The electricity market restructuring, and its worldwide evolution into regional and even continental scales, along with the increasing necessity for an adequate integration of renewable energy sources, is resulting in a rising complexity in power systems operation. Several power system simulators have been developed in recent years with the purpose of helping operators, regulators, and involved players to understand and deal with this complex and constantly changing environment. The main contribution of this paper is given by the integration of several electricity market and power system models, respecting to the reality of different countries. This integration is done through the development of an upper ontology which integrates the essential concepts necessary to interpret all the available information. The continuous development of Multi-Agent System for Competitive Electricity Markets platform provides the means for the exemplification of the usefulness of this ontology. A case study using the proposed multi-agent platform is presented, considering a scenario based on real data that simulates the European Electricity Market environment, and comparing its performance using different market mechanisms. The main goal is to demonstrate the advantages that the integration of various market models and simulation platforms have for the study of the electricity markets’ evolution.

Introdução: O crescimento da despesa com antidiabéticos orais tem levado a preocupações questionando os ganhos de saúde e vantagens para o sistema de saúde gerados por esses medicamentos. Este estudo contribui para responder a estas questões. Material e Métodos: Numa primeira fase apresentam-se estimativas das três variáveis centrais a utilizar na análise: 1) a prevalência tratada da diabetes por ano e por região de saúde baseadas nos consumos de antidiabéticos orais, 2) o número de episódios de internamento hospitalar atribuíveis à diabetes com base nos riscos relativos das várias patologias e os seus custos e 3) uma caracterização quantitativa dos antidiabéticos orais consumidos através do cálculo da sua vintage média. Através do uso de observações para 10 anos e cinco regiões, perfazendo um total de 50 observações, foi possível estimar um modelo econométrico explicando estatisticamente os internamentos e os custos hospitalares atribuíveis à diabetes por características regionais, pela prevalência tratada e pela vintage média dos antidiabéticos orais. Resultados: Os resultados dos modelos de regressão múltipla mostram que as despesas hospitalares são proporcionais à prevalência tratada, tudo o mais constante mas que quanto mais recente for a vintage dos antidiabéticos orais usados menores são os custos hospitalares. Os efeitos para o número de internamentos são similares. Discussão e Conclusões: Para uma observação média na amostra, se a vintage média dos antidiabéticos orais fosse um ano superior então os custos hospitalares seriam 5,3% inferiores (cerca de € 11 milhões em 2009) e o número de internamentos seria 3,8% menor (cerca de menos 3965 episódios em 2009). Um exercício contra factual permite estimar que para o ano de 2009 a introdução da classe dos inibidores de DPP IV permitiu reduzir o número de internamentos atribuíveis à diabetes em 8480 e com isso poupar € 23,3 milhões em custos hospitalares.

Introdução: Durante um internamento podem ser necessários procedimentos diagnósticos e terapêuticos, e as crianças e os pais contactam com vários profissionais de saúde. Será que as informações fornecidas pelos médicos são suficientes e claras? Objectivos: Avaliar a opinião dos pais relativamente às informações fornecidas pelos médicos e a influência que podem ter factores socioeconómicos e culturais na compreensão destas informações. Métodos: Recolheram-se dados demográficos e avaliou-se o grau de conhecimento sobre o diagnóstico, exames e tratamentos, data da alta, nome do médico assistente e opinião quanto à clareza e frequência das informações. Resultados: Considerámos 300 entrevistas. A maioria dos pais tinha uma correcta percepção sobre o diagnóstico(74,8%), tratamento (70,3%) e exames realizados (78,0%), mas apenas 18,7% sabiam a data provável da alta. Em 65,0% dos casos as informações tinham sido fornecidas pelo médico responsável e a maioria considerou a informação clara(87,0%) e suficiente (81,0%). Em serviços de Cirurgia registou-se uma melhor informação relativamente ao diagnóstico (80,4% vs 69,9%, p=0,042) e nome do médico responsável (70,6% vs 55,5%, p=0,008). Os entrevistados com maior escolaridade conheciam melhor o médico (70,4% vs 56,6%, p=0,016) e os tratamentos efectuados às crianças (79,6% vs 69,8%, p<0,001). Os pais de nacionalidade portuguesa e de meio socioeconómico mais elevado conheciam melhor o diagnóstico (79,7% vs 61,8%, p=0,004), (80,7% vs 65,7%, p=0,007) e tratamento (76,3% vs 50,0%, p<0,001), (81,3% vs 49,0%, p=<0,001). Adicionalmente, o grupo socialmente mais diferenciado conhecia melhor o médico responsável (55,9% vs 67,7%, p=0,050). Quando as informações eram transmitidas pelo médico responsável os entrevistados consideravam-nas mais claras (92,3% vs 81,1%, p=0,001) e suficientes (89,7% vs 64,2%, p<0,001).Conclusão: Este estudo identifica a importância do médico assistente na comunicação com os pais das crianças e a necessidade de adequar a informação ao nível sociocultural da família ao longo do internamento.

Introduction: Recent studies suggest that magnesium deficiency may play a role in inflammation. In diabetes and cardio-vascular diseases, conditions with a component of chronic inflammation, C–reactive protein levels are higher and associated with low serum magnesium. The objective of this study is to evaluate serum magnesium levels in patients with systemic lupus erythematosus and its potential association with inflammation and renal manifestations. Methods: All patients with systemic lupus erythematosus followed in a Systemic Immune Diseases Unit, from January 2012 until January 2014, were included in this cross sectional analysis. Patients with infection, neoplasia, liver failure and chronic kidney disease (stage > 3) were excluded. Clinical information and laboratory results (serum magnesium, C-reactive protein, erythrocyte sedimentation rate, serum creatinine and spot urine test) were collected. A multivariate analysis was performed to explore possible predictive factors for hypomagnesaemia. Results: One hundred and two patients were included (94.1% female, 21-86 years). 33.4% had hypertension, 8.8% had diabetes and 20.6% had hypomagnesaemia (< 1.8mg/dL). There were no significant differences between the inflammatory parameters of patients with hypomagnesaemia or normomagnesaemia. Serum magnesium was significantly lower with increasing comorbidities (p = 0.01). Leukocyturia was significantly higher in the hypomagnesaemia group (p = 0.03) and haematuria had a negative correlation with serum magnesium (-0.23, p < 0.05). Multivariate analysis showed that patients with hypertension and diabetes had higher risk of hypomagnesaemia: OR 42.29 (95% CI, 1.43-1249.31). Leukocyturia was also individually and independently associated with hypomagnesaemia: OR 8.37 (95% CI, 1.40-49.97). Conclusion: The presence of hypomagnesaemia in our patients with systemic lupus erythematosus was high. There was no association between the levels of serum magnesium and the inflammatory parameters. Increasing comorbidities and leukocyturia were independent predictors of lower serum magnesium. Finally, the association of leukocyturia and haematuria with lower serum magnesium may suggest a relationship with a higher disease activity.

Intestinal parasites are a problem for public health all over the world. The infection with Blastocystis, a protozoan of controversial pathogenicity, is one of the most common among them all. In this study, the occurrence of intestinal parasites, with emphasis on Blastocystis, in patients at the Universidade Federal do Triângulo Mineiro was investigated in Uberaba (MG) through microscopy of direct smears and fecal concentrates using Ritchie’s method. Feces of 1,323 patients were examined from April 2011 to May 2012. In 28.7% of them at least one intestinal parasite was identified, and the most frequent organisms were Blastocystis spp. (17.8%) and Giardia intestinalis (7.4%). The occurrence of parasitism was higher in children aged 6 -10 years old, and the infection with Blastocystis spp. was higher above the age of six (p < 0.001). The exclusive presence of G. intestinalis and of Blastocystis spp. was observed in 5.4% and 12.2% of the patients, respectively. Regarding patients with diarrheic feces, 8% revealed unique parasitism of Blastocystis spp. Other intestinal parasites observed in children were Ascaris lumbricoides(0.3%) and Entamoeba histolytica/dispar/moshkovskii (1.4%). The Ritchie’s method was more sensitive (92.8%) when compared to direct microscopy (89.8%), with high agreement between them (97.7%, kappa = 0.92). In conclusion, the occurrence of Blastocystis spp. in Uberaba is high and the presence of diarrheic feces with exclusive presence of the parasite of Blastocystis spp. was observed.

Os autores apresentam um caso de esteatohepatite não alcoólica numa mulher obesa, não diabética, que evoluiu para cirrose hepática. A propósito desta entidade revêem a literatura discutindo a sua história natural e os principais processos patológicos a ela associados. Destacam a importância da obesidade no desenvolvimento da esteatohepatite e da vantagem, em casos seleccionados, da execução de biópsia hepática, como técnica de diagnóstico fundamental.

Background: Indoor air quality (IAQ) is considered an important determinant of human health. The association between exposure to volatile organic compounds, particulate matter, house dust mite, molds and bacteria in day care centers (DCC) is not completely clear. The aim of this project was to study these effects. Methods --- study design: This study comprised two phases. Phase I included an evaluation of 45 DCCs (25 from Lisbon and 20 from Oporto, targeting 5161 children). In this phase, building characteristics, indoor CO2 and air temperature/relative humidity, were assessed. A children’s respiratory health questionnaire derived from the ISAAC (International Study on Asthma and Allergies in Children) was also distributed. Phase II encompassed two evaluations and included 20 DCCs selected from phase I after a cluster analysis (11 from Lisbon and 9 from Oporto, targeting 2287 children). In this phase, data on ventilation, IAQ, thermal comfort parameters, respiratory and allergic health, airway inflammation biomarkers, respiratory virus infection patterns and parental and child stress were collected. Results: In Phase I, building characteristics, occupant behavior and ventilation surrogates were collected from all DCCs. The response rate of the questionnaire was 61.7% (3186 children). Phase II included 1221 children. Association results between DCC characteristics, IAQ and health outcomes will be provided in order to support recommendations on IAQ and children’s health. A building ventilation model will also be developed. Discussion: This paper outlines methods that might be implemented by other investigators conducting studies on the association between respiratory health and indoor air quality at DCC.

Background: In the haemodynamically unstable patient the method of treatment of acute renal failure is still largely controversial. The purpose of our study was to compare slow extended dialysis with continuous haemodiafiltration in the critical patient with indication for renal replacement therapy and haemodynamic instability. Patients and Methods: This is a cohort study comparing in 63 ventilated critical patients a 12 month period when only continuous haemodiafiltration was used (n=25) with an equal period of slow extended dialysis (n=38). Our primary objective was to evaluate the impact of the dialytic procedure on cardiovascular stability in those patients. As secondary aims we considered system coagulation/thrombosis and predictors of mortality. In the two groups we analysed the first session performed, the second session performed and the average of all the sessions performed in each patient. Results: In these patients, mortality in the intensive care unit was high (68% in the continuous haemodiafiltration group and 63% in the slow extended dialysis group). We did not find any association between the dialytic technique used and death; only the APACHE score was a predictor of death. Slow extended dialysis was a predictor of haemodynamic stability, a negative predictor of sessions that had to be interrupted for haemodynamic instability, and a predictor of achieving the volume removal initially sought. Slow extended dialysis was also associated with less coagulation of the system. Conclusions: Our data suggested that slow extended dialysis use was not inferior to continuous haemodiafiltration use in terms of cardiovascular tolerability.

Foi realizada avaliação imunológica em 48 indivíduos com históriapregressa de leishmaniose visceral (L. V)eem seis pacientes durante a fase aguda da doença e após o tratamento. Títulos significativos de anticorpos determinados pela técnica de imunofluorescência e/ou ELISA foram observados em 32 (67%) dos 48 casos. A avaliação da resposta imune humoral e celular nos seis pacientes durante a fase ativa da doença demonstrou títulos elevados de anticorpos (média 9536 ± 7169) e resposta línfoproliferativa ausente (323 ± 24). Após o tratamento (3 e 6 meses) os títulos de anticorpos só caíram em três dos seis pacientes, ao passo que linfócitos passaram a responder "in vítro" a antígenos de leíshmânía (11909 ± 5637). Estes dados demonstram que os indivíduos que adquirem leishmaniose visceral não são geneticamente incapacitados de responder a antígeno de leíshmânía equea persistência de títulos elevados de anticorpos anos após o tratamento, sugere que o parasita permaneça no hospedeiro após a cura clínica da doença.

Em Mambaí, área endêmica de doença de Chagas, foram realizadas abreugrafias do esôfago com filme de 70mm, em 1.145 indivíduos do sexo masculino e 1.184 dofeminino, de 4 a 87 anos, dos quais 1.006eram soropositivos. Os pacientes foram posicionados em oblíqua anterior direita e tomadas duas abreugrafias: uma imediatamente à ingestão de 75 ml de suspensão baritada e outra 60 segundos após. Setenta e seis (3,2%) indivíduos apresentaram esofagopatia, sendo que 71(7%) entre os 1.006 soropositivos e, 5(0,37%) entre os 1.323 soronegativos. Dos 76 esofagopatas 47(61,8%) eram do sexo masculino e 29(38,1%) do sexo feminino. Segundo a classificação de Rezende e cols 48(63,1%) pertenciam ao grupo I, 18(23,7%) ao grupo II, 5(6,6%) ao grupo III e 5(6,6%) ao grupo IV. Entre os infectados a esofagopatia aumentou progressivamente com a idade, principalmente a partir dos 30 anos, alcançando a prevalência de 21,5% após os 59 anos.

In this work, a volumetric unit previously assembled by the research group was upgraded. This unit revamping was necessary due to the malfunction of the solenoid valves employed in the original experimental setup, which were not sealing the gas properly leading to erroneous adsorption equilibrium measurements. Therefore, the solenoid valves were substituted by manual ball valves. After the volumetric unit improvement its operation was validated. For this purpose, the adsorption equilibrium of carbon dioxide (CO2) at 323K and 0 - 20 bar was measured on two different activated carbon samples, in the of extrudates (ANG6) and of a honeycomb monolith (ACHM). The adsorption equilibrium results were compared with data previously measured by the research group, using a high-pressure microbalance from Rubotherm GmbH (Germany) – gravimetric. The results obtained using both apparatuses are coincident thus validating the good operation of the volumetric unit upgraded in this work. Furthermore, the adsorption equilibrium of CO2 at 303K and 0 - 10 bar on Metal-Organic Frameworks (MOFs) Cu-BTC and Fe-BTC was also studied. The CO2 adsorption equilibrium results for both MOFs were compared with the literature results showing good agreement, which confirms the good quality of the experimental results obtained in the new volumetric unit. Cu-BTC sample showed significantly higher CO2 adsorption capacity when compared with the Fe-BTC sample. The revamping of the volumetric unit included a new valve configuration in order to allow testing an alternative method for the measurement of adsorption equilibrium. This new method was employed to measure the adsorption equilibrium of CO2 on ANG6 and ACHM at 303, 323 and 353K within 0-10 bar. The good quality of the obtained experimental data was testified by comparison with data previously obtained by the research group in a gravimetric apparatus.
