Kidney transplantation is one of the most common transplantation operations in the world, accounting for up to 50 % of all transplantation surgeries. To curtail the damage to transplanted organs that is caused by ischemia-reperfusion injury and the recipient's immune system, small interfering RNA (siRNA) technology is being explored. Importantly, the kidney as a whole is a preferential site for non-specific systemic delivery of siRNA. To date, most attempts at siRNA-based therapy for transplantation-related conditions have remained at the in vitro stage, with only a few of them being advanced into animal models. Hydrodynamic intravenous injection of naked or carrier-bound siRNAs is currently the most common route for delivery of therapeutic constructs. To our knowledge, no systematic screens for siRNA targets most relevant for kidney transplantation have been attempted so far. A majority of researchers have arrived at one or another target of interest by analyzing current literature that dissects pathological processes taking place in transplanted organs. A majority of the genes that make up the list of 53 siRNA targets that have been tested in transplantation-related models so far belong to either apoptosis- or immune rejection-centered networks. There is an opportunity for therapeutic siRNA combinations that may be delivered within the same delivery vector or injected at the same time and, by targeting more than one pathway, or by hitting the same pathways within two different key points, will augment the effects of each other.

The electronic storage of medical patient data is becoming a daily experience in most of the practices and hospitals worldwide. However, much of the data available is in free-form text, a convenient way of expressing concepts and events, but especially challenging if one wants to perform automatic searches, summarization or statistical analysis. Information Extraction can relieve some of these problems by offering a semantically informed interpretation and abstraction of the texts. MedInX, the Medical Information eXtraction system presented in this document, is the first information extraction system developed to process textual clinical discharge records written in Portuguese. The main goal of the system is to improve access to the information locked up in unstructured text, and, consequently, the efficiency of the health care process, by allowing faster and reliable access to quality information on health, for both patient and health professionals. MedInX components are based on Natural Language Processing principles, and provide several mechanisms to read, process and utilize external resources, such as terminologies and ontologies, in the process of automatic mapping of free text reports onto a structured representation. However, the flexible and scalable architecture of the system, also allowed its application to the task of Named Entity Recognition on a shared evaluation contest focused on Portuguese general domain free-form texts. The evaluation of the system on a set of authentic hospital discharge letters indicates that the system performs with 95% F-measure, on the task of entity recognition, and 95% precision on the task of relation extraction. Example applications, demonstrating the use of MedInX capabilities in real applications in the hospital setting, are also presented in this document. These applications were designed to answer common clinical problems related with the automatic coding of diagnoses and other health-related conditions described in the documents, according to the international classification systems ICD-9-CM and ICF. The automatic review of the content and completeness of the documents is an example of another developed application, denominated MedInX Clinical Audit system.

Une étude récente a démontré qu’une rétroaction orientée vers le changement (ROC) qui est empathique, accompagnée de choix de solutions et de trucs, basée sur des objectifs clairs et atteignables, exempte d’énoncés portant sur la personne et donnée sur un ton respectueux, contribue à soutenir l’autonomie des athlètes (Carpentier & Mageau, 2013). À travers trois articles scientifiques, cette thèse vise à (1) démontrer qu’une telle ROC, offerte en situation réelle d’entrainement, soutient l’autonomie, (2) mieux comprendre ses impacts et (3) explorer ses déterminants. Alors que les études précédentes ont utilisé les perceptions des athlètes afin de définir une ROC soutenant l’autonomie et d’évaluer ses conséquences, l’article 1 utilise le codage de vidéos. La qualité et la quantité de ROC donnée par 53 entraineurs ont été codées et leurs 310 athlètes ont rapporté leurs perceptions d’autonomie. Les résultats soutiennent les résultats d’études passées en démontrant qu’une ROC caractérisée par les six dimensions proposées par Carpentier et Mageau (2013) favorise l’autonomie. À l’opposé, la quantité de ROC est négativement liée à l’autonomie. L’article 2 vise à mieux comprendre les impacts de la ROC en s’intéressant aux variations intra-individuelles. Quarante-neuf athlètes ont complété, immédiatement après 15 entrainements (N = 534), un questionnaire évaluant la ROC reçue au cours de l’entrainement et diverses conséquences situationnelles. Des analyses multiniveaux démontrent que la qualité de la ROC est positivement liée à la qualité de l’expérience de l’athlète. À l’opposé, la quantité de ROC est négativement liée à la motivation autonome et au besoin de compétence. Finalement, l’article 3 s’intéresse à la passion des entraineurs ainsi qu’à leur perception de la motivation de leurs athlètes en tant que déterminants de la qualité et de la quantité de ROC donnée. Au total, 280 athlètes et 48 entraineurs ont rempli un questionnaire. Les résultats démontrent que plus les entraineurs ont une passion obsessive, moins leur ROC soutient l’autonomie des athlètes. Aussi, plus les entraineurs ont une passion obsessive et plus ils perçoivent leurs athlètes comme étant motivés, plus ils donnent de la ROC. Cette thèse contribue à l’avancement des connaissances sur la ROC, à la théorie de l’autodétermination, et à la formation des entraineurs.

In der vorliegenden Arbeit wurde untersucht, wie Führung zur Bewältigung von Unternehmenskrisen und Umsetzung von Veränderungen im Kontext des Interim Management beitragen kann. Dazu wurde die Theorie transformationaler Führung um aufgabenbezogene und indirekte Führung und den Einsatz von Positionsmacht ergänzt. Damit wurden diese erfolgskritischen Führungsdimensionen erstmals in einem gemeinsamen theoretischen Rahmen verbunden und die Auswirkungen einer kombinierten Anwendung untersucht. Dieser neue Ansatz wurde als „Total Range of Leadership“ bezeichnet. In der vorliegenden Untersuchung konnten neue Erkenntnisse zum Kontext und den Erfolgsfaktoren des Interim Management sowie zu den Auswirkungen und Wirkungsmechanismen transformationaler Führung und des Total Range of Leadership gewonnen werden. Die Untersuchungsergebnisse deuten darauf hin, dass Interim Management ein hoch effektives Veränderungsinstrument ist. Als erfolgsrelevante Rahmenbedingungen wurden die Ausstattung der Interim Manager mit umfassenden Vollmachten sowie die Unterstützung durch die Auftraggeber, als wichtigster Erfolgsfaktor des Interim Management aber eindeutig die Anwendung transformationaler Führung und des Total Range of Leadership identifiziert. Die Anwendung transformationaler Führung im Rahmen des Total Range of Leadership wirkt sich positiv auf Persönlichkeit, Einstellungen, Kompetenz und Verhalten der Mitarbeiter sowie Organisationsstruktur, Unternehmenskultur, organisationale Lernfähigkeit und finanziellen und marktbezogenen Erfolg von Unternehmens aus. Dabei erzielt transformationale Führung die bei Weitem größte Wirkung aller Führungsdimensionen. Es konnte aufgezeigt werden, dass transformationale Führung diese Wirkung nur in Verbindung mit der kompetenten Anwendung von transaktionaler, aufgabenbezogener und indirekter Führung entfalten kann. Die Untersuchungsergebnisse deuten außerdem darauf hin, dass eine transformationale Führungsbeziehung nur dann entstehen kann, wenn die Mitarbeiter Vertrauen in die professionelle Kompetenz und die Integrität ihrer Führungskraft fassen. Als normative Grundlagen transformationaler Führung wurden die Gewährleistung der Wohlfahrt und des Nutzens der Geführten sowie der Bezug auf und die Berücksichtigung von geteilten und allgemein akzeptierte Werten, insbesondere der distributiven und prozeduralen Gerechtigkeit, identifiziert. Insgesamt deuten die Erkenntnisse der Untersuchung darauf hin, dass mit dem Total Range of Leadership ein ganzheitlicher Führungsansatz entwickelt wurde, der alle aktiven und positiven Führungsverhaltensweisen umfasst, die zur Bewältigung von Krisen und Durchführung von Veränderungen in Unternehmen sowie für effektive Führung überhaupt erforderlich sind.

Chemostat culture was used to determine the effects of the antimicrobial agents tetracycline and nystatin on predominant components of the human gut microflora. Their addition to mixed culture systems caused a non-specific, and variable, decrease in microbial populations, although tetracycline allowed an increase in numbers of yeasts. Both had a profound inhibitory effect upon populations seen as important for gut health (bifidobacteria, lactobacilli). However, a tetracycline resistant Lactobacillus was enriched from the experiments. A combination of genotypic and phenotypic characterisations confirmed its identity as Lactobacillus plantarum. This strain exerted powerful inhibitory effects against Candida albicans. Because of its ability to resist the effects of tetracycline, this organism may be useful as a probiotic for the improved management of yeast related conditions such as thrush and irritable bowel syndrome. The aim of using GPS for Alzheimer's Patients is to give carers and families of those affected by Alzheimer's Disease, as well as all the other dementia related conditions, a service that can, via SMS text message, notify them should their loved one leave their home. Through a custom website, it enables the carer to remotely manage a contour boundary that is specifically assigned to the patient as well as the telephone numbers of the carers. The technique makes liberal use of such as Google Maps.

Economic theory makes no predictions about social factors affecting decisions under risk. We examine situations in which a decision maker decides for herself and another person under conditions of payoff equality, and compare them to individual decisions. By estimating a structural model, we find that responsibility leaves utility curvature unaffected, but accentuates the subjective distortion of very small and very large probabilities for both gains and losses. We also find that responsibility reduces loss aversion, but that these results only obtain under some specific definitions of the latter. These results serve to generalize and reconcile some of the still largely contradictory findings in the literature. They also have implications for financial agency, which we discuss.

This quantitative survey aims to highlight the impact of the meeting with the grief for customer advisers and representatives of funeral homes connected to the SBF (Sveriges Begravningsbyråers förbund). It also aims to demonstrate any differences between the sexes on this subject. Issues include: How does the person experience expectations and requirements in relation to their professional role? Does the person experience a personal impact due to the encounter with grief and death in their profession, in what way and to what extent? What consequences does the expectations, reactions and the possible impact result in for that person? What conditions does the person have to help cope with the professional role? Is there a gender difference in this experience? The theory base, which consists of theories taken from The Managed Heart: The commercialization of Human Feeling by Arlie Hochschild (2003) and descriptions taken from Yrke: begravningsentreprenör. Om utanförskap, döda kroppar, riter och professionalisering by Anna Davidsson Bremborg (2002), has been fundamental in both the preparatory work and the analysis. The population consists of those with client contact at funeral homes connected to the SBF. The questionnaire was answered through an internet page and the contact with the particiants were made via e-mail. The questionnaire consisted of 44 claims which were answered by how well the respondents agreed with them. The responses were then coded and calculations were made of mean and significance of these figures. Comparisons were made on several levels between the different variables in order to find patterns and indications. The results showed trends that the respondents to some extent experienced high demands in their profession, stress connected with the customer interactions and that this to some extent resulted in negative consequences. The results suggests that respondents were expected to and strove to go the "extra mile" to please the customers. The results also showed trends that the respondents enjoyed their work, experienced relatively good conditions in their profession and that the benefits outweighed the sacrifices. The results also showed an interesting pattern of that women, more than men, experience pressures and stress due to the encouter with grief at work, and that the higher education the respondents had the greater the impact. This study resulted in some interesting indications that can be used in further research.
