A "Pneumatosis Cystoides Intestinalis" é uma situação clínica rara que se caracteriza pela presença de múltiplos quistos de conteúdo gasoso ao nível da submucosa ou subserosa na parede do tracto gastrointestinal. A pneumatose intestinal classifica-se em idiopática e secundária. Na última forma admite-se uma relação causal com doença pulmonar obstrutiva crónica, conectivites, amiloidose, colites infecciosas, oclusão intestinal, isquémia, doença de Crohn, fármacos e iatrogenia cirúrgica e endoscópica. O espectro de manifestações clínicas compreende dor abdominal, oclusão intestinal, diarreia e hemorragia digestiva. Todavia, é frequentemente assintomática ou constitui um achado incidental no decurso de uma investigação não relacionada. Os autores apresentam o caso clínico de uma doente com pneumatose quística intestinal associada à utilização terapêutica de um antidiabético oral - acarbose (inibidor da alfa-glucosidase).

A hipofaringe e o esófago cervical são áreas particularmente vulneráveis a traumatismos por entubação oro-traqueal. Estes acidentes anestésicos são raros, pouco relatados e com maior incidência em situações de emergência. As perfurações faringoesofágicas podem passar despercebidas até ao aparecimento de sinais e sintomas característicos. Estes incluem dor cervical, febre, disfagia, leucocitose, enfisema subcutâneo e pneumomediastino. O tratamento cirúrgico vs conservador é controverso. É no entanto consensual que o intervalo de tempo entre o traumatismo e o diagnóstico e tratamento é, por si só, o factor mais importante para o prognóstico. Os autores apresentam um caso de uma parturiente submetida a cesariana de urgência em que ocorreu um acidente anestésico por entubação difícil, com laceração do seio piriforme e posterior formação de abcesso mediastínico.

O laringocelo é uma dilatação anómala do sáculo ventricular, com comunicação com o lúmen laríngeo. Pode ser congénito ou adquirido, sendo a etiopatogénese do tipo adquirido multifactorial. O laringocelo adquirido pode estar relacionado com um aumento prolongado da pressão intralaríngea. Manifesta-se sobretudo em indivíduos do sexo masculino, caucasianos, na quinta ou sexta décadas de vida; é mais frequentemente unilateral e do tipo combinado ou misto. Habitualmente a sua evolução é lenta; no entanto, a sua forma de apresentação pode ser súbita, com dispneia laríngea que pode requerer intubação orotraqueal ou uma traqueotomia de urgência, como no caso da sobreinfecção, com formação de um laringopiocelo. Apresentamos o caso clínico de uma doente do sexo feminino, de 71 anos de idade, vendedora de flores na rua, que dá entrada na Urgência do Hospital de S. José por odinofagia e disfonia com um mês e meio de evolução e que, na sequência de primeira toma de antibiótico, tem reacção alérgica com necessidade de internamento por Otorrinolaringologia. A ulterior investigação revelou laringopiocelo misto. O laringocelo é uma patologia rara e que raramente se apresenta sob a forma de obstrução da via aérea. A sua sobreinfecção ocorre em apenas 8% dos casos, o que torna este caso clínico particularmente raro. No entanto consideramos importante ter em mente esta patologia no diagnóstico diferencial da obstrução aguda da via aérea superior.

INTRODUCTION: Obesity is a chronic disease and a serious health problem that leads to increased prevalence of diabetes, hypertension, dyslipidemia and gallbladder disease. OBJECTIVE: To evaluate the efficacy of orlistat for weight loss and improved lipid profile compared to placebo in obese patients with hypercholesterolemia, treated over a period of 6 months. METHODOLOGY: In a 6-month, multicenter (10 centers in Portugal), double-blind, parallel, placebo-controlled study, 166 patients, aged 18-65 years, body mass index (BMI) > or = 27 kg/m2, LDL cholesterol > 155 mg/dl, were randomized to a reduced calorie diet (600 kcal/day deficit) plus orlistat three times a day or placebo. Exclusion criteria included triglycerides > 400 mg/dl, severe cardiovascular disease, uncontrolled hypertension, type 1 or 2 diabetes under pharmacological treatment, and gastrointestinal or pancreatic disease. RESULTS: The mean difference in weight from baseline was 5.9% (5.6 kg) in the orlistat group vs. 2.3% (2.2 kg) in the placebo group. In the orlistat group 49% of patients achieved 5-10% weight loss and 8.8% achieved > 10%. The orlistat group showed a significant reduction in total and LDL cholesterol, with similar changes for HDL in both treatment groups. The frequency of gastrointestinal adverse events was slightly higher in the orlistat group than in the placebo group, leading to discontinuation in 7 patients. CONCLUSION: Treatment with orlistat plus a reduced calorie diet for 6 months achieved significant reductions in weight, BMI and lipid parameters.

We present a case of central nervous system (CNS) infection by a member of the Penicillium genera in a HIV-negative man in Brazil. The patient was admitted complaining of loss of visual fields and speech disturbances. CT scan revealed multiple brain abscesses. Stereothacic biopsies revealed fungal infection and amphotericin B treatment begun with initial improvement. The patient died few days later as a consequence of massive gastrointestinal bleeding due to ruptured esophageal varices. The necropsy and final microbiologic analyses disclosed infection by Penicillium sp. There are thousands of fungal species of the Penicillium genera. Systemic penicilliosis is caused by the P. marneffei and was formerly a rare disease, but now is one of the most common opportunistic infection of AIDS patients in Southeast Asia. The clinical presentation usually involves the respiratory system and the skin, besides general symptoms like fever and weight loss. Penicillium spp infection caused by species other than P. marneffei normally cause only superficial or allergic disease but rare cases of invasive disease do occur. We report the fourth case of Penicillium spp CNS infection.

Salmonellosis remains an important cause of diarrheal illness in humans in São Paulo State, Brazil. In this study were identified 3554 Salmonella isolates from human infections, during the period 1996-2003. Among 68 different serovars determined, S. Enteritidis was the most frequent one in gastrointestinal and extra-intestinal infections accounting for 67.4% of all isolates. S. Typhimurium and S. enterica subsp. enterica (4,5,12:i:-) were most frequently isolated from children aged < 1-4 year-old, in contrast, people with S. Enteritidis infections were most likely to be 20-50 year-old. In our geographic area the continued laboratorial surveillance of salmonellosis, including serotyping, has showed the trends in Salmonella serovars causing infections in humans throughout the time.

The authors report neuromuscular manifestations in a 45-year-old woman after consuming octopus meat (Octopus sp.). The patient presented malaise, paresthesias in perioral and extremity areas, intense muscular weakness and arterial hypotension, followed by severe itch and disseminated cutaneous rash. Gastrointestinal manifestations and fever were not observed, reducing the probability of alimentary poisoning. The presence of muscular and neurological symptoms suggests neurotoxin action, which could have been ingested by the victim from the octopus salivary glands or from an accumulation of toxins in the meat, or by an unknown mechanism. There is little known about toxins of the Octopus genus and this communication is important alert to the possibility of poisoning in humans that eat octopus and its differentiation from alimentary poisonings arising from incorrect conservation of seafood.

The distinction between primary melanoma and melanoma metastatic to the skin has major prognostic implications. We report a case of a 67-year-old male with a diagnosis of a superficial spreading melanoma (stage IB) rendered 6 years earlier who presented clinically with an atypical nevus on his left thigh. Histopathological examination showed an intraepidermal melanocytic proliferation that was interpreted as melanoma in situ. Subsequently, 45 additional pigmented macules appeared in crops over a 9-month period. Clinically and dermoscopically, these lesions were extremely polymorphic. Histopathological findings were compatible with melanoma in situ, as each lesion consisted of a wholly intraepidermal proliferation of markedly atypical melanocytes arranged singly and in nests. A complete gastrointestinal study showed multiple pigmented metastatic lesions throughout the stomach and small bowel, which supported a diagnosis of metastatic melanoma with gastrointestinal and epidermotropic skin involvement. Monosomy of chromosome 9 and a BRAF V600E mutation were detected in the primary tumor sample and in macro-dissected secondary lesions. No CDKN2A or CDK4 germline mutations were found. Intraepidermal epidermotropic metastases of melanoma have been rarely described in literature. In this case, histopathology alone was insufficient to distinguish metastatic melanoma from multiple in situ melanomas. The recognition of epidermotropic metastases should be based on the correlation between clinical, dermoscopic, histopathological and molecular findings.

The lack of specific laboratorial diagnosis methods and precise symptoms makes the toxocariasis a neglected disease in Public Health Services. This study aims to determine the frequency of Toxocara spp. infection in children attended by the Health Public Service of Hospital Municipal de Maringá, South Brazil. To evaluate the association of epidemiological and clinical data, an observational and cross-section study was carried out. From 14,690 attended children/year aged from seven month to 12 years old, 450 serum samples were randomly collected from September/2004 to September/2005. A questionnaire was used to evaluate epidemiological, clinical and hematological data. An ELISA using Toxocara canis larval excretory-secretory products as antigen detected 130 (28.8%) positive sera, mainly between children from seven month to five years old (p = 0.0016). Significant correlation was observed between positive serology for Toxocara, and frequent playing in sandbox at school or daycare center (p = 0.011) and the presence of a cat at home (p = 0.056). From the families, 50% were dog owners which exposed soil backyards. Eosinophilia (p = 0.776), and signs and symptoms analyzed (fever p = 0.992, pneumonia p = 0.289, cold-like symptoms p = 0.277, cough p = 0.783, gastrointestinal problems p = 0.877, migraine p = 0.979, abdominal pain p = 0.965, joint pain p = 0.686 and skin rash p = 0.105) could not be related to the presence of anti-Toxocara antibodies. Therefore, two asthmatics children showed titles of 1:10,240 and accentuated eosinophilia (p = 0.0001). The authors emphasize the needs of prevention activities.

O edema agudo do pulmão pós-extubação traqueal é um acontecimento raro (≈ 0,1%)1. A etiologia é multifactorial, sendo a obstrução da via aérea superior o factor desencadeante principal. O esforço inspiratório contra a glote encerrada causa pressões intra torácicas muito negativas, que se transmitem ao interstício pulmonar, condicionando uma transudação de fluidos a partir dos vasos capilares pulmonares1-5. Relatamos um caso de edema agudo do pulmão pós-extubação num doente de quinze anos, operado no serviço de urgência por amputação traumática da perna esquerda. Revemos a fisiopatologia, o padrão radiológico, potenciais factores de risco e medidas preventivas desta complicação respiratória pós-anestésica.

Cytomegalovirus (CMV) is a genus in the family Herpesviridae that has been associated with gastrointestinal syndromes. In this work we looked for a possible association of CMV infection with colorectal cancer and ulcerative colitis (UC). Blood and enteric tissue samples of 14 patients with colorectal cancer and of 21 with UC were subjected to a nested-PCR that amplifies part of the gB gene of CMV and also to immunohistochemistry using a specific monoclonal antibody to IE 76kDa protein of CMV. CMV was detected by nested-PCR in the blood and/or the enteric tissue of nine (64.3%) colorectal cancer and 16 (76.2%) ulcerative colitis patients. In the immunohistochemistry it was observed that 12 (12/21, 57.1%) positive enteric tissue samples of patients with UC and none from patients with colorectal cancer (0/14) were positive to CMV. The positivity of CMV infections in the UC patient group (12/21, 57.1%) showed by both techniques, was significantly higher (p = 0.015) than that observed for colorectal cancer patients (2/14, 14.3%). These results suggest an association of ulcerative colitis with CMV infection of the enteric tissue.

Doente do sexo feminino, 18 anos, raça negra, natural da Guiné-Bissau, com quadro clínico com 8 anos de evolução, de sinais inflamatórios ao nível de ambos os ângulos da mandíbula, com nódulos palpáveis e drenagem espontânea de material aquoso não purulento, após exodontia de molar mandibular. Para diagnóstico definitivo foi colocada indicação para biópsia mandibular sob anestesia geral. Na avaliação pré-operatória destacava-se uma via aérea previsivelmente muito difícil: mallampati IV, micrognátia com abertura da boca muito limitada (1 cm), distância tiromentoniana < 6 cm e mobilidade cervical limitada, pelo que se programou intubação endotraqueal guiada por fibroscopia. Procedeu-se à intubação nasotraqueal guiada por fibroscopia com tubo 7,0 com cuff, sob sedação endovenosa com midazolam e fentanil, sem intercorrências. A apresentação deste caso tem como objectivo realçar as dificuldades inerentes ao diagnóstico de actinomicose com frequente necessidade de procedimentos invasivos, sob anestesia geral, o que condiciona um risco anestésico importante associado à dificuldade na abordagem da via aérea.

Purple urine bag syndrome (PUBS) was first reported in 1978. PUBS is rare, occurs predominantly in constipated women, chronically catheterized and associated with some bacterial urinary infections that produce sulphatase/phosphatase. The etiology is due to indigo (blue) and indirubin (red) or to their mixture that becomes purple. A chain reaction begins in the gastrointestinal tract with tryptophan as described in the article.
