Objetivou-se descrever o perfil reprodutivo de mulheres com diabetes mellitus (DM) e verificar o nível de conhecimento destas quanto aos riscos maternos e fetais e os cuidados pré-concepcionais. Estudo exploratório, que contou com a participação de 106 mulheres, realizado no Centro Integrado de Hipertensão e Diabetes, de março a julho de 2009. As variáveis reprodutivas foram: número de gestações, partos e abortos e planejamento da gravidez. Os dados foram coletados por meio de entrevista que seguiu um formulário pré-estabelecido. O perfil reprodutivo de mulheres com DM mostrou-se permeado de riscos e repercussões reprodutivas negativas à saúde materna e fetal. Das 106 (100%) mulheres estudadas, 44 (41,5%) apresentaram conhecimento moderado sobre os cuidados pré-concepcionais e 58 (54,7%) conhecimento limitado sobre os riscos maternos e fetais. Faz-se necessário oferecer informações às mulheres a fim de promover o conhecimento sobre os riscos maternos e fetais e os cuidados pré-concepcionais.

It was prepared as an edocument and relies mainly on links to other documents which, when combined, make up the complete report. The outline that follows names the issues being presented this year by the CJJPAC and is both a table of contents and a “site-map” for the report.

Levels of circulating glucose are tightly regulated. To identify new loci influencing glycemic traits, we performed meta-analyses of 21 genome-wide association studies informative for fasting glucose, fasting insulin and indices of beta-cell function (HOMA-B) and insulin resistance (HOMA-IR) in up to 46,186 nondiabetic participants. Follow-up of 25 loci in up to 76,558 additional subjects identified 16 loci associated with fasting glucose and HOMA-B and two loci associated with fasting insulin and HOMA-IR. These include nine loci newly associated with fasting glucose (in or near ADCY5, MADD, ADRA2A, CRY2, FADS1, GLIS3, SLC2A2, PROX1 and C2CD4B) and one influencing fasting insulin and HOMA-IR (near IGF1). We also demonstrated association of ADCY5, PROX1, GCK, GCKR and DGKB-TMEM195 with type 2 diabetes. Within these loci, likely biological candidate genes influence signal transduction, cell proliferation, development, glucose-sensing and circadian regulation. Our results demonstrate that genetic studies of glycemic traits can identify type 2 diabetes risk loci, as well as loci containing gene variants that are associated with a modest elevation in glucose levels but are not associated with overt diabetes.

Estudo quase-experimental com objetivo de avaliar a qualidade de vida relacionada à saúde de pessoas com diabetes mellitus, antes e após participação em um programa educativo de cinco meses. Participaram 51 sujeitos, com predomínio do sexo feminino (56,9%), média e desvio-padrão de 57,65 ± 11,44 anos de idade, em um serviço de atenção primária no interior paulista, em 2008. Para obtenção dos dados utilizou-se o questionário SF-36. O instrumento mostrou-se confiável para a população estudada, sendo os valores de alpha de Cronbach para os componentes físico e mental 0,83 e 0,89, respectivamente. Os resultados mostraram melhora discreta em quase todos os domínios, embora apenas o estado geral de saúde antes (63,96 ± 19,03) e após (70,59 ± 17,82) o programa educativo tenha apresentado diferença estatisticamente significativa t(50) = -2,16, p < 0,05. A participação dos sujeitos no programa educativo em diabetes mellitus também contribuiu para a melhoria da percepção acerca de seu estado geral de saúde.

Este estudo objetivou descrever as variáveis sociodemográficas e comparar as morbidades e a qualidade de vida (QV) dos idosos com diabetes mellitus (DM) residentes nas zonas urbana e rural. A amostra foi composta de 271 idosos da zona urbana e 104 da rural que autorreferiram DM. Utilizou-se análise descritiva e, na comparação das localidades, realizou-se ajuste para a idade por meio de regressão logística e linear múltipla (p < 0,05). Os idosos da zona rural eram mais jovens, casados e possuíam maior escolaridade e renda em relação àqueles da área urbana. Além disso, apresentaram maior escore de QV nos domínios físico e relações sociais e nas facetas autonomia, atividades passadas, presentes e futuras e intimidade, em relação aos do espaço urbano, nos quais se verificou maior número de comorbidades. Os idosos com DM da zona rural apresentaram, de forma geral, melhores condições de saúde em comparação aos que residiam na área urbana.

OBJECTIVE: To develop a provisional definition for the evaluation of response to therapy in juvenile dermatomyositis (DM) based on the Paediatric Rheumatology International Trials Organisation juvenile DM core set of variables. METHODS: Thirty-seven experienced pediatric rheumatologists from 27 countries achieved consensus on 128 difficult patient profiles as clinically improved or not improved using a stepwise approach (patient's rating, statistical analysis, definition selection). Using the physicians' consensus ratings as the "gold standard measure," chi-square, sensitivity, specificity, false-positive and-negative rates, area under the receiver operating characteristic curve, and kappa agreement for candidate definitions of improvement were calculated. Definitions with kappa values >0.8 were multiplied by the face validity score to select the top definitions. RESULTS: The top definition of improvement was at least 20% improvement from baseline in 3 of 6 core set variables with no more than 1 of the remaining worsening by more than 30%, which cannot be muscle strength. The second-highest scoring definition was at least 20% improvement from baseline in 3 of 6 core set variables with no more than 2 of the remaining worsening by more than 25%, which cannot be muscle strength (definition P1 selected by the International Myositis Assessment and Clinical Studies group). The third is similar to the second with the maximum amount of worsening set to 30%. This indicates convergent validity of the process. CONCLUSION: We propose a provisional data-driven definition of improvement that reflects well the consensus rating of experienced clinicians, which incorporates clinically meaningful change in core set variables in a composite end point for the evaluation of global response to therapy in juvenile DM.

Physical activity has a beneficial effect on the cardiovascular risk and the well-being in patients with type 2 diabetes. Thereby, both aerobic physical activities and resistance activities are recommended. DIAfit is a programm for patients with type 2 diabetes that is being implemented in Switzerland. Its objective is to allow the initiation of a structured physical activity in the setting of a pluridisciplinary team to promote a healthy lifestyle.

We present a cross-sectional study that aims to describe the sociodemographic and clinical conditions of individuals with diabetes mellitus and to analyze their knowledge of treatment five years after the end of an educational program in which they took part. In 2010, 40 individuals who had participated in a diabetes educational program for 12 months in 2005 at a primary care service were interviewed. A form was used for data collection that included their knowledge of the notion, physiopathology, and treatment of the disease; exercise; nutrition; foot care; self-monitoring of capillary blood glucose at home; hypoglycemia; chronic complications; special situations; and family support. The results showed that the volunteers incorporated the information about the notion, physiopathology, and treatment of the disease; exercise; foot care; self-monitoring; care associated with hypoglycemia; chronic complications; and special situations. In contrast, nutrition and family support require further reinforcement. It is concluded that five years after the end of the educational program, the participants kept most of the information provided.

In the absence of works which would significantly change the perspective on the management of diabetes in the elapsed year, this article proposes a reflection on the integration of the evolving knowledge over the past decade into clinical practice. The major preventive impact of an approach targeting all the cardiovascular risk factors in diabetic patients will remain as the main lesson of this decade. The therapeutic goals need to be tailored to the individual patient's situation based on the evaluation of the benefit: inconvenience-ratio of the treatments. The process of their choice has to include the quest for a shared vision with the patient who is in charge of diabetes management in daily life.

This cross-sectional study aimed to analyze the adherence to drug and non-drug treatments in 17 Family Health Strategy units. A total of 423 patients with type 2 diabetes mellitus were selected through stratified random sampling in Family Health Strategy units of a city in the state of Minas Gerais, Brazil, in 2010. The results showed that the prevalence rate of adherence to drug therapy was higher than 60% in the 17 units investigated; in relation to physical activity, adherence was higher than 60% in 58.8% units; and for the diet plan, there was no adherence in 52.9% units. Therefore, we concluded that adherence to drug therapy in most units was high and the practice of physical activity was heterogeneous, and in relation to diet adherence, it was low in all units. We recommend strengthening of institutional guidelines and educational strategies, in line with SUS guidelines, so that, professionals may face the challenges imposed by the lack of adherence.
