A presente dissertação integra a problemática da relação entre saúde, mais especificamente saúde suplementar (planos e seguros de saúde), e a reprodução do modo de produção capitalista. Partindo do referencial teórico-metodológico do materialismo histórico, analisa a expansão dos planos e seguros de saúde, com foco na Região Metropolitana de Belém (RMB), nos anos 2000. Sustentando essa análise, apresenta as características principais da nova Divisão Internacional do Trabalho a partir da crise dos anos 1970 e seus desdobramentos na formação econômico-social brasileira. Analisa a crise constante, crônica, do capitalismo contemporâneo, e demonstra como a compreensão de que o excesso de capital, oriundo da crise de 1974, em conjunto com as mudanças na divisão internacional do trabalho e na economia brasileira, são fenômenos fundamentais para compreender a ampliação e o novo perfil assumido pelo setor suplementar dos serviços de saúde no Brasil e na RMB. Para isso, retoma e apresenta os conceitos de Estado, imperialismo, crise do capitalismo, supercapitalização, entre outros. Expõe também o desenvolvimento dos serviços de saúde no Brasil, em uma perspectiva histórica, demonstrando as transformações na oferta dos serviços de saúde, públicos ou privados, sempre na perspectiva da relação existente entre a reprodução do capital na formação brasileira e a análise da reprodução do capital no setor de serviços em saúde, especificamente, destacando o surgimento e expansão dos planos e seguros de saúde. Com esses pressupostos, apresenta as características centrais da saúde suplementar na RMB, a partir de quatro dimensões: os usuários, a conformação específica dos planos de saúde na região, as operadoras de planos na RMB e a distribuição da força de trabalho no setor. Destaca ainda as determinações principais, gerais e específicas, da expansão desse mercado na RMB e algumas prováveis tendências ao seu desenvolvimento.

Esta investigação teve como objetivo analisar as condições de trabalho e saúde dos professores de Educação Física do Município de Belém-PA. Para tanto, foi necessário entender como a saúde destes professores era afetada a partir dos processos de precariedade do trabalho no capitalismo contemporâneo. Foi organizado um quadro teórico a fim de dar suporte para o debate em questão. Refere-se inicialmente a crise do capitalismo na década de 70, o qual se caracterizou como a crise do petróleo. Em um segundo momento destacou-se as políticas de ajustes econômicos e políticos que refletiam na estrutura básica do funcionamento da educação. O reordenamento do capital exige modificações no entendimento do papel da educação de um país, logo foi mostrado como as políticas mais gerais afetam a educação brasileira e a vida dos professores. Foi neste bojo que se analisou a partir dos dados encontrados na pesquisa de campo, as condições de trabalho destes professores tendo como variáveis a jornada de trabalho, salário, quantidade de turmas, locais de trabalho etc. Por último, analisou-se a questão de saúde dos docentes, identificando os principais problemas como as patologias encontradas, os acidentes de trabalho, a relação que se tem com o plano de saúde oferecido pela prefeitura, a periodicidade de ida ao médico etc. Neste estudo foram aplicados 22 questionários, sendo 11 com professores do sexo feminino e 11 do sexo masculino, sendo que das 11 professoras colaboradoras, 02 participaram da entrevista. Como resultado desta investigação foi possível inferir que: a) O processo de precariedade do trabalho dos professores de Educação Física tem se mostrado extremamente evidente e com consequências para vida e saúde destes trabalhadores; b) O fenômeno da violência nas escolas é parte fundamental do processo de precariedade do trabalho dos professores; c) Problemas fonoaudiológicos foram os mais evidentes nos pesquisados, além de problemas mentais; d) Através da leitura de outras pesquisas sobre a condição de precariedade do trabalho e saúde, é possível conjecturar uma tendência crescente em níveis de maior proporção sobre este tema; e) Os maiores índices de doenças ocupacionais referem-se aos professores do sexo feminino, ratificando que no precário mundo do trabalho essa tendência de adoecimento é prevalente. Conclui-se que é necessário a criação de espaços nas escolas públicas para se pensar a questão do trabalho e da saúde dos professores e estes juntamente com outros profissionais da educação formem autonomamente suas propostas de viabilização destes ambientes. Para tanto, é preciso antes, pensar na possibilidade de formação e construção do professor militante, o qual possa ser sujeito destas transformações. Cabe então ao professor colaborar como sujeito histórico para o processo de ruptura necessária do capital em crise.

O interesse deste estudo foi, de modo geral, poder identificar como o modelo privatista influenciou as ações da política pública de saúde no Brasil, como se deram os impactos da política macroeconômica neste sentido. Um dos pontos chave a ser verificado gira em torno da desigualdade de acesso da população ao serviço de saúde, com a não concretização da universalidade, gerando um processo denominado “universalização excludente”. Esse processo que consiste na migração de usuários do SUS para as operadoras de planos de saúde privados contribui para a mudança da racionalidade da saúde como direito para a racionalidade da eficiência, a racionalidade burguesa. Parte-se do referencial da Reforma Sanitária brasileira, como um marco da luta dos movimentos sociais pela democratização no país e como ponto inicial do reconhecimento da saúde enquanto direito de todos e dever do Estado, buscando fazer um resgate histórico deste movimento. Tem, ainda, como referência o pressuposto da minimização da atuação do Estado no trato às políticas sociais e a interferência direta de grandes organismos financeiros internacionais na condução do modo de fazer política de saúde, a exemplo do Banco Mundial. Esta consiste em uma pesquisa qualitativa, de cunho teórico, com o objetivo de proporcionar subsídios para a discussão do tema da política de saúde no Brasil, bem como promover e ampliar o debate teórico acerca da função que o Estado desempenha no modo de pensar e executar essa política.

The sector of health insurance is the entrance of 42 millions Brazilian people for health services. Passed a lot of years until regulations appears. The objective of this article was made an analysis of dental institutions participation at the national agency of health insurance – ANS, response for regulation and investigation in Brazil. Through an documental analysis of texts described in meeting registration of ANS, from 1998 to 2006, was made an identification of dental citations, try to describe a standard of dental representation in the meetings. The dentistry was present in 135 citations, the dentistry representatives institutional made 77 citations (57%), 19 was about dentistry proceeds and 7 of those was to ask resolutions; others 35 citations was about manager and standard of services offer by insurance companies, 5 was ask resolutions; others 3 asks was about taxes and fees, and 10 others was about relationship between companies and employs. The results show that dentistry has a good presence by their institutional representatives in ANS meetings.

Aim: The present study investigated the fees paid by dental insurance companies to dental surgeons, referrent to groups of dental procedures. Materials and Methods: The fees offered by dental insurance companies were defined according to the average values from the price tables of 5 insurance plans that offered national coverage. These average values and the values paid by the Brazilian Unified Health System (SUS) were compared with those set forth in the VRCC table from Brazilian Federal Dental Council (CFO). For comparison, average values were obtained for the following groups of procedures: preventive, aesthetic, endodontic, and surgical. The values of these preventive procedures were compared with other groups of procedures. Results: The plans applied an average discount of approximately 54.51% upon the prices suggested in the CFO price list. The highest discount was observed within the group of preventive procedures (57.09%, on average). Conclusion: The evaluated health insurance companies in vestigated in the present study are applying an abusive discount upon the prices suggested in the CFO price list. It's necessary to identify alternatives to reverse this situation.

To commemorate the 10th anniversary of the Nebraska Rural Poll, rural Nebraskans were asked about changes they may have experienced during the past ten years. Where have they lived during the past decade? In what types of business activities have they been involved? Have they received any education or training during that time period? What has been their experience with the Internet? This report details 2,851 responses to the 2005 Nebraska Rural Poll, the tenth annual effort to understand rural Nebraskans’ perceptions. Respondents were asked a series of questions about changes they have experienced during the past ten years. For all questions, comparisons are made among different respondent subgroups, that is, comparisons by age, occupation, region, etc. Based on these analyses, some key findings emerged: One quarter of rural Nebraskans have lived somewhere other than their current community during the past ten years. Of those who have lived elsewhere, they have moved their primary residence an average of 2.2 times. Younger rural Nebraskans are more likely than older residents to have lived elsewhere during the past decade. Sixty-six percent of persons between the ages of 19 and 29 have lived in a different location, compared to only 12 percent of persons age 65 and older. Many rural Nebraskans who have lived in a different community during the past ten years have lived in another state. Forty-one percent of persons who have lived elsewhere during the past decade have lived in a different state. Forty-five percent have lived in a larger community (18% have lived in either Omaha or Lincoln and 27% have lived in or near a Nebraska community larger than their current one - other than Lincoln or Omaha). Thirty-six percent have lived in or near a Nebraska community smaller than their current one. Twenty percent of rural Nebraskans currently own a business. Thirteen percent started operating a business during the past ten years, 10 percent closed or stopped operating a business during this time period and four percent tried unsuccessfully to start a business. Persons living in or near the smallest communities are more likely than persons living in or near larger communities to currently own a business. Twenty-nine percent of persons living in or near communities with less than 500 people currently own a business, compared to 15 percent of persons living in or near communities with at least 10,000 persons. In general, rural Nebraskans have favorable opinions about self-employment but they also recognize the hardships and risks involved with this type of employment. Sixtyone percent agree that self-employment is desirable because they can be their own boss. Forty-four percent agree that self-employment provides a better quality of life than being an employee. However, 74 percent agree that self-employed individuals work longer hours than traditional employees and 70 percent agree that the cost of health insurance makes self-employment unappealing. Younger persons are more likely than older persons to agree that the cost of health insurance makes self-employment unappealing. Eighty percent of persons age 19 to 29 agree with that statement, compared to 55 percent of persons age 65 and older. One-half of rural Nebraskans have participated in formal education courses, workshops or other training activities during the past ten years. Sixty-nine percent of rural Nebraskans have Internet access either at home or at work. Sixty-six percent have acquired Internet access either at home or at work during the past ten years. An additional three percent had acquired access more than ten years ago. Persons with higher levels of income are more likely than persons with lower incomes to have acquired Internet access. Sixty-six percent of persons with household incomes of $60,000 or more have acquired Internet access at both home and work during the past ten years, compared to only 11 percent of persons with household incomes less than $20,000. Information searches and email are the most important reasons for having an Internet connection. Eighty-nine percent of rural Nebraskans with access to the Internet at either home or work say that information searches are an important or very important reason for having an Internet connection. Eighty-three percent say email is an important reason. In general, rural Nebraskans say their satisfaction with various features of their Internet connection has increased during the past ten years. Fifty-five percent of rural Nebraskans with an Internet connection at home say their satisfaction with the availability of service has increased during the past ten years and 50 percent report an increase in their satisfaction with the speed of their connection. Persons living in or near the larger communities are more likely than persons living in or near the smaller communities to say their satisfaction with the speed of their Internet connection has increased during the past ten years. Fifty-four percent of persons living in or near communities with populations of 5,000 or more say their satisfaction with the speed of their connection has increased over the past decade, compared to 43 percent of persons living in or near communities with less than 1,000 people.

This study aimed to assess the prevalence of dental pain among adults and older people living in Brazil's State capitals. Information was gathered from the Telephone Survey Surveillance System for Risk and Protective Factors for Chronic Diseases (VIGITEL) in 2009 (n = 54,367). Dental pain was the outcome. Geographic region, age, gender, race, schooling, private health coverage, smoking, and soft drink consumption were the explanatory variables. Multilevel Poisson regression models were performed. Prevalence of dental pain was 15.2%; Macapa and Sao Luis had prevalence rates greater than 20%; all capitals in the South and Southeast, plus Cuiaba, Campo Grande, Maceio, Recife, and Natal had prevalence rates less than 15%. Factors associated with increased prevalence of dental pain were the North and Northeast regions, female gender, black/brown skin color, lack of private health insurance, smoking, and soft drink consumption. Dental pain is a public health problem that should be monitored by health surveillance systems.

OBJETIVO: Analisar fatores epidemiológicos e sociodemográficos associados à saúde de idosos com ou sem plano de saúde. MÉTODOS: Foram realizadas entrevistas com 2.143 pessoas de 60 anos e mais, no município de São Paulo, em 2000 e 2006. A variável dependente, dicotômica, foi ter ou não plano de saúde. As variáveis independentes abrangeram características sociodemográficas e de condição de saúde. Foram descritas as proporções encontradas para as variáveis analisadas e desenvolvido modelo de regressão logística que considerou significantes as variáveis com p < 0,05. RESULTADOS: Houve diferenças, favoráveis aos titulares de planos, para renda e escolaridade. O grupo sem planos privados realizou menos prevenção contra neoplasias e mais contra doenças respiratórias; esperou mais para ter acesso a consultas de saúde; realizou menos exames pós-consulta; referiu menor número de doenças; teve maior proporção de avaliação negativa da própria saúde e relatou mais episódios de queda. Os titulares de planos relataram menor adesão à vacinação e, dentre os que foram internados, 11,1% em 2000 e 17,9% em 2006 tiveram esse procedimento custeado pelo Sistema Único de Saúde. A única doença associada à condição de titular de plano privado foi a osteoporose. CONCLUSÕES: Há diferenças representadas pela renda e pela escolaridade favoráveis aos titulares de planos e seguros privados, as quais estão relacionadas com o uso de serviços e com os determinantes sociais de saúde.

O presente estudo trata das políticas de saúde mental conduzidas pela Agência Nacional de Saúde Suplementar - ANS, no cenário da assistência dispensada pelos planos privados de assistência à saúde. Dessa forma, analisa o modelo de regulação econômica e assistencial do setor suplementar, a forma de atuação da ANS como organismo regulador e o tratamento dispensado à assistência à saúde mental nos normativos emanados pela Agência. Concluiu-se que, apesar de avanços como a obrigatoriedade de cobertura para todas as doenças listadas na CID-10, a inclusão do tratamento das tentativas de suicídio e das lesões autoinfligidas, o atendimento por uma equipe multiprofissional, a ampliação do número de sessões com psicólogo, com terapeuta ocupacional e de psicoterapia, e a inclusão do hospital-dia na rede credenciada da operadora, a assistência à saúde mental ainda é pouco normatizada pelos regramentos vigentes no sistema de atenção à saúde suplementar, existindo muitas lacunas a serem preenchidas. A regulamentação dos mecanismos de coparticipação e franquia, a coparticipação crescente como limitador da internação psiquiátrica sem o repensar em uma rede substitutiva e a limitação do número de sessões de psicoterapia de crise são alguns dos desafios colocados para a ANS, no sentido de que esta cumpra realmente o seu papel institucional de promoção da defesa do interesse público na assistência suplementar à saúde.

Trata-se de um trabalho com objetivo de apresentar os principais resultados alcançados pela regulação dos planos de saúde exercida pela Agência Nacional de Saúde Suplementar, autarquia instituída para promover o equilíbrio das relações entre operadoras e consumidores, tornando o mercado de planos de saúde mais eficiente. Como avanços mais significativos oriundos da regulação do setor podem ser citados: as barreiras à entrada e à saída das operadoras no mercado, a ampliação das coberturas assistenciais contratuais, o monitoramento e o controle dos reajustes, a indução a práticas de promoção da saúde e à qualificação do setor, e a possibilidade da portabilidade de carências. Como desafios a serem enfrentados, podem ser apontados: o monitoramento da qualidade da assistência prestada, a renúncia fiscal, a existência dos cartões de desconto, a operação de empresas como operadoras de planos de saúde sem o registro na ANS e a adoção de alguns mecanismos nocivos de regulação assistencial pelas operadoras de planos de saúde.

The SWISSspine registry is the first mandatory registry of its kind in the history of Swiss orthopaedics and it follows the principle of "coverage with evidence development". Its goal is the generation of evidence for a decision by the Swiss federal office of health about reimbursement of the concerned technologies and treatments by the basic health insurance of Switzerland. Recently, developed and clinically implemented, the Dynardi total disc arthroplasty (TDA) accounted for 10% of the implanted lumbar TDAs in the registry. We compared the outcomes of patients treated with Dynardi to those of the recipients of the other TDAs in the registry. Between March 2005 and October 2009, 483 patients with single-level TDA were documented in the registry. The 52 patients with a single Dynardi lumbar disc prosthesis implanted by two surgeons (CE and OS) were compared to the 431 patients who received one of the other prostheses. Data were collected in a prospective, observational multicenter mode. Surgery, implant, 3-month, 1-year, and 2-year follow-up forms as well as comorbidity, NASS and EQ-5D questionnaires were collected. For statistical analyses, the Wilcoxon signed-rank test and chi-square test were used. Multivariate regression analyses were also performed. Significant and clinically relevant reduction of low back pain and leg pain as well as improvement in quality of life was seen in both groups (P < 0.001 postop vs. preop). There were no inter-group differences regarding postoperative pain levels, intraoperative and follow-up complications or revision procedures with a new hospitalization. However, significantly more Dynardi patients achieved a minimum clinically relevant low back pain alleviation of 18 VAS points and a quality of life improvement of 0.25 EQ-5D points. The patients with Dynardi prosthesis showed a similar outcome to patients receiving the other TDAs in terms of postoperative low back and leg pain, complications, and revision procedures. A higher likelihood for achieving a minimum clinically relevant improvement of low back pain and quality of life in Dynardi patients was observed. This difference might be due to the large number of surgeons using other TDAs compared to only two surgeons using the Dynardi TDA, with corresponding variations in patient selection, patient-physician interaction and other factors, which cannot be assessed in a registry study.

Background Switzerland introduces a DRG (Diagnosis Related Groups) based system for hospital financing in 2012 in order to increase efficiency and transparency of Swiss health care. DRG-based hospital reimbursement is not simultaneously realized in all Swiss cantons and several cantons already implemented DRG-based financing irrespective of the national agenda, a setting that provides an opportunity to compare the situation in different cantons. Effects of introducing DRGs anticipated for providers and insurers are relatively well known but it remains less clear what effects DRGs will have on served populations. The objective of the study is therefore to analyze differences of volume and major quality indicators of care between areas with or without DRG-based hospital reimbursement from a population based perspective. Methods Small area analysis of all hospitalizations in acute care hospitals and of all consultations reimbursed by mandatory basic health insurance for physicians in own practice during 2003-2007. Results The results show fewer hospitalizations and a relocation of resources to outpatient care in areas with DRG reimbursement. Overall burden of disease expressed as per capita DRG cost weights was almost identical between the two types of hospital reimbursement and no distinct temporal differences were detected in this respect. But the results show considerably higher 90-day rehospitalization rates in DRG areas. Conclusion The study provides evidence of both desired and harmful effects related to the implementation of DRGs. Systematic monitoring of outcomes and quality of care are therefore essential elements to maintain in the Swiss health system after DRG's are implemented on a nationwide basis in 2012.

OBJECTIVES: To validate the Probability of Repeated Admission (Pra) questionnaire, a widely used self-administered tool for predicting future healthcare use in older persons, in three European healthcare systems. DESIGN: Prospective study with 1-year follow-up. SETTING: Hamburg, Germany; London, United Kingdom; Canton of Solothurn, Switzerland. PARTICIPANTS: Nine thousand seven hundred thirteen independently living community-dwelling people aged 65 and older. MEASUREMENTS: Self-administered eight-item Pra questionnaire at baseline. Self-reported number of hospital admissions and physician visits during 1 year of follow-up. RESULTS: In the combined sample, areas under the receiver operating characteristic curves (AUCs) were 0.64 (95% confidence interval (CI)=0.62-0.66) for the prediction of one or more hospital admissions and 0.68 (95% CI=0.66-0.69) for the prediction of more than six physician visits during the following year. AUCs were similar between sites. In comparison, prediction models based on a person's age and sex alone exhibited poor predictive validity (AUC or= 0.5) were 2.3 times as likely (95% CI=2.1-2.6) as low-risk individuals to have a hospital admission, and 2.1 times as likely (95% CI=2.0-2.2) to have more than six physician visits. CONCLUSION: The Pra instrument exhibits good validity for predicting future health service use on a population level in different healthcare settings. Administrative data have shown similar predictive validity, but in practice, such data are often not available. The Pra is likely of high interest to governments and health insurance companies worldwide as a basis for programs aimed at health risk management in older persons.
