Eficàcia predictiva de l’HCR-20 i del PCL:SV en contexts institucionals

L’estudi es dirigeix a avaluar la capacitat predictiva sobre la violència dels instruments de judici clínic estructurat, amb una metodologia que aporti resultats comparables a estudis de l’àmbit internacional. La investigació es va portar a terme en un hospital civil de salut mental i la mostra està composta per 114 pacients de les unitats de crònics i subaguts. A l’avaluació inicial, l’HCR-20, el PCL:SV i el Protocol 7 van ser els instruments utilitzats per a la recollida d’informació de les variables predictors. La variable depenent o resultat va ser registrada prospectivament per part de l’equip d’infermeria amb un instrument observacional de fàcil ús, el MOAS. Mitjançant índexs de correlació, càlcul de riscos relatius, i anàlisis de regressió logística i corbes ROC va ser possible conèixer que l’HCR-20 i el PCL-SV són mesures vàlides per a la predicció de la violència intrahospitalària en el curt i mig termini en una mostra espanyola de persones amb malaltia mental severa. L’HCR-20 i particularment els ítems clínics van ser els millors predictors de la violència física envers a persones i objectes. Tant la puntuació numèrica de l’HCR-20 com el judici clínic estructurat van demostrar una precisió predictiva alta i comparable a l'obtinguda amb la versió original de l'instrument. El PCL:SV va arribar una precisió predictiva moderada que va anar disminuint al llarg del seguiment. Altres factors de risc com les agressions o la ira prèvies a l’avaluació també van augmentar significativament el risc de violència durant l’any de seguiment.

La dynamique protestataire du Mouvement du 20 février à Casablanca

Au-delà du discours vantant « l'exception marocaine », un large mouvement de protestation s'est développé dès le mois de février 2011 dans l'ensemble du pays. Loin d'être le produit d'un effet domino, le Mouvement du 20 février est notamment tributaire d'un processus d'attribution de similarité, de la réactivation de relais organisationnels et de réseaux plus ou moins dormants. Une large coalition s'est constituée au sein d'un champ d'alliance et d'opposition à la jonction entre scène politique instituée et espace protestataire, par-delà les clivages entre réseaux de gauche et islamistes. Dans un jeu d'échelles entre le local, le national, le régional et l'international, un faisceau d'actions, d'interactions et d'événements a contribué autant à l'enracinement de la coalition qu'à sa désagrégation.

Impairment and disability in 20 CIDP patients according to disease activity status.

Twenty patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) meeting the EFNS/PNS criteria were examined in order to assess differences/similarities between the various grading systems according to CIDP disease activity status (CDAS). A principal component (PC) analysis and the correlations between the following scores were performed: Neurological Symptom Score; MRC sum score; Neurological Impairment Score; Hammersmith Functional Motor Scale; Inflammatory Neuropathy Cause and Treatment (INCAT) Sensory Sum Score; Overall Disability Sum Score; INCAT Disability Score; Rasch-built Overall Disability Scale. Our analysis outlined two main sets of scales, with high influence in the top two PCs. The first PC that best explained the variability within the cohort consisted of CDAS, general disability scores and motor scores; these parameters were also strongly correlated amongst each other. The second PC explained less the variability and consisted mainly of sensory scores and disease duration; these parameters did not correlate with the scores of the first PC or with the CDAS. Our findings suggest separating screening for motor and sensory deficits when evaluating CIDP patients, as only the motor scores correlate with CDAS.

Estado actual de pacientes trasplantados en edad pediátrica con más de 20 años de seguimiento

L'objectiu d'aquest treball és analitzar els portadors d'un empelt hepàtic trasplantats en edat pediàtrica amb més de 20 anys de seguiment i per a això hem analitzat un total de 27 pacients. La principal indicació del trasplantament va ser l'atrèsia de vies biliars. S'han analitzat les complicacions quirúrgiques mèdiques i immunològiques, la immunosupressió, infecció per VHC, dades antropromètriques i adaptació social. Hem conclòs que la majoria dels pacients trasplantats en edat pediàtrica amb més de 20 anys d'evolució presenten una adaptació social excel•lent i pocs efectes adversos de la immunosupressió comparada amb la població adulta.

Chemical and carbon isotopic evolution of hydrocarbons during prograde metamorphism from 100°C to 550°C: Case study in the Liassic black shale formation of Central Swiss Alps

Hydrocarbon distributions and stable isotope ratios of carbonates (delta(13)C(car), delta(18)O(car)), kerogen (delta(13)C(ker)), extractable organic matter (delta(13)C(EOM)) and individual hydrocarbons of Liassic black shale samples from a prograde metamorphic sequence in the Swiss Alps were used to identify the major organic reactions with increasing metamorphic grade. The studied samples range from the diagenetic zone (< 100 degrees C) to amphibolite facies (similar to 550 degrees C). The samples within the diagenetic zones (< 100 and 150 degrees C) are characterized by the dominance of C-< 20 n-alkanes, suggesting an origin related with marine and/or bacterial inputs. The metamorphic samples (200 to 550 degrees C) have distributions significantly dominated by C-12 and C-13 n-alkanes, C-14, C-16 and C-18 n-alkylcyclopentanes and to a lesser extend C-15, C-17 and C-21 n-alkylcyclohexanes. The progressive C-13-enrichment (up to 3.9 parts per thousand) with metamorphism of the C-> 17 n-alkanes suggests the occurrence of cracking reactions of high molecular weight compounds. The isotopically heavier (up to 5.6 parts per thousand) C-< 17 n-alkanes in metamorphic samples are likely originated by thermal degradation of long-chain homologous with preferential release of isotopically light C-1 and C-2 radicals. The dominance of specific even C-number n-alkylcyclopentanes suggests an origin related to direct cyclization mechanism (without decarboxylation step) of algal or bacterial fatty acids occurring in reducing aqueous metamorphic fluid conditions. The regular increase of the concentrations of n-alkylcycloalkanes vs. C-> 13 n-alkanes with metamorphism suggests progressive thermal release of kerogen-linked fatty acid precursors and degradation of n-alkanes. Changes of the steroid and terpenoid distributions are clearly related to increasing metamorphic temperatures. The absence of 18 alpha(H)-22,29,30-trisnorneohopane (Ts), the occurrence of 17 beta(H)-trisnorhopane, 17 beta(H), 21 alpha(H)-hopanes in the C-29 to C-31 range and 5 alpha(H),14 alpha(H),17 alpha(H)-20R C-27, C-29 steranes in the low diagenetic samples (< 100 degrees C) are characteristic of immature bitumens. The higher thermal stress within the upper diagenetic zone (150 degrees C) is marked by the presence of Ts, the disappearance of 17 beta(H)-trisnorhopane and thermodynamic equilibrium of the 22S/(22S + 22R) homohopane ratios. The increase of the alpha alpha alpha-sterane 20S/(20S + 20R) and 20R beta beta/(beta beta + alpha alpha) ratios (from 0.0 to 0.55 and from 0.0 to 0.40, respectively) in the upper diagenetic zone indicates the occurrence of isomerization reactions already at < 150 degrees C. However, the isomerization at C-20 (R -> S) reaches thermodynamic equilibrium values already at the upper diagenesis (similar to 150 degrees C) whereas the epimerisation at C-14 and C-17 (alpha alpha ->beta beta) arrives to constant values in the lower anchizone (similar to 200 degrees C). The ratios Ts vs. 17 alpha(H)-22,29,30-trisnorneohopane [(Ts/(Ts + Tm)] and 18 alpha(H)-30-norneohopane (C29Ts) vs. 17 alpha(H),21 beta(H)-30-norhopane [C29Ts/(C29Ts + C-29)] increase until the medium anchizone (200 to 250 degrees C) from 0.0 to 0.96 and from 0.0 to 0.44, respectively. An opposite trend owards lower values is observed in the higher metamorphic samples. The occurrence of specific hydrocarbons (e.g., n-alkylcyclopentanes, cadalene, hydrogenated aromatic compounds) in metamorphic samples points to kerogen degradation reactions most probably occurring in the presence of water and under reducing conditions. The changes of hydrocarbon distributions and carbon isotopic compositions of n-alkanes related to metamorphism suggest that the organic geochemistry may help to evaluate the lowest grades of prograde metamorphism. IPH response to A Healthier Future: A 20 year vision for health and wellbeing in Northern Ireland

The Institute of Public Health in Ireland aims to promote cooperation for public health between Northern Ireland and Ireland, to tackle inequalities in health and influence public polices in favour of health. In its work, the Institute emphasises a holistic model of health which recognises the interplay of a wide range of health determinants, including economic, social and environmental factors as well as health and social services.

IPH response to DOE Consultation 'Preparing a Road Safety Strategy for Northern Ireland 2010-20'

The Department of Environment (NI)  recently held a consultation on preparing a new road safety strategy for Northern Ireland which will cover the years 2010 to 2020.  The consultation ran from 16 March 2010 to 15 June 2010.  The consultation paper outlined key challenges to be addressed over the lifetime of the strategy and proposed a number of action measures which have been agreed by the statutory road safety partners.  Views were invited on preparing a new road safety strategy for Northern Ireland that will shape the way ahead for safety on roads over the next decade.

Selection of Trypanosoma cruzi clonal genotypes (clonet 20 and 39) isolated from Bolivian triatomines following subculture in liquid medium

Previous studies showed that two groups of Trypanosoma cruzi clonal genotypes named clonet 20 and clonet 39 were predominant in Triatoma infestans, the unique vector of Chagas disease in Bolivia. These groups of clones correspond to distinct genetic clusters. These clonets were detected in T. infestans and Rhodnius pictipes fecal samples before isolation and after culture by kDNA PCR (polymerase chain rreaction) and hybridization of the amplified products with clonet specific kDNA probes named 20 and 39 as previously reported. Forty eight T. infestans and three R. pictipes infected insects captured at random in different Bolivian departments were proceeded. As previously reported the direct identification of the two major clonets in fecal samples allowed the detection of abundant mixed infections: 41% in the original sample, however after culture, only 6% of mixed infections were detected. Among the 21 parasite stocks isolated from digestive tracts where mixed infections were initially detected (clonet 20 + 39) clonet 20 alone was detected in 81% of them. This result clearly showed that the culture step selected clonet 20 parasites over those belonging to clonet 39. The taxonomic status of the isolated stocks was also confirmed by isoenzyme typing, and correlation was observed between clustering topology and hybridization patterns with the probes 20 and 39.
