The aim of the study was to determine the occurrence of virulence genes expressing fimbriae, production of hemolysin, colicin and aerobactin among a hundred Escherichia coli isolates obtained from in-and outpatients of a tertiary-care teaching hospital, between July and August 2000, showing clinical and laboratory signs of urinary tract infection (UTI). The presence of genes (pap, afa, sfa) for fimbriae expression was assayed using specific primers in a polymerase chain reaction. Among the isolates studied, the prevalence of the virulence factors was 96.0%, 76.0%, 24.0%, for hemolysin, aerobactin and colicin, respectively; the prevalence of genes coding for fimbrial adhesive systems was 32.0%, 19.0% and 11.0% for pap, sfa and afa respectively. The strains isolated from the outpatients displayed a greater number of virulence factors compared to those from hospitalized subjects, emphasizing the difference between these two kinds of patients.

Os modelos de maturidade são instrumentos facilitadores da gestão das organizações, incluindo a gestão da sua função sistemas de informação, não sendo exceção as organizações hospitalares. Neste artigo apresenta-se uma investigação inicial que visa o desenvolvimento de um abrangente modelo de maturidade para a gestão dos sistemas de informação hospitalares. O desenvolvimento deste modelo justifica-se porque os modelos de maturidade atuais no domínio da gestão dos sistemas informação hospitalares ainda se encontram numa fase embrionária de desenvolvimento, sobretudo porque são pouco detalhados, não disponibilizam ferramentas para determinação da maturidade e não apresentam as características dos estágios de maturidade estruturadas por diferentes fatores de influência.

This is the report on a patient with chronic diarrhea caused by microsporidia. He is married, infected with HIV and has low CD4 cell count. The diagnosis was established through stool parasite search using concentration methods and Gram - chromotrope staining technique. Ileum biopsy was also performed in this case. The etiological diagnosis may be established in a clinical laboratory, by chromotrope staining technique in routine microscopic examination of stool specimens.

A Tuberculose é uma doença altamente contagiosa que atinge sobretudo idosos e pessoas com maior vulnerabilidade, embora afecte também a população activa, em particular os mais expostos como os profissionais de saúde. A sua transmissão é potenciada por factores demográficos, pela urbanização, pelo aparecimento de formas resistentes à medicação habitual e pela associação à infecção por VIH. Apresenta-se o modelo, estratégia, métodos e resultados da intervenção de prevenção do risco profissional de Tuberculose no CHL/Hospital S. José desenvolvida pelo Serviço de Saúde Ocupacional (SSO). O SSO é constituído por equipa multiprofissional composta por Medicina do Trabalho, Ergonomia, Higiene e Toxicologia, Psicologia e Segurança do Trabalho, e inclui uma componente técnica orientada para os problemas de saúde no trabalho e a prevenção dos riscos ocupacionais, e uma componente clínica vocacionada para os problemas de saúde individual e o atendimento na doença aguda e crónica, assegurando cuidados de saúde personalizados e integrados (primários, secundários e terciários), com ênfase na prevenção primária e promoção da saúde no trabalho. A equipa de Enfermagem intervém na mediação e personalização dos cuidados da Medicina do Trabalho e da área clínica, interagindo com a equipa técnica no que se refere aos problemas da saúde no trabalho. No Hospital trabalham cerca de 2500 profissionais (68%do sexo feminino), dos quais os grupos profissionais mais representativos correspondem à enfermagem (29%), auxiliares (25,8%) e médicos (19,5%), apresentando diferentes factores de vulnerabilidade, predisposição e doença associados a hábitos, comportamentos e estilos de vida, e a riscos particulares das tarefas da sua actividade. O SSO iniciou a intervenção sistemática de prevenção e controlo do risco de Tuberculose nos profissionais do Hospital em Abril de 1996 com a disponibilização do rastreio activo, em colaboração com o Centro de Diagnóstico Pneumológico da Praça do Chile, o qual representa, até ao presente, a maior adesão registada em hospitais do país. Reflecte-se o nível de risco de exposição ocupacional dos profissionais à tuberculose, particularidades de risco dos cenários e adequação da protecção individual, isolamento e ventilação; o rastreio activo (Mantoux, Rx tórax) inicial, periódico conforme o nível de risco, após exposição e em profissionais com vulnerabilidade – Mantoux negativos, imunocomprometidos (VIH, corticoterapia, imunossupressão, insuficiência renal, …); a informação personalizada aos profissionais, formação individual e de grupo, em sala e no local de trabalho; e os resultados (taxa de Tuberculose latente, taxa de viragem tuberculínica, quimioprofilaxia, Tuberculoses profissionais).

The aim of this study was to undertake a comparative analysis of the practices and information behaviour of European information users who visit information units specialising in European information in Portugal and Spain. The study used a quantitative methodology based on a questionnaire containing closed questions and one open question. The questions covered the general sociological profile of the respondents and their use of European Document Centres, in addition to analysing aspects associated with information behaviour relating to European themes. The study therefore examined data on the preferred means and sources for accessing European information, types of documents and the subjects investigated most. The use of European databases and the Internet to access material on Europe was also studied, together with the reasons which users considered made it easy or difficult to access European information, and the aspects they valued most in accessing this information. The questionnaire was administered in European Document Centres in 2008 and 2010.

An 80 years old man suffered a cardiac arrest shortly after arrival to his local health department. Basic Life Support was started promptly and nine minutes later, on evaluation by an Advanced Life Support team, the victim was defibrillated with a 200J shock. When orotracheal intubation was attempted, masseter muscle contraction was noticed: on reevaluation, the victim had pulse and spontaneous breathing.Thirty minutes later, the patient had been transferred to an emergency department. As he complained of chest pain, the ECG showed a ST segment depression in leads V4 to V6 and laboratory tests showed cardiac troponine I slightly elevated. A coronary angiography was performed urgently: significant left main plus three vessel coronary artery disease was disclosed.Eighteen hours after the cardiac arrest, a quadruple coronary artery bypass grafting operation was undertaken. During surgery, a fresh thrombus was removed from the middle left anterior descendent artery. Post-operative course was uneventful and the patient was discharged seven days after the procedure. Twenty four months later, he remains asymptomatic.In this case, the immediate call for the Advanced Life Support team, prompt basic life support and the successful defibrillation, altogether, contributed for the full recovery. Furthermore, the swiftness in the detection and treatment of the acute reversible cause (myocardial ischemia in this case) was crucial for long-term prognosis.

The study involved 100 yeast isolates, obtained from urine samples provided by a Public Pediatric Hospital of São Paulo, Brazil, from 1999 to 2004. The most frequent species was Candida albicans, followed by C. tropicalis, C. glabrata and C. parapsilosis. In regard to virulence, 97% of the isolates showed index 3 for proteinase and 63% index 2 for phospholipase. The most frequent killer biotypes were 511 and 888.

A very important part of the globally produced energy is consumed in buildings, being an important share frequently used in the HVAC systems. These ones are increasing both in performance and in complexity, taking advantage from the use of the recent advances in mechanical and power electronic devices, particularly in the speed variation field. However the improved efficiency only occurs while the HVAC unit is working in the conditions specified by the manufacturer, otherwise the energy consumption raises to values considerably higher than the nominal ones. The adequate maintenance enforces the system to run on its nominal performance and the contrary has undesirable impact both in the performance and in the system expected life time. Therefore, HVAC field maintenance assumes a very important role in the global building sustainability concept. This work presents some results of an incorrect use of HVAC and the associated electric energy overconsumption that can assume values 50% higher than those that occur when the installation is operated according to the adequate maintenance plan.

We sought to determine the frequency of serological markers of selected infections in a population of psychiatric patients in Durango City, Mexico, and to determine whether there are any epidemiological characteristics of the subjects associated with the infections. One hundred and five inpatients of a public psychiatric hospital of Durango were examined for HBsAg, anti-HCV antibodies, anti-HIV antibodies, anti-Brucella antibodies, rapid plasma reagin and anti-Cysticercus antibodies by commercially available assays. Anti-Cysticercus antibodies were confirmed by Western blot and HBsAg by neutralization assay. Epidemiological data from each participant were also obtained. Seroprevalences of HBsAg, anti-HCV, anti-HIV, anti-Brucella, rapid plasma reagin and anti-Cysticercus antibodies found were 0.0%, 4.8%, 0.9%, 0.0%, 1.9%, and 0.9%, respectively. Overall, 9 (8.6%) inpatients showed seropositivity to any infection marker. We concluded that our psychiatric inpatients have serological evidence of a number of infections. HCV is an important pathogen among our psychiatric inpatients. Health care strategies for prevention and control of infections in Mexican psychiatric patients should be considered.

O presente estudo teve como objectivo aferir a adequação da prescrição de albumina humana num hospital central à evidência científica nesta área actualmente disponível. O estudo foi retrospectivo e analisou os pedidos de albumina aos serviços farmacêuticos entre Setembro de 2007 e Agosto de 2008. Analisadas 761 requisições de 619 doentes com idade média de 58,1 +/- 14,7 anos, 510 (77%) dos quais do sexo masculino. Foram avaliados parâmetros analíticos globais (proteínas totais, albumina sérica, creatinina e INR) à data da requisição de albumina, grandes grupos nosológicos e os motivos de prescrição. Dos 556.831 frascos de albumina requisitados, foram efectivamente administrados 5.033 (0,9%). Os doentes com patologia hepática foram o grupo nosológico mais representado (60%). Verificou-se que 36% dos pedidos correspondiam aos critérios científicos com suporte de evidência, sendo a hipoalbuminémia o motivo mais frequente de pedido (53%). Concluímos que é necessário um maior cuidado no fundamento dos pedidos de albumina, de forma a adequá-los à evidência científica. Normas restritivas dos serviços farmacêuticos para o fornecimento de albumina encontram-se justificadas.

The incidence of Candida bloodstream infection has increased over the past years. In the Center-West region of Brazil, data on candidemia are scarce. This paper reports a retrospective analysis of 96 cases of Candida bloodstream infection at a Brazilian tertiary-care teaching hospital in the state of Mato Grosso do Sul, from January 1998 to December 2006. Demographic, clinical and laboratory data were collected from medical records and from the hospital's laboratory database. Patients' ages ranged from three days to 92 years, with 53 (55.2%) adults and 43 (44.8%) children. Of the latter, 25 (58.1%) were newborns. The risk conditions most often found were: long period of hospitalization, utilization of venous central catheter, and previous use of antibiotics. Fifty-eight (60.4%) patients died during the hospitalization period and eight (13.7%) of them died 30 days after the diagnosis of candidemia. Candida albicans (45.8%) was the most prevalent species, followed by C. parapsilosis (34.4%), C. tropicalis (14.6%) and C. glabrata (5.2%). This is the first report of Candida bloodstream infection in the state of Mato Grosso do Sul and it highlights the importance of considering the possibility of invasive Candida infection in patients exposed to risk factors, particularly among neonates and the elderly.
