Introdução: A importância da investigação da qualidade de vida de crianças está diretamente relacionada ao fato de que muitos problemas na vida adulta têm sua origem na infância. Objetivo: analisar a contribuição da percepção do relacionamento familiar e do estado nutricional sobre a qualidade de vida de crianças do município de Indaiatuba. Metodologia: Na etapa 1 foi realizado o estudo de validade do instrumento APGAR Familiar adaptado à crianças de 7 a 11 anos, utilizando como medida de confiabilidade a técnica do Teste-Reteste e para a avaliação da validade convergente foi utilizada a Escala de Avaliação de Qualidade de Vida Infantil. Na etapa 2 foram avaliados os determinantes da Qualidade de Vida de crianças sob aspectos familiares, estado nutricional e socioeconômicos e demográficos. Os dados foram analisados por meio de Regressão Linear Múltipla com método dos Mínimos Quadrados Ordinários. Resultados: Na etapa 1 a análise de confiabilidade obteve a correlação de 0,764 do coeficiente de Spearman-Brown. Na análise de Validade Convergente o coeficiente de correlação de Rô de Spearman entre os escores dos dois instrumentos foi de 0,570 (p<0,01). Na etapa 2 foi estimado um modelo dos determinantes da qualidade de vida a partir de uma amostra de 1028 crianças de 7 a 11 anos de ambos os sexos. As variáveis independentes foram capazes de explicar a Qualidade de Vida de crianças a uma significância de 1% (Z = 8,417), sendo o R² ajustado de 0,104. A idade da criança e a percepção do relacionamento familiar foram as variáveis estatisticamente significativas, ao passo que o estado nutricional, o tamanho da família, o sexo da criança, a classe social e a escolaridade do responsável, não foram estatisticamente significantes. Conclusão: O instrumento APGAR Familiar apresentou índices preliminares de validade e precisão; a idade e o relacionamento familiar foram as variáveis que explicaram a qualidade de vida percebida sob a perspectiva subjetiva de bem-estar.

Instrumentos de avaliação psicológica constituem-se em métodos sistemáticos de investigação e de compreensão de componentes estruturais e funcionais do comportamento humano, com diversificados objetivos e estratégias técnicas, respeitando-se especificidades das etapas do desenvolvimento. Em processos de avaliação psicológica de características da personalidade, os métodos projetivos, como o Método de Rorschach e o Desenho da Figura Humana, são recursos amplamente utilizados, contribuindo para a compreensão e elaboração de intervenções terapêuticas em variados campos de aplicação, como na área da obesidade infantil. Nesse contexto, este trabalho teve por objetivo identificar e comparar características psicológicas de crianças com obesidade em relação a eutróficas, a partir de métodos projetivos de investigação da personalidade. Foram examinadas 60 crianças de sete a 11 anos de idade, sendo 30 crianças diagnosticadas como obesas e em tratamento específico para o transtorno (Grupo 1 - G1) e 30 crianças com peso normal (Grupo 2 - G2), sem atraso acadêmico, sem limites cognitivos e sem histórico de outras doenças físicas. Os participantes de G1 foram recrutados em instituições de saúde voltadas ao tratamento da obesidade infantil e G2 foi constituído a partir de parceria estabelecida com instituição de ensino bem como a partir de contatos informais da pesquisadora e de seu grupo de pesquisa (técnica da \"bola de neve\"), buscando-se balanceamento dos grupos por sexo e idade. Os seguintes instrumentos de avaliação psicológica foram aplicados individualmente nas crianças: Teste das Matrizes Progressivas Coloridas de Raven (critério de seleção de participantes, incluindo-se na amostra apenas crianças com resultados intelectuais médios ou superiores), o Desenho da Figura Humana e o Método de Rorschach (Escola Francesa). Os pais das crianças participantes responderam ao Questionário de Capacidades e Dificuldades (SDQ) para caracterização da amostra. Os resultados foram examinados conforme padronização específica dos respectivos manuais técnicos dos instrumentos, realizando-se análises descritivas e inferenciais, a fim de examinar possíveis associações entre variáveis clínicas e demográficas e indicadores de características de personalidade das crianças. Foram efetuadas análises correlacionais entre resultados no DFH e no Rorschach, considerando também a classificação nutricional da criança. Os achados permitem compreender características do funcionamento psíquico envolvidas na obesidade infantil, de modo a favorecer estratégias futuras de intervenção terapêutica com crianças. (CAPES)

El objetivo de este trabajo consistió en conocer si existen diferencias en los cinco grandes rasgos de la personalidad (Extraversión, Cordialidad, Neuroticismo, Apertura a la experiencia y Conciencia) entre los participantes con altos y bajos niveles de Perfeccionismo Auto-Orientado (PAO) en población infantil española. Se empleó una muestra compuesta por 483 alumnos entre 8 y 11 años, así como la subescala PAO de la Child and Adolescent Perfectionism Scale (CAPS) y el Cuestionario Big Five de personalidad para niños (BFQ-N) que evalúa los cinco grandes rasgos de la personalidad. Los resultados de la prueba t de Student mostraron que los participantes con altos niveles de PAO obtuvieron medias significativamente más altas en Extraversión, Apertura a la experiencia y Cordialidad que sus iguales con bajos niveles de PAO. Contrariamente, no se observaron diferencias significativas entre ambos grupos en lo que respecta a las puntuaciones medias en Neuroticismo y Conciencia. Los resultados no coinciden con las evidencias empíricas previas, sugiriendo que la relación que se establece entre el PAO y la personalidad puede variar en función de la edad o la cultura.

PURPOSE: Malignant ascites is debilitating for patients with advanced cancer. As shown previously, tumour cell production of vascular endothelial growth factor might be a major cause of the formation of malignant ascites. Intraperitoneal bevacizumab could therefore be an option for symptom control in refractory ascites. PATIENTS AND METHODS: Patients with advanced gastrointestinal cancer and malignant ascites who had undergone paracentesis at least twice within the past 4 weeks were randomly assigned in a 2:1 ratio to intraperitoneal bevacizumab (400 mg absolute) or placebo after paracentesis. During the 8-week treatment period, a minimum interval of 14 d was kept between the applications of the study drug. Primary end-point was paracentesis-free survival (ParFS). RESULTS: Fifty-three patients (median age 63 years) were randomised. Forty-nine patients received at least one study drug application and qualified for the main analysis. The proportion of patients with at least one common toxicity criteria grade III-V event was similar with 20/33 (61%) on bevacizumab and 11/16 (69%) on placebo. Median ParFS was 14 d (95% confidence interval [CI]: 11-17) in the bevacizumab arm and 10.5 d (95% CI: 7-21) on placebo (hazard ratio 0.74, 95% CI: 0.40-1.37; P = 0.16). The longest paracentesis-free period was 19 d on bevacizumab (range 6-66 d) and 17.5 d in the placebo arm (range 4-42) (P = 0.85). Median overall survival was 64 d (95% CI: 45-103) on bevacizumab compared to 31.5 d (95% CI: 20-117) on placebo (P = 0.31). CONCLUSION: Intraperitoneal bevacizumab was well tolerated. Overall, treatment did not result in a significantly better symptom control of malignant ascites. However, patients defined by specific immune characteristics may benefit.

PURPOSE To quantitatively evaluate retinal layer thickness changes in acute macular neuroretinopathy (AMN). METHODS AMN areas were identified using near-infrared reflectance (NIR) images. Intraretinal layer segmentation using Heidelberg software was performed. The inbuilt ETDRS -grid was moved onto the AMN lesion and the mean retinal layer thicknesses of the central grid were recorded and compared with the corresponding area of the fellow eye at initial presentation and during follow-up. RESULTS Eleven patients were included (mean age 26±6 years). AMN lesions at baseline had a significantly thinner outer nuclear layer (ONL) (51±21 µm vs 73±17 µm, p=0.002). The other layers, including inner nuclear layer (37±8 µm vs 38±6 µm, p=0.9) and outer plexiform layer (OPL) (45±19 µm vs 33±16 µm, p=0.1) did not show significant differences between the study eyes and fellow eyes. Adjacent to NIR image lesions, areas of OPL thickening were identified (study eye: 50±14 µm vs fellow eye: 39±16 µm, p=0.005) with corresponding thinning of ONL (study eye: 52±16 µm vs fellow eye: 69±16 µm, p=0.002). CONCLUSIONS AMN presents with characteristic quantitative retinal changes and the extent of the lesion may be more extensive than initially presumed from NIR image lesions.

Duchenne muscular dystrophy (DMD) is a progressive neuromuscular disease with death usually occurring because of respiratory failure. Signs of early respiratory insufficiency are usually first detectable in sleep. Objective: To study the presentation of sleep-related breathing disorder (SRBD) in patients with DMD. Method:> A retrospective review of patients with DMD attending a tertiary paediatric sleep disorder clinic over a 5-year period. Symptoms, lung function and polysomnographic indices were reviewed. Results: A total of 34 patients with DMD were referred for respiratory assessment (1-15 years). Twenty-two (64%) reported sleep-related symptomatology. Forced vital capacity (FVC) was between 12 and 107% predicted (n = 29). Thirty-two progressed to have polysomnography of which 15 were normal studies (median age: 10 years) and 10 (31%) were diagnostic of obstructive sleep apnoea (OSA) (median age: 8 years). A total of 11 patients (32%) showed hypoventilation (median age: 13 years) during the 5-year period and non-invasive ventilation (NIV) was offered to them. The median FVC of this group was 27% predicted. There was a significant improvement in the apnoea/hypopnoea index (AHI) (mean difference = 11.31, 95% CI = 5.91-16.70, P = 0.001) following the institution of NIV. Conclusions: The prevalence of SRBD in DMD is significant. There is a bimodal presentation of SRBD, with OSA found in the first decade and hypoventilation more commonly seen at the beginning of the second decade. Polysomnography is recommended in children with symptoms of OSA, or at the stage of becoming wheelchair-bound. In patients with the early stages of respiratory failure, assessment with polysomnography-identified sleep hypoventilation and assisted in initiating NIV.

Objective To determine the long-term health and development of a cohort of children in whom confined placental mosaicism (CPM) was diagnosed at prenatal diagnosis. Methods A retrospective cohort study was performed comparing 36 children in whom CPM had been diagnosed prenatally with 195 controls subjects in whom a normal karyotype had been detected prenatally. Data comprising birth information, health, health service utilisation, growth, development, behaviour, and the family were collected by a maternal questionnaire administered when the subjects were aged between 4 and 11 years. Results CPM cases did not differ from controls across a broad range of health measures and there were no major health problems or birth defects among the CPM group. No increase was detected in the incidence of intrauterine growth retardation (IUGR) among CPM cases; however, postnatal growth was reduced compared with controls (p = 0.047). Development and behaviour in CPM cases was similar to that of controls. Conclusions The prenatal diagnosis of CPM is not associated with an increased risk of birth defects or developmental problems, but may be associated with decreased growth. Background We compared cost-effectiveness of pravastatin in a placebo-controlled trial in 5500 younger (31-64 years) and 3514 older patients (65-74 years) with previous acute coronary syndromes. Methods Hospitalizations and long-term medication within the 6 years of the trial were estimated in all patients. Drug dosage, nursing home, and ambulatory care costs were estimated from substudies. Incremental costs per life saved of pravastatin relative to placebo were estimated from treatment effects and resource use. Results Over 6 years, pravastatin reduced all-cause mortality by 4.3% in the older patients and by 2.3% in the younger patients. Older patients assigned pravastatin had marginally lower cost of pravastatin and other medication over 6 years (A$4442 vs A$4637), but greater cost offsets (A$2061 vs. A$897) from lower rates of hospitalizations. The incremental cost per life saved with pravastatin was A$55500 in the old and A$167200 in the young. Assuming no treatment effect beyond the study period, the life expectancy to age 82 years of additional survivors was 9.1 years in the older and. 17.3 years in the younger. Estimated additional life-years saved from pravastatin therapy were 0.39 years for older and 0.40 years for younger patients. Incremental costs per life-year saved were A$7581 in the older and A$1.4944 in the younger, if discounted at 5% per annum. Conclusions Pravastatin therapy was more cost-effective among older than younger patients, because of their higher baseline risk and greater cost offsets, despite their shorter life expectancy.

Background: Describe the recent evolution of cigarette smoking habits by gender in Geneva, where incidence rates of lung cancer have been declining in men but increasing in women. Methods: Continuous cross-sectional surveillance of the general adult ( 35 - 74 yrs) population of Geneva, Switzerland for 11 years ( 1993 - 2003) using a locally-validated smoking questionnaire, yielding a representative random sample of 12,271 individuals ( 6,164 men, 6,107 women). Results: In both genders, prevalence of current cigarette smoking was stable over the 11-year period, at about one third of men and one quarter of women, even though smoking began at an earlier age in more recent years. Older men were more likely to be former smokers than older women. Younger men, but not women, tended to quit smoking at an earlier age. Conclusion: This continuous ( 1993 - 2003) risk factor surveillance system, unique in Europe, shows stable prevalence of smoking in both genders. However, sharp contrasts in age-specific prevalence of never and former smoking and of ages at smoking initiation indicate that smoking continues a long-term decline in men but has still not reached its peak in women.

Purpose To evaluate the imaging characteristics of a cohort of patients with ocular adnexal lymphoproliferative disease (OALD). Methods A noncomparative retrospective review between 1992 and 1995 and prospective study from 1995 to 2005 of the clinical, imaging and treatment of 105 patients presenting to tertiary orbital referral centre presenting with OALD. Results One hundred and five patients (mean age 61 years, range 11-90 years) with equal gender distribution were included. Fifty-three were primary and 52 were secondary. Computed tomography (CT) usually showed a well-circumscribed lesion of greater than brain density, moulding to adjacent tissues with moderate enhancement. Aggressive histology was associated with bone destruction, while moulding was associated with indolent histology (P < 0.005). MRI in OALD showed intermediate signal intensity on T1- and T2-weighted images and moderate enhancement with gadolinium. Gallium scanning sensitivity to detect ocular adnexal disease was 25 and 57% for systemic involvement. Positron emission tomography (PET) upstaged (71%) of patients with systemic lymphoproliferative involvement, having a higher sensitivity than CT in detecting distant disease (86 vs 72%). Conclusions CT and/ or MRI are essential in the evaluation of OALD and can be used to establish that an orbital lesion may be lymphoprolifetaive in nature. Further, these imaging modalities may predict the behaviour of the lymphoma in certain cases. Gallium scanning provides no additional information to CT and does not influence patient treatment. PET represents an important addition to the assessment of OALD with real impact on patient management.

Theory of mind (ToM) was examined in late-signing deaf children in two studies by using standard tests and measures of spontaneous talk about inner states of perception, affect and cognition during storytelling. In Study 1, there were 21 deaf children aged 6 to 11 years and 13 typical-hearing children matched with the deaf by chronological age. In Study 2, there were 17 deaf children aged 6 to 12 years and 17 typical-hearing preschoolers aged 4 to 5 years who were matched with the deaf by ToM test performance. In addition to replicating the consistently reported finding of poor performance on standard false belief tests by late-signing deaf children, significant correlations emerged in both studies between deaf children's ToM test scores and their spontaneous narrative talk about imaginative cognition (e.g. 'pretend'). In Study 2, with a new set of purpose-built pictures that evoked richer and more complex mentalistic narration than the published picture book of Study 1, results of multiple regression analyses showed that children's narrative talk about imaginative cognition was uniquely important, over and above hearing status and talking of other kinds of mental states, in predicting ToM scores. The same was true of children's elaborated narrative talk using utterances that either spelt out thoughts, explained inner states or introduced contrastives. In addition, results of a Guttman scalograrn analysis in Study 2 suggested a consistent sequence in narrative and standard test performance by deaf and hearing children that went from (1) narrative mention of visible (affective or perceptual) mental states only, along with FB failure, to (2) narrative mention of cognitive states along with (1), to (3) elaborated narrative talk about inner states along with (2), and finally to (4) simple and elaborated narrative talk about affective/perceptual and cognitive states along with FIB test success. Possible explanations for this performance ordering, as well as for the observed correlations in both studies between ToM test scores and narrative variables, were considered.

Two-dimensional (2-D) strain (epsilon(2-D)) on the basis of speckle tracking is a new technique for strain measurement. This study sought to validate epsilon(2-D) and tissue velocity imaging (TVI)based strain (epsilon(TVI)) with tagged harmonic-phase (HARP) magnetic resonance imaging (MRI). Thirty patients (mean age. 62 +/- 11 years) with known or suspected ischemic heart disease were evaluated. Wall motion (wall motion score index 1.55 +/- 0.46) was assessed by an expert observer. Three apical images were obtained for longitudinal strain (16 segments) and 3 short-axis images for radial and circumferential strain (18 segments). Radial epsilon(TVI) was obtained in the posterior wall. HARP MRI was used to measure principal strain, expressed as maximal length change in each direction. Values for epsilon(2-D), epsilon(TVI), and HARP MRI were comparable for all 3 strain directions and were reduced in dysfunctional segments. The mean difference and correlation between longitudinal epsilon(2-D) and HARP MRI (2.1 +/- 5.5%, r = 0.51, p < 0.001) were similar to those between longitudinal epsilon(TVI), and HARP MRI (1.1 +/- 6.7%, r = 0.40, p < 0.001). The mean difference and correlation were more favorable between radial epsilon(2-D) and HARP MRI (0.4 +/- 10.2%, r = 0.60, p < 0.001) than between radial epsilon(TVI), and HARP MRI (3.4 +/- 10.5%, r = 0.47, p < 0.001). For circumferential strain, the mean difference and correlation between epsilon(2-D) and HARP MRI were 0.7 +/- 5.4% and r = 0.51 (p < 0.001), respectively. In conclusion, the modest correlations of echocardiographic and HARP MRI strain reflect the technical challenges of the 2 techniques. Nonetheless, epsilon(2-D) provides a reliable tool to quantify regional function, with radial measurements being more accurate and feasible than with TVI. Unlike epsilon(TVI), epsilon(2-D) provides circumferential measurements. (c) 2006 Elsevier Inc. All rights reserved.
