Not only the progress of the radiotherapy, but also the improve of the dose in the patient with adjusted radiotherapy plannings, calculation of shield, radiometric survey, among others, are responsibilities of the medical physic inside of a sector of radiotherapy. This monograph has as objective to above show some of these cited functions of the medical physicis, starting with a theoretical basement and at as a moment to present the calculations and the results found during a visit in the Institute of Radiotherapy of the Valley of the Paraíba, located in the city of São José dos Campos, and in the Regional Hospital of Taubaté in the period of 2006, 2007 and 2008

Health Care Waste (HCW) represents 1%, and it has presently gained a lot of importance. Adequate management is one of the great challenges to be faced by health care centers. It has gained distinction and been widely discussed by members of the sectors involved with sanitation, public health and environmental issues due to waste physical, chemical biological characteristics, which pose potential risk to the environment and public health. The present study aims at evaluating HCW internal management by following all its phases, determining indicators, classifying and quantifying, establishing production rates (kg /patient/day) for the sector and designing materials to disseminate appropriate HCW disposal in the Emergency Room of the UNESP University Hospital in the city of Botucatu according to the guidelines presently in force. : From June to October 2011, the waste flow was observed from its production to final disposal. Four weight measurements were performed on four consecutive days in the month of August by using a properly calibrated (in grams) digital scale at the times scheduled for collection of the produced waste. Hence, the daily and monthly amounts were estimated according to their classification. All the waste packaged in the bags in garbage cans in the Emergency Room for a 24-hour period was considered to be a sample. Separation was not adequately performed in that sector, and waste from Group A was mixed with that from Group D. The amount of infectious waste produced in the sector corresponded to 87.80 %, common waste to 10.93 % and recyclable waste to 1.27%. The mean daily HCW production was of 123.300 kg/day, and the total monthly production was of 3,822 kg/month, which was distributed as follows: Group A 3,355,750 kg/month; Group D 417,570 kg/month and recyclables 48,670 kg/month. As a result of the 20th-century industrial development, and particularly after World War II, cars have become an important consumer good, and this has led to a large number of circulating vehicles. To this day, the number of cars has increased exponentially worldwide, which results in a high number of traffic accidents (TAs) every day. TAs are responsible for 1.2 million deaths worldwide each year, and in Brazil, they are considered to be a public health problem, considering that its traffic is regarded as one of the worst and most dangerous in the world. In addition to temporary or permanent physical disabilities that bring great harm to individuals, accidents also cause psychological disorders, such as the post-traumatic stress disorder. Hence, considering the importance of TAs in the country and their outcomes, this study aimed at investigating the epidemiological aspects of car accidents occurring in 2010 who were assisted in the emergency room of a university hospital in São Paulo state. By means of records made by the nursing team and medical records analysis, it was possible to extract a stratified sample of 88 cases involving adults and eight cases involving children. The sample was analyzed by the SAS software for Windows, version 9.2. As regards the accidents involving adults, there was a predominance of males (67%), with an approximate 2:1 ratio in relation to females. The victims’ most frequent age range was 20-29 years (39%), and they were mostly single (56%). The month showing the largest number of occurrences was July, with 17%, and the weekday was Sunday, with 25%. Showing 32%, the time period when most accidents occurred was from 06:00 o’clock p.m. to 11:59 p.m. As regards vehicles types involved, motorcycles were in first place, with 47% of cases, and the most common accident type was crashing between vehicles, with 43%... Nuclear medicine uses non-sealed sources for exams and treatments, because the movement of these sources, source of patients injected or not, involve a wide range of environmental radiometric levels. To better control of these levels this study was aimed at monitoring points strategically distributed in all environments Sector Technical Nuclear Medicine, Hospital of the Botucatu Medical School, performing two measurements at random times daily for a period of three months, sampling the normal routine of the sector. The detector Geiger-Muller was used for the acquisition of exposure rates of the points, efficiency and reprodutibility test have shown good performance of this equipment for this purpose. The results allowed to make a projection of annual dose for each environment indicating higher risks for Occupationally Exposed Individuals as well as Public Individuals compared with the limits established by the National Commission of Nuclear Energy. Was developed a spatial map of dose that can be used as a visual warning to the regions with the highest exposure to ionizing radiation, enabling reduced risk of a potential exposure

The searches for a health service it’s a part of a process, searches for a qualified evaluation can arise the perception, in form of irritating factors taking for an alert state. The personal expectation, it makes influence in its perceptions, too. The objective was to evaluate the satisfaction degree of the users of “Seção Técnica de Ambulatório Geral do Hospital das Clínicas de Botucatu”. The work is about a quantitative and transversal study, with calculated sample of 366 users, chosen randomly and invited to participate through a questionnaire. The population was compounded by a majority of women (64,5%), between 50 to 59 years old and average degree of education (38,3%). 25 specialties were cited. The evaluate about comfort, cleaning, reception, waiting time, medical attention and nursing vary between “very good” and “good”. Obtained significant “regular” assessment, the comfort in the waiting rooms (24%), silence (25%) and cleaning bathroom (63%). Others negative evaluation was the reception about the waiting time (26%), waiting time between arrival and the consultation (34%), waiting time between the request and consultation (27%). 30,6% report to know the place for complaints and 79,4% of these pointed to the ombudsman. 62% consider their problems solved, 84,4% would indicate the hospital, 57,4% would consider the hospital better than imagined. 42,62% used the opened area for thanking, critical for staff, suggestion infrastructure improvements, cleaning and waiting time. The biggest complaint was the staff turnover in the treatment. There was disharmony between the objective and subjective questions, but, all of them must be considered to propose improvements. The satisfaction evaluation makes the service more effective, bringing credibility to the health service and the patients adhere better to the treatment

Accidents represent a major public health problem as they are frequent, preventable, and account for high morbidity and mortality rates. Children are the most vulnerable to accidents due to their inherent characteristics, and as a result of their physical, sensorial, psychomotor and cognitive limitations, which will only develop with time. Watchfulness and careful attention are of paramount importance, especially as children develop locomotor skills that are accompanied by curiosity about their surroundings. Child accidents, particularly those that could have been avoided, are the accidents most commonly seen in emergency and urgency departments. Urgency and emergency departments are the greatest allies in the attention to pediatric accidents as they offer adequate, immediate and specific high complexity care to patients at risk, viewing their vital stabilization. Investigating the causes and consequences of this insult is essential to establish a diagnosis and to contribute for the adoption of measures of prevention, control, and assistance. To identify the epidemiologic characteristics of the accidents involving children that received care at the pediatric emergency department of Botucatu Medical School Hospital and required hospitalization. This quantitative, retrospective, descriptiveanalytic epidemiologic study included all children aged 0-14 years who had had an accident and were seen at the Pediatric Emergency Department of Botucatu Medical School Hospital of São Paulo State University/UNESP between January 1/2008 and December 31/2009. A total of 227 medical charts were reviewed and 178 (78.4%) patients were included in this study. Of these, 116 (65.1%) were males and 62 (34.8%) were females. Children aged 5 - 9 years (38.9%) were the most affected, followed by those aged 10 - 14 years (37.5%). Fractures occurred in 138 (77.5%) of the cases, followed by foreign bodies... This article presents the results of research on the Hall-School installed in the Juqueri Hospital, São Paulo, in 1929, by initiative of the psychiatrist Antonio Carlos Pacheco e Silva, and organized by the pediatrician Vicente Baptista da Silva and professors Norberto de Souza Pinto and Celso Guimarães da Fonseca. It is regarded as a real pioneer project for the development of Special Education and of Psychology, formerly named as for the abnormals. One can neatly perceive the change in the treatment of a medical origin only to that of an inclusion of Psychology and Education in the development of children affected by mental deficiency or by psychopathies. The influence of the ideas of educators, such as the ones mentioned above, were fundamental for the changes.

Autologous hematopoietic stem cell transplantation is a conduct used to treat some hematologic diseases and to consolidate the treatment of others. In the field of nursing, the few published scientific studies on nursing care and early hospital discharge of transplant patients are deficient. Knowledge about the diseases treated using hematopoietic stem cell transplantation, providing guidance to patients and caregivers and patient monitoring are important nursing activities in this process. Guidance may contribute to long-term goals through patients' short-term needs. To analyze the results of early hospital discharge on the treatment of patients submitted to autologous transplantation and the influence of nursing care on this conduct. A retrospective, quantitative, descriptive and transversal study was conducted. The hospital records of 112 consecutive patients submitted to autologous transplantation in the period from January to December 2009 were revisited. Of these, 12 patients, who remained in hospital for more than ten days after transplantation, were excluded from the study. The medical records of 100 patients with a median age of 48.5 years (19-69 years) were analyzed. All patients were mobilized and hematopoietic stem cells were collected by leukapheresis. The most common conditioning regimes were BU12Mel100 and BEAM 400. Toxicity during conditioning was easily managed in the outpatient clinic. Gastrointestinal toxicity, mostly Grades I and II, was seen in 69% of the patients, 62% of patients had diarrhea, 61% of the patients had nausea and vomiting and 58% had Grade I and II mucositis. Ten patients required hospitalization due to the conditioning regimen. Febrile neutropenia was seen in 58% of patients. Two patients died before Day +60 due to infections, one with aplasia. The median times to granulocyte and platelet engraftment were 12 days and 15 days, respectively, with median red blood cell and platelet transfusions until discharge of three and four units, respectively. Twenty-three patients required rehospitalization before being discharged from the outpatient clinic. The median time to granulocyte engraftment was 12 days and during the aplasia phase few patients were hospitalized or suffered infections. The toxicity of the conditioning was the leading cause of rehospitalization. The nursing staff participated by providing guidance to patients and during the mobilization, transplant and outpatient follow-up phases, thus helping to successfully manage toxicity.

The indications for adenotonsillectomy in pediatric patients have changed considerably during the 90th decade. Local or systemic complications of the adenoid or tonsil hypertrophy itself have now been substituted by signs of obstructive ventilatory disturbances, including obstructive sleep apnea as the major indications for surgery. Objective: This study analyses the clinical profile of children submitted to adenotonsilectomy in their pre and postoperative state, at Botucatu Medical School-State University São Paulo, UNESP. Methods: 332 children of both genders, aged 1 to 12 years, who underwent adenotonsillectomy between 1999 and 2004, were studied, focused on epidemiological profile, pre and postoperative (1 month) symptoms, obtained from medical records. Height and weight were compared to brazilian normal age related values. Results: We found a predominance of the male gender, except in he group aged from 10 to 12 years. Considering wheight and height, we found important failure to thrive, mostly for height deficit. Among clinical aspects, we found a significant reduction in obstructive symptoms like snoring or apneia (p<0001) in the postoperative period. Conclusion: Our results were similar to the literature findings of patients clinical profile. The major indication for adenotonsillectomy in our service was clinical diagnosis of obstructive sleep apnea.

The introduction of more efficient diagnostic methods, new techniques in surgery and transplantation, antibiotics and chemotherapeutics more potent and novel materials for prostheses, catheters and probes significantly increased the life expectancy and quality of life of critically ill patients, on the other hand, hospital-acquired infections emerged as important iatrogenic complications. Invasive infections are a growing problem in public health hospitals in Brazil and worldwide. Among the various etiological agents found in the hospital environment, the genus Candida has been the third most frequently isolated pathogen. In general, invasive fungal infections are associated with high morbidity and mortality, difficulties in diagnosis, antimicrobial resistance, length of hospital stay and increased hospital costs. This mini review of the literature describes about epidemiology of hospital infection of Candida species, as well as its virulence factors and drugs resistance

Background: The search for alternative and effective forms of training simulation is needed due to ethical and medico-legal aspects involved in training surgical skills on living patients, human cadavers and living animals. Aims : To evaluate if the bench model fidelity interferes in the acquisition of elliptical excision skills by novice medical students. Materials and Methods: Forty novice medical students were randomly assigned to 5 practice conditions with instructor-directed elliptical excision skills' training (n = 8): didactic materials (control); organic bench model (low-fidelity); ethylene-vinyl acetate bench model (low-fidelity); chicken legs' skin bench model (high-fidelity); or pig foot skin bench model (high-fidelity). Pre- and post-tests were applied. Global rating scale, effect size, and self-perceived confidence based on Likert scale were used to evaluate all elliptical excision performances. Results : The analysis showed that after training, the students practicing on bench models had better performance based on Global rating scale (all P < 0.0000) and felt more confident to perform elliptical excision skills (all P < 0.0000) when compared to the control. There was no significant difference (all P > 0.05) between the groups that trained on bench models. The magnitude of the effect (basic cutaneous surgery skills' training) was considered large (>0.80) in all measurements. Conclusion : The acquisition of elliptical excision skills after instructor-directed training on low-fidelity bench models was similar to the training on high-fidelity bench models; and there was a more substantial increase in elliptical excision performances of students that trained on all simulators compared to the learning on didactic materials.
