Back pain is a considerable economical burden in industrialised countries. Its management varies widely across countries, including Switzerland. Thus, the University Hospital and University of Lausanne (CHUV) recently improved intern processes of back pain care. In an already existing collaborative context, the two university hospitals in French-speaking Switzerland (CHUV, University Hospital of Geneva), felt the need of a medical consensus, based on a common concept. This inter-hospital consensus produced three decisional algorithms that bear on recent concepts of back pain found in literature. Eventually, a fast track was created at CHUV, to which extern physicians will have an organised and rapid access. This fast track aims to reduce chronic back pain conditions and provides specialised education for general practitioners-in-training.

Peut-on accepter les progrès en sciences biomèdicales sans progrès en éthique ? [Can we take for granted medical progress without progress in ethics ?].

Since the mid 20th century progress in biomedical science has been punctuated by the emergence of bioethics which has fashioned the moral framework of its application to both research and clinical practice. Can we, however, consider the advent of bioethics as a form of progress marking the advances made in biomedical science with an adequate ethical stamp? The argument put forward in this chapter is based on the observation that, far from being a mark of progess, the development of bioethics runs the risk of favouring, like modern science, a dissolution of the links that unite ethics and medicine, and so of depriving the latter of the humanist dimensions that underlie the responsibilities that fall to it. Faced with this possible pitfall, this contribution proposes to envisage as a figure of moral progress, consubstantial with the development of biomedical science, an ethical approach conceived as a means of social intervention which takes the first steps towards an ethics of responsibility integrating the bioethical perspective within a hermeneutic and deliberative approach. By the yardstick of a prudential approach, it would pay particular attention to the diverse sources of normativity in medical acts. It is suggested that this ethical approach is a source of progress insofar as it constitutes an indispensable attitude of watchfulness, which biomedical science can lean on as it advances, with a view to ensuring that the fundamental link uniting ethics and medicine is maintained.

Psychotropic drug prescriptions in hospitalized ederly psychiatric patients: comparison with adult psychiatric patients

RÉSUMÉ Comparaison dés habitudes de prescription de médicaments psychotropes dans des cliniques de psychiatrie adulte et de psychogériatrie Afin de pouvoir comparer l'utilisation de médicaments psychotropes et non psychotropes, la proportion des nouveaux et celle des anciens antidépresseurs ou antipsychotiques, ce travail a eu pour but d'étudier les prescriptions médicamenteuses dans deux groupes de patients hospitalisés, l'un en milieu psychiatrique adulte (de 18 à 64 ans), l'autre en milieu psychogériatrique (plus de 64 ans). Lors d'un jour de référence en Mai 2000, toutes les prescriptions médicamenteuses dans deux hôpitaux psychiatriques universitaires abritant l'un une population adulte, l'autre gériatrique, ont été relevées chez tous les patients. Le coût financier total par patient a été comparé en tenant compte de la proportion des médicaments non psychotropes. La médication de 61 patients adultes et de 82 patients gériatriques a ainsi été analysée. Le nombre moyen de médicaments non psychotropes par patient était plus élevé dans la population âgée (p< 0.001), ce qui se reflète également par une prescription totale de médicaments par patient en moyenne plus élevée dans cette population (p<0.001). L'utilisation de benzodiazépines était inférieure dans là population psychogériatrique (p<0.001), même si l'on y additionne celle en association avec les antidépresseurs (p<0.001). Le coût financier du traitement pharmacologique quotidien d'un patient adulte était significativement inférieur à celui d'un patient gériatrique dont la comédication somatique est nécessairement plus importante (9.3 ± 7.2 CHF/patient contre 14.1 ± 9.5 CHF/patient) (p<0.009). En conclusion, cette étude confirme l'importance des habitudes locales dans la prescription médicamenteuse par les médecins, à l'exception de l'utilisation des benzodiazépines pour lesquelles les psychogériâtres semblent moins favorables.

La partie initiale de la consultation médicale, la phase sociale, constitue la première pierre dans la construction d'une relation médecin-patient de confiance et de qualité. Si, d'un point de vue structurel, la littérature a répondu de façon claire et concordante, des questions demeurent ouvertes d'un point de vue procédural. De quelle manière le médecin parvient-il à établir le premier contact ? Comment procède-t-il pour accueillir son patient ? Des pistes pour répondre à ces questions se repèrent dans le travail de révision des enregistrements vidéo des consultations de médecine générale qui sont régulièrement pratiqués à la Policlinique médicale universitaire (PMU) de Lausanne. The initial step of the medical consultation, the social phase, is the cornerstone in the construction of a doctor-patient relationship of trust and quality. If, in a structural point of view, the literature has responded in a clear and consistent way, questions remain openned in a procedural point of view. How successful is the physician to establish the first contact? How does he proceed to welcome his patient? We looked out ways to address these issues by the work of revising the video recordings of general medical consultations, which are regularly practiced at the Medical outpatient clinic of the University of Lausanne.

Implementation of a medical command and control team in Switzerland.

In case of a major incident or disaster, the advance medical rescue command needs to manage several essential tasks simultaneously. These include the rapid deployment of ambulance, police, fire and evacuation services, and their coordinated activity, as well as triage and emergency medical care on site. The structure of such a medical rescue command is crucial for the successful outcome of medical evacuation at major incidents. However, little data has been published on the nature and structure of the command itself. This study presents a flexible approach to command structure, with two command heads: one emergency physician and one experienced paramedic. This approach is especially suitable for Switzerland, whose federal system allows for different structures in each canton. This article examines the development of these structures and their efficiency, adaptability and limitations with respect to major incident response in the French-speaking part of the country.

Hemophilia Registry of the Medical Committee of the Swiss Hemophilia Society: Update and Annual Survey 2008.

The Swiss Haemophilia Registry of the Medical Committee of the Swiss Haemophilia Society was established in 2000. Primarily it bears epidemiological and basic clinical data (incidence, type and severity of the disease, age groups, centres, mortality). Two thirds of the questions of the WFH Global Survey can be answered, especially those concerning use of concentrates (global, per capita) and treatment modalities (on-demand versus prophylactic regimens). Moreover, the registry is an important tool for quality control of the haemophilia treatment centres. There are no informations about infectious diseases like hepatitis or HIV, due to non-anonymisation of the data. We plan to incorporate the results of the mutation analysis in the future.
