Background: Postnatal breastfeeding support in the form of home visits is difficult to accommodate in regional Australia, where hospitals often deal with harsh economic constraints in a context where they are required to provide services to geographically, dispersed consumers. This study evaluated a predominately telephone-based support service called the Infant Feeding Support Service. Methods: A prospective cohort design was used to compare data for 696 women giving birth in two regional hospitals (one public, one private) and participating in the support service between January and July 2003 with data from a cohort of 625 women who gave birth in those hospitals before the introduction of the support service. Each mother participating in the support service was assigned a lactation consultant. First contact occurred 48 hours after discharge, and approximately it weekly thereafter for 4 it weeks. Breastfeeding duration was measured at 3 months postpartum. Results: For women from the private hospital, the support service improved exclusive breastfeeding duration to 4.5 weeks postpartum, but these improvements were not evident at 3 months postpartum. No effects were observed for mothers from the public hospital. Quantitative and qualitative data demonstrated high levels of client satisfaction with the support service. Conclusions: This small-scale, predominately telephone-based intervention provided significant, although apparently context-sensitive, improvements to exclusive breastfeeding duration.

Drastic improvements in styrene yield and selectivity were achieved in the oxidative dehydrogenation of ethylbenzene by staged feeding of O2. Six isothermal packed bed reactors were used in series with intermediate feeding of O2, while all EB was fed to the first reactor, diluted with helium or CO2 (1:5 molar ratio), resulting in total O2:EB molar feed ratios of 0.2-0.6. The two catalyst samples, γ-Al 2O3 and 5P/SiO2, that were applied both benefitted from this operation mode. The ethylbenzene conversion per stage and the selectivity to styrene were significantly improved. The production of COX was effectively reduced, while the selectivity to other side products remained unchanged. Compared with co-feeding at a total O 2:EB molar feed ratio of 0.6, by staged feeding the EB conversion (+15% points for both catalysts), ST selectivity (+4% points for both samples) and O2 (ST) selectivity (+9% points for γ-Al2O 3 and +17% points for 5P/SiO2) all improved. The ethylbenzene conversion over 5P/SiO2 can be increased from 18% to 70% by increasing the number of reactors from 1 to 6 with each reactor a total amount of O2 of 0.1 without the loss of ST selectivity (93%). For 5P/SiO2 a higher temperature (500 C vs. 450 C for Al 2O3) is required. Essentially more catalyst (5P/SiO 2) was required to achieve full O2 conversion in each reactor. Staged feeding of O2 does not eliminate the existing issues of the catalyst stability both in time-on stream and as a function of the number of catalyst regenerations (5P/SiO2), or the relatively moderate performance (relatively low styrene selectivity for γ-Al2O 3). © 2014 Elsevier B.V.

Introduction: The nutrition of very low birth weight (VLBW) infants is aimed at promoting a similar growth to that occurring in the uterus. However, in practice this is difficult to achieve and extrauterine growth restriction is frequent. The current tendency is to avoid this restriction by means of early parenteral and enteral nutrition. Nonetheless, uncertainty about many of the practices related with nutrition has resulted in a great variation in the way it is undertaken. In 2009 and 2011 in our hospital there was an unexpected increase in necrotizing enterocolitis. To check to see whether our nutrition policy was involved, we undertook a systematic review and drew up clinical practice guidelines (CPG) about enteral feeding in VLBW infants. New considerations about the duration of the fortification and the use of probiotics have led to an update of these CPG. Methods: A total of 21 clinical questions were designed dealing with the type of milk, starting age, mode of administration, rate and volume of the increments, fortification, use of probiotics and protocol. After conducting a systematic search of the available evidence, the information was contrasted and summarized in order to draw up the recommendations. The quality of the evidence and the strength of the recommendations were determined from the SIGN scale Comment: These CPG aim to help physicians in their decision making. The protocolized application of well-proven measurements reduces the variation in clinical practice and improves results.

Broad nosed caiman are ectotherm sauropsids that naturally experience long fasting intervals. We have studied the postprandial responses by measuring oxygen consumption using respirometry, the size changes of the duodenum, the distal small intestine, and the liver, using repeated non-invasive ultrasonography, and by investigating structural changes on the level of tissues and cells by using light- and electron microscopy. The caimans showed the same rapid and reversible changes of organ size and identical histological features, down to the ultrastructure level, as previously described for other ectothermic sauropsids. We found a configuration change of the mucosa epithelium from pseudostratified during fasting to single layered during digestion, in association with hypertrophy of enterocytes by loading them with lipid droplets. Similar patterns were also found for the hepatocytes of the liver. By placing the results of our study in comparative relationship and by utilizing the phylogenetic bracket of crocodiles, birds and squamates, we suggest that the observed features are plesiomorphic characters of sauropsids. By extending the comparison to anurans, we suggest that morphological and physiological adjustments to feeding and fasting described here may have been a character of early tetrapods. In conclusion, we suggest that the ability to tolerate long fasting intervals and then swallow a single large meal as described for many sit-an-wait foraging sauropsids is a functional feature that was already present in ancestral tetrapods.

Objective: There are indications that the prevalence of exclusively breastfed preterm infants is decreasing in Sweden. The objective was to investigate trends in exclusive breast feeding at discharge from Swedish neonatal units and associated factors in preterm infants. Design, setting and participants: This is a register study with data from the Swedish Neonatal Quality Register. Data from 29 445 preterm infants (gestational age (GA) <37 weeks) who were born during the period 2004–2013 were retrieved. Data included maternal, perinatal and neonatal characteristics. Data were analysed for the whole population as well as for 3 GA groups. Results: From 2004 to 2013, the prevalence of exclusive breast feeding decreased, in extremely preterm (GA 22–27 weeks) from 55% to 16%, in very preterm (GA 28–31 weeks) from 41% to 34% and in moderately preterm infants (GA 32–36 weeks) from 64% to 49%. The decline was statistically significant (p<0.001) in all 3 GA groups. This decline remained significant when adjustments were made for factors negatively associated with exclusive breast feeding and which became more prevalent during the study period, that is, small for GA (all groups) and maternal mental illness (very preterm and moderately preterm infants). Conclusions: In the past 10 years, Sweden has experienced a lower rate of exclusive breast feeding in preterm infants, especially in extremely preterm infants. The factors analysed in this study explain only a small proportion of this decline. The decline in exclusive breast feeding at discharge from neonatal units raises concern and present challenges to the units to support and promote breast feeding.

Gastric evacuation rates of the gilthead sea bream, Sparus aurata, fed with commercial pelleted food and polychaetes (Nereis diversicolor) were determined under experimental conditions. The estimated gastric evacuation rate for pelleted food was 7.97% h(-1), with a total time of digestion of approximately 9 h. The respective values for the natural food were 6.24% h(-1), with a total digestion time of approximately 12 h. The daily consumption of fish reared in earth ponds in a semi-intensive aquaculture facility was estimated through 24h cycles performed between April and August. The daily consumption varied from 18.58 to 31.98 mg g(-1). There was a constant increase in the average daily consumption per individual of 1.8-4.6 g (dry weight). During these cycles, samples of stomachs were taken and the contents preserved for further observation. The feeding behaviour of the reared fish was compared with a fish sample caught in the Ria Formosa lagoon. No common species were found between samples. A total of 38 prey were identified, which suggests that the gilthead sea bream is a non-specific predator. Despite the high abundance of natural prey in the ponds, the dependence of sea bream on pelleted food was high.

Penelitian ini bertujuan untuk menyelidiki pengaruh suplementasi ransum dasar (BD, ransum kontrol yang disusun menurut NRC, 1988) dengan asam amino esensial lysine, methionin, threonine (SD1 dan SD2, ransum perlakuan) terhadap konsumsi ransum dan kinerja pada ternak babi. Bahan utama ketiga ransum terdiri dari tepung barley, gandum, dan kedele. Masing-masing ransum mengandung kadar protein kasar (18% untuk grower dan 16,5% untuk finisher) dan enerji (14,2 MJ/kg). Ransum diberikan dengan cara dipecah (splitted) menurut periode pertumbuhan (grower dan finisher) dan jenis kelamin (jantan dan betina ). Ransum kg BB dan SDI dipecah menjadi 2 fase pemberian (grower: 20 - 60 kg berat badan (BB), dan finisher: 60 - 105 kg BB), ransum SD2 dipecah menjadi 4 fase pemberian (grower I: 20 - 40 kg, grower II:  40 - 60 kg, finisher I : 60 - 80 kg, dan finisher II: 80 - 105 kg BB).Babi dengan berat awal 20 kg sebanyak 72 ekor (36 jantan dan 36 betina) dikandangkan dengan kondisi lingkungan yang serupa (200C) selama penelitian. Suplementasi ransum dasar dengan asam amino esensial tidak mempengaruhi konsumsi ransum harian (2,49, 2,43, dan 2,36 kg masing-masing untuk BD, SD1 dan SD2, P>0,05). Babi pada masa pertumbuhan finisher mengonsumsi ransum harian terbanyak (2,77 - 2,83 kg) dibanding masa pertumbuhan lainnya (P<0,01). Babi jantan cenderung mengonsumsi  ransum harian lebih banyak (P<0,11) dibanding babi betina (2,49 kg vs. 2,36 kg). Babi yang diberi ransum yang diperkaya dengan asam amino tumbuh lebih cepat (0,93 dan 0,96 kg/hari untuk SD1 dan SD2) dibanding babi yang diberi ransum kontrol (0,82 kg/hari), P<0,01. Pertumbuhan tercepat terjadi pada masa awal finisher (60 - 80 kg BB), yaitu 1,07 kg/hari, sedangkan kecepatan pertumbuhan pada masa pertumbuhan yang lain sebanding(0,85; 0,86; dan 0,83 kg/hari). Babi yang diberi ransum yang diperkaya dengan asam amino esensial dapat menggunakan ransum dengan lebih efisien (2,68 dan 2,58 kg ransum/kg PBB untuk SD1 dan SD2) dibanding babi yang diberi ransum kontrol (3,03 kg ransum/kg PBB), P<0.01. Diantara masa pertumbuhan, awal masa pertumbuhan grower (20-40kg BB) mempunyai efisiensi penggunaan ransum tertinggi(2,16 kg ransum/kg PBB) dan akhir masa pertumbuhan finisher (80-105 kg BB) mempunyai nilai efisiensi terendah(3,55 kg ransum/kg PBB),P<0.01. Babi jantan cenderung lebih efisien dalam menggunakan ransum dibanding babi betina (2,66 vs 2,87 kg ransum/kg PBB), P<0,09.
