Au Niger, le taux de mortalité maternelle est estimé à 535 décès pour 100 000 naissances vivantes (INS, 2013) et la probabilité pour un nouveau-né de mourir avant l’âge d’un mois est de 33 ‰. Depuis 2006, le Niger a mis en place une politique de gratuité des soins pour les femmes enceintes et les enfants de 0 à 5 ans, ce qui a contribué à une amélioration significative de la fréquentation des centres de santé. En mars 2012, un processus délibératif fut organisé pendant une conférence de trois jours pour échanger sur les acquis, limites et perspectives de cette nouvelle politique avec 160 participants dont des chercheurs, des humanitaires, des décideurs politiques et des intervenants sur le terrain. L’objectif de cette recherche est de comprendre les effets de cette conférence ainsi que d’explorer les activités du comité de suivi de la feuille de route. La recherche a été réalisée durant deux mois en été 2014 à Niamey et à N’guiguimi. Elle a reposé sur l’utilisation du cadre conceptuel de Boyko et al., (2012) qui permet de décrire les principales caractéristiques et les effets attendus des dialogues délibératifs et comprendre comment les dialogues délibératifs peuvent contribuer à l’élaboration de politiques sur la base de données probantes. Nous avons mis un accent particulier sur les trois formes d’utilisation des connaissances présentées par Dagenais et al., (2013) : instrumentale, conceptuelle et persuasive. Des entretiens semi-directifs ont été effectués avec 22 acteurs impliqués dans la mise en oeuvre des recommandations. Ils ont été enregistrés, retranscrits intégralement et traités avec le logiciel QDA Miner. Les résultats de l’analyse des discours recueillis révèlent une utilisation instrumentale des recommandations et plus visible chez les humanitaires que les décideurs et les acteurs de la société civile. Il ressort aussi de cette analyse une utilisation conceptuelle et persuasive des recommandations à un degré plus faible parmi tous les acteurs. Le comité de suivi de la feuille route de la conférence n’a pratiquement pas fonctionné, par conséquent, le processus n’a pas eu l’impact souhaité. Les principales raisons de cet échec sont liées au contexte de mise en oeuvre des recommandations (arrestation de plusieurs agents du ministère de la Santé publique qui sont des membres clés du comité de suivi à cause du détournement des fonds GAVI, manque de volonté technique et politique) et/ou aux conditions financières (absence de primes pour les membres du comité et de budget de fonctionnement.). Les iv résultats obtenus ont permis de comprendre les énormes défis (contextuels, financiers notamment) qui restent à relever en matière de transfert de connaissance dans le secteur de santé publique au Niger. En ce qui concerne la suite de la conférence, il faudrait accélérer la redynamisation du comité de suivi en le dotant d’un fonds de fonctionnement et en créant une agence autonome de gestion de la gratuité des soins; et renforcer le soutien politique autour de l’Initiative Santé Solidarité Sahel.

Taken as a policy framework, active aging ranks high on most supranational bodies’ agenda. The new political economy of aging portrays “active” citizenship amongst seniors as a key challenge for the years to come. Our research focuses on, first, elderly women’s everyday ‘active’ practices, their meaning and purpose, in the context of Quebec’s active aging policy framework; and second, their day-to-day practical citizenship experiences. Informed by discourse analysis and a narrative approach, the life stories of women 60 to 70 years of age allowed for the identification of a plethora of distinctive old age activity figures. More specifically, four activity figures were identified by which respondents materialize their routine active practices, namely: (1) paid work; (2) voluntary and civic engagement; (3) physical activity; and (4) caregiving. Set against Quebec’s active aging policy framework, these patterns and set of practices that underpin them are clearly in tune with government’s dominant perspectives. Respondents’ narratives also show that active aging connotes a range of ‘ordinary’ activities of daily living, accomplished within people’s private worlds and places of proximity. Despite nuances, tensions and opposition found in dominant public discourse, as well as in active aging practices, a form of counter-discourse does not emerge from respondents’ narratives. To be active is normally the antithesis of immobility and dependence. Thus, to see oneself as active in old age draws on normative, positive assumptions about old age quite difficult to refute; nevertheless, discourses also raise identity and relational issues. In this respect, social inclusion issues cut across all active aging practices described by respondents. Moreover, a range of individual aims and quests underpin activity pattern. Such quests express respondents’ subjective interactions with their social environment; including their actions’ meaning and sense of social inclusiveness in old age. A first quest relates to personal identity and social integration to the world; a second one concerns giving; a third centers on the search for authenticity; whereas the fourth one is connected to a desire for freedom. It is through the objectivising of active practices and related existential pursuits that elderly woman recognize themselves as active citizens, rooted in the community, and variously contributing to society. Accordingly, ‘active’ citizenship experiences are articulated in a dialogic manner between the dimensions of ‘doing’, ‘active’ social practices, and ‘being’ in relation to others, within a context of interdependence. A proposed typology allows for the modeling of four ‘active’ citizenship figures. Overall, despite the role played by power relations and social inequality in structuring aging experiences, in everyday life ‘old age citizenship’ appears as a relational process, embedded in a set of social relations and practices involving individuals, families and communities, whereby elderly women are able to express a sense of agency within their social world.

One of the known risk factors for abuse and neglect of the elderly is the decrease in functionat capacity, contributíng to self care dependency of instrumental actívities of daily living and basic activities of daily Itving (OMS, 2015). Methods: Cross-sectional study with non probabilistíc sample of 333 elderly, performed in a hospital, homes and day centers for the elderly. The data collectíon protocol tncluded socio-demographic data, Questíons to elicit Elder Abuse (Carney, Kahan B Paris, 2003 adap. By Ferreira Alves & Sousa, 2005), scale of instrumental actívi - ties of daily living Lawton and Brody and Katz index to assess the levei of independence in actívities of daily living. Objectives: To evaluate the assodation between abuse and neglect in the elderly, instrumental actívitíes of daily living and levei of independence in actívitíes of daily living. Results: Emotional abuse is signifícantty correlated with the levei of independence in activities of daity Uving (p = 0. 000), older peopie with less independence tend to have higher leveis of emotional abuse. The total abuse is signtficantly correlated with the leveis of independence in activittes of daily living (p = 0. 002), less independent elderty tend to suffer greater abuse and neglect. Contact details: catarinaisabeln.amaraliSsmaU.com Introduction: Breast câncer is one of the most common mahgnant pathology in European countries, as Portugal, where annual inddence is around 90 new cases per 100,000 women. Breast surgery is the usual treatment for this pathology, however such procedure can be complicated by the infection of surgical site. Objectives: To know the prevalence and determtnants of surgtcal wound infection after breast surgery. Methods: We conducted a systematic review by searching of the Web of Sdence electronic database for articles published over the last s1x years 1n developed countries. Over three hundred dtatíons were obtained and after excludtng citations with reasons, fíve artícles met our inclusion criteria and were included in the present review. Results: Prevalence of surgical wound infection varied across studies between 0. 1% and 12. 5%. Bilateral mastectomy is assodated with higher prevalence of wound infectíon than unilateral mastectomy (3. 6% vs 3, 3%), lumpectomy with immediate breast reconstruction (IBR) is related with higher frequency of wound infectíon than surgery with no IBR (0, 5% vs 0, 1%), also, mastectomy with IBR is associated with higher prevalence of wound infectíon than mastectomy wtth no IBR (1, 5% vs 0, 3%) and breast surgery followed by axiltary lymph nade dissectíon is related with higher prevalence of wound infection than surgical procedures wtth no axillary lymph node dissection (2, 82% vs 1, 66%). Conclusions: Nurses that provide post-operatíve care to women after breast surgery should be aware about risk of wound tnfectíon, partícularly after more invasive procedures.

Introdução: O burnout é uma síndrome psicológica, caracterizada por elevada exaustão emocional, elevada despersonalização e baixa realização profissional, que conduz à erosão dos valores pessoais, profissionais e de saúde. Este estudo reporta a prevalência do burnout em profissionais de saúde Portugueses. Material e Métodos: Os níveis de burnout foram estimados pelo Maslach Burnout Inventory - Human Services Survey numa escala ordinal de zero (nunca) a seis (sempre) pontos. A amostra foi constituída por 1 262 enfermeiros e 466 médicos com médias de idade de 36,8 anos (DP = 12,2) e 38,7 (DP = 11,0), respetivamente. Os participantes foram provenientes de todos os distritos nacionais (35% Lisboa; 18% Porto; 6% Aveiro, 6% Setúbal, 5% Coimbra; 5% regiões autónomas), com atuação em meio hospitalar (54%), centros de saúde (Unidade de Saúde Familiar - 30%; Unidades de Cuidados de Saúde Primários - 8%) e outras instituições públicas/privadas (8%). Resultados: A análise dos níveis de burnout revelou que ambas as categorias profissionais apresentaram níveis moderados a elevados de burnout (M = 3,0; DP = 1,7) não sendo significativas as diferenças entre as duas profissões. Vila Real (M = 3,8; SD = 1,7) e a Madeira (M = 2,5; DP = 1,5) são as regiões onde os níveis de burnout são mais e menos elevados, respetivamente. Os níveis de burnout não diferiram significativamente entre Hospitais, Unidades de Cuidados de Saúde Personalizados e Unidades de Saúde Familiares. Os profissionais com maior tempo na função são menos acometidos por burnout (r = -0,15) não ocorrendo associação significativa com a duração da jornada de trabalho (r = 0,04). A má qualidade das condições de trabalho foi o melhor preditor do burnout (r = -0,35). Discussão: A ocorrência da síndrome de burnout em profissionais de saúde portugueses é frequente, estando associada à percepção de más condições de trabalho e à menor duração do tempo de serviço. A incidência de burnout apresenta diferenças regionais que podem estar associadas ao aumento do stress imposto pelo exercício da profissão em condições sub-ótimas para a prestação dos cuidados de saúde. Os resultados alertam para a necessidade de intervenções para melhorar as condições de trabalho e formação inicial dos profissionais de saúde de forma a garantir a qualidade do serviço prestado aos utentes e o bem-estar pessoal destes profissionais. Conclusões: A nível nacional, entre 2011 e 2013, 21,6% dos profissionais de saúde apresentaram burnout moderado e 47,8% burnout elevado. A perceção de más condições de trabalho foi o principal preditor da ocorrência de burnout nos profissionais de saúde Portugueses.

nd-of-life care is not usually a priority in cardiology departments. We sought to evaluate the changes in end-of-life care after the introduction of a do-not-resuscitate (DNR) order protocol. Retrospective analysis of all deaths in a cardiology department in two periods, before and after the introduction of the protocol. Comparison of demographic characteristics, use of DNR orders, and end-of-life care issues between both periods, according to the presence in the second period of the new DNR sheet (Group A), a conventional DNR order (Group B) or the absence of any DNR order (Group C). The number of deaths was similar in both periods (n = 198 vs. n = 197). The rate of patients dying with a DNR order increased significantly (57.1% vs. 68.5%; P = 0.02). Only 4% of patients in both periods were aware of the decision taken about cardiopulmonary resuscitation. Patients in Group A received the DNR order one day earlier, and 24.5% received it within the first 24 h of admission (vs. 2.6% in the first period; P < 0.001). All patients in Group A with an implantable cardioverter defibrillator (ICD) had shock therapies deactivated (vs. 25.0% in the first period; P = 0.02). The introduction of a DNR order protocol may improve end-of-life care in cardiac patients by increasing the use and shortening the time of registration of DNR orders. It may also contribute to increase ICD deactivation in patients with these orders in place. However, the introduction of the sheet in late stages of the disease failed to improve patient participation.
