To evaluate the effectiveness of Cognitive Behavioral Therapy (CBT) in the success of postoperative weight loss after 2 years of CB. METHODS: A prospective observational study was conducted in consecutive patients with morbid obesity aged between 18 and 59 yrs and enrolled in the bariatric surgery program of the Obesity Surgery Unit of our hospital from June 2007 through June 2010, with two years postoperative follow-up. Participants were divided into two groups according to their participation in Cognitive Behavioral Therapy or not. Over a 3-month period, CBT was applied in 12 2-h sessions. The main dependent variables studied were body weight and height, from which we calculated BMI and percentage of excess weight lost (weight lost x 100)/(initial weight-ideal weight), classifying patients as successful (E ) those with EPP > 50%, and unsuccessful (NE) those with EPP <50%. Participants were also, assessed for general (stress, anxiety, depression and self-esteem) and specific (binge eating and food craving) psychopathology. RESULTS: Of the 35 patients with bariatric surgery, 30 responded postoperative evaluations, 16 underwent CBT before CB and 14 underwent surgery without receiving psycho-nutritional therapy, (76% female) with a mean age of 41 ± 9.5 years. The mean baseline BMI was 42 ± 10 and 45% of patients were classified as super obese (BMI: 56 ± 6). Mean excess weight loss (EPP) was 77%. According to the EPP were classified as "successful" (S) (59%) and "unsuccessful" (U) (41%). Of the patients assigned to S, 94% received CBT (15 individuals of 17 total), compared with only 12% who did not receive (2 individuals of the 17 total) with statistically significant differences (p < 0.05). Also, the S patients appeared to be significantly less anxious and stressed and have higher self-esteem (P < 0.05). Regarding specific psychopathology, the food craving guided by hunger, loss of control over food intake and guilt was lower in patients who achieved > 50% of EPP (p < 0.04, p < 0.001, p < 0.001, respectively). This paper examines key aspects of Allan Gibbard's psychological account of moral activity. Inspired by evolutionary theory, Gibbard paints a naturalistic picture of morality mainly based on two specific types of emotion: guilt and anger. His sentimentalist and expressivist analysis is also based on a particular conception of rationality. I begin by introducing Gibbard's theory before testing some key assumptions underlying his system against recent empirical data and theories. The results cast doubt on some crucial aspects of Gibbard's philosophical theory, namely his reduction of morality to anger and guilt, and his theory of 'normative governance'. Gibbard's particular version of expressivism may be undermined by these doubts.

INTRODUCTION: Congenital hypogonadotropic hypogonadism (CHH) is a rare, genetic, reproductive endocrine disorder characterized by absent puberty and infertility. Limited information is available on the psychosocial impact of CHH and psychosexual development in these patients. AIM: The aim of this study was to determine the impact of CHH on psychosexual development in men on long-term treatment. METHODS: A sequential mixed methods explanatory design was used. First, an online survey (quantitative) was used to quantify the frequency of psychosexual problems among CHH men. Second, patient focus groups (qualitative) were conducted to explore survey findings in detail and develop a working model to guide potential nursing and interdisciplinary interventions. MAIN OUTCOME MEASURES: Patient characteristics, frequency of body shame, difficulty with intimate relationships, and never having been sexually active were assessed. Additionally, we collected subjective patient-reported outcomes regarding the impact of CHH on psychological/emotional well-being, intimate relationships, and sexual activity. RESULTS: A total of 101 CHH men on long-term treatment (>1 year) were included for the analysis of the online survey (mean age 37 ± 11 years, range 19-66, median 36). Half (52/101, 51%) of the men had been seen at a specialized academic center and 37/101 (37%) reported having had fertility-inducing treatment. A high percentage of CHH men experience psychosexual problems including difficulty with intimate relationships (70%) and body image concerns/body shame (94/101, 93%), and the percentage of men never having been sexually active is five times the rate in a reference group (26% vs. 5.4%, P < 0.001). Focus groups revealed persisting body shame and low self-esteem despite long-term treatment that has lasting impact on psychosexual functioning. CONCLUSIONS: CHH men frequently experience psychosexual problems that pose barriers to intimate relationships and initiating sexual activity. These lingering effects cause significant distress and are not ameliorated by long-term treatment. Sleep disorders are a common health problem in western countries. Every third working age person suffers from sleep deprivation and that often leads to other health problems as well. One can end up in a vicious circle, which can further decrease mood and ability to function. The aim of this thesis is to illustrate how sleep deprivation affects the lives of working age population and to deepen our understanding of life with sleep deprivation. Study questions are: how does sleep deprivation affect a working age person’s life and what kind of experiences do people have about cognitive-behavioural therapy as a treatment to sleep disorders. Theoretical perspective is based on clinical nursing science theories and the humanist view of man, which sees human as an entity. The methodology used is phenomenological approach and data analysis is conducted by using Ricœur’s hermeneutic phenomenological interpretation method. The empirical part is divided into two different sections. The material of the study consists of interviews and surveys done by people who have experienced sleep deprivation or sleep disorders. Two interviewees talked about their lives with sleep disorders and there are 21 surveys conducted on people’s experiences on cognitive-behavioural therapy. The partakers in the two sections are different people. The results show that people with sleep disorders can end up in a vicious circle of sleep deprivation and in worst cases a sleep disorder can take charge of a person’s whole life. Sleep disorder can cause shame and fear of stigma. Nevertheless, someone suffering from a sleep disorder can find strength and solutions to control the difficult situation. This study proves that both nursing staff and other people have little information about difficulties in sleeping and awareness should be improved in clinical nursing. A health-care provider has an essential role in preventing someone ending up in a vicious circle of sleep deprivation and cognitive-behavioural therapy can contribute to good health. Reflection at the end of cognitive-behavioural sleep therapy course helps patients to continue their learning process. When someone is sleep deprived, it means that they have control over the situation, but when someone has a sleep disorder, that person does not have the strength to control the situation.

Autism is one of those human ambiguities that forces vigilant open-mindednesssometimes this open-mindedness comes without choice, for example when you become the mother of a child with autism. Recent reports indicate that Pervasive Developmental Disorders affect 1 in 150 children (Centre for Disease Control and Prevention, 2007). This also means that there are many families caring for children with autism. The purpose of this research was to explore the day to day lived experiences of mothers caring for a child with autism. With a drastic increase in children diagnosed with autism, and very little research on mothers themselves, assisting in articulating lived experiences from mothers themselves seemed like an acceptable first step. Mothers were asked to journal for a period of one month, once a week, as well as participate in a focus group. Findings from both of these techniques were analyzed using underpinnings from Amelio Giorgi and Max van Manen. General findings indicate that mothers present poignant narratives about living with their child. It becomes clear that mothers are stressed, and live a complicated and often contradictory existence. Many days are fraught with struggle, anticipation, watchful eyes, judgment and guilt. There is a constant battle waging; the one within themselves, and the one with an uninformed and uncooperative public. Given that this research contributes to an extremely small body of qualitative research on mothers, future research should continue to gain insight from mothers, without classifying or categorizing their words. Their words speak volumes. Professionals may know autism, but mothers know their children.

Dans les centres d’hébergement et de soins de longue durée (CHSLD), jusqu’à 80 % des personnes admises vivent avec une démence (Conseil des aînés, 2007). Parmi ces personnes âgées, plusieurs crient. Ce comportement a fait l’objet de quelques études, mais ces études ne fournissent pas une compréhension des sens de ces cris qui pourrait orienter le choix d’interventions face à ces personnes et leur entourage. À l’aide de la perspective infirmière de Leininger (2001) et de la théorie de la communication et des interactions à l’intérieur de triades d’Adams et Gardiner (2005), le but de cette étude était de décrire les sens des cris de personnes âgées vivant avec une démence et les facteurs les influençant en tenant compte de la perspective des personnes âgées, de leurs aidants familiaux et de leurs soignants. Pour atteindre ce but, l’ethnographie critique a été choisie comme méthode. L’étude s’est découlée dans un CHSLD au sein duquel sept triades, composées de personnes âgées vivant avec une démence qui crient, d’aidants familiaux principaux et de soignants, ont été recrutées selon un échantillonnage théorique. Diverses méthodes de collecte des données ont été utilisées dont l’observation des personnes âgées et du milieu et des entrevues semi-dirigées auprès des aidants et soignants. L’analyse des données a été effectuée à l’aide des quatre approches proposées par Spradley (1979) soit l’analyse : des domaines, taxonomique, componentielle et thématique. Les résultats ont permis d’identifier des thèmes sur les sens des cris et sur les facteurs influençant ceux-ci. Les cris peuvent avoir pour sens la vulnérabilité, la souffrance et la perte de sens vécues par la personne âgée. Ce comportement peut aussi indiquer diverses finalités, par exemple l’expression d’insatisfactions ou d’émotions. Ces finalités peuvent être distinguées à partir de critères de modulation tels que la prévisibilité des cris ou un faciès émotif. Par ailleurs, divers aspects rendent les sens des cris singuliers. La stabilité et la flexibilité dans la façon d’organiser les soins ainsi que l’effet réciproque entre les personnes âgées qui crient et les autres personnes dans le CHSLD se sont dégagées comme étant des facteurs qui influencent les sens des cris. Il s’est aussi révélé que les cris de chaque personne âgée peuvent être considérés comme un langage unique que les aidants et les soignants sont en mesure d’apprendre et qui influence l’interprétation des sens des cris. L’accompagnement de la personne âgée, en tenant compte de ses volontés, ses besoins et de sa personnalité, les fluctuations dans les relations de pouvoir au sein des triades personnes âgées-aidants-soignants et les sentiments d’impuissance et de culpabilité des aidants et des soignants sont d’autres facteurs qui influencent les cris. Les connaissances découlant de cette étude augmentent la compréhension sur les sens des cris des personnes âgées vivant avec une démence. Elles ont des implications pour les divers champs d’activités des infirmières et qui pourront contribuer à offrir des soins culturellement cohérents et caring pour les personnes âgées vivant avec une démence et leur entourage.

Dans Creep show, un narrateur présente ses creeps, les malades de son entourage, des schizophrènes inadéquatement nommés, afin de les ramener à la vie par ses mots. En se souvenant de certains moments où la folie se manifestait à lui, il veut déterrer ses ensevelis, les faire parler en leur prêtant son écriture. Dans un récit morcelé pouvant évoquer une galerie de portraits en mouvement, les protagonistes sont présentés comme des monstres, des rêveurs ou des sources d’inspiration selon le moment relaté par un narrateur affecté qui se replonge littéralement dans un passé s’échelonnant entre l’enfance et l’âge de dix-huit ans. Portant autant sur la maladie mentale que sur la honte et la peur des mots, Creep show est un texte sur le silence et l’impuissance, sur l’incapacité de nommer adéquatement la folie ; il s’agit d’un court récit de dix-sept scènes encadrées par un prologue et un épilogue où l’écriture d’un traumatisme se vit comme une histoire d’amour. L’essai intitulé “Je est des autres.” De l’esthétique borderline chez Marie-Sissi Labrèche décrit la genèse d’une esthétique « borderline ». Dans une approche à la fois psychanalytique et narratologique, fondée sur les concepts de la mélancolie, du kitsch et de la crypte, l’analyse tente de montrer quel type de construction thématique et formelle soutient cette esthétique. À partir d’éléments représentatifs de l’univers de Marie-Sissi Labrèche (la question de la limite, la pulsion de mort, le rapport au corps et l’instabilité), l’essai s’intéresse à la façon dont la narratrice de Borderline (2000) donne à lire une identité sédimentaire, un autoportrait masqué-fêlé, où « Je est [des] autre[s] ». En regard de ces éléments, l’hypothèse d’une machine textuelle fonctionnant – thématiquement et formellement – dans et par l’instabilité et l’altérité oriente la réflexion vers l’idée d’une écriture du trauma qui pourrait représenter une tentative de réappropriation identitaire passant par l’esthétique dite « borderline ».

Este documento ofrece un análisis sociológico del papel que cumplía el honor como valor en la sociedad neogranadina a finales de la Colonia a partir de algunos conceptos clave que ofrecen autores clásicos de la disciplina. El honor se puede enmarcar dentro de lo que algunos autores han denominado solidaridades sociales o conciencia colectiva. Es decir, dentro del conjunto de creencias y valores compartidos por los integrantes de una misma sociedad. Dado que para la época estudiada la herencia española en torno al honor ya había logrado un alto nivel de arraigo, es posible pensar que el interés por proteger el honor hacía parte del sentimiento común en la sociedad neogranadina. Y es que el hecho de que la noción de honor estuviese anclada en los valores religiosos, que tanto la Iglesia como la Corona promovieron, condujo a que el fundamento del honor tuviese un carácter moral, logrando así una alta difusión dentro del territorio americano. La idea del honor determinaba, en buena medida, los parámetros del orden social deseado.

This paper explores the identities projected in advertisements directed towards HIV positive individuals and people with AIDS. Fifty such advertisements were collected from three popular American magazines for gay men over a period of seven months. Analysis of the ads reveals a paradoxical presentation of people with HIV/AIDS, which offers simultaneous conflicting images of hope and fear, power and weakness, innocence and guilt. An interactive sociolinguistic model through which this contradictory discourse might be understood is presented, drawing on Goffman’s insights on stigma management and the presentation of the self in social interaction. Advertisements directed towards people with HIV/AIDS, it is suggested, present a contradictory discourse in which the advertisers are positioned as ‘the wise’, offering to mediate the conflicting identities of the stigmatized. The identity values enacted in this contradictory discourse are further measured against American conceptions of communication and the self as observed by Carbaugh and others. The possible consequences of these positionings on the roles made available to people with HIV/AIDS in the wider social context are discussed.

Ett använt begrepp när det gäller tonårstjejer och sexualitet är sexuell agens, att handla efter egna beslut. Avsikten med denna studie är att nå en ökad förståelse av vad som påverkar utvecklingen av sexuell agens hos tonårstjejer, utifrån berättelser av två professioner vid ungdomsmottagningar. Syftet är också att göra en kritisk analys och en sociologisk teoretisering av upplevelsen av sexuell agens samt utifrån ett maktperspektiv utreda dess disciplinerande och frigörande potential. Tidigare forskning visar främst att det finns en avsaknad av lust och kunskap hos tonårstjejer samt att deras förmåga att ta självständiga beslut i relation till sexualitet är avhängt deras agens. I denna studie genomfördes kvalitativa intervjuer med barnmorskor och kuratorer vid ungdomsmottagningar. Tematisk analysmetod användes vid tolkningsförfarandet. Det empiriska materialet visade bland annat att tonårstjejers agens är influerat av yttre påverkan, gränssättning, skam, självförtroende och kunskap. För att nå en sociologisk förståelse av det empiriska materialet har sociologiska perspektiv på sexualitet, makt samt Foucaults teori om kroppens disciplinering använts. Baserat på professionerna vid ungdomsmottagningarna visade analysen att utvecklingen av sexuell agens gynnas av kunskap och självförtroende, hämmas av påverkan och skam samt både gynnas och hämmas av gränssättning. Den fördjupade analysen visade att sexuell agens kan skapa autonoma beslut, vilket minskar sårbarheten för maktens disciplin och därmed verkar frigörande inom disciplinens egendefinierade gränser. En avslutande slutsats var att upplevelsen av sexuell agens bör förstås som främst disciplinerande.

The present work proposes an investigation of the treatment given to memory in Pinter’s latest play, Ashes to Ashes, and of its function in the development of Pinter’s work. In order to do that, different aspects of the construction of meaning in the theatre are analysed, so that the specificity of its reception is determined. A survey of techniques used to present information, time and space in the theatre is made. The analytical drama, the history drama, and the theatre of the absurd are defined. After that, the evolution of the author’s work is analysed to determine what characterises Pinter’s work, while at the same time determining how his treatment of themes like menace, memory, and political oppression of the individual has evolved. Finally, a detailed survey of the apparently disconnected elements that are mentioned in Ashes to Ashes is made. The intertextual analysis allied to a study of the analytical form as used in this play enables the discovery of several layers of meaning. Through the connection established between the Holocaust and man’s fall followed by expulsion from Eden, Pinter examines the use of memory as a way of dealing with personal and collective responsibility and guilt. It is through the recovery of memory (also through writing) that the present can establish a critical and responsible relation with the past.

A vergonha é uma emoção autoconsciente e avaliadora que depende de uma série de cognições relacionadas ao self para ser experimentada. Apesar de amplamente estudada na psicologia, no marketing pouco se sabe sobre os antecedentes da vergonha e seu efeito no comportamento do consumidor, em especial no consumo de produtos falsificados. O objetivo geral desse estudo é entender a relação entre vergonha e comportamentos de consumo de produtos falsificados. Dois procedimentos experimentais, numa amostra de 129 pessoas com idade entre 18 e 50 anos, foram realizados para analisar seis hipóteses apresentadas. O primeiro experimento verificou (a) se o risco social percebido alto aumenta a vergonha, (b) se a qualidade da falsificação (alta/baixa) modera a relação entre risco social percebido e vergonha, caso esta relação exista. Os resultados confirmaram o efeito esperado do risco social percebido na vergonha, mas não confirmaram a moderação da qualidade da falsificação nessa relação. Mostraram ainda um efeito inesperado do gênero na vergonha: homens sentem mais vergonha que mulheres no contexto de consumo de falsificados. O segundo experimento verificou (a) se a antecipação da vergonha reduz a intenção de compra de produtos falsificados, (b) se a antecipação de vergonha influencia a atitude (favorável/desfavorável) em relação a produtos falsificados, e (c) se estas duas últimas relações, caso existam, são moderadas pelo custo-benefício do produto. Os resultados não confirmaram a influência da antecipação da vergonha na atitude em relação a falsificados e na intenção de compra de falsificados, mas mostraram que quanto menor o custo-benefício, maior o efeito da antecipação da vergonha na redução da intenção de produtos falsificados. Os resultados mostraram ainda um efeito moderador inesperado da renda na relação entre antecipação da vergonha e intenção de compra de falsificados. A antecipação da vergonha reduz a intenção de compras em consumidores com renda mais alta.
