consommation d’alcool ne diffèrent pas entre 1994 et 2004 (volume, fréquence, quantité, quantité maximale), signifiant probablement l’influence des changements physiques qui accompagnent le vieillissement normal sur la modulation de la consommation d’alcool avec l’âge. Le deuxième article vise à dégager une typologie de buveurs et de non-buveurs chez les adultes âgés canadiens, à évaluer comment cette typologie est associée à l’état de santé ainsi qu’à comparer le rôle de l’alcool à celui d’un ensemble d’autres déterminants de la santé dans l’association à la santé (ici santé autoperçue). Les résultats issus de l’enquête GENACIS de 2004 montrent que les buveurs se perçoivent en meilleure santé que les non-buveurs, peu importe le profil de consommation d’alcool rapporté. Il est probable que l’état de santé autoperçue dicte le statut de buveur ou de non-buveur et non pas l’inverse. Les résultats montrent aussi que les caractéristiques démographiques, psychosociales et les comportements liés à la santé des adultes âgés sont davantage associés à la perception de leur santé que leur consommation (ou non-consommation) d’alcool seule. Les résultats de ce second article indiquent notamment que les bienfaits de la consommation d’alcool se définissent probablement par la biologie, mais pas seulement par la biologie. Il est recommandé de poursuivre l’étude du rapport à l’alcool chez les adultes âgés qui peut varier d’une cohorte et d’une période à l’autre. D’un point de vue de santé publique, la conceptualisation d’un modèle de santé qui cible les adultes âgés implique une meilleure compréhension de l’impact de la consommation d’alcool et d’autres déterminants de la santé sur l’état de santé. Les contributions et implications pratiques de la thèse ainsi que les principales limites et pistes de futures recherches sont présentées dans la discussion.

Les élèves aux problèmes de comportements externalisés comme l'opposition éprouvent généralement de la difficulté à s’engager dans leurs apprentissages (Janosz, 2000). Or, un faible engagement scolaire peut générer des problèmes d’ordre académique et mener au décrochage. Toutefois, certains facteurs environnementaux tels que la relation maître-élève semblent influer sur l’investissement des élèves dans les activités scolaires. La présente étude vise à examiner le lien entre le degré d'opposition d'élèves du 2e cycle du primaire et leur engagement comportemental et affectif en français. Elle tente aussi de vérifier si la chaleur ou les conflits présents dans la relation maître-élève agissent comme facteurs de protection ou comme facteurs aggravants face à l’engagement scolaire des élèves oppositionnels. Finalement, cette étude s’intéresse à l’effet différentiel de la relation maître-élève sur l’engagement scolaire des garçons et des filles. Des régressions linéaires multiples hiérarchiques effectuées sur 385 élèves québécois ont permis d’atteindre ces objectifs. Les résultats obtenus révèlent que la chaleur n’agit pas à titre de facteur de protection en ce qui a trait à l’engagement comportemental des élèves oppositionnels, mais qu’elle est bénéfique pour l’engagement comportemental des filles. Les résultats confirment également l’effet aggravant des conflits sur l’engagement comportemental des élèves oppositionnels. Finalement, cette étude indique que les conflits élèves-enseignants sont nuisibles pour l’engagement affectif des garçons. Les implications de ces résultats pour la recherche et pour l’intervention en psychoéducation seront présentées.

Des études récentes ont démontré une augmentation de la prévalence de l’infertilité au Canada ainsi qu’une augmentation fulgurante de l’utilisation de la procréation assistée. Le Québec s’est doté en 2010 d’un programme de financement de la procréation assistée visant un accès universel ainsi que la protection de la santé des mères et des enfants. Les diverses parties prenantes attribuent un certain nombre de lacunes à ce programme, incluant l’absence de mesures de prévention et de promotion de la santé visant à réduire la prévalence de l’infertilité. En effet, une proportion significative de cas d’infertilité découle de facteurs modifiables et relatifs aux modes de vie tels que le tabagisme, les infections transmises sexuellement et par le sang, les problèmes de poids, les toxines environnementales et l’âge. De plus, l’âge maternel avancé ainsi que l’usage de la procréation assistée comportent des risques pour la santé des mères et des enfants au sujet desquels la population ne possède pas une connaissance suffisante. Des approches en amont ont été proposées par diverses organisations et dans divers pays, toutefois, peu ont été adoptées. Force est de constater que ces initiatives représentent de grands défis au point de vue de l’acceptabilité sociale, en raison de la nature sensible du sujet et d’une grande valorisation sociale de l’autonomie reproductive. L’éthique des communications en santé permet d’identifier ces défis qui touchent l’usage de tactiques persuasives, le risque de stigmatisation et l’attribution indue d’une responsabilité. Si leur élaboration tient compte de ces enjeux, les campagnes de communications en santé ont le potentiel d’informer adéquatement la population afin de favoriser l’autonomie et la santé reproductive des individus, sans causer de dommage iatrogénique. L’éthique de l’ « empowerment », qui requiert l’attribution d’une responsabilité individuelle de nature prospective, l’apport de ressources concrètes et l’implication des communautés, permet d’identifier les besoins en termes de solutions législatives favorisant des contextes socioéconomiques qui soutiennent la santé reproductive et l’autonomie reproductive.

Le but premier des services de protection de la jeunesse est de mettre fin à une situation de compromission et d’éviter que celle-ci ne se reproduise. Cependant, une meilleure connaissance des facteurs de risque d’incidence LSJPA chez les jeunes pris en charge en raison de leurs comportements problématiques permettrait de mieux identifier et ainsi, d’adapter les interventions chez ceux se révélant être les plus à risque de délinquance juvénile. La présente étude propose donc de décrire l’ampleur et le risque d’incidence c’est-à-dire, l’application d’une sanction ou d’une mesure en vertu de la LSJPA chez ces jeunes et de déterminer, parmi les facteurs associés au jeune et ceux inhérents à l’intervention, quels en sont les meilleurs prédicteurs. Pour ce faire, des données clinico-administratives des 16 centres jeunesse du Québec ont été utilisées. Ainsi, tous les enfants et adolescents dont le dossier a été fermé à la suite d’une première intervention en protection de la jeunesse en raison de troubles de comportement entre le 1er janvier 2005 et le 31 décembre 2009 ont été observés (N = 6 630). Des analyses de survie (modèle Kaplan-Meier) ainsi que des régressions de Cox ont été effectuées. Les résultats indiquent qu’un jeune pris en charge en raison de troubles de comportement a 39,7% de risque de migrer vers les services judiciaires pour adolescents dans les cinq années qui suivent la fermeture de son dossier. Sans grande surprise, les garçons présentent un plus grand risque que leurs homologues féminins. Il appert également que le risque d’incidence varie en fonction du sexe et de la présence de maltraitance lors de la prise en charge initiale. De plus, les facteurs associés à l’usager lui-même semblent avoir un impact plus important sur l’incidence que ceux associés à la prise en charge.
