Objetivo: Identificar el nivel de conocimiento, satisfacción y capacitación de las enfermeras con relación a la prescripción enfermera tras la aprobación de la Ley 28/2009. Método: Estudio observacional descriptivo transversal realizado en la Escuela de Enfermería de la Universidad de Barcelona. Los sujetos a estudio fueron 485 enfermeras en formación máster o especialización y docentes de dicha escuela. Las variables principales de estudio fueron: satisfacción, conocimiento e información de la aprobación de la Ley, conocimiento de los elementos autorizados a prescribir, percepción de capacidad prescriptora de las enfermeras y opinión de la necesidad formativa e informativa respecto a la Ley. Como secundarias: años de experiencia profesional, ámbito laboral, curso formativo y sociodemográficas. La recogida de datos se realizó entre los meses de marzo y abril de 2010 mediante un cuestionario autoaplicable ad hoc de 29 ítems, probado previamente. Resultados: Participaron 345 enfermeras, el 87,2% (301) alumnas en formación máster. El 72,3% (238) de las enfermeras conocían la aprobación de la ley, pero un 95,9% (303) indicó tener insuficiente información. La satisfacción media con la aprobación de la Ley se situó en 6,24 ± 2,08. El 57,6% (189) de las enfermeras opinaron estar capacitadas para prescribir productos sanitarios y un 41,3% (134), para prescribir fármacos. Conclusiones: Hay satisfacción con la aprobación de la Ley 28/2009. Actualmente muchas enfermeras prescriben productos farmacológicos y sanitarios de forma autónoma.

Persistent schizophrenias and delusional disorders are classified as primary psychiatric pathologies amongst the elderly. It is crucial to distinguish them from secondary psychotic disorders associated with physical illnesses, such as acute confusion and psychotic symptoms caused by dementia or other somatic pathologies. Employing the concept of a primary psychiatric disorder occurring in an elderly patient is not simple, and each term used to define the concept refers back to an array of various criteria in clinical, psychological, biological, neurological, and cognitive fields. What about very late-onset schizophrenia, occurring after the age of 60 years, for instance? Is this a primary psychiatric illness occurring very late or a secondary pathology caused by brain disease, particularly a degenerative one? Studies reveal controversial results and it is still being debated as to whether the disease has neurodevelopmental or neurodegenerative causes. Due to the variable symptoms and psychiatric, somatic, and cognitive comorbidities associated with psychosis in elderly patients, patient healthcare must not be limited to prescribing an antipsychotic. Once it has been determined whether the psychosis is secondary or primary (old-agerelated schizophrenia, late-onset or very late-onset schizophrenia, or late-onset delusional disorder), an aetiological or symptomatic treatment must follow, including a psychotherapeutic approach, close surveillance of the drug treatment and its potential side-effects, rehabilitation steps through community-based care, and psychoeducational support for the family and other professionals in charge of the patient. Our article's aim has been restricted to summarising our understanding regarding late-onset schizophrenias and delusional disorders amongst the elderly.

Heterozygosity-fitness correlations (HFCs) have been used to understand the complex interactions between inbreeding, genetic diversity and evolution. Although frequently reported for decades, evidence for HFCs was often based on underpowered studies or inappropriate methods, and hence their underlying mechanisms are still under debate. Here, we used 6100 genome-wide single nucleotide polymorphisms (SNPs) to test for general and local effect HFCs in maritime pine (Pinus pinaster Ait.), an iconic Mediterranean forest tree. Survival was used as a fitness proxy, and HFCs were assessed at a four-site common garden under contrasting environmental conditions (total of 16 288 trees). We found no significant correlations between genome-wide heterozygosity and fitness at any location, despite variation in inbreeding explaining a substantial proportion of the total variance for survival. However, four SNPs (including two non-synonymous mutations) were involved in significant associations with survival, in particular in the common gardens with higher environmental stress, as shown by a novel heterozygosity-fitness association test at the species-wide level. Fitness effects of SNPs involved in significant HFCs were stable across maritime pine gene pools naturally growing in distinct environments. These results led us to dismiss the general effect hypothesis and suggested a significant role of heterozygosity in specific candidate genes for increasing fitness in maritime pine. Our study highlights the importance of considering the species evolutionary and demographic history and different spatial scales and testing environments when assessing and interpreting HFCs.

Initiated within the first 72hours of the rash, prescribing antiviral drugs reduces both acute neuralgia (AN) and later complications and especially postherpetic neuralgia (PHN). But their analgesic as well as preventative effect on AN and PHN is modest. Combination with analgesic drugs is more often needed for pain management. However, the pharmacological management of pain, in the context of old patients' frailty, co-morbidities and often polypharmacy, must be carefully considered. Based on analyses of the evidences from the literature, this review presents the therapeutic options we have at one's disposal and proposes a stepwise management for both AN and PHN specifically designed for aged population.

OBJECTIVE: To perform a critical review focusing on the applicability in clinical daily practice of data from three randomized controlled trials (RCTs): SWOG 8794, EORTC 22911, and ARO/AUO 96-02. METHODS AND MATERIALS: An analytical framework, based on the identified population, interventions, comparators, and outcomes (PICO) was used to refine the search of the evidence from the three large randomized trials regarding the use of radiation therapy after prostatectomy as adjuvant therapy (ART). RESULTS: With regard to the inclusion criteria: (1) POPULATION: in the time since they were designed, in two among three trial (SWOG 8794 and EORTC 22911) patients had a detectable PSA at the time of randomization, thus representing de facto a substantial proportion of patients who eventually received salvage RT (SRT) at non-normalised PSA levels rather than ART. (2) INTERVENTIONS: although all the trials showed the benefit of postoperative ART compared to a wait-and-see approach, the dose herein employed would be now considered inadequate; (3) COMPARATORS: the comparison arm in all the 3 RCTs was an uncontrolled observation arm, where patients who subsequently developed biochemical failure were treated in various ways, with up to half of them receiving SRT at PSA well above 1ng/mL, a level that would be now deemed inappropriate; (4) OUTCOMES: only in one trial (SWOG 8794) ART was found to significantly improve overall survival compared to observation, with a ten-year overall survival rate of 74% vs. 66%, although this might be partly the result of imbalanced risk factors due to competing event risk stratification. CONCLUSIONS: ART has a high level of evidence due to three RCTs with at least 10-year follow-up recording a benefit in biochemical PFS, but its penetrance in present daily clinics should be reconsidered. While the benefit of ART or SRT is eagerly expected from ongoing randomized trials, a dynamic risk-stratified approach should drive the decisions making process.

The government of Catalonia has developed a planning framework that seeks to establish the provision of cultural facilities throughout the country. The Cultural Facilities Plan of Catalonia (PECCAT) is based on an analysis of historical gaps and establishes a minimum spatial scheme. The plan responds to problems associated with the absence of a former similar instrument, which has led to an inconsistent and inappropriate cultural infrastructure that fails to fulfill its fundamental mission of securing the cultural rights of the population. The paper sets forth the aims of this policy and describes the objectives and basic characteristics of the plan and the expected outcomes. With the plan, the government of Catalonia seeks to rebalance the infrastructure within the territory and to ensure universal access to basic cultural services, while avoiding a logic of standardization and taking local communities into account. With the development of local plans in the municipalities, local governments encourage community participation processes to adapt and decide on priorities for action based on needs assessments and cultural opportunities for local sustainable development. The local plans focus on local cultural strengths, take advantage of opportunities, and aim to realize the cultural dynamics of a place through establishing an infrastructure that can best respond to the needs and cultural demands of the local communities, taking into account economic, social, and environmental sustainability.

QUESTION UNDER STUDY: The aim of this study was to assess the prevalence of chronic kidney disease (CKD) among type 2 diabetic patients in primary care settings in Switzerland, and to analyse the prescription of antidiabetic drugs in CKD according to the prevailing recommendations. METHODS: In this cross-sectional study, each participating physician was asked to introduce anonymously in a web database the data from up to 15 consecutive diabetic patients attending her/his office between December 2013 and June 2014. Demographic, clinical and biochemical data were analysed. CKD was classified with the KDIGO nomenclature based on estimated glomerular filtration rate (eGFR) and urinary albumin/creatinine ratio. RESULTS: A total of 1 359 patients (mean age 66.5 ± 12.4 years) were included by 109 primary care physicians. CKD stages 3a, 3b and 4 were present in 13.9%, 6.1%, and 2.4% of patients, respectively. Only 30.6% of patients had an entry for urinary albumin/creatinine ratio. Among them, 35.6% were in CKD stage A2, and 4.1% in stage A3. Despite prevailing limitations, metformin and sulfonylureas were prescribed in 53.9% and 16.5%, respectively, of patients with advanced CKD (eGFR <30 ml/min). More than a third of patients were on a dipeptidyl-peptidase-4 inhibitor across all CKD stages. Insulin use increased progressively from 26.8% in CKD stage 1-2 to 50% in stage 4. CONCLUSIONS: CKD is frequent in patients with type 2 diabetes attending Swiss primary care practices, with CKD stage 3 and 4 affecting 22.4% of cases. This emphasizes the importance of routine screening of diabetic nephropathy based on both eGFR and urinary albumin/creatinine ratio, the latter being largely underused by primary care physicians. A careful individual drug risk/benefit balance assessment is mandatory to avoid the frequently observed inappropriate prescription of antidiabetic drugs in CKD patients.

Adjusting behavior following the detection of inappropriate actions allows flexible adaptation to task demands and environmental contingencies during goal-directed behaviors. Post-error behavioral adjustments typically consist in adopting more cautious response mode, which manifests as a slowing down of response speed. Although converging evidence involves the dorsolateral prefrontal cortex (DLPFC) in post-error behavioral adjustment, whether and when the left or right DLPFC is critical for post-error slowing (PES), as well as the underlying brain mechanisms, remain highly debated. To resolve these issues, we used single-pulse transcranial magnetic stimulation in healthy human adults to disrupt the left or right DLPFC selectively at various delays within the 30-180ms interval following false alarms commission, while participants preformed a standard visual Go/NoGo task. PES significantly increased after TMS disruption of the right, but not the left DLPFC at 150ms post-FA response. We discuss these results in terms of an involvement of the right DLPFC in reducing the detrimental effects of error detection on subsequent behavioral performance, as opposed to implementing adaptative error-induced slowing down of response speed.

Abstract Gastrointestinal bleeding represents a common medical emergency, with considerable morbidity and mortality rates, and a prompt diagnosis is essential for a better prognosis. In such a context, endoscopy is the main diagnostic tool; however, in cases where the gastrointestinal hemorrhage is massive, the exact bleeding site might go undetected. In addition, a trained professional is not always present to perform the procedure. In an emergency setting, optical colonoscopy presents limitations connected with the absence of bowel preparation, so most of the small bowel cannot be assessed. Scintigraphy cannot accurately demonstrate the anatomic location of the bleeding and is not available at emergency settings. The use of capsule endoscopy is inappropriate in the acute setting, particularly in the emergency department at night, and is a highly expensive method. Digital angiography, despite its high sensitivity, is invasive, presents catheterization-related risks, in addition to its low availability at emergency settings. On the other hand, computed tomography angiography is fast, widely available and minimally invasive, emerging as a promising method in the diagnostic algorithm of these patients, being capable of determining the location and cause of bleeding with high accuracy. Based on a critical literature review and on their own experience, the authors propose a computed tomography angiography protocol to assess the patient with gastrointestinal bleeding.

OBJECTIVES: Polypharmacy is one of the main management issues in public health policies because of its financial impact and the increasing number of people involved. The polymedicated population according to their demographic and therapeutic profile and the cost for the public healthcare system were characterised. DESIGN: Cross-sectional study. SETTING: Primary healthcare in Barcelona Health Region, Catalonia, Spain (5 105 551 inhabitants registered). PARTICIPANTS: All insured polymedicated patients. Polymedicated patients were those with a consumption of ≥16 drugs/month. MAIN OUTCOMES MEASURES: The study variables were related to age, gender and medication intake obtained from the 2008 census and records of prescriptions dispensed in pharmacies and charged to the public health system. RESULTS: There were 36 880 polymedicated patients (women: 64.2%; average age: 74.5±10.9 years). The total number of prescriptions billed in 2008 was 2 266 830 (2 272 920 total package units). The most polymedicated group (up to 40% of the total prescriptions) was patients between 75 and 84 years old. The average number of prescriptions billed monthly per patient was 32±2, with an average cost of 452.7±27.5. The total cost of those prescriptions corresponded to 2% of the drug expenditure in Catalonia. The groups N, C, A, R and M represented 71.4% of the total number of drug package units dispensed to polymedicated patients. Great variability was found between the medication profiles of men and women, and between age groups; greater discrepancies were found in paediatric patients (5-14 years) and the elderly (≥65 years). CONCLUSIONS: This study provides essential information to take steps towards rational drug use and a structured approach in the polymedicated population in primary healthcare.

The agricultural sector has always been characterized by a predominance of small firms. International competition and the consequent need for restraining costs are permanent challenges for farms. This paper performs an empirical investigation of cost behavior in agriculture using panel data analysis. Our results show that transactions caused by complexity influence farm costs with opposite effects for specific and indirect costs. While transactions allow economies of scale in specific costs, they significantly increase indirect costs. However, the main driver for farm costs is volume. In addition, important differences exist for small and big farms, since transactional variables significantly influence the former but not the latter. While sophisticated management tools, such ABC, could provide only limited complementary useful information but no essential allocation bases for farms, they seem inappropriate for small farms

Això qüestiona els resultats d’anteriors estudis de demanda i la majoria de les bases de dades, on l’observació del nivell d’agregació adequat dels productes no es té en compte.

Water Quality Index (WQI) was used during summer and winter of 1994 and 1995 in the final section of Onça and Feijão streams, downstream Broa Reservoir (São Carlos/SP) to evaluate agricultural and catlleman effects. In Onça stream water quality was "acceptable" in winter and "inappropriate to conventional treatment" in the summer. In Feijão stream the water had an "excellent" quality in winter and "good" in the summer. A MANOVA (Multivariate Analysis of Variance) has used to discriminate seasons and streams. Correlation (p<0,05) among the variables was tested. Water temperature, turbidity, pH and fecal bacteria are highly correlated and can be one of the factors that cause WQI change by seasons.
