Increasingly, patients with unhealthy alcohol and other drug use are being seen in primary care and other non-specialty addiction settings. Primary care providers are well positioned to screen, assess, and treat patients with alcohol and other drug use because this use, and substance use disorders, may contribute to a host of medical and mental health harms. We sought to identify and examine important recent advances in addiction medicine in the medical literature that have implications for the care of patients in primary care or other generalist settings. To accomplish this aim, we selected articles in the field of addiction medicine, critically appraised and summarized the manuscripts, and highlighted their implications for generalist practice. During an initial review, we identified articles through an electronic Medline search (limited to human studies and in English) using search terms for alcohol and other drugs of abuse published from January 2010 to January 2012. After this initial review, we searched for other literature in web-based or journal resources for potential articles of interest. From the list of articles identified in these initial reviews, each of the six authors independently selected articles for more intensive review and identified the ones they found to have a potential impact on generalist practice. The identified articles were then ranked by the number of authors who selected each article. Through a consensus process over 4 meetings, the authors reached agreement on the articles with implications for practice for generalist clinicians that warranted inclusion for discussion. The authors then grouped the articles into five categories: 1) screening and brief interventions in outpatient settings, 2) identification and management of substance use among inpatients, 3) medical complications of substance use, 4) use of pharmacotherapy for addiction treatment in primary care and its complications, and 5) integration of addiction treatment and medical care. The authors discuss each selected articles' merits, limitations, conclusions, and implication to advancing addiction screening, assessment, and treatment of addiction in generalist physician practice environments.

La interacció de l’home amb les diferents espècies amb les quals cohabita ha creat casos com el del Voltor Negre (Aegypius monachus), l’au necròfaga més gran d’Europa, que actualment es troba en perill d’extinció. La utilització del territori per a ús antropogènic (ramaderia, xarxes elèctriques, zones urbanes,...) ha fet disminuir la població d’aquesta espècie. Així com la incursió, de manera indirecta, en aquestes poblacions degut a l’acció de l’home podem destacar-ne’n dos, el més rellevant és la intoxicació per verí, degut a l’ús il·legal d’aquestes substàncies per al control de depredadors fa que els tòxics vagin pujant per la piràmide tròfica. La segona causa rellevant és la disminució d’aliment degut a la mixomatosis i a la normativa existent d’obligatorietat de retirada dels animals morts a muntanya per sota dels 1400m. Al fer l’anàlisi de problemàtiques, s’observa que la gran majoria són antròpiques, degudes a la desinformació i al desconeixement, per tant; per a realitzar un bon pla de reintroducció és necessari fer un treball previ de socialització de les poblacions de la zona per a que el projecte sigui eficient. El projecte consisteix en fer una conscienciació social als diferents grups implicats (turisme, escoles i població; ramaders i caçadors), mitjançant documents gràfics, material educatiu, material de suport i materials i mètodes de divulgació. Per a que la vall d’Alinyà i Boumort passin a ser destacades per la presencia establerta de les quatre aus necròfagues més rellevants d’Europa; l’Aufrany, el Trencalòs, el voltor comú i el nostre protagonista: el Voltor Negre.

Contrary to what Felipe Pedrell indicates, the second Ave maris stella in his Victoria's collected works (vol. V, 1908, pp. 100-3, n° 33) doesn't appear in the collection published in 1600 in Madrid by the composer, nor in any other of the musician's books. In the 1600 edition, Victoria reissues the two first verses (plainchant followed by polyphony) of the Ave maris stella published in 1576 and then again in 1581. The earliest source of the problematic Ave maris stella is Munich, Bayerische Staatsbibliothek, Musik-Abteilung, 2 Mus. pr. 23 handschriftlicher Beiband, dating from the third quarter oft he seventeenth century. This source is a manuscrit that runs as an appendix to the 1581 edition of Victoria's hymns. No attributions are given in the manuscript. The first attributions of the piece to Victoria arise in the nineteenth century, in manuscripts copied by Johann Michael Hauber, Johann Caspar Aiblinger, August Baumgartner and Carl Proske, and preserved in Munich and Regensburg. Proske pubished the piece in his Musica divina in 1859 (Annus primus, vol. III, pp. 419-24). The most probable hypothesis ist that Pedrell had knowledge of the second Ave maris stella, under the spanish composer's name, via Proske's Musica divina. In all likelihood the piece is not by Victoria, not least because the composer has never written odd polyphonic verses of hymns. In his Studies in the Music of Tomás Luis de Victoria (2001), Eugene Casjen Cramer relies on the supposed authenticity of the work to ascribe the others pieces of Munich, Bayerische Staatsbibliothek, Musik-Abteilung, 2 Mus. pr. 23 handschriftlicher Beiband to the composer. These attributions should therefore be refuted.

OBJECTIVE: To report a novel phenotype of autosomal dominant atypical congenital cataract associated with variable expression of microcornea, microphthalmia, and iris coloboma linked to chromosome 2. Molecular analysis of this phenotype may improve our understanding of anterior segment development. DESIGN: Observational case study, genome linkage analysis, and gene mutation screening. PARTICIPANTS: Three families, 1 Egyptian and 2 Belgians, with a total of 31 affected were studied. METHODS: Twenty-one affected subjects and 9 first-degree relatives underwent complete ophthalmic examination. In the Egyptian family, exclusion of PAX6, CRYAA, and MAF genes was demonstrated by haplotype analysis using microsatellite markers on chromosomes 11, 16, and 21. Genome-wide linkage analysis was then performed using 385 microsatellite markers on this family. In the 2 Belgian families, the PAX6 gene was screened for mutations by direct sequencing of all exons. MAIN OUTCOME MEASURES: Phenotype description, genome-wide linkage of the phenotype, linkage to the PAX6, CRYAA, and MAF genes, and mutation detection in the PAX6 gene. RESULTS: Affected members of the 3 families had bilateral congenital cataracts inherited in an autosomal dominant pattern. A novel form of hexagonal nuclear cataract with cortical riders was expressed. Among affected subjects with available data, 95% had microcornea, 39% had microphthalmia, and 38% had iris coloboma. Seventy-five percent of the colobomata were atypical, showing a nasal superior location in 56%. A positive lod score of 4.86 was obtained at theta = 0 for D2S2309 on chromosome 2, a 4.9-Mb common haplotype flanked by D2S2309 and D2S2358 was obtained in the Egyptian family, and linkage to the PAX6, CRYAA, or MAF gene was excluded. In the 2 Belgian families, sequencing of the junctions and all coding exons of PAX6 did not reveal any molecular change. CONCLUSIONS: We describe a novel phenotype that includes the combination of a novel form of congenital hexagonal cataract, with variably expressed microcornea, microphthalmia, and atypical iris coloboma, not caused by PAX6 and mapping to chromosome 2. In recent decades, technological advances have made extensive documentation available to us. But the philologist must be aware of the dangers of poor use of the documentary corpus in order to avoid creating dreaded ghost words. In this paper we recall the main sources of this type of error: folk etymology phenomena among speakers, copyists" errors, transcribers" errors in the interpretation of some abbreviations and graphic variants of the manuscripts, onomastic changes introduced by cartographers" ignorance of linguistic variants, gaps in the dating of some documents, confusion in the processes of lemmatization and the evaluation of texts... All these sources of error contribute, to a greater or lesser degree, to the distortion or to the masking of the data on which the research of philologists is based. Hence the importance of philological rigour in the transmission and study of ancient texts.

Insect attack triggers changes in transcript level in plants that are mediated predominantly by jasmonic acid (JA). The implication of ethylene (ET), salicylic acid (SA), and other signals in this response is less understood and was monitored with a microarray containing insect- and defense-regulated genes. Arabidopsis thaliana mutants coi1-1, ein2-1, and sid2-1 impaired in JA, ET, and SA signaling pathways were challenged with the specialist small cabbage white (Pieris rapae) and the generalist Egyptian cotton worm (Spodoptera littoralis). JA was shown to be a major signal controlling the upregulation of defense genes in response to either insect but was found to suppress changes in transcript level only in response to P. rapae. Larval growth was affected by the JA-dependent defenses, but S. littoralis gained much more weight on coi1-1 than P. rapae. ET and SA mutants had an altered transcript profile after S. littoralis herbivory but not after P. rapae herbivory. In contrast, both insects yielded similar transcript signatures in the abscisic acid (ABA)-biosynthetic mutants aba2-1 and aba3-1, and ABA controlled transcript levels both negatively and positively in insect-attacked plants. In accordance with the transcript signature, S. littoralis larvae performed better on aba2-1 mutants. This study reveals a new role for ABA in defense against insects in Arabidopsis and identifies some components important for plant resistance to herbivory.

De nombreuses méthodes de classification automatique de textes (supervisée ou non) existent. Elles se sont particulièrement développées depuis la création du web en vue d'améliorer les moteurs de recherche. Au delà de ce champs d'application, elles peuvent aussi être utiles pour « découvrir » ou « redécouvrir » ce qui caractérise une classe de textes aussi simplement que possible. Dans l'approche statistique développée ici, il s'agira de partitionner des parties de textes en types de discours tels que narratif, explicatif, argumentatif etc. en utilisant un minimum d'information, à savoir les catégories morphosyntaxiques (CMS). Les premiers résultats obtenus mettent en évidence des liens significatifs entre les CMS, les types de discours et des textes annotés par un expert humain ; ces relations sont toutefois encore incomplètes et difficiles à interpréter d'un point de vue linguistique.

A morte súbita dos citros é uma doença que afeta cultivares de laranjas e tangerinas enxertadas nos limões-Cravo e Volkameriano. Ela foi observada em plantas com dois a seis anos de idade que, após mostrarem sintomas gerais de declínio, entraram em colapso e morreram. A retirada da casca dos porta-enxertos suscetíveis revela o amarelecimento na região cambial, sendo esse o sintoma-diagnóstico da doença e que precede os sintomas da copa. As plantas enxertadas nas tangerinas Cleópatra e Sunki, no trifoliata e no citrumelo Swingle, não mostram sintomas da doença. A transmissão por borbulha e a evolução espacial sugerem que a morte súbita dos citros seja causada por patógeno transmitido por vetor alado. Com o objetivo de selecionar porta-enxertos tolerantes à doença, laranjeiras Valência enxertadas em 254 porta-enxertos foram plantadas em maio de 2003 e 2004 em solos onde foram erradicados pomares afetados pela morte súbita dos citros e próximos a pomares afetados pela doença. Em novembro de 2006, o sintoma-diagnóstico da doença estava presente em dez seleções de limão-Cravo: Santa Barbara red lime, Borneo red lime, Limão-Cravo Taquaritinga, Rangpur India C-26-1, Rangpur rose lemon, Rangpur Kusaie lime, Rangpur red lime D-33-40, Rangpur Egyptian lime, Rangpur lemon India e Japanshe citroen. A presença de interenxerto de trifoliata ou de tangerina Cleópatra, entre o limão-Cravo e a laranja Valência, não impediu a manifestação da doença.
