Introducción: El Alzhéimer tiene una prevalencia de 1,6% en las personas mayores de 65 años. Se caracteriza por la pérdida de memoria y otras funciones cognoscitivas, pero más del 70% de los afectados presentan síntomas no cognitivos entre los que se encuentran alteraciones del afecto, comportamiento y psicosis. Estos síntomas están asociados a mayores tasas de institucionalización, morbilidad y mortalidad. Métodos: Revisión sistemática de la literatura de artículos que proporcionaron información sobre la eficacia de los inhibidores selectivos de recaptación de serotonina en síntomas no cognitivos del Alzhéimer. Resultados: La búsqueda inicial arrojó un total de 495 resultados, 64 artículos fueron preseleccionados y 7 se incluyeron en la revisión; éstos se clasificaron como nivel de evidencia Ib; citalopram mostró ser más eficaz que el placebo y similar a antipsicóticos para síntomas comportamentales y psicóticos asociados al Alzhéimer con un perfil de efectos adversos más tolerable; sertralina no mostró eficacia en depresión y sí una mayor incidencia de efectos adversos con respecto al placebo. Discusión: Se obtuvieron resultados favorables en síntomas comportamentales y psicóticos pero no en afectivos. Las diferencias metodológicas de los estudios le dan complejidad a la interpretación de los resultados. Conclusión: La evidencia sugiere que el tratamiento con citalopram es eficaz en síntomas comportamentales y psicóticos asociados al Alzhéimer; para el tratamiento de la depresión asociado a esta demencia aún no existe un antidepresivo que pueda considerarse de elección.

Objetivos. Caracterizar el perfil neuropsicológico de una muestra de 22 pacientes diagnosticados con VIH/SIDA, de un hospital de 4to nivel de Bogotá. Materiales y métodos. Estudio descriptivo de tipo exploratorio. Se hizo una descripción de las características neuropsicológicas de las personas con VIH/SIDA. Los resultados de la evaluación neuropsicológica de los sujetos se analizaron con el programa SPSS. Las variables registradas fueron edad, genero, escolaridad, tiempo de diagnóstico y funciones cognitivas superiores. Se incluyeron en el estudio los sujetos con diagnóstico de VIH/SIDA y con reportes de quejas subjetivas de memoria. No se excluyeron aquellos sujetos con antecedentes o presencia de alteraciones psiquiátricas. Resultados. Se creó una base de datos de 22 sujetos, de los cuales predominaron participantes del sexo masculino (77.3%); edad promedio 53,5 años. Se encontró que las funciones con mayor compromiso, sin importar tiempo de diagnóstico, fueron la atención sostenida, la memoria declarativa y la función ejecutiva (control inhibitorio). Las funciones más preservadas fueron las visoespaciales. Conclusiones. Es fundamental que los sujetos con diagnóstico de VIH/SIDA, sean valorados desde un inicio por neuropsicología para incluirlos en un protocolo de prevención y rehabilitación cognitiva específico para dicha población. Se recomienda que el protocolo sea diseñado por un equipo multidisciplinar de diferentes profesionales de la salud.

En las últimas dos décadas el crecimiento de la ciudad ha generado una serie de problemas relacionados con la movilidad y el transporte para los bogotanos. La incapacidad de los distintos gobiernos distritales al proponer soluciones efectivas para mejorar dichas problemáticas, ha provocado que los ciudadanos busquen distintas alternativas para mejorar su movilización. El mercado y la ciudadanía encontraron en las motos una respuesta a los problemas del transporte en la ciudad, sin embargo, esta alternativa viene acompañada de una serie de efectos que por sí mismas repercuten y ahondan el problema de movilidad. Este trabajo pretende determinar las causas y consecuencias del uso de la moto en Bogotá como alternativa de movilización, ello a través de la mirada tanto de conductores de motocicletas como de los demás participantes de las vías y calles de la ciudad.

El presente estudio analiza la práctica del cuidado respiratorio en cinco regiones del país, por medio de una investigación de carácter descriptivo-exploratoria, que busca revisar las tendencias de formación, las condiciones laborales, los roles que ejercen sus practicantes, los dilemas del ejercicio profesional y la caracterización de las funciones de los fisioterapeutas y terapeutas respiratorios. La información se obtuvo mediante encuesta a fisioterapeutas, terapeutas respiratorios, entrevista a profesionales de enfermería y médicos intensivistas que trabajan en instituciones de salud de tercer y cuarto nivel de complejidad en las ciudades seleccionadas para el estudio. La información recopilada incluye desde el quehacer diario, los conocimientos y las preferencias respecto a la práctica de los profesionales, hasta los aspectos relacionados con las condiciones laborales y las características de su ejercicio profesional. El estudio permitió concluir que hay una idealización de los roles, vacíos conceptuales para ejercer eficientemente el cuidado respiratorio sin una formación adicional tipo posgrado o cursos de entrenamiento en el área, debilidad en las funciones administrativas, investigativas, de atención domiciliaria y de evaluación social del entorno, falta de posicionamiento del quehacer ante el equipo interdisciplinario y condiciones laborales precarias.

The electron localization function (ELF) has been proven so far a valuable tool to determine the location of electron pairs. Because of that, the ELF has been widely used to understand the nature of the chemical bonding and to discuss the mechanism of chemical reactions. Up to now, most applications of the ELF have been performed with monodeterminantal methods and only few attempts to calculate this function for correlated wave functions have been carried out. Here, a formulation of ELF valid for mono- and multiconfigurational wave functions is given and compared with previous recently reported approaches. The method described does not require the use of the homogeneous electron gas to define the ELF, at variance with the ELF definition given by Becke. The effect of the electron correlation in the ELF, introduced by means of configuration interaction with singles and doubles calculations, is discussed in the light of the results derived from a set of atomic and molecular systems

O impacto da obesidade na fisiopatologia da pele humana parece relacionar-se com diversas dermatoses, resultado da alteração da sua fisiologia normal, incluindo alterações na função barreira e na função de “envelope”. Contudo, a informação disponível é ainda escassa devido às diversas complexidades do tema. Este trabalho pretende contribuir para a definição de uma metodologia de abordagem experimental para estudar, de forma objectiva, as alterações funcionais que caracterizam a pele obesa. O presente estudo, transversal, incluiu 28 voluntárias, do sexo feminino, saudáveis, com idade média 23±5 anos de idade, após consentimento informado. Foi realizada uma única medição de caracterização das diversas funções cutâneas obtidas por meios não invasivos em condições controladas. As variáveis consideradas relevantes foram, a hidratação (superficial e profunda) a função de barreira e o comportamento biomecânico, medidos em 4 áreas anatómicas distintas. Através do SPSS (v 20.0) realizámos uma análise estatística univariada com cálculo de medidas de tendência central e de dispersão. Recorremos aos testes de Pearson e de Spearman, para as variáveis que seguiam, ou não, uma distribuição normal, respectivamente, adoptando um grau de confiança de 95% . Os resultados permitem propor uma metodologia para o estudo da pele, aplicável ao doente obeso, incluindo a escolha das áreas anatómicas e das variáveis adequadas ao objetivo pretendido.

O objetivo deste estudo foi de analisar os modelos e funções dos planejamentos estratégicos adotados nas instituições de ensino superior (IES) de Palmas, Tocantins. Procuramos, para tanto, enfatizar três grandes práticas empresariais globais que fazem parte do contexto das universidades: a competitividade, visualizada pela introdução da inovação e de novas tecnologias; a mercantilização da educação, que emergiu da forma como a educação é tratada - ‘mercadoria’- e, assim, reflete sobre a autonomia universitária; e, por último, o planejamento estratégico, que tomou grande proporção de uso no ensino superior nas últimas décadas. Frente ao exposto, a questão central que conduziu a pesquisa foi: como as práticas empresariais globais (planejamento estratégico, competitividade e mercantilização da educação) têm influenciado os modelos de gestão adotados pelas universidades frente aos desafios que enfrentam no tocante à oferta da educação superior e como os planejamentos estratégicos expressam essa influência? Nesse sentido, analisamos os documentos dos planejamentos estratégicos de duas IES, uma pública e outra privada, de forma a evidenciar suas diferenças e similaridades por meio dos modelos de gestão e suas principais funções, bem como entrevistamos os gestores das IES sobre suas concepções em relação à influência das práticas empresariais globais quanto ao planejamento estratégico. Como resultado das análises e das entrevistas realizadas, concluímos que o planejamento estratégico é utilizado, pelas IES, como importante ferramenta para formar a imagem institucional, pensar a universidade no futuro, evitar retrabalhos e, principalmente, melhorar os serviços prestados à sociedade.

This report provides a forecast of the potential direct and indirect influences of various kinds of technologies on the LTC milieu, answering the following question: from a technology-driven perspective: “Consider each technological solution. What could be its future usage in the LTC sector?” Future technological deployments will induce changes in the respective roles of the care recipient and of the formal and informal carers, with an impact on three major concerns: the transformation of the care recipient into a proactive subject, the augmented potentiality for home care and the new functions that informal carers could assume.

The skill of numerical Lagrangian drifter trajectories in three numerical models is assessed by comparing these numerically obtained paths to the trajectories of drifting buoys in the real ocean. The skill assessment is performed using the two-sample Kolmogorov–Smirnov statistical test. To demonstrate the assessment procedure, it is applied to three different models of the Agulhas region. The test can either be performed using crossing positions of one-dimensional sections in order to test model performance in specific locations, or using the total two-dimensional data set of trajectories. The test yields four quantities: a binary decision of model skill, a confidence level which can be used as a measure of goodness-of-fit of the model, a test statistic which can be used to determine the sensitivity of the confidence level, and cumulative distribution functions that aid in the qualitative analysis. The ordering of models by their confidence levels is the same as the ordering based on the qualitative analysis, which suggests that the method is suited for model validation. Only one of the three models, a 1/10° two-way nested regional ocean model, might have skill in the Agulhas region. The other two models, a 1/2° global model and a 1/8° assimilative model, might have skill only on some sections in the region

The current energy requirements system used in the United Kingdom for lactating dairy cows utilizes key parameters such as metabolizable energy intake (MEI) at maintenance (MEm), the efficiency of utilization of MEI for 1) maintenance, 2) milk production (k(l)), 3) growth (k(g)), and the efficiency of utilization of body stores for milk production (k(t)). Traditionally, these have been determined using linear regression methods to analyze energy balance data from calorimetry experiments. Many studies have highlighted a number of concerns over current energy feeding systems particularly in relation to these key parameters, and the linear models used for analyzing. Therefore, a database containing 652 dairy cow observations was assembled from calorimetry studies in the United Kingdom. Five functions for analyzing energy balance data were considered: straight line, two diminishing returns functions, (the Mitscherlich and the rectangular hyperbola), and two sigmoidal functions (the logistic and the Gompertz). Meta-analysis of the data was conducted to estimate k(g) and k(t). Values of 0.83 to 0.86 and 0.66 to 0.69 were obtained for k(g) and k(t) using all the functions (with standard errors of 0.028 and 0.027), respectively, which were considerably different from previous reports of 0.60 to 0.75 for k(g) and 0.82 to 0.84 for k(t). Using the estimated values of k(g) and k(t), the data were corrected to allow for body tissue changes. Based on the definition of k(l) as the derivative of the ratio of milk energy derived from MEI to MEI directed towards milk production, MEm and k(l) were determined. Meta-analysis of the pooled data showed that the average k(l) ranged from 0.50 to 0.58 and MEm ranged between 0.34 and 0.64 MJ/kg of BW0.75 per day. Although the constrained Mitscherlich fitted the data as good as the straight line, more observations at high energy intakes (above 2.4 MJ/kg of BW0.75 per day) are required to determine conclusively whether milk energy is related to MEI linearly or not.

Insulin is a prebiotic food ingredient, which suppresses colon tumour growth and development in rats. In the gut lumen, it is fermented to lactic acid and short chain fatty acids (SCFA). Of these, butyrate has suppressing agent activities, but little is known concerning cellular responses to complex fermentation samples. To investigate the effects of fermentation products of insulin on cellular responses related to colon carcinogenesis. Fermentations were performed in anaerobic batch cultures or in a three-stage fermentation model that simulates conditions in colon-segments (proximal, transverse, distal). Substrate was insulin enriched with oligofructose (Raftilose® Synergy1), fermented with probiotics (Bifidobacterium lactis Bb12, Lactobacillus rhamnosus GG), and/or faecal inocula. HT29 or CaCo-2 cells were incubated with supernatants of the fermented samples (2.5%-25% v/v, 24-72 hours). Cellular parameters of survival, differentiation, tumour progression, and invasive growth were determined. Fermentation supernatants derived from probiotics and Synergy1 were more effective than with glucose. The additional fermentation with faecal slurries produced supernatants with lower toxicity, higher SCFA contents, and distinct cellular functions. The supernatant derived from the gut model vessel representing the distal colon, was most effective for all parameters, probably on account of higher butyrate-concentrations. Biological effects of insulin upon colon cells may be mediated not only by growth stimulation of the lactic acid-producing bacteria and/or production of butyrate, but also by other bacteria and products of the gut lumen. These newly reported properties of the supernatants to inhibit growth and metastases in colon tumour cells are important mechanisms of tumour suppression.

Complement-mediated inflammation exacerbates the tissue injury of ischaemic necrosis in heart attacks and strokes, the most common causes of death in developed countries. Large infarct size increases immediate morbidity and mortality and, in survivors of the acute event, larger non-functional scars adversely affect long-term prognosis. There is thus an important unmet medical need for new cardioprotective and neuroprotective treatments. We have previously shown that human C-reactive protein (CRP), the classical acute-phase protein that binds to ligands exposed in damaged tissue and then activates complement(1), increases myocardial and cerebral infarct size in rats subjected to coronary or cerebral artery ligation, respectively(2,3). Rat CRP does not activate rat complement, whereas human CRP activates both rat and human complement(4). Administration of human CRP to rats is thus an excellent model for the actions of endogenous human CRP2,3. Here we report the design, synthesis and efficacy of 1,6-bis(phosphocholine)-hexane as a specific small-molecule inhibitor of CRP. Five molecules of this palindromic compound are bound by two pentameric CRP molecules, crosslinking and occluding the ligand-binding B-face of CRP and blocking its functions. Administration of 1,6-bis(phosphocholine)-hexane to rats undergoing acute myocardial infarction abrogated the increase in infarct size and cardiac dysfunction produced by injection of human CRP. Therapeutic inhibition of CRP is thus a promising new approach to cardioprotection in acute myocardial infarction, and may also provide neuroprotection in stroke. Potential wider applications include other inflammatory, infective and tissue-damaging conditions characterized by increased CRP production, in which binding of CRP to exposed ligands in damaged cells may lead to complement-mediated exacerbation of tissue injury.

Aims: To study the development of resistance responses in Campylobacter jejuni to High Hydrostatic Pressure (HHP) treatments after the exposure to different stressful conditions that may be encountered in food processing environments, such as acid pH, elevated temperatures and cold storage. Methods and Results: C. jejuni cells in exponential and stationary growth phase were exposed to different sublethal stresses (acid, heat and cold shocks) prior to evaluate the development of resistance responses to HHP. For exponential-phase cells, neither of the conditions tested increased nor decreased HHP resistance of C. jejuni. For stationary-phase cells, acid and heat adaptation sensitized C. jejuni cells to the subsequent pressure treatment. On the contrary, cold-adapted stationary-phase cells developed resistance to HHP. Conclusions: Whereas C. jejuni can be classified as a stress sensitive microorganism, our findings have demonstrated that it can develop resistance responses under different stressing conditions. The resistance of stationary phase C. jejuni to HHP was increased after cells were exposed to cold temperatures. Significance and Impact of the Study: The results of this study contribute to a better knowledge of the physiology of C. jejuni and its survival to food preservation agents. Results here presented may help in the design of combined processes for food preservation based on HHP technology.

The problem of identification of a nonlinear dynamic system is considered. A two-layer neural network is used for the solution of the problem. Systems disturbed with unmeasurable noise are considered, although it is known that the disturbance is a random piecewise polynomial process. Absorption polynomials and nonquadratic loss functions are used to reduce the effect of this disturbance on the estimates of the optimal memory of the neural-network model.
