With the ageing of the population, articular prosthetic replacements are becoming more and more frequent. One of the most feared complications is prosthetic infection, mostly due to bacteria of the cutaneous flora. Listeria monocytogenes is rarely the cause. This paper describes the management of a hip prosthetic infection due to Listeria monocytogenes. The patient was cured with antimicrobial therapy and a two-stage exchange. This case report creates an opportunity to review the literature in the aim of determining the risk factors and the optimal care.

Objetivou-se compreender o discurso das pessoas com deficiência acerca da concretude do processo de inclusão social e articular o discurso desses sujeitos com as diretrizes do Decreto nº 3.298/99. Foram entrevistadas seis pessoas com deficiência física, seis auditivos e seis visuais Os dados foram padronizados e categorizados em dois núcleos de sentidos: diretrizes pré-inclusivistas da pessoa com deficiência, e diretrizes inclusivistas da pessoa com deficiência. A perspectiva metodológica foi a análise de discurso. Conclui-se que esses sujeitos conquistaram legislação que assegura direitos de cidadania, mas existem desafios no campo da saúde, educação, profissionalização e da inserção no mercado de trabalho. Os enfermeiros reabilitadores precisam conhecer a vivência desses sujeitos e dos seus familiares para ajudá-los a buscar estratégias de enfrentamento dos problemas que afetam a sua inclusão e, conseqüentemente, a sua saúde.

Pensar a formação de enfermeiros pressupõe articular essa questão às expressões de referenciais teóricos, na perspectiva de uma vertente pedagógica que passe pelo construtivismo e por competências. O objetivo foi caracterizar, numa visão longitudinal, a constituição das competências assistenciais nos cursos de graduação em Enfermagem contidas nos Planos de Ensino. O estudo teve um caráter exploratório-descritivo, com abordagem qualitativa. Foi realizada uma análise documental dos 9 planos de ensino das disciplinas assistenciais da graduação. Os aspectos ético-legais foram garantidos, sendo os dados coletados após aprovação pelo Comitê de Ética em Pesquisa. Os dados evidenciaram uma organização curricular centrada em disciplinas, mantendo lógicas internas aparentemente refratárias às organizações somativas. Daí emergem sinalizações de uma aprendizagem com vínculos pouco substantivos entre os conhecimentos prévios e a potencialização do julgamento crítico e do raciocínio clínico. Como proposta, o estudo trouxe reconsiderações para o processo de ensino-aprendizagem e a influência da concepção construtivista na proposição das competências clínicas.

Slightly displaced clavicular midshaft fracture is currently treated nonoperatively. There is considerable debate about whether acute displaced fractures should be treated operatively. Nonunion rate after displaced fracture has been underestimated for a long time, and malunion clinical impact often minimized. It is known that operative treatment decreases these rates. However, operating all displaced fractures may lead to overtreatment. Acute operative treatment of midshaft fractures with delayed treatment of established nonunion showed no significant difference in the outcome, but malunion surgical management is technically demanding. Consequently these fractures could be treated surgically, but operative indication should be adapted to patient expectations.

PURPOSE: To describe osseous, chondral and tendinous lesions associated with fissures of the posterior labrum. To better understand the pathological processes leading to fissure of the posterior labrum. Materials and methods. Retrospective study of 43 CT arthrograms performed in 43 patients that showed a fissure of the posterior labrum. The following associated lesions were noted: osseous and chondral remodeling of the humeral head and/or glenoid and articular surface fissures of the rotator cuff. Based on type of associated lesions, patients were separated into one of four pathological subgroups: posterior instability, posterosuperior or internal impingement, anterior instability and isolated fissure of the posterior labrum. RESULTS: Sixteen patients (37.2%) of patients showed posterior instability, 12 (27.9%) showed lesions of internal impingement, and 11 (25.6%) showed lesions of anterior instability. Only 4 patients (9.3%) had an isolated fissure of the posterior labrum. CONCLUSION: Posterior instability, internal impingement and anterior instability are the main pathologies leading to fissure of the posterior labrum, which seldom occurs in isolation. Evaluation of these associated lesions allows understanding of the underlying pathological processes leading to fissure of the posterior labrum.

Glypicans are a family of glycosylphosphatidylinositol (GPI)-anchored, membrane-bound heparan sulfate (HS) proteoglycans. Their biological roles are only partly understood, although it is assumed that they modulate the activity of HS-binding growth factors. The involvement of glypicans in developmental morphogenesis and growth regulation has been highlighted by Drosophila mutants and by a human overgrowth syndrome with multiple malformations caused by glypican 3 mutations (Simpson-Golabi-Behmel syndrome). We now report that autosomal-recessive omodysplasia, a genetic condition characterized by short-limbed short stature, craniofacial dysmorphism, and variable developmental delay, maps to chromosome 13 (13q31.1-q32.2) and is caused by point mutations or by larger genomic rearrangements in glypican 6 (GPC6). All mutations cause truncation of the GPC6 protein and abolish both the HS-binding site and the GPI-bearing membrane-associated domain, and thus loss of function is predicted. Expression studies in microdissected mouse growth plate revealed expression of Gpc6 in proliferative chondrocytes. Thus, GPC6 seems to have a previously unsuspected role in endochondral ossification and skeletal growth, and its functional abrogation results in a short-limb phenotype.

OBJECTIVES: The aim of our study was to evaluate the role of cell-membrane expressed TLRs and the signaling molecule MyD88 in a murine model of OA induced by knee menisectomy (surgical partial removal of the medial meniscus [MNX]). METHODS: OA was induced in 8-10weeks old C57Bl/6 wild-type (WT) female (n=7) mice and in knockout (KO) TLR-1 (n=7), -2 (n=8), -4 (n=9) -6 (n=5), MyD88 (n=8) mice by medial menisectomy, using the sham-operated contralateral knee as a control. Cartilage destruction and synovial inflammation were evaluated by knee joint histology using the OARSI scoring method. Apoptotic chondrocytes and cartilage metabolism (collagen II synthesis and MMP-mediated aggrecan degradation) were analyzed using immunohistochemistry. RESULTS: Operated knees exhibited OA features at 8weeks post-surgery compared to sham-operated ones. In menisectomized TLR-1, -2, -4, and -6 deficient mice, cartilage lesions, synovial inflammation and cartilage metabolism were similar to that in operated WT mice. Accordingly, using the same approach, we found no significant protection in MyD88-deficient mice in terms of OA progression as compared to WT littermates. CONCLUSIONS: Deficiency of TLRs or their signalling molecule MyD88 did not impact on the severity of experimental OA. Our results demonstrate that MyD88-dependent TLRs are not involved in this murine OA model. Moreover, the dispensable role of MyD88, which is also an adaptor for IL-1 receptor signaling, suggests that IL-1 is not a key mediator in the development of OA. This latter hypothesis is strengthened by the lack of efficiency of IL-1β antagonist in the treatment of OA.

O Grupo de Estudos e Pesquisas de Tecnologia da Informação nos Processos de Trabalho em Enfermagem (GEPETE) visa produzir e socializar o conhecimento na área de tecnologia da informação e comunicação na saúde e enfermagem, articular a integração com grupos de pesquisas desta área e propiciar a participação de alunos. O estudo realizado pelos tutores teve como objetivo relatar a construção do ambiente virtual de aprendizagem (AVA) e a experiência dos tutores como mediadores de um grupo de pesquisa na plataforma Moodle. Para tanto, foi construído um AVA, realizada a mediação pedagógica sob a temática tutoria e o diagnóstico inicial em relação às dificuldades na utilização da tecnologia na interatividade e na comunicação, o que permitiu a proposição da continuidade da utilização da plataforma como um recurso para apoiar as atividades de pesquisa, oferecer aos líderes pesquisadores mecanismos para socialização dos projetos e possibilidades de orientação à distância.

O artigo é uma reflexão teórica que faz parte de um estudo denominado Em Defesa da Sociedade: a invenção dos Cuidados Paliativos. Para articular tal discussão, utilizamos o manual de Cuidados Paliativos, publicado no ano de 2007 pela Organização Mundial da Saúde (OMS), entendendo-o como parte de um artefato capaz de produzir subjetividades e governar condutas. Nesse sentido, pretendemos descobrir como os discursos sobre os Cuidados Paliativos se articulam e efetuam a invenção de uma nova disciplina que funcionaria como uma estratégia biopolítica para defender a sociedade. A partir da análise textual do discurso instituído pelo manual, com o auxílio do referencial dos Estudos Culturais e sob a inspiração dos escritos de Michel Foucault, articulamos uma das possíveis leituras deste guia. Desta maneira observamos a (re)organização e a (re)invenção de uma disciplina que investe na subjetividade dos indivíduos constituindo aparatos de verdade que regulam e governam a população.

Se evaluó la seguridad y efectividad de la asociación de Metotrexato (MTX) y Leflunomida (LFN) en pacientes con Artritis Idiopática Juvenil (AIJ) en la práctica clínica habitual. Se recogieron datos: demográficos, seguridad y efectividad (articular, ocular). Realizamos un estudio retrospectivo (19 pacientes). Las variables e índices JADAS mejoraron. A los 18 meses las tasas de mejoría del 30%, mínima enfermedad, inactividad y remisión eran elevadas. Los 8 niños con uveítis presentaron remisión. No hubo EA graves. En nuestros pacientes con AIJ el tratamiento MTX+LFN demostró ser seguro y efectivo pudiendo considerarse una alternativa en pacientes sin buena respuesta en monoterapia.

PURPOSE: To compare examination time with radiologist time and to measure radiation dose of computed tomographic (CT) fluoroscopy, conventional CT, and conventional fluoroscopy as guiding modalities for shoulder CT arthrography. MATERIALS AND METHODS: Glenohumeral injection of contrast material for CT arthrography was performed in 64 consecutive patients (mean age, 32 years; age range, 16-74 years) and was guided with CT fluoroscopy (n = 28), conventional CT (n = 14), or conventional fluoroscopy (n = 22). Room times (arthrography, room change, CT, and total examination times) and radiologist times (time the radiologist spent in the fluoroscopy or CT room) were measured. One-way analysis of variance and Bonferroni-Dunn posthoc tests were performed for comparison of mean times. Mean effective radiation dose was calculated for each method with examination data, phantom measurements, and standard software. RESULTS: Mean total examination time was 28.0 minutes for CT fluoroscopy, 28.6 minutes for conventional CT, and 29.4 minutes for conventional fluoroscopy; mean radiologist time was 9.9 minutes, 10.5 minutes, and 9.0 minutes, respectively. These differences were not statistically significant. Mean effective radiation dose was 0.0015 mSv for conventional fluoroscopy (mean, nine sections), 0.22 mSv for CT fluoroscopy (120 kV; 50 mA; mean, 15 sections), and 0.96 mSv for conventional CT (140 kV; 240 mA; mean, six sections). Effective radiation dose can be reduced to 0.18 mSv for conventional CT by changing imaging parameters to 120 kV and 100 mA. Mean effective radiation dose of the diagnostic CT arthrographic examination (140 kV; 240 mA; mean, 25 sections) was 2.4 mSv. CONCLUSION: CT fluoroscopy and conventional CT are valuable alternative modalities for glenohumeral CT arthrography, as examination and radiologist times are not significantly different. CT guidance requires a greater radiation dose than does conventional fluoroscopy, but with adequate parameters CT guidance constitutes approximately 8% of the radiation dose.

Universal standard goniometer is an essential tool to measure articulations' range of motion (ROM). In this time of technological advances and increasing use of smartphones, new measurement's tools appear as specific smartphone applications. This article compares the iOS application "Knee Goniometer" with universal standard goniometer to assess knee ROM. To our knowledge, this is the first study that uses a goniometer application in a clinical context. The purpose of this study is to determine if this application could be used in clinical practice.

Este presente trabalho, de carácter científico foi feito no âmbito de uma investigação sobre o uso de novas tecnologias de comunicação na sala de aula nas duas escolas do ensino básico integrado (EBI), sendo uma pública (escola Hermen Gmeiner `SOS` e outra privada (colégio da turminha), situadas no concelho da Praia. Para este trabalho foram aplicados inquéritos aos professores e alunos da terceira fase (6º ano de escolaridade) das duas escolas e ainda entrevista aplicada a dirigente do Ministério da Educação e Ensino Superior – Directora do Ensino Básico e Integrado e Pré-Escolar. A utilização do computador entrou no quotidiano das duas escolas por intermédio de projecto educativo que as mesmas elaboraram dentro das actividades curriculares. É preciso levar em conta, ao adoptar-se o computador em sala de aula, as possibilidades que professores e alunos terão com a sua utilização. A introdução da informática na escola favorece o aprofundamento das discussões pedagógicas e dá ao professor a oportunidade para repensar suas concepções sobre ensino e aprendizagem. O aluno entre outras vantagens, ganha um meio moderno de se apropriar do conhecimento O computador pode auxiliar o trabalho nas diferentes áreas curriculares, alem de ampliar os horizontes da unidade escolar, por meio do uso da rede electrónica. Essa facilidade permite á escola manter-se “plugada” com o mundo e articular-se a processos em andamento, seja no âmbito da produção da cultura, seja no do exercício da cidadania. Esta ampliação dos espaços escolares abre desafios e perspectivas para todos, na medida em que coloca perguntas novas e surpreendentes. Essas possibilidades fazem com que se ultrapassem, em muito, os limites do computador como um instrumento individual.

A new method of measuring joint angle using a combination of accelerometers and gyroscopes is presented. The method proposes a minimal sensor configuration with one sensor module mounted on each segment. The model is based on estimating the acceleration of the joint center of rotation by placing a pair of virtual sensors on the adjacent segments at the center of rotation. In the proposed technique, joint angles are found without the need for integration, so absolute angles can be obtained which are free from any source of drift. The model considers anatomical aspects and is personalized for each subject prior to each measurement. The method was validated by measuring knee flexion-extension angles of eight subjects, walking at three different speeds, and comparing the results with a reference motion measurement system. The results are very close to those of the reference system presenting very small errors (rms = 1.3, mean = 0.2, SD = 1.1 deg) and excellent correlation coefficients (0.997). The algorithm is able to provide joint angles in real-time, and ready for use in gait analysis. Technically, the system is portable, easily mountable, and can be used for long term monitoring without hindrance to natural activities.

OBJECTIVES: In vitro mechanical injury of articular cartilage is useful to identify events associated with development of post-traumatic osteoarthritis (OA). To date, many in vitro injury models have used animal cartilage despite the greater clinical relevance of human cartilage. We aimed to characterize a new in vitro injury model using elderly human femoral head cartilage and compare its behavior to that of an existing model with adult bovine humeral head cartilage. DESIGN: Mechanical properties of human and bovine cartilage disks were characterized by elastic modulus and hydraulic permeability in radially confined axial compression, and by Young's modulus, Poisson's ratio, and direction-dependent radial strain in unconfined compression. Biochemical composition was assessed in terms of tissue water, solid, and glycosaminoglycan (GAG) contents. Responses to mechanical injury were assessed by observation of macroscopic superficial tissue cracks and histological measurements of cell viability following single injurious ramp loads at 7 or 70%/s strain rate to 3 or 14 MPa peak stress. RESULTS: Confined compression moduli and Young's moduli were greater in elderly human femoral cartilage vs adult bovine humeral cartilage whereas hydraulic permeability was less. Radial deformations of axially compressed explant disks were more anisotropic (direction-dependent) for the human cartilage. In both cartilage sources, tissue cracking and associated cell death during injurious loading was common for 14 MPa peak stress at both strain rates. CONCLUSION: Despite differences in mechanical properties, acute damage induced by injurious loading was similar in both elderly human femoral cartilage and adult bovine humeral cartilage, supporting the clinical relevance of animal-based cartilage injury models. However, inherent structural differences such as cell density may influence subsequent cell-mediated responses to injurious loading and affect the development of OA.
