Background. Morphological and dentofacial alterations have been attributed to impaired respiratory function. Objective. To examine the influence of mouth breathing (MB) on children facial morphology before and after adenoidectomy or adenotonsillectomy. Methods. Thirty-three MB children who restored nasal breathing (NB) after surgery and 22 NB children were evaluated. Both groups were submitted to lateral cephalometry, at time 1 (T1) before and at time 2 (T2) 28months on average postoperatively. Results. Comparison between the MB and NB groups at T1 showed that mouth breathers had higher inclination of the mandibular plane; more obtuse gonial angle; dolichofacial morphology; and a decrease in the total and inferior posterior facial heights. Twenty-eight months after the MB surgical intervention, they still presented a dolichofacial morphologic pattern. During this period, MB altered the face growth direction and decreased their mandible plane inclination, with reduction in the SN.GoGn, PP.MP, SNGn, and ArGo.GoMe parameters as well as an increase in BaN.PtGn. Conclusion. After the MB rehabilitation, children between 3 and 6years old presented significant normalization in the mandibular growth direction, a decrease in the mandible inclination, and an increase in the posterior facial height. Instead, they still persisted with a dolichofacial pattern when compared with nasal breathers. © 2011 The Authors. Aim: To analyse factors potentially associated with molar incisor hypomineralisation (MIH) development. METHODS: A population-based study was carried out with 903 children aged from 6-12 years old, born and residing in rural and urban areas of the town of Botelhos, State of Minas Gerais, Brazil. Their mothers completed a structured medical history questionnaire, from pregnancy to the child's 3rd year of life. Two examiners evaluated children for MIH according to criteria suggested by the European Academy of Paediatric Dentistry. Descriptive analyses of the data and odds ratios (OR) with 95% test-based confidence intervals (CI) were estimated. Chisquare test was used to evaluate the differences between groups. RESULTS: The prevalence of MIH in children from rural area (RA) was significantly higher than those from the urban area (UA) (24.9% versus 17.8%, p= 0.01). In urban children, neither significant associations with MIH nor medical problems were found. In rural children, however, MIH was significantly more common among those whose mothers had experienced medical problems during pregnancy (OR=2.11; 1.01-4.37 CI 95%; p=0.04), who had throat infections (OR=2.93; 1.47-5.87 CI 95%; p=0.01), who had high fever (OR=1.91; 1.07-3.39 CI 95%; p=0.02), and who had used amoxicillin associated with other antibiotics (OR=1.92; 1.02-3.62 CI 95%; p=0.04) during the first 3 years of life. CONCLUSION: This study suggests a link between MIH and health problems during pregnancy, as well as environmental factors.

Our in vitro study evaluated calcium fluoride formation in enamel and the anticaries effect of seven resin-based varnishes under cariogenic challenge. Methods. Enamel blocks were subjected to pH cycling. The experimental groups received fluoride varnish application, the positive control received topical fluoride gel treatment, and the negative control did not receive any treatment. The pH cycling surface hardness (SH1) and integrated loss of subsurface hardness (ΔKHN) were then determined. We measured the amount of fluoride released into the demineralizing and remineralizing (DE-RE) solutions used in pH cycling. The fluoride concentration in the enamel was determined 24h after application of the products as loosely bound fluoride and firmly bound fluoride. Results. Higher deposits of loosely bound fluoride were observed for Duofluorid, followed by Biophat. For Duraphat, Bifluorid, Duraflur, and Duofluorid, no difference was observed in the SH1 and ΔKHN values, with the lowest mineral loss compared to the other groups. The Bifluorid and Duofluorid groups released high fluoride amounts into the DE-RE, and statistically significant difference was noted between them. Conclusions. The anticaries effect showed no correlation with higher deposited fluoride amounts, resin type, or fluoride source. © 2012 John Wiley & Sons Ltd, BSPD and IAPD.

Objective. This cross-sectional study assessed the prevalence and severity of the enamel defects, known as Molar-Incisor Hypomineralization (MIH) and its relationship to dental caries. Materials and methods. A sample of 1157 schoolchildren (population based), aged 6-12 years, of the Araraquara city-Brazil, was evaluated according to the European Academy of Paediatric Dentistry (EAPD) criteria by two trained examiners. The dental impact caused by MIH was evaluated with the Decayed, Missing and Filled Teeth (DMFT) index (WHO). Data were analyzed using ANOVA and Chi-square tests (p < 0.05). The socioeconomic status was collected using a questionnaire answered by parents. Results. The prevalence of MIH was 12.3%. Mild impairment was the most frequent diagnosis. DMFT of children with MIH was 0.89 (±1.18), which are higher than those of the unaffected group (0.43 ± 1.01). An association was found between dental caries only in the permanent dentition of children with MIH (p = 0.0001). Family income was considered low in 85% of the families of children with MIH in the public system, while in private school it was 18% (p < 0.05). Conclusion. The prevalence of MIH in Araraquara was associated with greater caries experience in the permanent dentition. © 2013 Informa Healthcare.

Aim: To determine the potential aetiological factors related to molar-incisor hypomineralisation (MIH) in Brazilian children. Methods: A total of 1,151 children aged 7-12 years (mean 8.86 ± 1.28), born and living in the urban area of Araraquara, Brazil, were examined by two examiners evaluating the presence of MIH according to criteria suggested by the European Academy of Paediatric Dentistry (2003). Their mothers completed a structured questionnaire about medical history, from pregnancy to the first 3 years of the children's life. Descriptive analyses of data and odds ratios (OR) with 95 % test-based confidence intervals (CI) were estimated. Chi-square test was used to evaluate the differences between groups. Results: The prevalence of MIH in the children was 12.3 %. The interviewing response rate was 90.4 %. The prevalence of miscarriage history (25 vs. 15.4 %; OR = 1.21; 95 % CI 0.30-4.92) and occurrence of anaemia (23 vs. 12.4 %; OR = 2.07; 95 % CI 0.50-8.63) were higher in mothers from MIH group than those from non-MIH group. However, these associations were not statically significant. In the children's medical history, rhinitis, bronchitis (56.5 vs. 52.5 %; OR = 1.17; 95 % CI 0.82-1.68), and high fever (20.4 vs. 18.2 %; OR = 1.14; 0.73-1.76) were more prevalent in MIH group, but there were no significant differences between the groups (p > 0.05). Conclusions: No possible aetiological factor investigated was associated with MIH. Prospective studies are needed to define the aetiological factors involved with MIH. © 2013 European Academy of Paediatric Dentistry.

Os rotavírus se constituem nos principais agentes causadores de gastroenterite grave entre crianças com idades inferiores a 5 anos, tanto nos países desenvolvidos quanto naqueles em desenvolvimento, com pico de incidência entre 6 e 24 meses de vida. Em termos globais, estima-se que pelo menos 500.000 óbitos por ano se associem a esse enteropatógeno. Um extenso ensaio clínico de fase 111, randomizado na proporção de 1 :1, controlado por placebo e duplo-cego, envolvendo 11 países da América Latina e a Finlândia se levou a efeito objetivando-se avaliar a eficácia e segurança de uma vacina atenuada, de origem humana, contra rotavírus, denominada RIX4414. Na totalidade, recrutaram-se mais de 63.000 crianças. Em Belém, Pará, tais estudos envolveram 3.218 indivíduos aos quais se administraram duas doses de vacina ou placebo, no segundo e quarto meses de idade. Desse total avaliou-se um subgrupo de 653 crianças quanto à eficácia da vacina, com acompanhamento ao longo de 1 a 2 anos, quando se registraram 37 episódios de GE grave por rotavírus, 75,6% (28/37) dos quais no grupo placebo e 24,3% (9/37) entre os vacinados, daí se inferindo eficácia da vacina de 68,8% (IC95% 32.0-87,0) nos primeiros dois anos de vida. No que se refere à intensidade desses episódios, notou-se maior eficácia contra os classificados como muito graves (escore de Ruuska & Vesikari ≥ 15), alcançando níveis de 83% (IC95% 22-96). No grupo placebo observou-se risco cumulativo, quanto ao desenvolvimento de gastroenterite grave por rotavírus, 4 vezes superior em relação ao vacinado. Quanto aos sorotipos de rotavírus G1 e não-G1, evidenciou-se proteção de 51 % (IC95% -30 - 81) e 82% (IC95% 37-95), respectivamente, denotando-se proteção tanto homotípica quanto heterotípica. De particular relevância se constituiu a proteção frente ao G9 [93% (IC95% 47-99)], dado o caráter emergente global desse sorotipo, além do seu potencial quanto a desencadear quadros diarréicos rotineiramente mais graves. A eficácia da vacina contra episódios de GE de qualquer etiologia alcançou 35,3% (IC95% 11,6-52,9), do que se depreende o expressivo impacto em potencial da vacinação contra rotavírus em termos de saúde pública. No que se refere à segurança desse imunizante, não se observaram diferenças significativas do ponto de vista estatístico, entre os grupos vacina e placebo, no que concerne à ocorrência de eventos adversos graves. Não se registrou qualquer caso de intussuscepção entre os sujeitos participantes, mercê de extensiva vigilância ativa nos hospitais de referência. Os resultados encontrados nesse estudo corroboram os já descritos em ensaios multicêntricos como um todo, em vários continentes, consolidando os indicadores quanto à eficácia e segurança da vacina RIX4414 quando administrada em duas doses a crianças saudáveis.

Background: This is a position paper from the 2nd International Bone Research Association (IBRA) Symposium for Condylar Fracture Osteosynthesis 2012 was held at Marseille, succeeding the first congress in Strasbourg, France, in 2007. The goal of this IBRA symposium and this paper was to evaluate current trends and potential changes of treatment strategies for mandibular condylar fractures, which remain controversial over the past decades.Methods: Using a cross-sectional study design, we enrolled the consensus based on the panel of experts and participants in the IBRA Symposium 2012. The outcomes of interest were the panel and electronic votes on management of condylar base, neck and head fractures, and panel votes on endoscopic and paediatric condylar fractures. Appropriate descriptive and univariate statistics were used.Results: The consensus derived from 14 experts and 41 participant surgeons, using 12 case scenarios and 27 statements. The experts and participants had similar decision on the treatment of condylar base, neck and head fractures, as well as similar opinion on complications of condylar fracture osteosynthesis. They had a parallel agreement on using open reduction with internal fixation (ORIF) as treatment of choice for condylar base and neck fractures in adults. Endoscopic approaches should be considered for selected cases, such as condylar base fractures with lateral displacement. There was also a growing tendency to perform ORIF in condylar head fractures. The experts also agreed to treat children (> 12 years old) in the same way as adults and to consider open reduction in severely displaced and dislocated fractures even in younger children. Nevertheless, non-surgical treatment should be the first choice for children <6 years of age. The decision to perform surgery in children was based on factors influencing facial growth, appropriate age for ORIF, and disagreement to use resorbable materials in children.Conclusions: The experts and participating surgeons had comparable opinion on management of condylar fractures and complications of ORIF. Compared to the first Condylar Fracture Symposium 2007 in Strasbourg, ORIF may now be considered as the gold standard for both condylar base and neck fractures with displacement and dislocation. Although ORIF in condylar head fractures in adults and condylar fractures in children with mixed dentition is highly recommended, but this recommendation requires further investigations. Paediatric diagnostic radiology can be considered as a separate specialty and with distinct characteristics of the radiology applied in adult patients. This in reason of the variability in the anatomical structures size and bigger sensitivity of tissues. The literature present in its majority methodologies for segmentation and tissue classification in adult patients, and works on tissue quantification are rare. This work had for objective the development of a biological tissue classifier and quantifier algorithm, from histograms, and that converts the quantified average thickness of these tissues for its respective simulator materials. The results will be used in the optimization process of paediatrics images, in future works, since these patients are frequently over exposed to the radiation in the repeated attempts of if getting considered good quality radiographic images. The developed algorithm was capable to read and store the name of all the archives, in the operational system, to filter artifacs, to count and quantify each biological tissues from the histogram of the examination, to obtain the biological tissues average thicknesses and to convert this value into its respective simulator material. The results show that it is possible to distinguish bone, soft, fat and pulmonary tissues from histograms of tomographic examinations of thorax. The quantification of the constituent materials of anthropomorphic phantom made by the algorithm, compared with the data of literature shows that the biggest difference was of 21,6% for bone. However, the literature shows that variations of up to 30% in bone thickness do not influence of significant form in the radiographic image quality. The average thicknesses of biological tissues, quantified for paediatrics patients, show that one phantom can simulate patients with distinct DAP ranges, since variations... To evaluate the influence of examiner's clinical experience on detection and treatment decision of caries lesions in primary molars. Design Three experienced dentists (Group A) and three undergraduate students (Group B) used the International Caries Detection and Assessment System (ICDAS) criteria and bitewing radiographs (BW) to perform examinations twice in 77 primary molars that presented a sound or carious occlusal surface. For the treatment decision (TD), the examiners attributed scores, analyzing the teeth in conjunction with the radiographs. The presence and the depth of lesion were validated histologically, and reproducibility was evaluated. The sensitivity, specificity, accuracy, and area under the ROC curve values were calculated for ICDAS and BW. The associations between ICDAS, BW, and TD were analyzed by means of contingency tables. Results Interexaminer agreement for ICDAS, BW, and TD were excellent for Group B and moderate for Group A. The two groups presented similar and satisfactory performance for caries lesion detection using ICDAS and BW. In the treatment decision, Group A was shown to have a less invasive approach than Group B. Conclusion The examiner's experience was not determinant for the clinical and radiographic detection of occlusal lesions in primary teeth but influenced the treatment decision of initial lesions.
