BACKGROUND: The unimodal approach of using pentazocine as post-cesarean section pain relief is inadequate, hence the need for a safer, easily available and more effective multimodal approach. AIM: To evaluate the effectiveness of rectal diclofenac combined with intramuscular pentazocine for postoperative pain following cesarean section. METHODS: In this double blind clinical trial, 130 pregnant women scheduled for cesarean section under spinal anesthesia were randomly assigned to two groups. Group A received 100mg diclofenac suppository and group B received placebo suppository immediately following surgery, 12 and 24h later. Both groups also received intramuscular pentazocine 30mg immediately following surgery and 6 hourly postoperatively in the first 24 h. Postoperative pain was assessed by visual analogue scale at end of surgery and 2, 12 and 24 h after surgery. Patient satisfaction scores were also assessed. RESULTS: One hundred and sixteen patients completed the study. Combining diclofenac and pentazocine had statistically significant reduction in pain intensity at 2, 12, and 24 hours postoperatively compared to pentazocine alone (p <0.05). No significant side effects were noted in both groups. The combined group also had significantly better patient satisfaction scores. CONCLUSION: The addition of diclofenac suppository to intramuscular pentazocine provides better pain relief after cesarean section and increased patient satisfaction.

We report results from the analysis of intact polar lipids (IPLs) in sediments from Ocean Drilling Program Sites 1257 and 1258. IPLs, constituting the cell membranes of living organisms, were detected in organic-lean sediments but not in underlying organic-rich black shales. Microbial activity in organic-lean sediments is likely due to sulfate-dependent oxidation of methane whereas difficulties detecting IPLs in black shales are interpreted to result from unfavorable signal-to-noise ratios due to low cell concentrations in combination with extremely high analytical noise created by uncharacterized organic matrix. IPLs found are consistent with a low-diversity community of archaea and bacteria. The concentrations of IPLs are more than one order of magnitude lower than those in Neogene deep subsurface sediments at the Peruvian margin, suggestive of significantly lower cell concentrations in Demerara Rise. This finding is consistent with inferred low rates of subsurface microbial activity.

Study objective: To examine the relationship between work stress, as indicated by the job strain model and the effort-reward imbalance model, and smoking. Setting: Ten municipalities and 21 hospitals in Finland. Design and Participants: Binary logistic regression models for the prevalence of smoking were related to survey responses of 37 309 female and 8881 male Finnish public sector employees aged 17-65. Separate multinomial logistic regression models were calculated for smoking intensity for 8130 smokers. In addition, binary logistic regression models for ex-smoking were fitted among 16 277 former and current smokers. In all analyses, adjustments were made for age, basic education, occupational status, type of employment and marital status. Main results: Respondents with high effort-reward imbalance or lower rewards were more likely to be smokers. Among smokers, an increased likelihood of higher intensity of smoking was associated with higher job strain and higher effort-reward imbalance and their components such as low job control and low rewards. Smoking intensity was also higher in active jobs in women, in passive jobs and among employees with low effort expenditure. Among former and current smokers, high job strain, high effort-reward imbalance and high job demands were associated with a higher likelihood of being a current smoker. Lower effort was associated with a higher likelihood of ex-smoking. Conclusions: This evidence suggests an association between work stress and smoking and implies that smoking cessation programs may benefit from the taking into account the modification of stressful features of work environment. Size and shape tuneable ZnO nanostructures were prepared by a low frequency ultrasound (42 kHz) route using various organic solvents as the reaction media. The crystalline nature, lattice parameters and microstructural parameters such as microstrain, stress and energy density of the prepared ZnO nanostructures were revealed through X-ray diffraction (XRD) analysis. The organic solvents influenced the size and morphology of the ZnO nanostructures, and interesting morphological changes involving a spherical to triangular shaped transition were observed. The visible emission properties and lattice vibrational characteristics of the nanostructures were drastically modified by the changes in size and shape. Raman spectral measurements revealed the presence of multiphonon processes in the ZnO nanostructures. The intensity of the visible emission band was found to vary with the size and morphology of the structures. The strongest visible emission band corresponded to the structure with the largest surface/volume ratio and could be attributed to surface oxygen vacancies. The control over the size and morphology of ZnO nanostructures has been presented as a means of determining the intensity of the visible emission band

Animals show behavioral and physiological changes that emerge in response to environmental perturbations (i.e., emergency life-history stages). In this study, we investigate the effects of light intensity on aggressive encounters and social stability in groups of adult male Nile tilapia, Oreochromis niloticus (Linnaeus, 1758). The study compared the behavior observed under low (280.75 ± 50.60 lx) and high (1394.14 ± 520.32 lx) light intensities, with 12 replicates for each treatment. Adult fish were isolated in 36-L aquaria for 96 hours, and three males were grouped for 11 days in 140-L aquaria. Agonistic behavior was video-recorded (10 min/day) on the 3rd, 5th, 7th, and 9th day to quantify aggressive interactions and social stability. There was an effect of light intensity and day of observation on the total number of agonistic behaviors performed by the fish group. Besides, increased frequency of aggressive interactions (the sum of the four sessions) by the alpha, beta and gamma fish occurred at the higher light intensity. The dominance ranks of the fish remained unchanged across the observation sessions under both the low and high light intensities. We concluded that enhanced light intensity has a cumulative effect that increases the aggressiveness of the Nile tilapia but that this effect is not sufficiently strong to destabilize the social hierarchy.

In this study, we considered earthquakes with a maximum observed intensity (MOI) Io≥V that occurred throughout the Portuguese mainland and its adjacent Atlantic region to produce the map of maximum intensities of Portugal. This map is based on a wide variety of historical and recent sources, including 175 earthquakes, over the period of 1300–2014. This MOI map allows the regions of high, medium, and low levels of seismic hazard to be highlighted. The entirety of Portugal has been affected by major seismic events, some of which have caused serious damage. In addition, offshore earthquakes have had significant impacts on the coastal cities located in the central and southern regions of the country. Because the southern region of mainland Portugal is more active than the northern region, the largest concentrations of events with Io≥VI are in the southern region, especially on the mainland near the city of Évora, in the Lisbon region, in the Lower Tagus Valley region, and along the Algarve coast, especially in the southwest region in Cape of São Vicente and the Gorringe Bank.

A low temperature synthesis method based on the decomposition of urea at 90°C in water has been developed to synthesise fraipontite. This material is characterised by a basal reflection 001 at 7.44 Å. The trioctahedral nature of the fraipontite is shown by the presence of a 06l band around 1.54 Å, while a minor band around 1.51 Å indicates some cation ordering between Zn and Al resulting in Al-rich areas with a more dioctahedral nature. TEM and IR indicate that no separate kaolinite phase is present. An increase in the Al content however, did result in the formation of some SiO2 in the form of quartz. Minor impurities of carbonate salts were observed during the synthesis caused by to the formation of CO32- during the decomposition of urea.

The behavior of the hydroxyl units of synthetic goethite and its dehydroxylated product hematite was characterized using a combination of Fourier transform infrared (FTIR) spectroscopy and X-ray diffraction (XRD) during the thermal transformation over a temperature range of 180-270 degrees C. Hematite was detected at temperatures above 200 degrees C by XRD while goethite was not observed above 230 degrees C. Five intense OH vibrations at 3212-3194, 1687-1674, 1643-1640, 888-884 and 800-798 cm(-1), and a H2O vibration at 3450-3445 cm(-1) were observed for goethite. The intensity of hydroxyl stretching and bending vibrations decreased with the extent of dehydroxylation of goethite. Infrared absorption bands clearly show the phase transformation between goethite and hematite: in particular. the migration of excess hydroxyl units from goethite to hematite. Two bands at 536-533 and 454-452 cm(-1) are the low wavenumber vibrations of Fe-O in the hematite structure. Band component analysis data of FTIR spectra support the fact that the hydroxyl units mainly affect the a plane in goethite and the equivalent c plane in hematite.
