A paralisia de prega vocal (PPV) decorre da lesão do nervo vago ou de seus ramos, podendo levar a alterações das funções que requerem o fechamento glótico. O tempo máximo de fonação (TMF) é um teste aplicado rotineiramente em pacientes disfônicospara avaliar a eficiência glótica e freqüentemente utilizado em casos de PPV, cujos valores encontram-se diminuídos. A classificação clínica clássica da posição da prega vocal paralisada em mediana, para-mediana, intermediária e em abdução ou cadavérica tem sido objeto de controvérsias. OBJETIVO: Verificar a associação e correlação entre os TMF e posição da prega vocal paralisada (PVP), TMF e ângulo de afastamento da PVP, medir o ângulo de afastamento da linha média das diferentes posições da PVP e correlacioná-lo com a sua classificação clínica FORMA DE ESTUDO: Clínico retrospectivo. MATERIAL E MÉTODO: Foram revisados os prontuários e analisados os exames videoendoscópicos de 86 indivíduos com paralisia de prega vocal unilateral e medido o ângulo de afastamento da PVP por meio de um programa computadorizado. RESULTADOS: A associação e correlação entre os TMF em cada posição assumida pela PVP têm significância estatística somente para /z/ na posição mediana. A associação e correlação entre TMF com ângulo de afastamento da PVP guardam relação para /i/, /u/. Ao associar e correlacionar medidas de ângulo com posição observa-se significância estatística em posição de abdução. CONCLUSÕES: Neste estudo não foi possível determinar as posições assumidas pela PVP por meio dos TMF nem correlacioná-las com medidas do ângulo.

Verificar se os potenciais evocados miogênicos vestibulares podem apresentar anormalidades na orelha comprometida e na orelha assintomática em pacientes com hipótese diagnóstica de doença de Ménière definida unilateral. FORMA DE ESTUDO: Transversal coorte. MATERIAL E MÉTODO: Foram avaliados os potenciais evocados miogênicos vestibulares de 20 pacientes com doença de Ménière definida unilateral. A seleção dos indivíduos baseou-se na história e na avaliação clínica sugestivas de doença de Ménière definida unilateral, e eletrococleografia com anormalidades na orelha comprometida. Os potenciais evocados miogênicos vestibulares foram avaliados em ambas as orelhas de cada paciente por meio da latência absoluta de p13 e n23, diferença interaural da latência dos picos p13 e n23 e índice de assimetria da amplitude de p13-n23. RESULTADO: Os potenciais evocados miogênicos vestibulares estavam alterados em 35,0% das orelhas comprometidas e em 25,0% das orelhas assintomáticas. As alterações foram: ausência de resposta em sete casos, aumento da latência absoluta de p13 em três casos, e aumento do índice de assimetria da amplitude em um caso. CONCLUSÃO: Os potenciais evocados miogênicos vestibulares podem apresentar anormalidades nas orelhas comprometida e assintomática de pacientes com hipótese diagnóstica de doença de Ménière definida unilateral.

O schwannoma vestibular, também conhecido como neuroma ou neurinoma do acústico, é o tumor mais freqüente do ângulo pontocerebelar correspondendo a aproximadamente a 9% de todos os tumores intracranianos. APRESENTAÇÃO DO CASO: Os autores apresentam um relato de caso de surdez súbita e zumbido unilateral com melhora dos sintomas através de tratamento clínico com Prednisona e Pentoxifilina e posterior diagnóstico por exame de imagem de schwannoma vestibular. DISCUSSÃO: A surdez súbita pode ser descrita como uma perda neurossensorial abrupta e intensa. Costuma ser acima de 30 dB, em três ou mais freqüências contíguas e se desenvolve em menos de três dias. CONCLUSÃO: É de grande importância a pesquisa da etiologia nos casos de surdez súbita para a boa condução do caso e orientação terapêutica.

As intervenções humanitárias (Bósnia 1995-2003, Somália 1992-1993, Kosovo 1999-2003) e de substituição de regimes ditatoriais (Haiti 1994, Afeganistão 2001-2003, Iraque 2003) na política internacional têm sido objeto de muitas controvérsias acadêmicas durante a última década. O presente artigo, à luz dos teóricos das relações internacionais e da ciência política, analisa a legitimidade dessas atitudes por meio do unilateralismo norte-americano.

Aim: Visual acuity outcome of amblyopia treatment depends on the compliance. This study aimed to determine parental predictors of poor visual outcome with occlusion treatment in unilateral amblyopia and identify the relationship between occlusion recommendations and the patient's actual dose of occlusion reported by the parents. Methods: This study comprised three phases: refractive adaptation for a period of 18 weeks after spectacle correction; occlusion of 3 to 6 hours per day during a period of 6 months; questionnaire administration and completion by parents. Visual acuity as assessed using the Sheridan-Gardiner singles or Snellen acuity chart was used as a measure of visual outcome. Correlation analysis was used to describe the strength and direction of two variables: prescribed occlusion reported by the doctor and actual dose reported by parents. A logistic binary model was adjusted using the following variables: severity, vulnerability, self-efficacy, behaviour intentions, perceived efficacy and treatment barriers, parents' and childrens' age, and parents' level of education. Results: The study included 100 parents (mean age 38.9 years, SD approx 9.2) of 100 children (mean age 6.3 years, SD approx 2.4) with amblyopia. Twenty-eight percent of children had no improvement in visual acuity. The results showed a positive mild correlation (kappa = 0.54) between the prescribed occlusion and actual dose reported by parents. Three predictors for poor visual outcome with occlusion were identified: parents' level of education (OR = 9.28; 95%CI 1.32-65.41); treatment barriers (OR = 2.75; 95%CI 1.22-6.20); interaction between severity and vulnerability (OR = 3.64; 95%CI 1.21-10.93). Severity (OR = 0.07; 95%CI 0.00-0.72) and vulnerability (OR = 0.06; 95%CI 0.05-0.74) when considered in isolation were identified as protective factors. Conclusions: Parents frequently do not use the correct dosage of occlusion as recommended. Parents' educational level and awareness of treatment barriers were predictors of poor visual outcome. Lower levels of education represented a 9-times higher risk of having a poor visual outcome with occlusion treatment.

OBJETIVO: Analisar a sobrevida e os principais fatores prognósticos entre os pacientes com tumor de Wilms unilateral. MÉTODOS: A coorte de estudo incluiu 132 casos de tumor de Wilms unilateral em menores de 15 anos de idade matriculados em serviço de oncologia pediátrica, de janeiro de 1990 a dezembro de 2000. Curvas de sobrevida foram confeccionadas utilizando-se o método de Kaplan-Meier e fatores prognósticos foram analisados pelo modelo de riscos proporcionais de Cox. RESULTADOS: A estimativa de sobrevida global em cinco anos foi 84,6%. As probabilidades de sobrevida para os estádios I, II, III e IV foram de 100%; 94,2%; 83,2% e 31,3%, respectivamente. A taxa de sobrevida para os pacientes com: histologia favorável foi de 89,4%, para aqueles com anaplasia focal 66,7 % e com anaplasia difusa 40%. Todos os pacientes com doença em estádio IV e anaplasia difusa foram a óbito (n=4). Todos os pacientes com doença em estádio I, independente da histologia, permaneceram vivos até o final do período de seguimento. CONCLUSÕES: Entre as variáveis escolhidas para o modelo final apenas o estadiamento e a histologia permaneceram associados ao elevado risco de óbito enquanto que os casos na faixa etária entre 24 e 47 meses apresentaram melhor prognóstico que os demais. Esses resultados mostram a importância do diagnóstico em fases iniciais da doença e que a histologia é fundamental para orientar a terapia adequada.

A simple procedure to measure the cohesive laws of bonded joints under mode I loading using the double cantilever beam test is proposed. The method only requires recording the applied load–displacement data and measuring the crack opening displacement at its tip in the course of the experimental test. The strain energy release rate is obtained by a procedure involving the Timoshenko beam theory, the specimen’s compliance and the crack equivalent concept. Following the proposed approach the influence of the fracture process zone is taken into account which is fundamental for an accurate estimation of the failure process details. The cohesive law is obtained by differentiation of the strain energy release rate as a function of the crack opening displacement. The model was validated numerically considering three representative cohesive laws. Numerical simulations using finite element analysis including cohesive zone modeling were performed. The good agreement between the inputted and resulting laws for all the cases considered validates the model. An experimental confirmation was also performed by comparing the numerical and experimental load–displacement curves. The numerical load–displacement curves were obtained by adjusting typical cohesive laws to the ones measured experimentally following the proposed approach and using finite element analysis including cohesive zone modeling. Once again, good agreement was obtained in the comparisons thus demonstrating the good performance of the proposed methodology.

In order to correctly assess the biaxial fatigue material properties one must experimentally test different load conditions and stress levels. With the rise of new in-plane biaxial fatigue testing machines, using smaller and more efficient electrical motors, instead of the conventional hydraulic machines, it is necessary to reduce the specimen size and to ensure that the specimen geometry is appropriated for the load capacity installed. At the present time there are no standard specimen’s geometries and the indications on literature how to design an efficient test specimen are insufficient. The main goal of this paper is to present the methodology on how to obtain an optimal cruciform specimen geometry, with thickness reduction in the gauge area, appropriated for fatigue crack initiation, as a function of the base material sheet thickness used to build the specimen. The geometry is optimized for maximum stress using several parameters, ensuring that in the gauge area the stress is uniform and maximum with two limit phase shift loading conditions. Therefore the fatigue damage will always initiate on the center of the specimen, avoiding failure outside this region. Using the Renard Series of preferred numbers for the base material sheet thickness as a reference, the reaming geometry parameters are optimized using a derivative-free methodology, called direct multi search (DMS) method. The final optimal geometry as a function of the base material sheet thickness is proposed, as a guide line for cruciform specimens design, and as a possible contribution for a future standard on in-plane biaxial fatigue tests. © 2014, Gruppo Italiano Frattura. Relata-se o caso de um paciente vítima de envenenamento loxoscélico associado a paralisia diafragmática direita reversível. O diagnóstico de envenenamento loxoscélico baseou-se nas informações prestadas pelo paciente de que havia encontrado uma aranha marrom em sua cama no dia seguinte à picada e no quadro clínico típico deste tipo de envenenamento: lesão cutânea necrótica acompanhada de erupção escarlatiniforme e comprometimento sistêmico sob a forma de insuficiência renal aguda, distúrbios da coagulação sangüínea, hemólise intravascular e hemoglobinúria. Estas alterações regrediram completamente com o tratamento conservador. O diagnóstico da paralisia diafragmática baseou-se na elevação da hemicúpula diafragmática direita na radiografia de tórax em inspiração forçada e em sua completa imobilidade no exame radioscópico. A paralisia frênica não existia na radiografia realizada previamente ao acidente e desapareceu completamente trinta dias após o mesmo, o que permitiu associá-la à toxocidade do veneno loxoscélico ou a outras manifestações sistêmicas produzidas por ele.

The container loading problem (CLP) is a combinatorial optimization problem for the spatial arrangement of cargo inside containers so as to maximize the usage of space. The algorithms for this problem are of limited practical applicability if real-world constraints are not considered, one of the most important of which is deemed to be stability. This paper addresses static stability, as opposed to dynamic stability, looking at the stability of the cargo during container loading. This paper proposes two algorithms. The first is a static stability algorithm based on static mechanical equilibrium conditions that can be used as a stability evaluation function embedded in CLP algorithms (e.g. constructive heuristics, metaheuristics). The second proposed algorithm is a physical packing sequence algorithm that, given a container loading arrangement, generates the actual sequence by which each box is placed inside the container, considering static stability and loading operation efficiency constraints.

The Container Loading Problem (CLP) literature has traditionally evaluated the dynamic stability of cargo by applying two metrics to box arrangements: the mean number of boxes supporting the items excluding those placed directly on the floor (M1) and the percentage of boxes with insufficient lateral support (M2). However, these metrics, that aim to be proxies for cargo stability during transportation, fail to translate real-world cargo conditions of dynamic stability. In this paper two new performance indicators are proposed to evaluate the dynamic stability of cargo arrangements: the number of fallen boxes (NFB) and the number of boxes within the Damage Boundary Curve fragility test (NB_DBC). Using 1500 solutions for well-known problem instances found in the literature, these new performance indicators are evaluated using a physics simulation tool (StableCargo), replacing the real-world transportation by a truck with a simulation of the dynamic behaviour of container loading arrangements. Two new dynamic stability metrics that can be integrated within any container loading algorithm are also proposed. The metrics are analytical models of the proposed stability performance indicators, computed by multiple linear regression. Pearson’s r correlation coefficient was used as an evaluation parameter for the performance of the models. The extensive computational results show that the proposed metrics are better proxies for dynamic stability in the CLP than the previous widely used metrics.

PURPOSE: This study was designed to compare baseline data and clinical outcome between patients with prostate enlargement/benign prostatic hyperplasia (PE/BPH) who underwent unilateral and bilateral prostatic arterial embolization (PAE) for the relief of lower urinary tract symptoms (LUTS). METHODS: This single-center, ambispective cohort study compared 122 consecutive patients (mean age 66.7 years) with unilateral versus bilateral PAE from March 2009 to December 2011. Selective PAE was performed with 100- and 200-μm nonspherical polyvinyl alcohol (PVA) particles by a unilateral femoral approach. RESULTS: Bilateral PAE was performed in 103 (84.4 %) patients (group A). The remaining 19 (15.6 %) patients underwent unilateral PAE (group B). Mean follow-up time was 6.7 months in group A and 7.3 months in group B. Mean prostate volume, PSA, International prostate symptom score/quality of life (IPSS/QoL) and post-void residual volume (PVR) reduction, and peak flow rate (Qmax) improvement were 19.4 mL, 1.68 ng/mL, 11.8/2.0 points, 32.9 mL, and 3.9 mL/s in group A and 11.5 mL, 1.98 ng/mL, 8.9/1.4 points, 53.8 mL, and 4.58 mL/s in group B. Poor clinical outcome was observed in 24.3 % of patients from group A and 47.4 % from group B (p = 0.04). CONCLUSIONS: PAE is a safe and effective technique that can induce 48 % improvement in the IPSS score and a prostate volume reduction of 19 %, with good clinical outcome in up to 75 % of treated patients. Bilateral PAE seems to lead to better clinical results; however, up to 50 % of patients after unilateral PAE may have a good clinical outcome.
