Introdução A presença de mulheres no transporte rodoviário de cargas tem sido cada vez mais crescente e as repercussões do trabalho na vida das motoristas de caminhão ainda são desconhecidas pela comunidade científica. Objetivo - Caracterizar e analisar o trabalho de mulheres motoristas de caminhão e suas repercussões sobre sua saúde, a partir do relato de homens e mulheres motoristas de caminhão. Metodologia - O estudo com abordagem qualitativa utilizou a técnica do grupo focal, entrevistas individuais e observação não participante. Os grupos focais foram realizados em uma empresa transportadora localizada no estado de São Paulo e as entrevistas individuais em evento realizado na cidade de Itupeva/SP. Em oito encontros, grupos de motoristas de caminhão, discutiram a temática trabalho e saúde conduzida por meio de questões semiabertas. As mesmas questões foram utilizadas para as entrevistas individuais. Os relatos foram gravados, sendo o conteúdo das gravações transcrito e analisado por meio da metodologia Análise de Conteúdo de Bardin. A partir dos dados obtidos, construíram-se as seguintes categorias: A trajetória profissional de motoristas de caminhão; As mulheres no Transporte Rodoviário de Cargas; O trabalho; Um momento inesquecível na profissão e A saúde das mulheres motoristas na estrada. Resultados Tornar-se motorista de caminhão, para a maioria das mulheres, ocorreu por acaso, ou devido à uma necessidade financeira, ou a falta de perspectiva de emprego. Para as mulheres no Transporte Rodoviário de Cargas, o cotidiano de trabalho das profissionais está atravessado por aspectos como: a força física; dúvidas e preconceitos quanto à sua orientação sexual; o desafio em conciliar a vida dentro e fora do caminhão; conflitos na vida conjugal decorrentes da vida profissional; a discriminação sexual e a necessidade do reconhecimento no trabalho, bem como a falta de infraestrutura dedicada às trabalhadoras nas empresas e postos de parada nas rodovias brasileiras. O momento inesquecível nesta profissão, considerado por homens e mulheres, foi a primeira viagem. Os impactos do trabalho sobre a saúde das trabalhadoras recaíram sobre dores lombares e na coluna; problemas relacionados ao sono; necessidade de recorrer ao uso de drogas como anfetaminas e cocaína para manterem-se acordadas durante o trabalho; estresse; infecção urinária e uso ininterrupto de anticoncepcionais. Conclusões - A inserção das mulheres no transporte rodoviário de cargas desafia empresas e a infraestrutura existente nas rodovias do país a acompanharem as transformações sociais no mundo do trabalho, incluindo as demandas de um novo perfil de trabalhadoras. Introducción: Los manuales de obstetricia citan numerosos signos y síntomas músculo-esqueléticos pero los porcentajes de mujeres que los presentan no aparecen registrados. Y en los trabajos no se estudian todos ellos simultáneamente. Objetivo: describir el comportamiento de los síntomas músculo-esqueléticos durante el embarazo. Material y Métodos: 745 gestantes, entrevistadas entre las 8 y 12 semanas de gestación, entre las 18 y 22 y entre las 37 y 42, procedentes de centros de salud de Esmeraldas (Ecuador). Resultados: Un 31,8% tuvo lumbalgia en algún momento de la gestación y un 15,6% ciatalgia, la lumbalgia aparece en un 40% en el tercer trimestre mientras que la ciatalgia desciende con el curso de la gestación. El dolor púbico y el de miembros inferiores se presenta en un 42,7% y un 46,6% de todas las mujeres. No se han encontrado diferencias en función de la edad, paridad, talla, peso, estado nutricional, raza o actividad. Conclusiones: Las gestantes tienen dificultad para diferenciar los procesos dolorosos. Han sobreinformado la presencia de ciatalgia en detrimento de la lumbalgia. El dolor de miembros inferiores es el más frecuente. La sintomatología músculo-esquelética dificulta el desempeño de las actividades de la vida diaria a más de un 10% de las mujeres.

1.1 Introduction and Purpose: Adequate postoperative analgesia in the opioid tolerant with chronic non-malignant pain is challenging. Multimodal pain relief regimens include regional anesthesia but opioid tolerant patients report increased postoperative pain and opioid consumption. This study compared analgesia in opioid naïve and tolerant patients receiving postoperative sciatic nerve blockade for foot and ankle surgery. 1.2 Method: Preoperative pain scores, trauma, maintenance and intraoperative opioid doses and following postoperative sciatic nerve blockade, patient self-reported pain scores and opioid consumption at discharge from the post-anesthesia unit and 24 hours were recorded. 1.3 Results: 191 patients enrolled. 40.3% were opioid tolerant and 33% had lower extremity trauma. Preoperative, immediate and delayed postoperative pain scores and intraoperative, immediate and 24 hour postoperative consumption of opioids were increased in opioid tolerant patients. Trauma and continuous infusion in opioid naïve and tolerant groups did not result in differences in 24 hour opioid consumption. 1.4 Limitations: Small subgroups and use of the pain score limited the accuracy of results. 1.5 Conclusion: Opioid tolerant patients require greater analgesic doses following sciatic nerve blockade for foot and ankle surgery. 24 hour opioid consumption for opioid naïve and tolerant patients is neither influenced by lower extremity injury nor continuous infusion.

Background: In clinical trials, at the group level, results are usually reported as mean and standard deviation of the change in score, which is not meaningful for most readers. Objective: To determine the minimal clinically important improvement (MCII) of pain, patient's global assessment of disease activity, and functional impairment in patients with knee and hip osteoarthritis (OA). Methods: A prospective multicentre 4 week cohort study involving 1362 outpatients with knee or hip OA was carried out. Data on assessment of pain and patient's global assessment, measured on visual analogue scales, and functional impairment, measured on the Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) function subscale, were collected at baseline and final visits. Patients assessed their response to treatment on a five point Likert scale at the final visit. An anchoring method based on the patient's opinion was used. The MCII was estimated in a subgroup of 814 patients ( 603 with knee OA, 211 with hip OA). Results: For knee and hip OA, MCII for absolute ( and relative) changes were, respectively, ( a) -19.9 mm (-40.8%) and -15.3 mm (-32.0%) for pain; ( b) -18.3 mm ( - 39.0%) and -15.2 mm ( -32.6%) for patient's global assessment; ( c) -9.1 ( -26.0%) and -7.9 ( -21.1%) for WOMAC function subscale score. The MCII is affected by the initial degree of severity of the symptoms but not by age, disease duration, or sex. Conclusion: Using criteria such as MCII in clinical trials would provide meaningful information which would help in interpreting the results by expressing them as a proportion of improved patients.

In this article we review and critique the current body of scientific knowledge regarding the use of team lifting including: (a) psychophysical studies of team lifting capacity, and (b) studies of manual handling, patient handling, and stretcher carriage performed by lifting teams. The consensus of the research literature is that team-lifting capacity is greater than the lifting capacity of an individual, but that the capacity of lifting teams is less than the summed capacity of individual team members. Further, biomechanical, psychophysical, and physiological stress tends to be reduced compared to the equivalent lifts and transfers performed by individuals. However, the stress associated with team lifting depends on a broad range of individual team member, load, task and environmental factors, which can interact in unexpected ways. Caution is therefore recommended against making broad assumptions regarding the use of team lifting. Future studies are needed to examine how effort and load are distributed among lifting team members, with emphasis on identifying factors that may increase the risk of injury.

Highly selective N-type voltage-gated calcium (Ca-V) channel inhibitors from cone snail venom (the omega-conotoxins) have emerged as a new class of therapeutics for the treatment of chronic and neuropathic pain. Earlier in 2005, Prialt ( Elan) or synthetic omega-conotoxin MVIIA, was the first omega-conotoxin to be approved by Food and Drug Administration for human use. This review compares the action of three omega-conotoxins, GVIA, MVIIA and CVID, describing their structure-activity relationships and potential as leads for the design of improved N-type therapeutics that are more useful in the treatment of chronic pain.

Musculoskeletal diseases are one of the major causes of disability around the world and have been a significant reason for the development of the Bone and Joint Decade. Rheumatoid arthritis, osteoarthritis and back pain are important causes of disability-adjusted-life years in both the developed and developing world. COPCORD studies in over 17 countries around the world have identified back and knee pain as common in the community and are likely to increase with the ageing population. Musculoskeletal conditions are an enormous cost to the community in economic terms, and these figures emphasise how governments need to invest in the future and look at ways of reducing the burden of musculoskeletal diseases by encouraging exercise and obesity prevention campaigns.

We hypothesised that the inflammation seen around the nerve root in patients with sciatica may be caused by microbial infection. We used a newly developed serological test to diagnose deep-seated infections caused by low virulent gram-positive microorganisms. 43 of 140 (31%) patients with sciatica tested positive. Intervertebral disc material from a further 36 patients with severe sciatica who had undergone microdiscectomy was cultured for the presence of microorganisms. 19 of these patients (53%) had positive cultures after long-term incubation. Propionibacterium acnes was isolated from 16 of the 19 (84%) positive samples. Low virulent microorganisms, in particular P acnes, might be causing a chronic low-grade infection in the lower intervertebral discs of patients with severe sciatica. These microorganisms could have gained access to the spinal disc after previous minor trauma.

Non-steroidal anti-inflammatory drugs (NSAIDs) cause peptic ulcer disease, but whether they interact with Helicobacter pylori to promote damage is controversial. Moreover, the reported induction of apoptosis in gastric cells by H. pylori lipopolysaccharide (LPS) (10-9 g /ml) contrasts with studies showing low immunological potency of this LPS. Therefore, the effects of LPS from H. pylori NCTC 11637 and Escherichia coli 0111:B4 on apoptosis in a primary culture of guinea-pig gastric mucous cells were investigated in the presence and absence of the NSAID, ibuprofen. Cell loss was estimated by a crystal violet assay, and apoptosis determined from caspase activity and from condensation and fragmentation of nuclei. Exposure to E. coli LPS for 24 h caused cell loss and enhanced apoptotic activity at concentrations ≥ 10-9 g/ml, but similar effects were only obtained with H. pylori LPS at concentrations ≥10-6 g/ml. Although ibuprofen (250 μM) caused cell loss and apoptosis, addition of either E. coli or H. pylori LPSs further enhanced these effects. In conclusion, LPS and ibuprofen interact to enhance gastric cell loss and apoptosis. In such interactions, E. coli LPS is more potent than that of H. pylori. The low potency of H. pylori LPS may contribute to a chronic low-grade gastritis that can be enhanced by the use of NSAIDs. © W. S. Maney & Son Ltd.

The polycystic ovary syndrome (PCOS) is considered the most common endocrine disorder in reproductive age women, with a prevalence ranging from 15 to 20%. In addition to hormonal and reproductive changes, it is common in PCOS the presence of risk factors for developing cardiovascular disease (CVD) and diabetes mellitus, insulin resistance (IR), visceral obesity, chronic low-grade inflammation and dyslipidemia. Due to the high frequency of obesity associated with PCOS, weight loss is considered as the first-line treatment for the syndrome by improving metabolic and normalizes serum androgens, restoring reproductive function of these patients. Objectives: To evaluate the inflammatory markers and IR in women with PCOS and healthy ovulatory with different nutritional status and how these parameters are displayed after weight loss through caloric restriction in with Down syndrome. Methods: Tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP) were assessed in serum samples from 40 women of childbearing age. The volunteers were divided into four groups: Group I (not eutrophic with PCOS, n = 12); Group II (not eutrophic without PCOS, n = 10), Group III (eutrophic with PCOS, n = 08) and Group IV (eutrophic without PCOS, n = 10). The categorization of groups was performed by body mass index (BMI), according to the World Health Organization (WHO) does not eutrophic, overweight and obesity (BMI> 25 kg / m²) and normal weight (BMI <24.9 kg / m²). IR was determined by HOMA-IR index. In the second phase of the study a controlled dietary intervention was performed and inflammatory parameters were evaluated in 21 overweight and obese women with PCOS, before and after weight loss. All patients received a low-calorie diet with reduction of 500 kcal / day of regular consumption with standard concentrations of macronutrients. Results: Phase 1: PCOS patients showed increased levels of CRP (p <0.01) and HOMAIR (p <0.01). When divided by BMI, both not eutrophic group with PCOS (I) as eutrophic with PCOS (III) showed increased levels of CRP (I = 2.35 ± 0,55mg / L and 2.63 ± III = 0,65mg / L; p <0.01) and HOMA-IR (I = 2.16 ± 2.54 and III = 1.07 ± 0.55; p <0.01). There were no differences in TNF-α and IL-6 between groups. Step 2: After the weight loss of 5% of the initial weight was reduced in all of the components of serum assessed inflammatory profile, PCR (154.75 ± 19:33) vs (78.06 ± 8.9) TNF α (10.89 ± 5.09) vs (6:39 ± 1:41) and IL6 (154.75 ± 19:33) vs (78.06 ± 08.09) (p <0:00) in association with improvement some hormonal parameters evaluated. Conclusion: PCOS contributed to the development of chronic inflammation and changes in glucose metabolism by increasing CRP, insulin and HOMA-IR, independent of nutritional status. The weight loss, caloric restriction has improved the inflammatory condition and hormonal status of the evaluated patients.

The increasing prevalence of Repetitive Strain Injury / Work-Related Musculoskeletal Disorders (RSI / MSDs) has been explained by changes in the work that has been characterized by setting goals and productivity. This fact does not take into account the psycho-physiological characteristics of workers who become ill as a result of professional activities engaged in and also the adverse conditions in which his work was done. This study aimed to analyze the RSI / MSDs reported in the state of Rio Grande do Norte, Brazil, 2010-2014 compared to the profile of the population, epidemiological aspects and features of these diseases. It is a descriptive epidemiological cross-sectional study using secondary data obtained in the Notifiable Diseases Information System (SINAN) in May 2015 was used as the area covered by the Rio Grande do Norte. Data were collected regarding the grievances of work-related RSI / MSDs, assigned by the Worker's Health Reference Center (CEREST) of the State Health Department. Data were processed with the help of Microsoft Excel® 2013 and presented in tables in absolute frequency (n) and relative frequency (%). The results showed a total of 403 notifications of RSI / MSDs, where the vast majority 72% (290) was recorded in Natal. Regarding the profile of notified employees, 88.59% (357) have aged between 25 and 54 years, 62.78% (253) were female, and in relation to education, 31.51% (127) had high school complete. The most affected occupations were seamstresses (the clothing industry) with 24.97% (97), followed by masons, construction with 3.23% (13) and cashiers with 2.99% (12). About the work situation, 75.93% (306) had a formal contract, however, were sent to Communications Occupational accidents (CAT) only 67% (270) of the cases. Regarding the signs and symptoms, pain and limitation of movement were the most mentioned respectively with 98.01% (395) and 95.04% (383) of notifications. Notifications, 94.29% (380) showed exposure to repetitive movements in their workplace. The most frequent specific diagnoses were synovitis and tenosynovitis (CID F 65), with 30.02% (121) of notifications, followed by back pain (CID F 54) to 19.35% (78) and shoulder injuries (ICD M 75) with 15.88% (64). They were away from work 81.64% (329) of workers reported with RSI / MSDs. Evolution more prevalent among cases was temporary disability with 75.68% (305). It was concluded that the SINAN is a database of potential to characterize the profile of RSI / MSDs, requiring, however, an improvement in the coverage of records and data quality. In addition, this study reflects the need to implement protection strategies to workers by companies, signaling promotion, prevention and rehabilitation aimed at reducing these injuries and the improvement of occupational health indicators in Rio Grande do Norte.

Contexte: La douleur chronique non cancéreuse (DCNC) génère des retombées économiques et sociétales importantes. L’identification des patients à risque élevé d’être de grands utilisateurs de soins de santé pourrait être d’une grande utilité; en améliorant leur prise en charge, il serait éventuellement possible de réduire leurs coûts de soins de santé. Objectif: Identifier les facteurs prédictifs bio-psycho-sociaux des grands utilisateurs de soins de santé chez les patients souffrant de DCNC et suivis en soins de première ligne. Méthodologie: Des patients souffrant d’une DCNC modérée à sévère depuis au moins six mois et bénéficiant une ordonnance valide d’un analgésique par un médecin de famille ont été recrutés dans des pharmacies communautaires du territoire du Réseau universitaire intégré de santé (RUIS), de l’Université de Montréal entre Mai 2009 et Janvier 2010. Ce dernier est composé des six régions suivantes : Mauricie et centre du Québec, Laval, Montréal, Laurentides, Lanaudière et Montérégie. Les caractéristiques bio-psycho-sociales des participants ont été documentées à l’aide d’un questionnaire écrit et d’une entrevue téléphonique au moment du recrutement. Les coûts directs de santé ont été estimés à partir des soins et des services de santé reçus au cours de l’année précédant et suivant le recrutement et identifiés à partir de la base de données de la Régie d’Assurance maladie du Québec, RAMQ (assureur publique de la province du Québec). Ces coûts incluaient ceux des hospitalisations reliées à la douleur, des visites à l’urgence, des soins ambulatoires et de la médication prescrite pour le traitement de la douleur et la gestion des effets secondaires des analgésiques. Les grands utilisateurs des soins de santé ont été définis comme étant ceux faisant partie du quartile le plus élevé de coûts directs annuels en soins de santé dans l’année suivant le recrutement. Des modèles de régression logistique multivariés et le critère d’information d’Akaike ont permis d’identifier les facteurs prédictifs des coûts directs élevés en soins de santé. Résultats: Le coût direct annuel médian en soins de santé chez les grands utilisateurs de soins de santé (63 patients) était de 7 627 CAD et de 1 554 CAD pour les utilisateurs réguliers (188 patients). Le modèle prédictif final du risque d’être un grand utilisateur de soins de santé incluait la douleur localisée au niveau des membres inférieurs (OR = 3,03; 95% CI: 1,20 - 7,65), la réduction de la capacité fonctionnelle liée à la douleur (OR = 1,24; 95% CI: 1,03 - 1,48) et les coûts directs en soins de santé dans l’année précédente (OR = 17,67; 95% CI: 7,90 - 39,48). Les variables «sexe», «comorbidité», «dépression» et «attitude envers la guérison médicale» étaient également retenues dans le modèle prédictif final. Conclusion: Les patients souffrant d’une DCNC au niveau des membres inférieurs et présentant une détérioration de la capacité fonctionnelle liée à la douleur comptent parmi ceux les plus susceptibles d’être de grands utilisateurs de soins et de services. Le coût direct en soins de santé dans l’année précédente était également un facteur prédictif important. Améliorer la prise en charge chez cette catégorie de patients pourrait influencer favorablement leur état de santé et par conséquent les coûts assumés par le système de santé.

Chronic low-grade inflammation has been implicated in the processes leading to the development of type 2 diabetes (T2D) and its progression. Non-Hispanic Blacks bear a disproportionate burden of T2D and are highly susceptible to inflammation. This cross-sectional study assessed and compared the serum levels of established adipocytokines; interleukin-6 (IL-6), C-reactive protein (CRP), leptin, and novel adipocytokines; chemerin and omentin in Haitian and African Americans with and without T2D. The relationships of these adipocytokines with metabolic syndrome (MetS), anthropometric and HOMA2 measures by ethnicity and diabetes status were also assessed. Serum levels of IL-6, CRP, leptin, chemerin and omentin were determined by the ELISA method. HOMA2 measures were calculated for insulin sensitivity (HOMA2-IS) and insulin resistance (HOMA2-IR). Analyses of available data for 230 Haitian Americans and 241 African Americans (240 with and 231 without T2D) for the first study showed that Haitian Americans with and without MetS had lower levels of IL-6 and CRP compared to African Americans with and without MetS (P Ethnic-specific diabetes intervention and treatment programs must be designed to target Haitian Americans and African Americans as separate unique groups, in order to reduce the burden of T2D among the non-Hispanic Black community. Further research is needed to gain better understanding of the role of inflammation and T2D in this population.
