OBJECTIVE: To describe the microsurgical anatomy, branches, and anatomic relationships of the posterior cerebral artery (PCA) represented in three-dimensional images. METHODS: Seventy hemispheres of 35 brain specimens were studied. They were previously injected with red silicone and fixed in 10% formalin for at least 40 days. Four of the studied specimens were frozen at -10 degrees to -15 degrees C for 14 days, and additional dissection was done with the Klingler`s fiber dissection technique at x6 to x40 magnification. Each segment of the artery was measured and photographed to obtain three-dimensional stereoscopic images. RESULTS: The PCA origin was in the interpeduncular cistern at the pontomesencephalic junction level in 23 specimens (65.7%). The PCA was divided into four segments: P1 extends from the PCA origin to its junction with the posterior communicating artery with an average length of 7.7 mm; P2 was divided into an anterior and posterior segment. The P2A segment begins at the posterior communicating artery and ends at the most lateral aspect of the cerebral peduncle, with an average length of 23.6 mm, and the P2P segment extends from the most lateral aspect of the cerebral peduncle to the posterior edge of the lateral surface of the midbrain, with an average length of 16.4 mm; P3 extends from the posterior edge of the lateral surface of the midbrain and ends at the origin of the parieto-occipital sulcus along the calcarine fissure, with an average length of 19.8 mm; and the P4 segment corresponds to the parts of the PCA that run along or inside both the parieto-occipital sulcus and the distal part of the calcarine fissure. CONCLUSIONS: To standardize the neurosurgical practice and knowledge, surgical anatomic classifications should be used uniformly and further modified according to the neurosurgical experience gathered. The PCA classification proposed intends to correlate its anatomic segments with their required microneurosurgical approaches.

Tentorial meningiomas account for 2% to 3% of all intracranial meningiomas. The authors present their experience with posterior fossa tentorial meningiomas, and discuss the main features, which influence approaches and complications of the different surgical techniques. Twenty-four patients had meningiomas localized predominantly in posterior fossa. Their historical records and radiologic examinations were reviewed in accordance with Simpson`s classification. The extension of tumor removal was Simpson grade I in 12 patients (50%), grade II in 12 patients (50%), and grades III and IV in none of the patients. In 22 patients (91.66%), the meningioma was classified as grade I and in 2 cases (8.33%) classified as grade 11 (atypical meningioma). The combined supra/infiratentorial was employed in 12 cases, and complete resections were most common with this approach compared with retrosigmoid technique. Postoperative complications occurred in 10 patients (41.6%) with major deficits in 3 patients (12.5%). The authors believe that careful preoperative choice of the surgical approach should be based Oil tumor location and extension. It is then possible to achieve the best radical microsurgical tumor resection, avoiding additional injury to neurovascular structures.

Introdução: A ruptura uterina leva a consequências graves materno-fetais. A maioria dos casos ocorre em grávidas com cesarianas anteriores ou incisões uterinas prévias como miomectomia, raramente ocorrendo em úteros sem cicatrizes. Um dos principais factores correlacionado com o risco de ruptura é o tipo de incisão da histerotomia prévia: clássica (4-9%), em T (4-9%), vertical (1-7%); transversa (0,2-1,5%). Outros factores de risco são: ausência de parto vaginal anterior, indução do trabalho de parto, gravidez de termo, macrossomia fetal, multiparidade, sutura simples vs.dupla na histerorrafia prévia e intervalo curto entre gestações. 1-Caso clínico: Grávida, 28 anos, IO 2002 (cesariana em 2002 por apresentação pélvica; PTE em 2009), enviada ao nosso hospital para esclarecimento de anemia às 21 semanas. A gravidez decorreu normalmente; entrando espontaneamente em trabalho de parto em Agosto/2010. No período expulsivo a grávida referiu dor pélvica súbita com irradiação lombar. Teve um parto eutócico com distócia de ombros leve. Duas horas após, a puérpera apresentava-se inquieta, pálida e hipotensa comHb de 7,3g/dl. Decidiu-se laparotomia, constatando-se ruptura uterina no segmento inferior com prolongamento para a parede posterior, realizando-se histerorrafia.Pós-operatório sem intercorrências. 2-Caso clínico: Grávida, 41 anos, IO 0000, antecedentes pessoais de miomectomia por laparoscopia sem entrada na cavidade em 2008 e 2009, enviada ao nosso hospital para Consulta de DPN. Foi internada às 17+3 semanas para IMG por alteração do cariótipo fetal (Trissomia 21). Iniciou-se o protocolo para IMG aplicando-se unicamente 100 microg de misoprostol;24 horas após, a doente encontrava-se agitada e hipotensa, com episódio de lipotímia. Realizou-se laparotomia com visualização de ruptura uterina fúndica, corrigida com histerorrafia sem intercorrências Conclusão: Dada amorbi/mortalidade materno-fetal associada à ruptura uterina é fundamental reconhecer os factores de risco e os sintomas associados a esta, tal como o seu diagnóstico atempado e resolução imediata, minimizando os riscos materno-fetais.
