The purpose of this study is to examine the prevalence of drug abuse among welfare recipients in Houston, TX and compare the work activities and employment barriers of drug abusers in order to better understand the potential effects of welfare reform for this population. Four hypotheses were tested comparing the work activities and employment barriers of drug abusers to others on welfare and the relative importance of drug abuse and employment barriers in predicting work activity. ^ This cross-sectional study examined the characteristics and work activities of 447 welfare recipients (81 drug abusers and 366 non-abusers) who were surveyed between October 1998 and April 1999 in Houston, TX. Subjects were introduced and recruited to participate in the study through a flyer, door to door visits, and peer driven recruitment/referral. ^ About 18% were found to be drug abusers, which is consistent with the national average (10–33%) among welfare recipients. Compared to others on welfare, drug abusers were less involved in work activities, and had more employment barriers. Employment barriers were found to be more predictive of welfare to work activities than drug abuse. The results suggest that alleviating employment barriers should be stressed in programs aimed at welfare recipients with drug abuse problems. ^

The purpose of this thesis is to identify "best practice" recommendations for successful implementation of the EPSDT outreach program at Memorial Health System's Hospital for Children in Colorado Springs through a policy analysis of Medicaid EPSDT services in Colorado. A successful program at Memorial will increase education and awareness of EPSDT services, enrollment, and access to and utilization of health care services for eligible children. Methodology utilized in this study included questionnaires designed for the EPSDT contract administrator and outreach coordinators/workers; analysis of current federal and state policies; and studies conducted at the federal and state level, and by various advocacy groups. The need for this analysis of EPSDT came about in part through an awareness of increasingly high numbers of children in poverty and who are uninsured. Though the percentage of children living in poverty in Colorado is slightly below the national average (see Table 2), according to data analyzed by The Annie E. Casey Foundation, the percentage of children (0-18) living in poverty in Colorado increased from 10% in 2000 to 16% in 2006, a dramatic increase of 60% surpassed by only one other state in the nation (The Annie E. Casey Foundation, 2008). By comparison, the U.S. percentage of children in poverty during the same time frame rose from 17% to 18% (The Annie E. Casey Foundation, 2008). What kind of health care services are available to this vulnerable and growing group of Coloradans, and what are the barriers that affect their enrollment in, access to and utilization of these health care services? Barriers identified included difficulty with the application process; system and process issues; a lack of providers; and a lack of awareness and knowledge of EPSDT. Fiscal restraints and legislation at the federal and state level are also barriers to increasing enrollment and access to services. Outreach services are a critical component of providing EPSDT services, and there were several recommendations regarding outreach and case management that will benefit the program in the future. Through this analysis and identification of a broad range of barriers, a clearer picture emerged of current challenges within the EPSDT program as well as a broad range of strategies and recommendations to address these challenges. Through increased education and advocacy for EPSDT and the services it encompasses; stronger collaboration and cooperation between all groups involved, including providing a Medical Home for all eligible children; and new legislation putting more money and focus on comprehensive health care for low-income uninsured children; enrollment, access to and utilization of developmentally appropriate and quality health care services can be achieved. ^

Screening for latent tuberculosis infection (LTBI) is an integral component of an effective tuberculosis control strategy, but one that is often relegated to the lowest priority. In a state with higher than national average rates of tuberculosis, due consideration should be given to LTBI screening. Recent large scale contact investigations in the middle school of Del Rio, Texas, raised questions about the status of school screening for LTBI. An evidence based approach was used to evaluate school screening in high risk areas of Texas. A review of the literature revealed that the current recommendations for LTBI screening in children is based on administration of a risk factor questionnaire that should be based on the four main risk factors for LTBI in children that have been identified. Six representative areas in Texas were identified for evaluation of the occurrence of contact investigations in schools for the period of 2006 to 2009 and any use of school screening programs. Of the five reporting areas that responded, only one utilized a school screening program; this reporting area had the lowest percentage of contact investigations occurring in schools. Contact investigations were most common in middle schools and least common in elementary schools. In metropolitan areas, colleges represented up to 42.9% of contact investigations. The number of contact investigations has increased from 2006 to 2008. This report represents a small sample, and further research into the frequency, distribution and risk for contact investigations in schools and the efficacy of screening programs should be done. ^

Since the tragic events of September, 11 2001 the United States bioterrorism and disaster preparedness has made significant progress; yet, numerous research studies of nationwide hospital emergency response have found alarming shortcomings in surge capacity and training level of health care personnel in responding to bioterrorism incidents. The primary goals of this research were to assess hospital preparedness towards the threat of bioterrorist agents in the Southwest Region of the United States and provide recommendations for its improvement. Since little formal research has been published on the hospital preparedness of Oklahoma, Arizona, Texas and New Mexico, this research study specifically focused on the measurable factors affecting the respective states' resources and level of preparedness, such as funding, surge capacity and preparedness certification status.^ Over 300 citations of peer-reviewed articles and 17 Web sites were reviewed, of which 57 reports met inclusion criteria. The results of the systematic review highlighted key gaps in the existing literature and the key targets for future research, as well as identified strengths and weaknesses of the hospital preparedness in the Southwest states compared to the national average. ^ Based on the conducted research, currently, the Southwest states hospital systems are unable fully meet presidential preparedness mandates for emergency and disaster care: the staffed beds to 1,000 population value fluctuated around 1,5 across the states; funding for the hospital preparedness lags behind hospital costs by millions of dollars; and public health-hospital partnership in bioterrorism preparedness is quite weak as evident in lack of joint exercises and training. However, significant steps towards it are being made, including on-going hospital preparedness certification by the Joint Commission of Health Organization. Variations in preparedness levels among states signify that geographic location might determine a hospital level of bioterrorism preparedness as well, tending to favor bigger states such as Texas.^ Suggested recommendations on improvement of the hospital bioterrorism preparedness are consistent with the existing literature and include establishment and maintenance of solid partnerships between hospitals and public health agencies, conduction of joint exercises and drills for the health care personnel and key partners, improved state and federal funding specific to bioterrorism preparedness objectives, as well as on-going training of the clinical personnel on recognition of the bioterrorism agents.^

Ad-hoc population dynamics in Krugman’s type core and periphery models adjust population share of a region, based on its real wage rate deviation from national average, at pre-specified speed of population mobility. Whereas speed of population mobility is expected to be different across countries, for geographical, cultural, technological, etc. reasons, one common speed is often applied in theoretical and simulation analysis, due to spatially patchy, and temporally infrequent, availability of sub-national regional data. This article demonstrates how, increasingly available, high definition spatio-temporal remote-sensing data, and their by-products, can be used to measure speed of population mobility in national and sub-national level.

As intervenções de saneamento básico, ou seja, as obras para implantação ou ampliação de sistemas de abastecimento de água e de esgotamento sanitário têm sido relacionadas à redução das taxas de mortalidade infantil ocorrida no estado de São Paulo nas últimas décadas. De fato, a relação entre saneamento básico e saúde pública é antiga, principalmente no que se refere ao controle das doenças de veiculação hídrica. Entretanto, ela se mostra muito fraca, ou até nula, para regiões onde os índices de cobertura por redes de água e esgotos já se encontram em patamares acima da média nacional, como é o caso de grande parte dos municípios do estado de São Paulo. Uma abordagem do saneamento básico, sob o ponto de vista da complexidade de suas relações, considerando aspectos multidimensionais, faz-se necessária como forma de repensar o setor de forma mais ampla, fugindo do senso comum. O objetivo desta pesquisa foi identificar, analisar e descrever indicadores de sustentabilidade que pudessem estabelecer uma nova base para avaliação de efeitos de intervenções de saneamento básico, melhorando a determinação e acompanhamento dos impactos decorrentes, constituindo, então, uma matriz de indicadores específicos para a área, a partir da investigação dos conceitos de sustentabilidade e seus desdobramentos para o setor saneamento básico e o ambiente empresarial. Pesquisas em bases de dados públicas permitiram a identificação e seleção de indicadores multidimensionais para o saneamento, assim como a análise de correlação entre os indicadores selecionados possibilitou identificar possíveis benefícios de curto, médio e longo prazos, oriundos das intervenções de saneamento para os municípios do Vale do Ribeira. Utilizando modelos de organização de indicadores, como o Triple Bottom Line (TBL) e a matriz de indicadores FPSEEA, da Organização Mundial da Saúde (OMS), com algumas adaptações, foi possível agrupar os indicadores de forma a permitir uma visão integrada das dimensões da sustentabilidade e, dessa maneira, determinar que as intervenções de saneamento básico possuem relações sistêmicas com indicadores sociais, ambientais e econômicos, que possibilitem, a partir de sua execução, o estabelecimento de um círculo virtuoso para os municípios

Fragmentation of wildlife habitat is a primary driver of global species decline. A major contributor to habitat fragmentation in the United States is rural residential development. Rural development in Colorado is occurring at rates far greater than the national average. Additionally, the lack of state-level planning control coupled with a lack of comprehensive, effective planning tools at the local level creates conditions that contribute to habitat fragmentation in many rural counties. Greater oversight and involvement in land use planning is needed by the state level to assist county governments. This study provides five recommendations to strengthen Colorado state land use policy in order to reduce habitat fragmentation.

En este trabajo se analizan los cambios en el patrón de las migraciones interiores españolas a lo largo del período 1960-1989. De forma preliminar y de acuerdo con las teorías explicativas de los flujos migratorios, se contrasta econométricamente la consistencia de estos cambios con la existencia de sistemas regionales económicamente desequilibrados versus sistemas regionales con diferencias compensadoras. Como resultado, se encuentra que a pesar de la reciente intensificación de las migraciones interiores y a diferencia de lo sucedido en otras etapas y bajo otras circunstancias, ahora no se debería confiar en que los movimientos migratorios puedan contribuir al acercamiento a la media nacional de las regiones con tasas de desempleo más elevadas. Esta vía podría haberse agotado en la medida en que el peso de las barreras procedentes de los mercados de trabajo y de los mercados inmobiliarios es creciente y, paradójicamente, ha generado flujos inte-rregionales equilibrados y no polarizados. Si, además, la posibilidad de que la emigración cumpla un papel dinamizador del ajuste económico se ve afectada por el componente de desempleo de los trabajadores que se desplazan, entonces las políticas públicas no deberían centrarse en la incentivación de los flujos de forma indiscriminada sino, más bien, en los aspectos microeconómicos referentes a la disponibilidad de información, la orientación y la capacitación de los potenciales trabajadores emigrantes.

Este estudo objetivou identificar e descrever uma possível relação entre suporte social e qualidade de vida, em pessoas idosas participantes de um programa de educação permanente, o qual é promovido por órgão público em parceria com uma universidade, instalada em um dos municípios que integram a região do ABC Paulista, em São Paulo. A amostra consistiu de 106 idosos, com idade a partir de 60 anos, de ambos os sexos, e que efetivamente participam do referido programa. Para a coleta de dados, a amostra foi dividida em dois subgrupos, onde o primeiro grupo constou de 54 idosos que freqüentam o programa há menos de 1 ano, e o outro grupo foi formado por 52 idosos que freqüentam o programa há mais de 1 ano, com isso objetivamos identificar uma possível existência de diferentes percepções entre os grupos, das variáveis estudadas, em função do tempo de participação no programa, o que não foi confirmado, com os resultados apontando para uma homogeneidade entre os grupos, tanto nos aspectos socioeconômicos quanto nas percepções de suporte social e qualidade de vida, independente do tempo de participação no programa. Este estudo utilizou método descritivo exploratório, de caráter quantitativo e comparativo. Para a coleta de dados foram utilizadas a Escala de Percepção de Suporte Social (EPSS), que avalia percepção de suporte social em suas dimensões emocional e prático; o instrumento de avaliação da qualidade de vida: WHOQOL Bref e Old, e um questionário com dados socioeconômicos que auxiliaram na caracterização do perfil da amostra e na análise estatística dos resultados. Os resultados apontaram que a amostra pesquisada caracteriza-se por possuir um perfil socioeconômico diferenciado, no que se refere a uma maior escolaridade e rendimento mensal, quando comparado a media nacional que mostra o perfil do idoso brasileiro mais vulnerável, com baixa escolaridade e rendimento. Os resultados das avaliações das percepções de suporte social e da qualidade de vida demonstraram tratar-se de idosos que se sentem satisfeitos com seu momento de vida; que percebem apoio emocional, sentindo-se objeto de afeto em sua rede social. Com relação a percepção de suporte prático, os resultados demonstraram que os idosos possuem uma percepção relativa, apontando dúvidas e incertezas quanto ao recebimento deste tipo de apoio de sua rede social. Diante deste resultado, percepção de dúvidas e incertezas em receber suporte prático, e a característica socioeconômica diferenciada da amostra, podemos supor que esses idosos possuem estas percepções, por se sentirem ou por serem de fato provedores e não dependentes da sua rede social. Não foi evidenciada correlação entre as variáveis suporte social e qualidade de vida, sugerindo que o construto suporte social talvez seja percebido pelos idosos da amostra como fator de diminuição da funcionalidade biopsicossocial ou da competência comportamental; ou ainda, pode-se supor que diante dos sentimentos de satisfação com a vida atual, a amostra de idosos volta-se menos aos aspectos protetores do suporte social.
