To supplement other environmental monitoring programs and to protect the health of people consuming fish from waters within this state, the state of Iowa conducts fish tissue monitoring. Since 1980, the Iowa Department of Natural Resources (IDNR), the United States Environmental Protection Agency Region VII (U.S. EPA), and the State Hygienic Laboratory (SHL) have cooperatively conducted annual statewide collections and analyses of fish for toxic contaminants. Beginning in 1983, this monitoring effort became known as the Regional Ambient Fish Tissue Monitoring Program (RAFT). Currently, the RAFT program is the only statewide fish contaminant-monitoring program in Iowa. Historically, the data generated from the RAFT program have enabled IDNR to document temporal changes in contaminant levels and to identify Iowa lakes and rivers where high levels of contaminants in fish potentially threaten the health of fish-consuming Iowans (see IDNR 2006). The Iowa RAFT monitoring program incorporates five different types of monitoring sites: 1) status, 2) trend, 3) random, 4) follow-up and 5) turtle.

The authors pooled data from 15 case-control studies of head and neck cancer (9,107 cases, 14,219 controls) to investigate the independent associations with consumption of beer, wine, and liquor. In particular, they calculated associations with different measures of beverage consumption separately for subjects who drank beer only (858 cases, 986 controls), for liquor-only drinkers (499 cases, 527 controls), and for wine-only drinkers (1,021 cases, 2,460 controls), with alcohol never drinkers (1,124 cases, 3,487 controls) used as a common reference group. The authors observed similar associations with ethanol-standardized consumption frequency for beer-only drinkers (odds ratios (ORs) = 1.6, 1.9, 2.2, and 5.4 for < or =5, 6-15, 16-30, and >30 drinks per week, respectively; P(trend) < 0.0001) and liquor-only drinkers (ORs = 1.6, 1.5, 2.3, and 3.6; P < 0.0001). Among wine-only drinkers, the odds ratios for moderate levels of consumption frequency approached the null, whereas those for higher consumption levels were comparable to those of drinkers of other beverage types (ORs = 1.1, 1.2, 1.9, and 6.3; P < 0.0001). Study findings suggest that the relative risks of head and neck cancer for beer and liquor are comparable. The authors observed weaker associations with moderate wine consumption, although they cannot rule out confounding from diet and other lifestyle factors as an explanation for this finding. Given the presence of heterogeneity in study-specific results, their findings should be interpreted with caution.

Background: It is unknown whether cerebral perfusion in geriatric and younger patients under general anaesthesia differs. Methods: We compared 2 groups of patients undergoing elective major non-cardiac surgery under standardized general anaesthesia (thiopental, sevoflurane, fentanyl, atracurium). Group 1: 18-40 yrs (n = 20), Group 2: >65 yrs (n = 37). Cerebral perfusion was investigated with transcranial Doppler and near-infrared spectroscopy (NIRS). Arterial blood pressure was monitored continuously with a Finapres device. Mx, an index allowing continuous monitoring of cerebrovascular autoregulation based on the changes in mean arterial blood pressure (MAP) and cerebral blood flow velocity was calculated. Data are shown as mean } SD. Results: MAP (86 } 9.6 vs 79 } 10.9 mm Hg, p = 0.02), end-tidal concentration of sevoflurane (1.9 } 0.3 vs 1.6 } 0.3%, p <0.01), and the cerebral tissue oxygenation index measured by NIRS (72 } 4 vs 68 } 5%, p = 0.01), were significantly lower in Group 2. The end-tidal concentration of O2 was significantly higher in Group 2 (46 } 4 vs 48 } 4% p = 0.04). There were no significant differences between Group 1 and 2 for cerebral blood flow velocity (41 } 10 vs 43 } 18 cm/s), end tidal CO2 (4.7 } 0.3 vs 4.6 } 0.3 kPa) and cerebrovascular autoregulation (Mx 0.42 } 0.2 vs 0.48 } 0.2). In Group 1 35% and in Group 2 43% of the patients had an index of autoregulation suggesting disturbed cerebrovascular autoregulation (p = n.s.). Conclusions: In elderly patients under general anaesthesia with sevoflurane the cerebral tissue oxygenation index was significantly lower than in younger patients despite higher end-tidal oxygen concentrations. Dose kernel convolution (DK) methods have been proposed to speed up absorbed dose calculations in molecular radionuclide therapy. Our aim was to evaluate the impact of tissue density heterogeneities (TDH) on dosimetry when using a DK method and to propose a simple density-correction method. METHODS: This study has been conducted on 3 clinical cases: case 1, non-Hodgkin lymphoma treated with (131)I-tositumomab; case 2, a neuroendocrine tumor treatment simulated with (177)Lu-peptides; and case 3, hepatocellular carcinoma treated with (90)Y-microspheres. Absorbed dose calculations were performed using a direct Monte Carlo approach accounting for TDH (3D-RD), and a DK approach (VoxelDose, or VD). For each individual voxel, the VD absorbed dose, D(VD), calculated assuming uniform density, was corrected for density, giving D(VDd). The average 3D-RD absorbed dose values, D(3DRD), were compared with D(VD) and D(VDd), using the relative difference Δ(VD/3DRD). At the voxel level, density-binned Δ(VD/3DRD) and Δ(VDd/3DRD) were plotted against ρ and fitted with a linear regression. RESULTS: The D(VD) calculations showed a good agreement with D(3DRD). Δ(VD/3DRD) was less than 3.5%, except for the tumor of case 1 (5.9%) and the renal cortex of case 2 (5.6%). At the voxel level, the Δ(VD/3DRD) range was 0%-14% for cases 1 and 2, and -3% to 7% for case 3. All 3 cases showed a linear relationship between voxel bin-averaged Δ(VD/3DRD) and density, ρ: case 1 (Δ = -0.56ρ + 0.62, R(2) = 0.93), case 2 (Δ = -0.91ρ + 0.96, R(2) = 0.99), and case 3 (Δ = -0.69ρ + 0.72, R(2) = 0.91). The density correction improved the agreement of the DK method with the Monte Carlo approach (Δ(VDd/3DRD) < 1.1%), but with a lesser extent for the tumor of case 1 (3.1%). At the voxel level, the Δ(VDd/3DRD) range decreased for the 3 clinical cases (case 1, -1% to 4%; case 2, -0.5% to 1.5%, and -1.5% to 2%). No more linear regression existed for cases 2 and 3, contrary to case 1 (Δ = 0.41ρ - 0.38, R(2) = 0.88) although the slope in case 1 was less pronounced. CONCLUSION: This study shows a small influence of TDH in the abdominal region for 3 representative clinical cases. A simple density-correction method was proposed and improved the comparison in the absorbed dose calculations when using our voxel S value implementation.

L'agitation postopératoire chez les patients adultes dans les premières heures après le réveil¦est un domaine peu étudié. Nous avons effectué des recherches pour trouver des études¦concernant l'agitation postopératoire sur Pubmed pour les 10 dernières années. Les termes de¦recherche étaient agitation, emergence delirium, facteurs de risques (anglais : risk factors),¦agitation treatment, Riker scale. Ceci n'a permis de trouver que 2 études à propos de ce sujet.¦Pourtant, elle concerne environ 5% (1) (2) des patients adultes opérés, ce qui n'est pas¦négligeable.¦En effet, elle peut avoir de sérieuses conséquences en terme de lésions des patients¦(arrachement de cathéter, de tube d'intubation) ou du personnel soignant, ainsi qu'en terme de¦coûts occasionnés par les différentes complications.¦Selon l'étude de Lepousé (1) et l'étude de Radtke (2), certains facteurs seraient déterminants¦dans la survenue de l'agitation postopératoire, dont quelques-uns sont évitables.¦Dans l'étude observationnelle prospective de Radtke (2), il a déjà été démontré que l'âge était¦en lien avec l'agitation postopératoire. Les tranches d'âge les plus à risque sont : les moins de¦40 ans (18-39 ans) (OR=2.1, CI=1.1-3.8, p=0.02) et les plus de 64 ans (OR=2.1, CI=1.1-3.4,¦p=0.02). Les études de Lepousé (1) et de Radtke (2) ont montré que la prémédication par¦benzodiazépines pourrait aussi être un facteur de risque (Lepousé (1): CI=1.1-3.3, p=0.02)¦(Radtke (2): OR=2.4, CI=1.0-5.6, p≤0.05). L'intensité de la douleur est également liée à la¦survenue d'une agitation postopératoire (OR=1.8, CI=1.0-3.0, p<0.04) (2). D'autres facteurs¦comme certains médicaments utilisés lors de l'anesthésie (l'étomidate) (2), le type de¦chirurgie [abdominale (1) (2), musculosquelettique (2), de la tête et du cou(2), de la¦poitrine(1)] seraient en lien avec la survenue d'une agitation postopératoire.¦4¦Il restait toutefois beaucoup de facteurs dont la relation avec l'agitation postopératoire n'avait¦pas été évaluée ou dont l'étude n'a pas eu de résultat significatif [par exemple, les¦benzodiazépines à long terme (1), ou l'anesthésie locorégionale(2)].¦Le but de cette étude était premièrement de déterminer si la fréquence de l'agitation¦postopératoire dans la population prise en compte ici est comparable à celle observée dans les¦études de Radtke et de Lepousé.¦Ensuite il s'agissait d'identifier, lors d'agitation postopératoire chez des patients adultes,¦parmi certains facteurs possibles, ceux pouvant être en lien avec l'agitation postopératoire, et¦s'ils étaient similaires à ceux retrouvés dans les études déjà publiées.¦Les comorbidités sont un de ces facteurs. La dose de benzodiazépines dans les 24 heures¦précédent l'intervention, ainsi que les benzodiazépines en médication chronique, par un effet¦paradoxal, pourraient être en lien avec un état d'agitation. Dans le même ordre d'idée, nous¦avons comparé le type d'anesthésie utilisé, locorégionale versus générale ou combinée, avec¦la survenue de cet état. Nous nous sommes aussi penchés sur la dose intraopératoire totale¦d'opioïdes et l'administration intraopératoire d'atropine, médicaments largement utilisés lors¦des interventions, et qui seraient des facteurs modifiables si une relation avec l'agitation était¦démontrée. Enfin, l'hypotension intraopératoire provoquant une hypoperfusion cérébrale¦relative pourrait être liée à la survenue d'une agitation postopératoire.¦Nous avons également observé d'autres variables, comme l'intensité de la douleur, déjà¦démontrée comme étant en rapport avec l'agitation postopératoire, et l'hypoxie¦intraopératoire, qui pourrait favoriser une hypoxie cérébrale, et par là une agitation.¦Enfin, nous avons cherché s'il existe un lien entre la survenue d'une agitation postopératoire¦et d'un état confusionnel postopératoire, c'est-à-dire se développant dans la semaine suivant¦l'opération, une question qui n'avait pas encore été étudiée.

Background: The possible additional risk of infection in patients receiving induction with both basiliximab (Ba) and thymoglobulin (Th) is unclear. We assessed the 1-year incidence of infectious complications in 3 groups of kidney transplant recipients according to the type of induction therapy received.Methods: We compared the incidence of infection at 1 year in 3 groups of patients at our institution: fi rst transplant recipients received Ba 20mg at days 0 and 4 (Group Ba); in case of retransplantation or if PRA was >20% patients received Th 1 mg/kg for 3-5 days (Group Th); in case of delayed graft function (DGF), Ba was discontinued and Th was initiated (Group Ba+Th) or prolonged in Group Th. Kaplan-Meier curves were used to calculate the incidence of infection. A Cox analysis was used to identify risk factors for the development of infection.Results: Over 5 years, 170 consecutive kidney transplant recipients were performed:n=113 in Group Ba, n=39 in Group Th and n=18 in Group Ba+Th. As expected, more patients in Group Th received a second transplant (p<0.001). No differences in CMV serostatus were observed between groups (p=0.9). Incidences of CMV infection, CMV disease, BK viremia, BK nephropathy and urinary tract infection (UTI) is shown in Table 1. There was a trend towards higher incidence of BK viremia, but not nephropathy in Group Ba+Th (OR 2.2, p=0.23). There were no signifi cant differences in kidney function or graft loss at 1 year between groups.Conclusion: By multivariate analysis, we observed a lower risk of CMV infection andUTI in patients receiving Ba. The group Ba+Th had a similar risk for infection than the group receiving Th alone. Larger studies are needed to clarify whether combining Ba+Th in the setting of DGF may increase the risk of infectious complications, in particular BK infection.

OBJECTIVE: To compare the efficacy and safety of misoprostol (prostaglandin E(1) (PGE(1))) with dinoprostone (prostaglandin E(2) (PGE(2))) for third trimester cervical ripening and labor induction. STUDY DESIGN: Patients requiring induction of labor were randomly assigned to receive either 50 microg of intravaginal misoprostol every 4 h or 0.5 mg of intracervical dinoprostone gel every 6 h. Eligibility criteria included gestation = 36 weeks. Primary outcome was the time interval from induction to delivery; secondary outcomes were mode of delivery, perinatal outcome, and interpretation of cardiotocogram (CTG) records. RESULTS: Two hundred women were randomly enrolled to receive either misoprostol (n = 100) or dinoprostone (n = 100). Time induction-to-delivery at 12, 24 and 48 h and the need for oxytocin were reduced with misoprostol (P < 0.05). Pathological CTG tracing according to FIGO and Melchior scores were more frequent in the misoprostol-treated group (P < 0.001). CONCLUSION: Misoprostol shortened the induction-to-delivery interval, but is associated with a higher incidence of abnormal CTG than prostaglandin E(2).

Prospective comparative evaluation of patent V blue, fluorescein and (99m)TC-nanocolloids for intraoperative sentinel lymph node (SLN) mapping during surgery for non-small cell lung cancer (NSCLC). Ten patients with peripherally localised clinical stage I NSCLC underwent thoracotomy and peritumoral subpleural injection of 2 ml of patent V blue dye, 1 ml of 10% fluorescein and 1ml of (99m)Tc-nanocolloids (0.4 mCi). The migration and spatial distribution pattern of the tracers was assessed by direct visualisation (patent V blue), visualisation of fluorescence signalling by a lamp of Wood (fluorescein) and radioactivity counting with a hand held gamma-probe ((99m)Tc-nanocolloids). Lymph nodes at interlobar (ATS 11), hilar (ATS 10) and mediastinal (right ATS 2,4,7; left ATS 5,6,7) levels were systematically assessed every 10 min up to 60 min after injection, followed by lobectomy and formal lymph node dissection. Successful migration from the peritumoral area to the mediastinum was observed for all three tracers up to 60 min after injection. The interlobar lympho-fatty tissue (station ATS 11) revealed an early and preferential accumulation of all three tracers for all tumours assessed and irrespective of the tumour localisation. However, no preferential accumulation in one or two distinct lymph nodes was observed up to 60 min after injection for all three tracers assessed. Intraoperative SLN mapping revealed successful migration of the tracers from the site of peritumoral injection to the mediastinum, but in a diffuse pattern without preferential accumulation in sentinel lymph nodes.

BACKGROUND: Takayasu arteritis (TA) is a rare form of chronic inflammatory granulomatous arteritis of the aorta and its major branches. Late gadolinium enhancement (LGE) with magnetic resonance imaging (MRI) has demonstrated its value for the detection of vessel wall alterations in TA. The aim of this study was to assess LGE of the coronary artery wall in patients with TA compared to patients with stable CAD. METHODS: We enrolled 9 patients (8 female, average age 46±13 years) with proven TA. In the CAD group 9 patients participated (8 male, average age 65±10 years). Studies were performed on a commercial 3T whole-body MR imaging system (Achieva; Philips, Best, The Netherlands) using a 3D inversion prepared navigator gated spoiled gradient-echo sequence, which was repeated 34-45 minutes after low-dose gadolinium administration. RESULTS: No coronary vessel wall enhancement was observed prior to contrast in either group. Post contrast, coronary LGE on IR scans was detected in 28 of 50 segments (56%) seen on T2-Prep scans in TA and in 25 of 57 segments (44%) in CAD patients. LGE quantitative assessment of coronary artery vessel wall CNR post contrast revealed no significant differences between the two groups (CNR in TA: 6.0±2.4 and 7.3±2.5 in CAD; p = 0.474). CONCLUSION: Our findings suggest that LGE of the coronary artery wall seems to be common in patients with TA and similarly pronounced as in CAD patients. The observed coronary LGE seems to be rather unspecific, and differentiation between coronary vessel wall fibrosis and inflammation still remains unclear.

Abstract:The objective of this work was to evaluate the effect of limestone particle sizes in the diet and of lighting regimes on the egg and bone quality and on the performance of commercial laying hens. Three hundred Hissex White layers, at 18 weeks of age, were distributed in a completely randomized design, in a 5×2 factorial arrangement (coarse limestone in the diet at 0, 25, 50, 75, and 100%; with or without artificial light), with five replicates of six birds. No significant interaction was observed between particle sizes and lighting regime for the evaluated parameters. There was no significant effect of coarse limestone level in the diet on the performance and egg quality of hens; however, bone deformity (3.23 to 4.01 mm), strength (5.19 to 6.70 kgf cm-2), and mineral matter (51.09 to 59.61%) improved as the proportion of coarse limestone increased. For lighting regime, the treatment with artificial light yielded higher Haugh unit values (87.17 vs. 85.54) than that with natural light only. Greater limestone particles improve bone quality of laying hens, and the use of artificial light can benefit the albumen quality of the eggs.

It has been shown that for the reaction catalyzed by "biodegradative" L-threonine dehydratase from E. coli strains K-12 and 980 in 0.5 M phosphate-carbonate buffer, pH 8.4 and pH 9.5, the plots of initial reaction rate (v) versus the initial substrate concentration ([S]0 are characterized by several inflection points, i. e. an intermediate plateau. The plot of v versus the allosteric activator (AMP) concentration have very complicated shapes: there are several inflection points, and also the maximum at L-threonine concentration equal to 3-10(2) and 5-10(-2) M. High AMP concentrations inhibit the enzyme at high substrate concentrations. The reduced glutathion dose not influence the enzyme and does not alter the activating effect of AMP. On the basis of the data obtained it is proposed that the substrate and AMP shift the equilibrium between multiple oligomeric enzyme forms differing in catalytic activity and kinetic manifestations of allosteric interactions between the active and allosteric AMP-binding sites towards polymerization. Thus, the functioning the enzyme under study is discussed in the frames of the model of dissociating regulatory enzymes with multiple intermediate oligomeric forms.

O trabalho teve como objetivo avaliar um sistema de micropropagação para a cultivar Prata-Anã, observando-se os principais fatores de eficiência e limitação no processo produtivo da muda. Os explantes foram estabelecidos em meio nutritivo MS, suplementado com 5mg/L de BAP (benzilaminopurina) e 30g/L de sacarose solidificado com 8g/L de ágar e pH ajustado em 5,7. A fase de multiplicação foi composta pelo mesmo meio de cultura e a de enraizamento constituída pela metade da concentração dos sais de MS e de sacarose, sem regulador de crescimento. Perdas por contaminação bacteriana foram maiores durante o estabelecimento in vitro e por fungos, nos dois últimos subcultivos. As maiores taxas de multiplicação ocorreram entre o 4º e o 5º subcultivos e, ao final do processo, observou-se maior proporção de brotos na classe entre 30 e 60mm. Com relação à eficiência da utilização da mão-de-obra no laboratório, observou-se que o processamento diário de frascos por operador foi baixo nas fases de estabelecimento, primeiro subcultivo e enraizamento, quando comparado com as fases de multiplicação exponencial dos explantes. Apesar de não terem sido constatadas perdas na fase de aclimatação, foi observada a ocorrência de 1% do total das plântulas com anomalias morfológicas.

Tässä tutkimuksessa tarkastellaan, mitä sidosryhmät odottivat voittoa tavoittelemattomalta case-yritykseltä Lappeenranta Innovation Oy:ltä sen perustamisen yhteydessä ja reilun vuoden toiminnan jälkeen. Tutkimuksessa selvitetään myös niitä kehittämisehdotuksia, joita sidosryhmillä on yritykselle. Tutkimus on laadullinen tapaustutkimus jasen aineisto koostuu dokumenteista, mediateksteistä ja haastatteluista. Menetelminä on käytetty teemoittelua ja diskurssianalyysiä. Tutkimuksen tuloksena löytyi kuusi alkuvaiheen odotuskokonaisuutta: 1) yrittäjien auttaminen; 2) tutkimuksen ja liiketoiminnan yhdistäminen; 3) rahoituksen hakeminen; 4) public-private -yhteistyön kehittäminen; 5) Venäjä-yhteistyön kehittäminen ja6) verkostojen luominen. Reilun vuoden toiminnan jälkeen odotukset ovat muuttuneet jonkin verran: public-private-yhteistyön odotuksen merkitys on kasvanut ja uutena odotuksena on tullut IP-oikeuksien suojaaminen. Kehittämisehdotuksia tutkimuksessa löytyi 10 eri toiminta-alueelta: yritys-kehitys; rahoitus; Venäjä-yhteistyö; verkostot; maakunnallisuus; toiminnan laajuus; yhteistyö sidosryhmien kanssa; tiedottaminen, asioista kertominen, selkeys; fokusointi ja priorisointi; sisäisen toiminnan kehittäminen ja hallituksen rakenne.

O trabalho foi desenvolvido no laboratório de Frutas e Hortaliças do Departamento de Gestão e Tecnologia Agroindustrial da Faculdade de Ciências Agronômicas - UNESP - Câmpus de Botucatu, tendo como objetivo principal a caracterização do comportamento da radiação gama, na conservação pós-colheita da nectarina cv. "Sunred". Os frutos foram colhidos no início do estádio de maturação, selecionados, limpos, pré-resfriados (4ºC por 12 horas) e submetidos a diferentes doses de radiação gama, constituindo assim os tratamentos: 1 0,0 kGy, 2 - 0,2 kGy, 3 - 0,4 kGy, 4 - 0,6 kGy, 5 - 0,8 kGy, sendo após armazenados em câmara fria com temperaturas de 0ºC e 90-95% de UR, por 28 dias. As análises foram realizadas a cada sete dias, determinando-se o aspecto visual dos frutos, a perda de massa fresca, a firmeza de polpa, a acidez total titulável (ATT), os sólidos solúveis totais (STT) e a razão STT/ATT. Após 28 dias de armazenamento, verificou-se que os frutos submetidos à dose de 0,4 kGy apresentaram o melhor aspecto visual, as menores perdas de massa fresca, e a maior firmeza de polpa, não ocorrendo, entretanto, variações significativas nos teores de acidez total titulável, sólidos solúveis totais e nos valores da razão SST/ATT.
